경두개 색조 도플러 초음파검사를 이용한 대뇌동맥 파열 이후 지속적인 혈관 연축 증례 분석 (Case Analysis of Persistent Vasospasm after Cerebral Artery Aneurysm Rupture by Using Transcranial Color Coded Doppler Ultrasonography)

  • In this case analysis, a patient was diagnosed with subarachnoid hemorrhage (SAH) in a 49-year-old female and showed persistent vasospasm after coil emboilzation in an aneurysm. The patient suffered from persistent vasospasm and performed angioplasty a total of 6 times. Transcranial color coded doppler (TCCD) was performed 12 times to monitor vasospasm. As a result, repetitive cerebral blood flow tests were low cost and safely performed without exposure to invasive radiation through the TCCD, and the repeatability and reproducibility of the test were confirmed with the capabilities of a trained professional radiological technologist.

Spontaneous resolution of new coronary artery aneurysm following guideline-directed medical therapy after drug-eluting stent implantation

  • Here, we present a case of a 56-year-old man with acute myocardial infarction. The patient underwent percutaneous coronary intervention (PCI) at the left main bifurcation and mid-left anterior descending artery using drugeluting stents. Four months after the PCI, the patient was readmitted for cardiac arrest. Coronary angiography (CAG) revealed stent thrombosis in the left main-to-proximal left anterior descending artery and in-stent restenosis in the left main-to-proximal left circumflex artery. We performed balloon angioplasty at the left main to mid-left anterior descending artery and left main to proximal left circumflex artery stents; subsequently, blood flow was fully restored. However, contrast agent extravasation was observed outside the mid-portion of the left main artery to the proximal left anterior descending artery stent, indicating the presence of a coronary artery aneurysm (CAA) outside the stent. After guideline-directed medical therapy with dual antiplatelet agents and high-intensity statins, follow-up CAG revealed near-resolution of the CAA, absence of stenotic lesions, and good blood flow.

  • Purpose : This purpose of this study is to make a better public hospital patient room to reflect the views of the patient. Methods: Survey to examine patient experience on healing environment in public hospital have been conducted for the data collection. 229 open-ended answers have been analysed statistically by MS Excel program. Results : 1) If there is no toilet space in the six-patient room, installation of toilet in the patient room is necessary. 2) The most Frequently used patient furnitures are necessary to accomodate the patients needs. Depending on the patient condition, installation of a portable or fixed the room furniture for the convenience to the using furniture. 3) According to the flow of modern times, wireless internet access is required to install of expand the space. Also be used to establish a regulations of internet space. 4) To create healing environment infection free environment should be provided. 5) Need for improvements of patient room for relatives of patient. It is necessary to secure a comfortable space to stay. 6) Separation is needed among the patients through therapeutic classification. Implication : In order to provide a better patient room environment, it is necessary to reflect the patient's opinion.

Endovascular Treatment of Symptomatic High-Flow Vertebral Arteriovenous Fistula as a Complication after C1 Screw Insertion

  • High-flow vertebral arteriovenous fistulas (VAVF) are rare complications of cervical spine surgery and characterized by iatrogenic direct-communication of the extracranial vertebral artery (VA) to the surrounding venous plexuses. The authors describe two patients with VAVF presenting with ischemic presentation after C1 pedicle screw insertion for a treatment of C2 fracture and nontraumatic atlatoaxial subluxation. The first patient presented with drowsy consciousness with blurred vision. The diffusion MRI showed an acute infarction on bilateral cerebellum and occipital lobes. The second patient presented with pulsatile tinnitus, dysarthria and a subjective weakness and numbness of extremities. In both cases, digital subtraction angiography demonstrated high-flow direct VAVFs adjacent to C1 screws. The VAVF of the second case occurred near the left posterior inferior cerebellar artery originated from the persistent first intersegmental artery of the left VA. Both cases were successfully treated by complete occlusion of the fistulous portion and the involved segment of the left VA using endovascular coil embolization. The authors reviewed the VAVFs after the upper-cervical spine surgery including C1 screw insertion and the feasibility with the attention notes of its endovascular treatment.

