Cephalometrically assessing the validity of superior, middle and inferior tragus points on ala-tragus line while establishing the occlusal plane in edentulous patient

  • PURPOSE. The purpose of this study was to decide the most appropriate point on tragus to be used as a reference point at time of marking ala tragus line while establishing occlusal plane. MATERIALS AND METHODS. The data was collected in two groups of subjects: 1) Dentulous 2) Edentulous group having sample size of 30 for each group with equal gender distribution (15 males, 15 females each). Downs analysis was used for base value. Lateral cephalographs were taken for all selected subjects. Three points were marked on tragus as Superior (S), Middle (M), and Inferior (I) and were joined with ala (A) of the nose to form ala-tragus lines. The angle formed by each line (SA plane, MA plane, IA plane) with Frankfort Horizontal (FH) plane was measured by using custom made device and modified protractor in all dentulous and edentulous subjects. Also, in dentulous subjects angle between Frankfort Horizontal plane and natural occlusal plane was measured. The measurements obtained were subjected to the following statistical tests; descriptive analysis, Student's unpaired t-test and Pearson's correlation coefficient. RESULTS. The results demonstrated, the mean angle COO (cant of occlusal plane) as $9.76^{\circ}$, inferior point on tragus had given the mean angular value of IFH [Angle between IA plane (plane formed by joining inferior point-I on tragus and ala of nose- A) and FH plane) as $10.40^{\circ}$ and $10.56^{\circ}$ in dentulous and edentulous subjects respectively which was the closest value to the angle COO and was comparable with the values of angle COO value in Downs analysis. Angulations of ala-tragus line marked from inferior point with occlusal plane in dentulous subject had given the smallest value $2.46^{\circ}$ which showed that this ala-tragus line was nearly parallel to occlusal plane. CONCLUSION. The inferior point marked on tragus is the most appropriate point for marking ala-tragus line.

  • Since the late 1980s, there have been radical changes in the managerial environment of Y University Medical Center(YUMC). Externally, the competition among hospitals has intensified due to the establishment of universal health insurance in 1939 and the entrance of large enterprises into the health care industry in the early 1990s. In addition, government regulation of medical institution is becoming stricter. Also, consumer groups have continued to demand the respect for patient rights and improvement of the quality of medical services. Internally, the financial condition of YUMC has worsened, not only because weak control and poor mediation in its large-scale structure have made its operation inefficient, but also because the rates of increase in the prices of goods and labor have grown faster than any increases in revenues. This study on materials management at YUMC presents a way for YUMC to reduce costs and increase its productivity, thereby overcoming its financial difficulties and dealing with external pressures. This study utilized the case studies of the materials purchasing and medical supply management in the United States and the comparative analysis of management to suggest short-term and long-term alternatives for innovation in YUMC. The goals of the short-term alternatives for innovation are to centralize the purchasing and supply departments and to simplify the decision-making processes. Through these attempts, it is estimated that YUMC's costs could be reduced by $600,000 per year. In the long-term, it is necessary to consider introducing a Supply Processing Distribution(SPD) system and setting up a centralized electronic system for supply and inventory management, although it is difficult to estimate the effect of cost-cutting because of the lack of analysis data. Thus, YUMC should thoroughly analyze initial investment costs and economical efficiency generated from long-term alternatives.

Satisfaction to College Life and Stress of Clinical Practice among Nursing Students (간호학생의 임상실습 스트레스와 대학생활만족에 대한 융합연구)

  • This study was to investigate the relationship among clinical practice stress, major satisfaction, campus life satisfaction of nursing students. Data was collected from university students in C. do province and college students in S city. All 81 subjects was analyzed by SPSS 21.0ver. Campus life satisfactions were differences on gender and major satisfaction. Satisfied nursing students with campus life had lower level of clinical practice stress than unsatisfied students, especially in health care giver's attitude, patient's attitude, clinical circumstances and mental factors. This study suggested that the convergent extracurriculum program should be paid to interpersonal relationship competence before clinical practice.

A Case of Alagille's Syndrome (Alagille 증후군의 1예)

  • This is a case report of a 5-month-old male who was brought in to hospital for evaluation of jaundice from birth. The baby had a history of ileal atresia operated 2 days after birth. At the age of one month, Tc-99m DISIDA hepatobiliary scintigraphy was performed at other hospital and reported to show good hepatic uptake of the tracer but no uptake in the biliary tree, gall bladder, or intestine for 24 hours post injection. He was judged to have biliary atresia. However, subsequent exploratory laparotomy revealed that the hepatobiliary tree appeared intact and that there was a gall bladder. Additionally, the patient had central aorto-pulmonary shunt for the right ventricular septal defect with pulmonary stenosis of a peripheral type at the age of 4 months. The second hepatobiliary scintigraphy was performed on admission at the age of 5 months, showing a gall bladder but no intestinal uptake up to 24 hours. Retrospectively, the histological specimen of the liver obtained at the exploratory laparotomy was re-evaluated, and by the histological findings coupled with clinical data, arteriohepatic dysplasia (Alagille's syndrome) was diagnosed. In this report, we emphasize the diagnostic limitation of hepatobiliary scintigraphy and the importance of overall clinical and histologic evaluation in a case of Alagille's syndrome.

