  • The purpose of this study is to investigate patient satisfaction for the spacial environment of the rehabilitative facilities, and by this to examine areas of patients' needs to develop patient-friendly improvement in spacial environment. 308 patients were surveyed at physical therapy centers of 6 hospitals or rehabilitative institutions in Daejeon with patient satisfaction questionnaire. The spatial environment satisfaction was categorized into 7 constructs and the data were statistically analyzed and compared by t-test and ANOVA according to the patient characteristics. As a result, inter-floor transfer was found to be the area with the least satisfaction, especially in the in-patient group. Locating floor level was the second lowest satisfied construct. Comfort was relatively highly satisfied construct overall, but patients with neurological disease had less satisfaction than with musculoskeletal disease. Patients in 20's were less satisfied in wayfinding than 60's. Satisfaction for interior design was lower in subject in 50's than 70's. These findings can be used as evidences to improve spacial environment design for rehabilitative facilities to the better patient satisfaction.

  • Recently, problems regarding marital adjustment of cancer patients are recognized as serious matter of concern for it had been reported that these issues have severe negative effects on relationship stability within marriage. It is noteworthy how marital adjustment is found to be critical for the cancer patient's mental stability, healing process and chances of survival as reported in the research. Thus, the ultimate goal of this research is to contribute to improvement of the stable relationship and also to formulate the social service related strategy for marriage adjustment for married cancer patient married couples in Korea. Actor and Partner Interdependence Model was formed in order to observe how the communication of the married couple affects marital adjustment and how this has an actor effect as well as partner effect. In order to authenticate this research model, dyadic data which sample the cancer patient and the spouse as a pair. Survey was conducted as the cancer patients who had been diagnosed with cancer and their respective spouses. The survey was taken from 160 married couples which totals to 320 people all together. Results indicated the cancer patient and the spouse have a significant relationship in marital adjustment. Through this, it had been concluded that marital adjustment is not separated but co-dependent with dynamic relationship. And the mutual constructive communication had proven to have a significant positive effect on one's marital adjustment. In demand-withdrawal communication, its negative effect on the self effect was found to be significant in both the patient and the spouse to both of their marital adjustment. Mutual avoidance communication had shown to only cause significant negative effect on self effect to the patient. In verifying the partner effect, significant negative effect was caused by patient's demand-withdrawal communication in partner effect on the spouse's marital adjustment. The results of this study are discussed in term of their implications for clinical interventions for the betterment of marital adjustment among cancer patients and their spouses. The suggestions for future research are discussed also.

  • This study is a descriptive research study to compare the safety perception between patients and nurses and to identify the factors affecting the nurses' safety management activities in tertiary hospitals. Data were collected with structured questionnaires from 147 patients and 147 nurses from a university hospital in J city, Gyeongnam, from July 24 to September 23, 2019, and analyzed using SPSS 23.0. The result showed that the patient's safety perception was significantly higher than the nurse's perception. The significant factors affecting patient management activity were nurse's educational level, position, effective communication, patient safety perception, and perception of importance on patient safety management. In the multiple regression analysis, knowledge about 'effective communication', and 'concerned about patient safety management', explained 54.8% of the nurses' patient management activity. Based on the results of this study, it is emphasized that nurses need to develop strategies to improve the communication and attention between patients and hospital members to improve patient safety management activities in tertiary hospitals.

