A Characteristic EEG Pattern of Angelman Syndrome

  • The two new female cases of Angelman syndrome (AS) were described, which diagnosed on the basis of clinical features (dysmorphic facial features, severe mental retardation with absent speech, peculiar jerky movements, ataxic gait and paroxysms of inappropriate laughter) and neurophysiological findings. Failure to detect the deletion of the long arm of chromosome 15 or the absence of epileptic seizure were not considered sufficient to exclude a diagnosis of AS. Feeding problems, developmental delay and early signs of ataxia, especially tremor on handling objects and unstable posture when seated, proved effective as the clinical markers for early diagnosis of AS. Most of the authors agreed about the existence of three main EEG patterns in AS which may appear in isolation or in various combinations in the same patient. The most frequently observed pattern in children has prolonged runs of high amplitude rhythmic 2-3 Hz activity predominantly over the frontal region with superimposed interictal epileptiform discharges. High amplitude rhythmic 4-6 Hz activity, prominent in the occipital regions, with spikes, which can be facilitated by eye closure, is often seen in children under the age of 12 years. The EEG findings are characteristic of AS when seen in the appropriate clinical context and can be helpful to identify AS patients at an early age when genetic counselling may be particularly important.

Unexpected Isolation of Leclercia Adecarboxylata in Dermatitis of a Dog

  • A two-year-old intact male Labrador retriever was presented with generalized erythema, pustule and pruritus. A skin screening test revealed that there were no fleas but bacteria and dermatophytes were present. Blood testing revealed no remarkable findings. The patient was prescribed systemic medication of enrofloxacin 30 mg/kg once a day and itraconazole 10 mg/kg once a day and topical medication of 2% chlorhexidine shampoo twice a week for 2 weeks. Two weeks after the prescription, aerobic culture confirmed that the bacteria were Leclercia Adecarboxylata and Pseudomonas putida was sensitive to enrofloxacin. Therefore, more medicine was prescribed for 4 weeks to alleviate clinical signs. After six weeks of medication, clinical signs were alleviated and skin screening test revealed no remarkable findings. Bacterial and fungal skin infections are common in dogs. However, there are no reports of Leclercia Adecarboxylata infection even in gastrointestinal tract in veterinary medicine. This is the first report of Leclercia Adecarboxylata infection in dogs. This report proved that Leclercia Adecarboxylata can cause skin problem in dogs.

한국인의 치은연하 치태에서 Fusobacterium nucleatum의 분리 (Isolation of Fusobacterium nucleatum from subgingival plaque in Korean)

  • The purpose of this study was to isolate and characterize the Fusohacrerium nucleatum (F. nucleatum) from subgingival plaque in Korean periodontitis patients. The subgingival plaque samples of periodontitis patient were collected with sterilized paper point. The paper point was put into reduced transfer medium and then immediately transferred to laboratory. The subgingival samples were diluted by 10,000 folds and plated on F. nucleatum-selective media agar plate. The plates were incubated at 37$^{\circ}C$ in an anaerobic chamber for 3 days. The violet-colored colonies were selected and subjected to further verification whether those are F. nucleatum or not. For further confirmation, 16S rRNA genes (rDNA) were cloned from each of bacterial clones and determined sequence of 16S rDNA. In this study, we found 17 distinct clinical isolates of F. nucleatum from subgingival plaque. The clinical isolates will be a useful in various studies in periodontology.

Pedicled Perforator Flaps for Reconstruction of Bilateral Knee Defects: A Case Report

  • Reconstruction of soft tissue defects of the knee has always been a challenging task for plastic surgeons. Various reconstructive choices are available depending on the location, size, and depth of the defect relative to the knee joint. Defects on the knee joint have several characteristic features. The use of a free flap is preferred for reconstructions involving obliteration of large-cavity defects, but recipient pedicle isolation can be difficult because of the extent of the injury zone. Furthermore, the true defect during knee joint flexion is larger than during knee joint extension, and a durable flap is necessary for joint movement. We report for the first time on the use of pedicled perforator flaps for reconstruction of bilateral knee defects in a 76-year-old woman. The operative procedure required skeletonizing the perforators of an antero-lateral thigh flap and antero-medial thigh flap and rotating the flap in the defect. The patient returned to normal daily activity and had a full range of motion two months after the accident. The shorter operating time with decreased donor site morbidity and its durability make this flap a valuable alternative for soft tissue reconstruction of the knee.

  • COVID-19 (Corona Virus Disease-2019) is an infectious respiratory disease caused by the most recently discovered coronavirus, SARS-CoV-2 (Severe Acute Respiratory Syndrome Corona virus-2). This new viral disease was unknown before the outbreak began in Wuhan, China, in December 2019. As of November 16th 2020, it affects about 54.3 million populations, death troll increased to 1.32 million cases in worldwide. Whereas in India 8.85 cases are infected with COVID-19, of which 1, 30, 112 cases were died. Till now there has been no specific anti-virus drug or vaccines are available for the treatment of this disease, the supportive care and non-specific treatment to the symptoms of the patient are the only options in Biomedicine, the entire world turns its attention towards alternative medicine or Traditional medicine. Siddha medicine is one of the primordial systems of medicine practiced in the southern part of India, it dealt a lot about pandemic, and its management. This review provides an insight into Pandemic in Siddha system and its management in both ancient history and modern history, National and state level Government policies related to current pandemic, World Health Organization (WHO) guidelines on usage of unproven drug during infectious disease outbreak, Preparedness of Siddha system during a pandemic outbreak Challenges and Recommendations.

