류머티스 환자 이력 데이터에 기반한 환자 플로우 모니터링 시스템 (Patient Flow Monitoring System based on Rheumatic Patient History Data)

  • 최근 병원에서 수집되는 데이터를 전자적으로 저장, 관리 및 공유하기 위하여 병원정보시스템이 널리 활용되고 있다. 이러한 시스템들은 현대 병원들의 업무 효율성 제고에 크게 기여해왔으나, 건강관리 서비스 제공자들의 의사 결정을 지원하기 위해서는 수집된 환자 관련 데이터를 적절히 가공하고 재사용하는 것이 필요하다. 특히, 본 논문은 만성 질환과 관련된 외래진료과 운영관리를 위한 환자 플로우 모니터링을 제안하고, 이와 관련된 사항들에 대해 토의하고자 한다. 제안하는 시스템은 환자 이력 데이터에 프로세스 마이닝 기법을 적용하여 추출한 표준 프로세스 모형 및 여러 가지 평가 지표들을 시각화하며, 이를 통해 관리자들은 외래진료과의 운영을 평가하고 개선할 수 있다. 본 논문에서는 환자 플로우 모니터링 시스템을 류머티스 진료과에 적용하였으며, 아이패드 기기에 최적화된 프로토타입 시스템을 예시로 보이고자 한다.

  • Maxillary sinus infection following Le Fort I osteotomy is rare in patients without a history of preexisting nasal symptoms. A case of a 19-year-old male patient who suffered from preoperative chronic non-allergic rhinitis and developed repetitive postoperative maxillary sinus infection after Le Fort I osteotomy is reported.

부분 심내막상 결손증의 치험 1예 (Partial Endocardial Cushion Defect with Unusual Clinical Course)

  • This is a case report of spontaneous regression of elevated pulmonary vascular resistance after the age of 6 years in a patient with partial endocardial cushion defect. The patient was first evaluated and considered to be highly risky for surgical correction because of obstructive type of pulmonary hypertension and presisting congestive heart failure at the age of 6 years, during which time medical treatment for congestive heart failure and sufferring from respiratory infection were only provided. Finally the patient was reevaluated at the age of 16 years, in January of 1980 when cardiac catheterization was revealed markedly reduced pulmonary vascular resistance and pulmonary hypertension as well. The patient was operated upon with uneventful postoperative recovery. So we report this case with review of the literatures regarding natural history regarding natural history of partial endocardial cushion defect.

  • Traditionally, nurses keep the written patient records, which are referred as nursing care plan. Nursing care plan reports are one of the most important documents in the application of nursing processes. Typically, nurses prepare the plans by including general patient information as well as the patient's medical history information. In addition, the patient's developmental history and other specific health related information are part of the plans. The plans are usually concluded with the goals of the nursing care plan, nursing diagnoses, expected outcomes of the care, and possible nursing interventions. The nursing diagnoses, outcomes, and interventions are defined by North American Nursing Diagnosis Association (NANDA). This means that the nurses will select the appropriate diagnoses, outcomes, and interventions from an approved set. We developed a web-based nursing care plan generation system. In this paper, we report our work on developing a visual interface to the NANDA nursing diagnoses, outcomes, and interventions database as a part of the web-based nursing care plan generation system.

방사선사진 검사 선택기준에 대한 고찰 (Review of Radiographic Selection Criteria : The update is necessary)

  • Radiographic examinations are used to discover and define the type and extent of disease in many clinical situations. Currently, it is common for dental patients to receive dental radiographic examinations on a routine schedule with little variation in spite of different patient signs and symptoms. Such routine scheduling of radiographic examinations may result in unnecessary patient exposure. There is also the possibility of under-utilization of radiography; this can result in inadequate or excessively delayed diagnosis. Patient selection criteria are descriptions of clinical conditions derived from patient signs, symptoms and history that identify patients who are likely to benefit from a particular radiographic examination. It makes possible making individual decisions based on history, clinical examination and risk factors. The radiographic selection criteria of dentistry have been revised in accordance with recent guidelines and peer-reviewed research in USA and Europe, but the studies of these subjects are not sufficient in our nation. In this review article, the relating factors and update necessity of radiographic selection criteria are discussed.