Identification of Fusobacterium nucleatum isolated from Korean by F. nucleatum subspecies-specific DNA probes (Dot blot hybridization법을 이용한 Fusobacterium nucleatum 아종-특이 DNA 프로브의 특이성 평가)

  • The purpose of this investigation was to evaluate of the specificity of Fusobacterium nucleatum subspecies-specific DNA probes using dot blot hybridization. To confirm whether the clinical isolates were F. nucleatum or not, 16S rDNA of them were cloned and sequenced. The sequencing data were used in homology search with database of GenBank. When the homology was above 98% compared with the nucleotide sequence of a certain bacteria, it was judged as the same species with the bacteria. 23 strains of F. nucleatum were isolates from subgingival plaque of periodontitis patient. The clinical isolates of F. nucleatum were classified into 10 groups using phylogenetic analysis of 16S rDNA sequence. F. nucleatum subspecies nucleatum-specific DNA probe Fu4(1.3 kb) reacted with genomic DNAs from 8 type strains of F. nucleatum and it reacted strongly with those from 8 clinical isolates. The Fp4(0.8 kb) reacted with F. nucleatum subsp. polymorphum ATCC 10953 and one clinical isolates. Fv35(1.9 kb) and Fs17(8.2 kb) probes reacted with genomic DNAs from F. nucleatum subsp. vincentii ATCC 49256 and F. nucleatum subsp. fusiform ATCC 51190, respectively. Our results showed that it is not enough to evaluate the specificity of F. nucleatum subspecies-specific DNA probes with only dot blot hybridization. Therefore, Southern blot analysis will be necessary to confirm the specificity of F. nucleatum subspecies-specific DNA probes.

A Study on the Clothing Inconvenience Levels for the Clothing Improvement of Female Arthritis Patients (여성관절염질환자의 의복개선을 위한 의복불편정도에 관한 연구)

  • The purpose of this study is in the investigation to relieve into the clothing inconvenience levels and to need by arthritis patients who experience clothing difficulties in daily living even without showing conspicuous physical disabilities. For this study, personal interviews were carried out with 151 female arthritis patients, using a questionnaire form consisting of a total of 206 items. Data analysis were done with spss 12.0 for the frequency analysis, cross tab analysis, t-test, and ANOVA. The findings are as follows. 1. The survey subjects were diverse in their age from 20s to 60s and 18.5% of them have need of other's assistance at the time of getting dressed or undressed of clothing. The knee part was felt to be the most uncomfortable, causing the inconveniences in wearing pants or panties. 2. The investigation into the inconvenience levels at the time of dressing or undressing of clothing was indicated that the wearing of pants was the hardest movement to arthritis patients, as the items relating to the movements for putting feet into or pulling them out of pants and those for raising pants over the hip were also accompanied by the feeling of pains. In addition, the aged arthritis patients felt the clothes-wearing movements more difficult. 3. The survey on the kind and inconvenience levels for the clothes currently being worn showed the highest wearing ratio for the pants, which were pointed out to be the most inconvenient. 4. As for the clothing improvement, the needs were located as a whole in the sufficiency in measures of pants crotch and circumference items, the flexibility in materials, the short length of pants, or the front clearance.

Effects of δ-Catenin on APP by Its Interaction with Presenilin-1

  • Alzheimer's disease (AD) is the most frequent age-related human neurological disorder. The characteristics of AD include senile plaques, neurofibrillary tangles, and loss of synapses and neurons in the brain. ${\beta}-Amyloid$ ($A{\beta}$) peptide is the predominant proteinaceous component of senile plaques. The amyloid hypothesis states that $A{\beta}$ initiates the cascade of events that result in AD. Amyloid precursor protein (APP) processing plays an important role in $A{\beta}$ production, which initiates synaptic and neuronal damage. ${\delta}-Catenin$ is known to be bound to presenilin-1 (PS-1), which is the main component of the ${\gamma}-secretase$ complex that regulates APP cleavage. Because PS-1 interacts with both APP and ${\delta}-catenin$, it is worth studying their interactive mechanism and/or effects on each other. Our immunoprecipitation data showed that there was no physical association between ${\delta}-catenin$ and APP. However, we observed that ${\delta}-catenin$ could reduce the binding between PS-1 and APP, thus decreasing the PS-1 mediated APP processing activity. Furthermore, ${\delta}-catenin$ reduced PS-1-mediated stabilization of APP. The results suggest that ${\delta}-catenin$ can influence the APP processing and its level by interacting with PS-1, which may eventually play a protective role in the degeneration of an Alzheimer's disease patient.