  • 호흡 측정기는 기도를 통해 환자의 폐에 드나드는 기체의 양을 측정하여 호흡에 관련된 기능을 판단하는 의료기기이다. 수술실에서 전신 마취시 사용하는 마취기용 인공호흡기는 미미한 양의 마취제 과용으로도 환자의 생명을 앗아갈 수 있으므로 정확한 호흡량 측정이 특히 중요하다. 본 연구에서는 호흡량 측정에 영향을 주는 요인들로부터 정확한 유량을 산출해내는 방법에 대해 고찰하고. 마취용 인공호흡기에 적용하여 수술환자의 호흡특성을 알아내는 호홉측정기를 구현하였다. 인공호흡기에 많이 사용되고 있는 차동압력형 유량센서를 이용하여 압력, 온도, 가스구성의 상관관계에 따라 유랑을 계측하였으며 유량-타동압력의 비선형적 관계를 선형화하였다. 또한, 휴대형 기기에 적합하도록 전력 효율을 최대화 할 수 있게 시스템을 설계하였다

  • The patient characters in medical emergency facilities is in emergent situation and very wide medical area. So, the spatial composition of this facilities should be more efficient than general medical ones. For this reason, patient who visit medical emergency center should be triaged by each property of patient. And then the clinical space is to be composed in response to flow of each patient group that triaged. Therefore this study aims to find method of the spatial composition and architectural details of medical emergency center.

노인 우울증 환자의 진단에서 뇌 SPECT의 효율성 (Utility of Brain SPECT in Diagnosis of Elderly Depressed Patient)

  • Primary depression with cognitive impairment, referred to as depressive pseudodementia, may be mistaken for a progressive degenerative dementia. Recognition of primary depression is clinically important because of its treatability. To differentiate depression from degenerative dementia, author used brain SPECT. By the result the regional cerebral blood flow(rCBF) in elderly depressed patient was decreased in the right frontal cortex. The pattern of rCBF was different from that of dementia which shows decreased rCBF in bifrontal cortex. By using brain SPECT in depressed elderly patient with cognitive impairment, the discrimination from dementia will be more effective and accurate.

  • Pulsatile Extracorporeal Membrane Oxygenation(ECMO) can mitigate the heart load and raise the patient's blood perfusion. But If the ECMO pulsate the blood flow during the systolic period, It can burden to the patient's heart. To avoid the heart injury, we have to consider the relation between output of ECMO, hemodynamic states and heart movement. To raise the efficacy of the pulsatile ECMO, we investigated the coronary perfusion, cardiac muscle tension and hemodynamic states during the ECMO perfusion by using the mathematical model of human blood circulatory system and ECMO. The outflow data of the pulsatile ECMO(T-PLS, Bioheartkorea, Korea) was obtained in vitro experiments. According to the phase and pumping rate of the ECMO, the heart's load and coronary perfusion could be adjusted to the proper levels. The results of the human- ECMO lumped parameter model showed that the synchronizing operation of the pulsatile ECLS can be helpful at stabilizing the patient's hemodynamic states.

Patient Flow Optimization for Outpatient Department Using Discrete-Event Simulation

  • The patient's waiting time and length of stay have been reported as a factor decreasing their satisfaction in the hospital, especially in developing countries. This paper focuses on modeling hospital's outpatient department workflow in a developing country and optimizing the patient waiting time as well as total length of stay. By using discrete-event simulation, many alternative scenarios have raised, such as adding more working time, altering human resources, and adjusting the staff's responsibility, those scenarios will be examined to explore better settings for the hospital. The results show that possible to achieve a 9.6% reduction in patient total length of stay and it could be accomplished without adding more resources to the hospital.
