The Effect of Acupuncture Therapy of Tian-zong (SI11) on Pulmonary Function Test of Patients with Dyspnea (천종혈(SI11) 자침이 자각적 호흡곤란 환자의 폐기능변화에 미치는 영향)

  • Objectives : Dyspnea is one of the most common symptoms that we meet, but so far there has been no research about the effects of acupuncture therapy on it. This study was performed in order to evaluate the effect of acupuncture therapy on dyspnea by means of using objective indexes. Methods : The effect of acupuncture therapy was assessed by analyzing the pulmonary function test (PFT) in 10 patients, who had suffered from dyspnea and had pain at the acupuncture point Tian-zong (SI11) when it was compressed by a finger, at our outpatient department. Each patient was treated with acupuncture therapy at both sides of Tian-zong. We compared the results of pre-treatment and post-treatment PFTs, including forced vital capacity (FVC), forced expiratory volume in one second (FEV 1.0), and peak expiratory flow rate (PEFR). Results : FVCs of pre-treatment and post-treatment PFTs were $2.23{\pm}0.34(L)$ and $2.40{\pm}0.36(L)$ respectively (P<0.01). FEV1.0s of pre-treatment and post-treatment PFTs were $1.54{\pm}0.37(L)$ and $1.65{\pm}0.41(L)$ respectively (p<0.05). PEFRs of pre-treatment and post-treatment PFTs were $3.41{\pm}1.26(L)$ and $3.93{\pm}1.42(L)$ respectively (p<0.05). Conclusions From the above data, we conclude that FVC, FEV1.0, and PEFR in dyspneic patients have been improved after acupuncture therapy of Tian-zong, so acupuncture therapy of Tian-zong may relieve the dyspnea symptom.

Comparative Study for Therapeutic Effects of the Low Back Pain Patients According to the Bee Venom Pharmacopuncture-induced Skin Hypersensitivity Reaction and the Sasang Constitution (봉약침으로 유발된 피부 과반응과 사상체질에 따른 요통환자의 치료효과에 대한 비교연구)

  • Objectives : The aim of this study was to analyze the interrelation of therapeutic effects according to the bee venom pharmacopuncture-induced skin hypersensitivity reaction and the sasang constitution. Methods : Data were collected from 62 patients who had been treated with bee venom pharmacopuncture due to low back pain. The patient's symptoms were evaluated with visual analogue scale(VAS), oswestry disability index(ODI). And we classified the patients according to the manifestation of skin hypersensitivity reaction, sasang constitution by QSCC II. Results : 22 patients had the skin hypersensitivity reaction, 40 patients hadn't. And 12 patients were Soeumin, 30 patients were Soyangin, 17 patients were Taeeumin. After treatment with bee venom pharmacopuncture 6 times(three times per week for 2 weeks), VAS was changed from $6.15{\pm}1.63$ to $4.00{\pm}1.64$. ODI was changed from $19.61{\pm}8.15$ to $14.76{\pm}8.07$. And there were significant associations. By the change of VAS and ODI between skin hypersensitivity reaction group and not, there were no significant associations. By the occurrence of skin hypersensitivity reaction between the sasang constitution, there were no significant associations. By the change of VAS and ODI between the Sasang constitution, there were no significant associations. Conclusion : This study shows that there were no significant associations in the frequency of skin hypersensitivity reaction according to the Sasang constitution. And the therapeutic effects of bee venom pharmacopuncture had no concern with skin hypersensitivity reaction and the Sasang constitution.

Acupuncture Experience in Patients with Chronic Low Back Pain(2): A Qualitative Study - Focused on Participants in Randomized Controlled Trial (만성 요통 환자의 침 치료 경험(2): 질적 연구 - 무작위 대조군 연구 피험자를 대상으로)

  • Objectives : To explore the experiences of patients with chronic low back pain(CLBP) in a randomized controlled trial(RCT) of acupuncture. Methods : Five qualitative focus group interviews were conducted at three Korean Medicine Hospitals. Two to 4 participants from the same group(real or sham acupuncture) in the RCT of acupuncture for CLBP discussed their experiences and perceptions of the clinical trial and the acupuncture treatment. Transcribed data were read independently by researchers and analyzed to categorize information and identify themes. Results : A total of 14 participants were included. Most of them discovered positive aspects of being a study subject and a patient. They recognized the differences between experimental and real-world clinical settings such as formal procedures of treatment, and different acupuncture device. Participants also expressed the weaker sensation of acupuncture compared to the previous experience. Especially, they were well aware of the 'subjects' role themselves, thus they observed their changes of symptom closely. As subjects were generally satisfied with their treatment and they had a good feeling to acupuncture after the trial, they expressed their willingness to participate in the future clinical trial of acupuncture. Conclusions : Our finding suggests that the Korean patients' experience of participating in an RCT was generally positive. Their tendency to perform the 'subject' role might affect the trial's process or overall results.