  • The concept of precision medicine-prevention and treatment strategies that take individual variability into account-is hot issue of US in the year 2015. Precision medicine is a new concept that approach patients individually by there characteristics, such as genome, life style, environmental exposure, etc. For developing the precision medicine, National Institute of Health of US has been prepared the Precision Medicine Initiative Cohort Program, at least 1 million people cohort. The US President Obama announced the Precision Medicine Initiative on 30th January 2015. He announced that he will pioneer a new model of patient-powered research that promises to accelerate biomedical discoveries and provide clinicians with new tools, knowledge, and therapies to select which treatments will work best for which patients. Most medical treatments have been designed for the 'average patient.' As a result of this 'one-size-fits-all-approach,' treatments can be very successful for some patients but not for others. This is changing with the emergence of precision medicine, an innovative approach to disease prevention and treatment that takes into account individual differences in people's genes, environments, and lifestyles. Precision medicine gives clinicians tools to better understand the complex mechanisms underlying a patient's health, disease, or condition, and to better predict which treatments will be most effective. The healthcare researcher should prepare the new medicine era such as bio-information technology convergence, big data study.

  • When unapproved users access to healthcare system and use medical information for other malicious purposes, it could severely threaten important information related to patients' life, because in ubiquitous environment healthcare service makes patient's various examination results, medical records or most information of a patient into data. Therefore, to solve these problems, we design RBAC(Role Based Access Control) for U-healthcare that can access control with location, time and context-awareness information like status information of user and protect patient's privacy. With implementation of the proposed model, we verify effectiveness of the access control model for healthcare in ubiquitous environment.

  • Purpose: This study was conducted to examine the experience of nurse in comprehensive nursing care. Methods: Experiential data collected from 9 nurses through in-depth interviews. Participants were selected from nurses working in the comprehensive nursing care unit at general hospital. The main question was "Can you describe your experience in the comprehensive nursing care unit?" All interviews were recorded and transcribed, then analyzed using Colaizzi's method. Results: Nine themes were derived from the analysis: 'Practice nursing care', 'Feel thankful of the client', 'Difficulty in nursing due to absence of patients' guardian', 'Tired of over-demanding patient and distrust of guardian', 'Confusion regarding one's identity as a nurse', 'Not enough to support system', 'Insufficient pre-training for nurse and client', 'Requirement of work establishment for nurse and nurse aid', 'Concerns about low rewards and high safety accidents'. Conclusion: As a comprehensive nursing service, the nurses provided total patient care, and patient satisfaction and expression of appreciation increased. However, disadvantages were identified, such as patients' excessive needs, communication difficulties, lack of support systems, low compensation, and a high number of safety accidents. Therefore, systematic comprehensive nursing will be achieved if these shortcomings are addressed.

  • In an effort to obtain preventive, diagnostic and therpeutic medical and exercise training information of rheumatoid arthritis as well as to provide pertinent data to be used in development of exercise program for the management of rheumatoid arthritis, this study was conducted by using literature review. Because RA is a disease that involves both joints and muscles, its activity in the different functional classes influences the patient's ability to exercise. A patient in Functional Class 1 may perform any type of exercise because the disease involvement has not yet reached major importance. Exceptions may include hard physical exercise, running, and individual racket sports. In almost all cases, bicycle exercise is possible. Patients in Functional Class 2 and a few in Class 3 can perform most types of exercise (especially cycling, walking, heated pool exercise and even jogging) in low activity phases of RA. Although a few patients in Functional Class 3 can walk, jog, and perform similar types of exercise, most persons in this class can swim or exercise on a bicycle if the type of exercise, its intensity, and its range of motion are modified according to the patient's anatomic and pathologic conditions. Most patients in Functional Class 4 are not able to carry out complicated movements. We conclude that physical training for persons with RA has physical, psychological, and social consequences that are clearly beneficial. We recommend training be one part of the many involved in the complicated treatment of RA.

  • The rupture of the aorta commonly follows major blunt trauma to the thorax. It has markedly increased in recent years, paralleling the rising number of vehicular accidents. The most frequent site of rupture is the area of the isthmus, with the ascending aorta second. The diagnosis of the condition from clinical data is difficult, and aortography is used whenever aortic tear is suspected. We are presenting a case of patient who had intimal tearing of the thoracic aorta with multiple injuries. The patient underwent surgical repair 28 days after injury with left heart [LA-Femoral artery] bypass.