Paradoxical Response of Giant Left Atrial Appendage Aneurysm after Catheter Ablation of Atrial Fibrillation

  • We report the case of a 43-year-old male with both giant left atrial appendage (LAA) aneurysm and drug-refractory atrial fibrillation (AF). The patient was treated with percutaneous electrical isolation of cardiac arrhythmogenic substrate, and has been free of AF symptom over one year. Although the surgical resection of giant LAA aneurysm is mostly used to prevent systemic thromboembolism, we have performed follow-up of the giant LAA aneurysm using cardiac magnetic resonance (CMR) imaging and transesophageal echocardiography (TEE) after the successful catheter ablation of refractory AF. At one-year follow-up CMR, the giant LAA aneurysm showed remarkable enlargement as well as decreased contractility. Additionally, one-year follow-up TEE showed spontaneous echo contrast as an indicator of blood stasis in the giant LAA aneurysm. Those findings of giant LAA aneurysm suggest that the risk of thromboembolism may be high despite termination of AF.

응급실 간호사의 결핵감염관리에 대한 인식과 수행 (Emergency Nurses' Perception and Performance of Tuberculosis Infection Control Measures)

  • Purpose: The purpose of this study was to investigate the perception and performance of tuberculosis infection control measures among emergency department nurses. Methods: A structured questionnaire was administered to a convenience sample of 227 emergency department nurses from 20 hospitals in B City. Results: The mean perception score was 4.52 out of a possible 5, and the mean performance score was 3.31 out of a possible 5. There were big differences between the perception and performance score on the items regarding wearing a gown or mask, while there were small differences on the items regarding dealing with patients' articles or medication. Conclusion: Emergency nurses showed high perception and low performance score on the items related to facilities and regulation such as visitation restriction or patient isolation. There was a positive correlation between perception and performance score. Conclusion: For emergency nurses, perception is higher than performance of tuberculosis infection control measures. The findings indicate the importance of providing continuing education for emergency nurse professionals seeking to update and increase their knowledge and to develop evidence-based nursing protocols on tuberculosis infection control in emergency department.

An Atypical Case of Middle East Respiratory Syndrome in a Returning Traveler to Korea from Kuwait, 2018

  • We report a case of Middle East respiratory syndrome coronavirus (MERS-CoV) infection in a 61-year-old businessman returning from Kuwait. The patient arrived there on August 16, 2018, developed watery diarrhea on August 28 (day 0), and came back to Korea on September 7 (day 10) as his condition worsened. Upon arrival, he complained of diarrhea and weakness, but denied any respiratory symptoms, and he directly went to visit an emergency room. Chest radiography revealed interstitial infiltrates in the lungs, and he was immediately transferred to an isolation unit. Quantitative real-time PCR analysis of sputum samples taken on day 11 returned positive for MERS-CoV. No secondary MERS-CoV infection was identified among people who had close contact with him. This case underscores the importance of a high index of suspicion of MERS-CoV infection in any febrile patients who present after a trip to the Middle East.

  • Purpose: This study aims to analyze design guidelines for hospice facilities in the US, UK, and Canada focused on design considerations and space requirements, and utilizes them as baseline data for establishing standards for Korean hospice facilities. Methods: Comparative review was carried out to investigate hospice care models, design consideration, and room sizes and requirements for design guideline of hospice facilities in United States, UK and Canada identified on electronic database and review articles, and to examine major characteristics and tendencies of hospice facilities. Results: The hospice care models characteristics in design guidelines is generally largely divided into hospital-based hospice facility, Nursing home-based hospice facility, and daycare hospice. The design considerations in hospice facilities focused on medical efficiency, flexibility, barrier-free environment, person-centered care, and stability. There is also a need for single resident room, rooms for the patient's family, and isolation room for infection control. Implications: it is recommended to establish standards for the installation and operation of required and recommended rooms and considerations when establishing the standards of hospice facilities in Korea. This Study is limited to a simple comparative analysis of the framework of guideline.

X-Rays through the Looking Glass: Mobile Imaging Dosimetry and Image Quality of Suspected COVID-19 Patients

  • Background: This paper aims to evaluate the clinical utility and radiation dosimetry, for the mobile X-ray imaging of patients with known or suspected infectious diseases, through the window of an isolation room. The suitability of this technique for imaging coronavirus disease 2019 (COVID-19) patients is of particular focus here, although it is expected to have equal relevance to many infectious respiratory disease outbreaks. Materials and Methods: Two exposure levels were examined, a "typical" mobile exposure of 100 kVp/1.6 mAs and a "high" exposure of 120 kVp/5 mAs. Exposures of an anthropomorphic phantom were made, with and without a glass window present in the beam. The resultant phantom images were provided to experienced radiographers for image quality evaluation, using a Likert scale to rate the anatomical structure visibility. Results and Discussion: The incident air kerma doubled using the high exposure technique, from 29.47 µGy to 67.82 µGy and scattered radiation inside and outside the room increased. Despite an increase in beam energy, high exposure technique images received higher image quality scores than images acquired using lower exposure settings. Conclusion: Increased scattered radiation was very low and can be further mitigated by ensuring surrounding staff are appropriately distanced from both the patient and X-ray tube. Although an increase in incident air kerma was observed, practical advantages in infection control and personal protective equipment conservation were identified. Sites are encouraged to consider the use of this technique where appropriate, following the completion of standard justification practices.