A Study of Perceived Discomfort of The Patients Under Local Anesthesia - Centering Around the Development of Measures - (국소마취 수술환자의 불편감에 관한 연구 - 도구개발을 중심으로 -)

  • Background : This study was attempted to provide a basic information necessary for the development of measures evaluating perceived discomfort of the patients under local anesthesia and the developments of nursing intervention and nursing standard. Method : The study subjects were a convenience sample of 100 surgical patients under local anesthesia. The researcher visited the patients directly and explained the purpose of the study to them. Then they were asked to fill out questionnaires which were developed and structured by the researcher, under their prior permission. Out patient's data were collected at recovery room after operation. Results : Factor analysis on measures evaluating the patients under local anesthesia showed that the measures consisted of eight factors including "explanation", "humane treatment", "motion limitation", "pain", "no togetherness", "waiting", "the environment of operating room", and "unkindness". When analyzing each of the factors regarding their degree of contribution to perceived discomfort, it was found that "waiting" was highest in terms of the degree. According to general chara-cteristics of the sampled patients, the factor of "explanation" contributed to perceived discomfort more in male patients than female ones. Regarding parts of medical examination, the patients of otorhinolaryngology felt perceived discomfort most because of the factor of "pain". Conclusion : To relieve perceived discomfort the patients under local anesthesia require providing individually systematized nursing services which can reduce perceived discomfort according to each of the factors. In doing so, an unlimited attention and much effort should be given to realize CQI reaching the dimension of increase in comfort as well as decrease perceived discomfort.

  • Various methods have been used in the past to indirectly analyze the craniofacial region. Among these, the lateral and posterior-anterior cephalometircs are used for the evaluation of the dentofacial deformities. However, cephalometircs create inaccuracies because of the inherent enlargement and distortion of the image. The interpretation of cephalometric films is also problematic: the number of anatomic landmarks that can be identified accurately is limited, and the overlap of structures on a radiograph making locating these landmarks difficult. To overcome these problems, computed tomography(CT) has been recommended as an useful modality in the diagnosis, surgical planning, and follow-up of craniofacial anomalies. There is no significant enlargement or distortion of the image, overlap of structure, or tracing error. And the number of anatomic landmarks is vast. The purpose of this study was to examine the orbit and midfacial region using Occlusal Maxillary CT, consisted of slices parallel to the occlusal plane. Based on these CT scan, we provide the data that could be applied to monitor an individual patient's skeletal pattern and the guide to the maxillary osteotomy.

Learning Needs of Registered Nurse for Insertive Education (실무교육에 대한 간호원의 학습요구)

  • The advanced knowledge is increasing day by day due to the progress of medicine and tectonics, the increase of nursing research nowadays. In parallel with these, nursing profession has a need of the nursing care with improvement of new Knowledge, tectonics and nursing procedure through the insertive education. Therefore, this study was attempted to investigate that to grasp learning need of nurses about inservice educational play a pivotal role in the progress of inservice education. This study was made from 242 clinical nurse at 4 hospitals in Seoul and through question- are consisting of 1) ideological part 2) basic sciences 3) nursing part 4) administrative part 5) the others from 10 July '75 to 7 Oct. '75. The data were treated by percentage, Licker method, arid chi- square examination. Significant difference p is 0.05. The results of this studies are as follows : A. learning needs of nurses 1) Generally the learning needs of nurses is very high. 2) Of the learning need of inservice education for nurses, the highest learning need is as follow (Table 2 ) 1 st : Charting 2 nd : The ability of grasping patient's needs and problems. 3 rd ; The contents and the methods of the advanced nursing skill. 4 th ; The importance of team work 5 th ; The general knowledge of the various disease 6 th : The decision of a propriety nursing diagnosis under the general condition. 7 th ; The introduction of the new nursing theory. 8 th : The Nurses role and the responsibility in emergency and disaster. 9 th : The improvement of nursing skill for the perfect interpersonal relationship. l0th ; Cultural education: B . Estimation of Hazard Function and its Associated Factors in Gastric Cancer Patients using Wavelet and Kernel Smoothing Methods

