Investigation of Standard Evaluation for the Quality Control of General X-ray Systems

  • Thanks to the great development of technology in radiation, we are now able to reduce radiation exposure to the patients, and the radiographer and expenses in medical sector. We are also trying to produce ideal images which maintain useful information. These kinds of effort are increasing over the world. For that reason, we should get images which include necessary data of patients. Then it also can help to reduce radiation exposure to the patients. Therefore, we need to know the problems that cause a falling off in image's quality and check on generator in case of their electronic and mechanical errors. And moreover, we should anticipate the possibility of devices errors and prevent them with regular quality control. This investigation was conducted in medical institutions, institute of educations and hospitals. They are all in Seongnam-City. We used PMX-III, kVp meter to implement kVp test, mR / mAs output test, light fiel / beam alignment test, Reproducibility of exposure dose, half value layer test, reproducibility of exposure time test. in the case of hospitals, they perceive the importance of regular quality control and organize the regular quality control team so they can be satisfied with the error standard in most experiments. On the other hand, when it comes to medical institutions and institute of educations, they perceive the importance of regular quality control less than hospitals do. Radiographer need to understand the importance of regular quality control and practice it so they can get the fine ideal image with the lower dose to the patient.

Effect of Dual Task Training in Visual Control and Unstable Base on the Gait of Stroke Patient

  • This study examines changes in walking ability among patients with stroke after applying dual-task training under the condition of visual control and unstable supporting ground; the purpose is to provide reference data for selecting intervention methods that enhance the walking ability of patients with stroke. Among the patients with stroke who received rehabilitation treatment(at Rehabilitation Hospital B in Gyeonggi, South Korea from May 2014 to July 2014), 29 patients were selected as research subjects; all of them understood the purpose and contents of this research and agreed to participate in the experiment. The research subjects were divided into a visual control and unstable supporting ground dual-task(VUDT) group(10 patients), a visual control dual-task(VDT) group(10 patients), and an unstable supporting ground dual-task(UDT) group(9 patients); all of the subjects received 30-minute trainings, three times a week for a total of four weeks. A Timed-Up-and-Go(TUG) test was performed to investigate the change of walking function among the subjects, and a 10m walking test was conducted to measure their walking speed. According to the study results, all three groups showed significant differences after dual-task training; the dual-task training group under the condition of visual control and unstable supporting ground showed the most prominent change. This study confirmed that dual-task training using visual control and unstable supporting ground has a positive impact on the walking ability of patients with stroke. Through the study results, we found that implementing dual-task training under the condition of visual control and unstable supporting ground can more effectively improve the walking ability of patients with stroke, rather than performing visual control dual-task training or unstable supporting ground dual-task training only.

  • Dual-energy X-ray absorptiometry (DEXA) is the most widely used technical instrument for evaluating bone mineral content (BMC) and density (BMD) in patients of all ages. In 2016, DEXA devices operating is 5617 in Korea. In this study we investigated the quality of management practices survey for DEXA equipment and we analyzed it. We got a survey response rate of 12.6%. Accurate bone densitometry test is used data for estimation a patient's risk of fracture. However, improper bone densitometry will increase the possibility of causing a false positive. Therefore. it is essential to use the proper aids accurate bone densitomenty to be performed, and the quality control of the device to reduce the error factor of the tester through the training to reduce error for the device and the attitude.

Car-tire-related Crushing Injury of the Lower Leg in Children (자동차 바퀴에 의한 소아 아래다리의 압궤 손상)

  • Purpose: Crushing injuries by car tires result from a combination of friction, shearing, and compression forces and the severity of injury is influenced by the acceleration. Because car-tire injuries of the lower leg in children are common these days but they have received little attention; thus, our purpose was to look closely into this problem. Methods: A retrospective analysis was conducted of data from children under 15 years old age who visited an emergency department because of a car-tire-related crushing injury to the lower leg in pedestrian traffic accident from January 2008 to September 2012. The patient's age, sex, site of injury, degree of injury, associated injuries, type of surgery, and complications were reviewed. Results: There were 39 children, the mean age was 8.0 years, and 71.8% were boys. The dorsal part of the leg was involved most frequently. According to the severity classification, 15 children were grade I, 6 were grade II, and 18 were grade III. Among 24 patients, 13 were treated with skin graft and 3 were treated using a sural flap. Twelve patients developed complications, such as hypertrophic scarring, contractures, and deformities with significant bone loss. Conclusion: Various degrees of skin or soft tissue defects were caused in children by car tires. In this study, patients were often also had tendon or bone damage. Proper and timely initial treatments are needed to reduce the incidence of infection, the number of operative procedures, and the hospital stay.