Analysis of Prevalence of Anemia according to Severity of Atopic Dermatitis (아토피 피부염 심각도에 따른 빈혈 유병률 비교 분석)

  • Background: Inflammatory diseases can increase the prevalence of anemia. Recent studies confirmed that the prevalence of anemia is increased by atopic dermatitis (AD), a chronic inflammatory disease. Therefore, we aimed to elucidate the correlation between AD severity and prevalence of anemia. Methods: We used data of pediatric patients from the Health Insurance Review and Assessment Service (HIRA-PPS-2016). We included pediatric patients (<18 years) with AD diagnosis who were prescribed medications for AD. We applied a propensity score method with inverse probability of treatment weighting (IPTW) adjusting for differences in prevalence of confounders and performed IPTW logistic regression to evaluate associations between the anemia and severity of AD. Results: In total, 91,501 patients (mild AD: 47,054 patients; moderate-to-severe AD: 44,447 patients) <18 years who were prescribed drugs for AD were analyzed. Analysis of the probability of patients with mild AD and prevalence of anemia as a reference revealed an odds ratio (OR) of 1.159 (95% CI, 1.109-1.212; p<0.001) in moderate-to-severe AD patients, indicating a correlation between anemia prevalence and AD severity. Subgroup analysis according to gender, age group, and type of health insurance revealed there was an association between AD severity and anemia except in patients equal or older than 7 years. Conclusion: The prevalence of anemia increased with AD severity despite adjusting for confounding factors. Our results support the hypothesis that AD can cause anemia, and anemia prevalence could be increased in severe AD patients. Further studies are needed to establish a pathological basis.

A Study on the Emotional Characteristics of Adult with Fatigue Using the Core Seven Emotions Inventory-Short Form (피로를 호소하는 성인의 핵심칠정척도 단축형을 활용한 정서적 특성 연구)

  • Objectives: This study was conducted to investigate the emotional characteristics of adults who complained of fatigue using the Core Seven Emotion Inventory-Short Form (CSEI-s) followed by correlation analysis with the Chalder fatigue scale (CFQ) and Psychosocial Well-being Index-Short form (PWI-SF). Methods: In this study, the medical records of 45 participants who complained of fatigue and completed the CSEI-s, CFQ, and PWI-SF were evaluated. Records of a total of 45 adults were analyzed using SPSS (Statistical Package for the Social Sciences, Version 25.0). Frequency Analysis, Descriptive Analysis, Independent t-test, One Way ANOVA, and Correlation analysis were used for data analysis. Results: 1. According to the educational status of the participants, Thought (思) was identified to be significantly higher in below college graduate than in graduate school and above. 2. As the period of complaining of fatigue increased, a significant difference was observed in Depression (憂) and Fright (驚). 3. According to the degree of fatigue, there was a significant difference in Depression (憂) and Fright (驚), and especially Depression (憂) was higher in the middle fatigue group than in the low fatigue group. 4. PWI-SF, Anger (怒), Depression (憂), and Sorrow (悲) showed a positive correlation with CFQ scores. Conclusions: The results suggest that CSEI-s can be used to measure the psychological symptoms of a patient with fatigue.

The Association between Medication Use for Dyslipidemia and Osteoporosis (국내 성인환자에서 이상지질혈증 약물치료와 골다공증 상관성)

  • Background: Osteoporosis is a disease that affects the quality of life and imposes a high socioeconomic burden. Studies have reported that statins, a HMG CoA reductase inhibitor, have a positive or negative effect on osteoporosis. The purpose of this study was to analyze the correlation between statins and osteoporosis risk. Methods: We used the total patient sample data of the Health Insurance Review and Assessment Service (HIRA-NPS-2018). We analyzed the prevalence of osteoporosis in adult patients of Korea who were diagnosed with dyslipidemia and were prescribed statins at the same time. The odds ratio (OR) according to the intensity and type of statin was used to confirming the prevalence. Results: Among the 1,138,899 patients included in the study, 143,895 patients used statins and 27,524 patients (19.13%) were diagnosed with osteoporosis in the statin group. The OR value of statin group was 0.96 (95% CI 0.94-0.98), confirming that the prevalence of osteoporosis decreased, and a significant decrease was seen in all statin intensity. Some of the moderate-intensity statins rather increased the prevalence of osteoporosis, but atorvastatin and rosuvastatin obtained positive results at both medium- and high-intensity doses, and lovastatin, a low-intensity statin, showed the greatest reduction in the prevalence of osteoporosis. Conclusion: We found that the prevalence of osteoporosis was reduced in the statin group, and there was a constant correlation regardless of gender or age. However, a large, prospective, double-blind and randomized study is needed for a long period of time to demonstrate the effectiveness of statins.