  • Background and Objectives: Increase of mortality rates of gastric cancer in Iran and the world in recent years reveal necessity of studies on this disease. Here, hazard function for gastric cancer patients was estimated using Wavelet and Kernel methods and some related factors were assessed. Materials and Methods: Ninety-five gastric cancer patients in Fayazbakhsh Hospital between 1996 and 2003 were studied. The effects of age of patients, gender, stage of disease and treatment method on patient's lifetime were assessed. For data analyses, survival analyses using Wavelet method and Log-rank test in R software were used. Results: Nearly 25.3% of patients were female. Fourteen percent had surgery treatment and the rest had treatment without surgery. Three fourths died and the rest were censored. Almost 9.5% of patients were in early stages of the disease, 53.7% in locally advance stage and 36.8% in metastatic stage. Hazard function estimation with the wavelet method showed significant difference for stages of disease (P<0.001) and did not reveal any significant difference for age, gender and treatment method. Conclusion: Only stage of disease had effects on hazard and most patients were diagnosed in late stages of disease, which is possibly one of the most reasons for high hazard rate and low survival. Therefore, it seems to be necessary a public education about symptoms of disease by media and regular tests and screening for early diagnosis.

  • This review is a clinical and research update of recent literature related to childhood onset schizophrenia (with an onset of psychosis by age 12). Childhood onset schizophrenia(COS) is a rare disorder, but that may represent a more homogeneous patient population in which to search for risk or etiologic factors of schizophrenia. These overview data show that COS shares the same clinical and neurobiological features as later onset forms of the disorder. Compared with later onset schizophrenia, however, this subgroup of patients appear to have more severe premorbid neurodevelopmental abnormalities, more cytogenic abnormalies, poor outcome, and potentially greater family histories of schizophrenia and associated spectrum disorders. Future studies of this subgroup may provide important clues as to the genetic basis for schizophrenia and how gene products influence certain feature of the disease, such as age of onset and mode of inheritance.

Claimed Adverse Events of Korean Medicine in South Korea: Analysis of Cases in the Korea Medical Dispute Mediation and Arbitration Agency Databases (한의 의료와 연관된 조정 요청 이상반응: 한국의료분쟁조정중재원 접수사례 분석)

  • Objectives : The purpose of this study was to describe the type of claimed adverse events related to Korean Medicine practices in South Korea. Methods : Claims with regard to the Korean Medicine practice submitted to the Korea Medical Dispute Mediation and Arbitration Agency from April 2012 to December 2016 were collected. We analyzed claims that explicitly reported the type of Korean medicine intervention and were deemed as being adverse events as defined by the Korea Good Clinical Practice. Claims that did not mention the Korean medicine practice explicitly or those related to the patient's dissatisfaction to the service rather than adverse health outcomes were excluded. Types, related interventions and the suspected severity of claimed adverse events were summarized. Results : Of 197 claims obtained, 140 claim cases were eligible and 144 claimed events were deemed as possible adverse events of the Korean medicine practice. Pain(16%), local infection/inflammation(12%) and neurological symptoms(11%) were the most frequently reported types of claimed adverse events. Thirty-nine claimed serious adverse events(SAE) were identified, including pneumothorax(28.2%) and death(17.9%). Conclusions : A wide range of claimed adverse events were identified. Routine monitoring of claims data may provide undetected safety information with regard to the Korean medicine practice. High risk of misclassification of the intervention and claimed adverse events due to insufficient information is the main caveat of this study.

Characteristics of Fall Events and Fall Risk Factors among Inpatients in General Hospitals in Korea (입원 환자의 낙상실태 및 위험요인 조사연구: 국내 500병상 이상 종합병원을 중심으로)

  • Purpose: The purpose of this study was to investigate the present status of falls among inpatients in general hospitals and to identify the fall risk factors that reflect the characteristics of domestic hospitals. Methods: Data were collected between December 15, 2016 and January 15, 2017 from 32 Korean hospitals having 500 or more beds. First, 42 risk factors were extracted based on literature review and expert opinions. Then the importance of each factor was evaluated by 223 nurses from medical and surgical adult ward and intensive care units in 40 hospitals. Results: The incidence rate of falls in 18 hospitals was 3.87 per 1,000 total discharged patients and 0.55 per 1,000 patient-days, and the rate of injury-related falls was 40.5%. Major risk factors for falling were identified as the following: being over 65 years of age, history of falls during admission, physical mobility disorders requiring assistance, physical factors (dizziness or vertigo, unstable gait, general weakness, walking aids, visual problems), cognitive factors (delirium, lack of understanding on limitations), neurological disease, CNS medications Conclusion: The findings provide information that can be in the development of a fall risk assessment tool for inpatients in general hospitals in Korea.