Diagnostic Imaging Features of Asymptomatic Extrahepatic Portosystemic Shunt Detected by CT in Dogs (개에서 컴퓨터단층촬영술을 이용하여 진단한 임상증상이 없는 간외성 전신문맥단락의 영상학적 평가)

  • This study was performed to compare clinical and diagnostic imaging features between asymptomatic and symptomatic extrahepatic portosystemic shunts in dogs. The data of thirty patients diagnosed with extrahepatic PSS by multi-detector CT were reviewed, and the dogs were divided into asymptomatic (9/30) and symptomatic (21/30) groups. Signalments, hematologic results, liver size, morphologic classifications and main portal vein to abdominal aortic ratio (PV/AO) at the porta hepatis level from CT images were evaluated in two groups. Shih-tzu (5/9) was the most frequent breed in asymptomatic group, and various breeds were presented in symptomatic group. Mean age of asymptomatic group ($9.2{\pm}3.2$ years) was significantly higher than that of symptomatic group ($4.5{\pm}3.2$ years). The most morphologic form of shunt vessel was the splenophrenic shunt (16/30). PV/AO of asymptomatic group ($1.1{\pm}0.19$) was significantly higher than the values of symptomatic group ($0.55{\pm}0.19$). Clinical signs, hematologic results and diagnostic imaging findings of asymptomatic PSS are too nonspecific to suspect PSS. Therefore, considering of patient's age and CT examination with application of PV/AO ratio could be useful for the diagnosis of asymptomatic PSS.

A Comparison on the Operative Results of Benign Esophageal Disease by Video-Assisted Thoracic Surgery and Thoracotomy (양성 식도질환에서 개흉술과 비디오 흉강경을 이용한 수술 성적의 비교)

  • Background: Video-assisted thoracic surgery(VATS) is being used as a therapeutic modality in many diseases in which thoracotomy has been used. We studied that the VATS can substitute the thoracotomy in benign esophageal disease. Material and Method: Group I (n=18) underwent video-assisted thoracic surgery, and group II(n=19) thoracotomy. Group I includes 14 leiomyomas and 4 achalasias. Group II includes 16 leiomyomas and 3 achalasias. Operative technique is enucleation in the leiomyoma and modified Heller's myotomy in the achalasia. Analyzing factors of operation-efficacy are anesthetic time, operation time, hospital stay, chest tube drainage amount and chest tube removal day. The degree of the postoperative pain is assessed by the frequency of opioid analgesics injection. Result: There was no death in both groups. There were 5 complications in the group I and 2 in the group II. Prolonged pleural effusion and restenosis of achalasia occurred to 1 patient in each group. In the group I, there were 1 temporaty vocal cord palsy and 2 mucosal tear leading to thoracotomy. There were no differences in anesthesia time, operation time, hospital stay, total chest tube drainage amount, chest tube removal day and frequency of opioid analgesics injection. The amount of the chest tube drainage at POD 1 day was significantly lower in group I(155.6$\pm$77.8cc) than in group II(572.8$\pm$280.1cc)(p<0.05). Conclusion: The results of our data showed that video-assisted thoracic surgery for benign esophageal disease is as effective as thoracotomy and in addition, cosmetic effect is much better. We concluded VATS may be a substitute for thoracotomy in benign esophageal disease.

A Study on the Electrical Difference for The Limbs and Thoracic Impedance using Real-Time Bio-impedance Measurement System (실시간 생체임피던스 측정 시스템을 이용한 사지와 흉부 임피던스에 대한 전기적인 차이 연구)

  • Bio-impedance measurement system(BMS) is non-invasive and easy to implement a measurement method that allows determining the water content of a patient. The measurement conditions, the hardware specifications and the configurations of BMS devices must be well chosen in order to get correct and reproducible results. BMS was then conducted for the limbs and the thoracic using a lock-in amplifier and LabView control system with a frequency range of 1kHz-100kHz. From both the measurement data and the simulation results, we verified that the parameters in the proposed equivalent model and the trend of impedance variation according to the multi-frequency of applied current source are similar to those of human body. We believe that the real-time BMS developed in this study is highly reliable and applicable to the research on the clinical characteristics of the human being's impedance.