Mycobacterium Avium-intracellulare Complex와 M. Fortuitum에 의한 폐항산균증(肺抗酸菌症) 3례(例) (Nontuberculous Pulmonary Infection in Two Patients with Mycobacterium avium-intracellulare Complex and a Patient with M. fortuitum)

  • Two cases of pulmonary disease in a 54 year-old female and a 70 year-old male patient due to Mycobacterium avium-intracellulare complex(MAIC) and a case of pulmonary infection ina 69 year-old male patient due to M. fortuitum(MF) were found recently in this institute. All three patients had a long history of anti-tuberculous chemotherapy because they were initially diagnosed as pulmonary tuberculosis. A 70 year-old male patient infected with MAIC had an unsuccessful chemotherapy history of isoniazid(INH), para-aminosalicylic acid(PAS) and streptomycin(SM) with an incomplete, temporary, symptomatic improvement, for three years since 1964 when he was first diagnosed as pulmonary tuberculosis on physical examination. A 54 year-old female patient infected with MAIC also had an unsuccessful chemotherapy history with the various anti-tuberculous drugs since 1958. Both patients discharged large number of MAIC in their sputum specimens for at least more than one year, but no M. tuberculosis at all. A 69 year-old male patient infected with MF was diagnosed as moderately advanced pulmonary tuberculsis in 1977. Combined chemotherapy with INH+PAS+pyrazinamide(PZA) improved his clinical symptoms, however, his chest radiograph was deteriorated again in 1980 one year after he stopped therapy. Therefore he started chemotherapy again with INH+ethionamide(TH)+cycloserine(CS) but no improvement was noticed. MF was cultured from his sputum in August 1981 and he continuously discharged the same bacilli until last examination of January 1982. Whether all three patients were initially !infected with nontuberculous mycobacteria or complicated with predisposing tuberculosis was not clear because there were no reliable bacteriological examination records.

  • Chronic disease is linked to patient's' lifestyle. Therefore, doctor has to monitor his/her patient over time. This may involve reviewing many reports, finding any changes, and modifying several treatments. One solution to optimize the burden is using a visualizing tool over time such as a timeline-based visualization tool where all reports and medicine are integrated in a problem centric and time-based style to enable the doctor to predict and adjust the treatment plan. This solution was proposed by Bui et. al. [2] to observe the medical history of a patient. However, there was limitation of studying the diabetes patient's history to find out what was the cause of the current development in patient's condition; moreover what would be the prediction of current implication in one of the diabetes' related factors (such as fat, cholesterol, or potassium). In this paper, we propose a Grid-based Interactive Diabetes System (GIDS) to support bioinformatics analysis application for diabetes diseases. GIDS used an agglomerative clustering algorithm as clustering correlation algorithm as primary algorithm to focus medical researcher in the findings to predict the implication of the undertaken diabetes patient. The algorithm was Chronological Clustering proposed by P. Legendre [11] [12].

  • 저자들은 임신 중에 수두를 앓거나 수두 환자와 접촉한 과거력이 없는 산모로부터 출산된 신생아가 생후 7일에 수두를 앓고 난 직후의 아이와 접촉한 후 생후 9개월에 우측 11번 흉곽 신경절을 따라 발생된 대상포진을 임상 및 검사로 확진하였고, acyclovir로 치유한 경험을 하였기에 문헌 고찰과 함께 보고하는 바이다.

Hereditary protein S deficiency presenting acute pulmonary embolism

  • Protein S deficiency is one of the several risk factors for thrombophilia and can cause blood clotting disorders such as deep vein thrombosis and pulmonary embolism. A 54-year-old man was admitted with the complaint of dyspnea and was diagnosed with pulmonary embolism. The patient had very low level of free protein S, total protein S antigen, and protein S activity (type I protein S deficiency). In history taking, we found that his mother, 78 year old, had a history of same disease 10 years ago, and confirmed the pronounced low level of protein S. The patient's son also had very low level of protein S, however there had not been any history of pulmonary embolism yet. This case study suggests that asymptomatic persons with a family history of protein S deficiency and pulmonary embolism should be checked regularly for early detection of the disease, as protein S deficiency can be suspected.

치과 외래 환자의 초진시 병력에 관한 조사 연구 (A Study on Medical History of Dental Outpatient with a Brief Questionnaire)

  • The need for a brief questionaire, written in language that the patient can readily understand, to provide the dentist with an adequate current medical history of his patient has been estabished. So this study with brief medical questionaire for routine use in dental practice is discussed in relation to the findings from its use among a survey group of 513 patients. The results are that : 1. In 58.3% percent of the patients surveyed, at least one question was answered posi tively. 2. The frequency of positive responses in the various group was: medical treatment within recent 2 years, 27.5%; hospitalization, 27.1%;drug history within recent year, 25.9%. 3. The important systemic diseases being considerd in dental practice are: anemia, 4.9%; hypertension, 4.1%; tuberculosis, 3.9%.