A Study on the Wearing and Manufacturing Condition of Artificial Breast in Breast Cancer Patients with a Mastectomy (유방절제술을 시행한 유방암 환자의 시판 인조유방 착용 및 판매실태)

  • The study surveyed patients undergoing a mastectomy to understand their experience and requirements on artificial breasts and sales status. We retrieved 149 of the 150 surveyed questionnaires; subsequently, 144 questionnaires, excluding missing values, were used in the analysis. According to the results of the study, about 67% of respondents said that artificial breasts were not used after undergoing a mastectomy, and more than half of the breast cancer patients were not aware of the importance of artificial breasts. Breast cancer patients wearing artificial breast felt uncomfortable and hot during daily activities as well as experienced difficulty in size selection and purchase price burdens. The three most responded brands were selected based on the results of the survey that also investigated the sales status of the commercial artificial breast. Company A was selling artificial breast to reflect the consumer's functional requirements, and Company C was selling artificial breast with various kinds of reduced weight. However, it is thought that consumers will have less choice since artificial breasts have more limited forms than various types. Therefore, it is believed that the patient needs artificial breasts that satisfy the shape of various types of mastectomy. This study can be used as basic data for artificial breast development studies.

Various Applications of Deep Inferior Epigastric Artery Perforator Free Flap (심부하복벽천공지 유리피판의 다양한 임상적 적용)

  • Purpose: The deep inferior epigastric perforator(DIEP) free flap is well known as an ideal donor site for the breast reconstruction. The flap can provide huge amount of fat tissue for breast and buttock contour, while it is also very useful as a thin skin flap to reconstruct the upper and lower extremities. We used a DIEP free flap in various site reconstructions besides the breast and would like to reinsure the usefulness of this flap. Methods: Twenty nine consecutive patients who underwent DIEP free flap surgery from 2001 January to 2007 December were reviewed. The case constituted seven male patients and twenty two female patients. There were sixteen breast reconstructions, five face reconstructions, five lower extremity reconstructions, two upper extremity reconstructions, and one buttock contour reconstruction. All clinical data were based on the patient's medical records. Results: All DIEP free flaps survived without major complications. There was no hematoma, seroma, or partial necrosis. The donor sites were closed primarily with linear scar on lower abdomen. The thinnest part of flap was 0.7 cm in thickness. The size of the largest flap was $38{\times}13cm$. The flaps were used in various types of skin and adipose tissue, adipose tissue only, and skin only according to the requirement of recipient site. Conclusion: The DIEP free flap was enough to provide a thin and huge flap for both breast and extremity reconstructions. It was able to provide versatile designs with sufficient adipose tissue. So we use it for 3 - dimentional face and buttock contour reconstructions. The DIEP free flap is a valuable reconstructive donor for face, upper and lower extremity in addition to breast without compromising the integrity of abdominal wall.

Quantitative Analysis of Lower Nose and Upper Lip Asymmetry in Patient with Unilateral Cleft Lip Nose Deformity using 3D camera (3D camera를 이용한 일측성 구순비변형환자에서의 비하부 및 상구순 비대칭의 정량적 분석)

  • Purpose: Analysis of lower nose and upper lip asymmetry in patients with unilateral cleft lip nose deformity has been proceeded through direct measurement and photo analysis. But there are limitation in presenting real image because of its 2 dimensional trait. The authors analyzed such an asymmetry using 3D VECTRA system (Canfield, NJ, USA) in quantitative way. Methods: In 25 Patients with unilateral cleft lip nose deformity(male 12, female 13, age ranging from 4 to 19), patients with right side deformity were 10 and left were 15. Analysis of asymmetry was proceeded through 3D VECTRA system. After taking 3 dimensional photo, alar area, upper lip area, nostril perimeter, nostril area, Cupid's bow length, nostril height and nostril width were measured. Correlation coefficient and inter data quotients were calculated. Results: In nostril perimeter, maximal difference of cleft side and non - cleft side was 39.3%, asymmetric quotient Qasy = Qcl/Qncl(Qcl, value of cleft side; Qncl, value of non - cleft side) was ranged from 0.84 to 1.85 and in seven cases the length of cleft side was smaller. In nostril area, maximal difference was 69.6% and in 13 cases cleft side was smaller. In lower nasal area, maximal difference was 37.2% asymmetric quotient Qasy = Qcl/Qncl was ranged from 0.47 to 2.03 and in 20 cases cleft side was smaller. The correlation coefficients of nostril perimeter and area were 0.8345. Conclusion: Using 3D VECTRA system, the authors can measure nostril perimeter and lower nasal area that could not been measured with previous methods. Asymmetry of midface was analyzed through area comparison in quantitative way. Futhermore, post operative change can be measured in quantitative method.
