EXTEMPORANEOUS MICELLAR SOLUBILIZATION OF TITRATED EXTRACT OF CENTELLA ASIATICA IN AQUEOUS MEDIA

  • Titrated Extract of Centella asiatica (TECA) is a poorly water-soluble extract from the Centella asiatica. Despite excellent wound preparation causes pain due to a low aqueous solubility and high hypertonicity and therefore, the patient's compliance of this drug is low. The objective of this study is to design a formulation of TECA with an improved therapeutic applicability via an adequate solubilization. A mixture of propylene glycol and ethoxylated hydrogenated caster oil achieved an acceptable solubilization of TECA (i.e. 10%) via a formulation of micelle. A preparation of extemporaneous TECA micelle was achieved by a dilution of the original micellar formulation with either water or saline. An estimated distribution of particle size was between 15.9 and 32.6 ㎜. The osmotic pressure of the formulation was found to be much lower than that found In a commercially available injectable (i.e. Madecassole). The erthrocytic hemolysis of micellar solution was lower than that with the conventional dosage form, consistent with the osmotic pressure data. Pain score after an injection of the micellar solution was assessed by the hind-paw writhing test using ICR mice and compared with that found with the conventional injectable. The data indicated that the injection of the micellar solution was a significantly less painful. These results indicated that a micellar solubilization, followed by an extemporaneous dilution, is a novel formulation of TECA with an improved therapeutic applicability.

Estimation of Chest Compression Depth during Cardiopulmonary Resuscitation by using Single Frequency Analysis (단일주파수분석을 이용한 심폐소생술 흉부압박깊이 추정)

  • During the emergency situation such as cardiac arrest, cardiopulmonary resuscitation(CPR) is the most important treatment to maintain patient's blood circulation. Since the quality of CPR can not be easily measured or evaluated by the eye, an assistive device with an accelerometer can help to assess the pressure depth of CPR. In this study, we propose a single frequency analysis method to reduce the error of the accelerometer by extracting only one frequency component from the Fourier transform process. To verify the effectiveness of the single frequency analysis, acceleration data at CPR conditions were measured at a sampling rate of 50 / sec using a wristband equipped with an acceleration sensor. Then, We compared the existing distance estimation method and the single frequency analysis method using the measured data. The amplitude value proportional to the compression depth was obtained by applying the single frequency analysis method.

Factors on the Gap between Predicted Cesarean Section Rate and Real Cesarean Section Rate in Tertiary Hospitals (상급종합병원의 예측 제왕절개분만율과 실제 제왕절개분만율의 격차 관련요인)

  • Purpose: This study was aimed to examine the gap between predicted cesarean section rate and real cesarean section rate and it's determining factors of 44 tertiary hospitals. Method: This study is a cross-sectional analysis using the data of 25,623 deliveries in 2009 drawn from homepage of Health Insurance Review and Assessment Service. Data were analyzed with t-test, F-test, Scheff$\acute{e}$ test, and logistic regression. Result: There were statistically significant differences in the gap of cesarean section rate (more gap indicates higher quality of delivery) by grade of nurse staffing and delivery cases. Hospitals with nurse staffing grade 1 to 2 had more possibility to be classified into higher grade in quality of delivery (OR 5.67, 95% CI 1.07-30.08). Also hospitals with over 500 delivery cases had more possibility be classified into higher grade in quality of delivery (OR 4.92, 95% CI 1.14-21.23, respectively). Conclusion: The finding suggests that grade of nurse staffing may influence the real cesarean section rate because nurses do a vital role to prevent unnecessary cesarean section. Further study is required to provide evidence that nurse staffing influence on patient outcome and cost-effectiveness in order to obtain adequate number of nursing staffs.

  • This study aims to develop a mobile-based portable u-Health Monitoring System which provides a personal medical service on demand by processing patients' data intellectually achieved through sensing technique of non-restriction/non-consciousness oriented and deciding. To do this, we composed a USN-based portable monitoring unit. It is the one, that contains a somatometry sensor which is attached to patient's body and detects bio information, a portable wireless terminal which receives information from the sensor and transmits it to monitor server, and a monitor server which interprets received data through wireless network and processes. Also, it tries to develop a non-restriction /non-consciousness oriented sensing technique which is related to glycosuria and cardiovascular diseases.

  • 1. Objectives This is the comparative study with hand-operated measurement method and Automatic measurement method, in order to convert the automatic measurement method. 2. Methods We measured the body of patients(hand-operated Width 5 Places and hand-operated circumference 8 place,Automatic Width 5 and automatic circumference 8 place by 3D body measuring instrument) and analyzed the anthropometric data divding into sex&age. 362 patient's data are used in the analysis. 3. Results and Conclusions 1) 1th circumference variable which standing was not a sasang constitutional difference. 2) Diagnostic accuracy rate of the body measurement was 50-80%. 3) Diagnostic accuracy rate of man is higher than Diagnostic accuracy rate of women 4) Diagnostic accuracy rate of Automatic & hand-operated measuring was not a big difference.

ICU Patients' Experience Process of Physical Restraint (중환자실 환자의 억제대 경험과정)

  • Purpose: The purposes of this study were to explore and describe the use of restraint on patients and to generate a grounded theory of how the use of restraint affects patients who have been restrained. Methods: Interview data from seven patients with physical restraint was analyzed using the Strauss and Corbin's grounded theory method. Data were collected and analyzed simultaneously. Unstructured and in-depth interviews were conducted retrospectively with patients recalling their memories of ICU following their transfer to general unit. Results: 'Safety belt' was emerged as a core category and it reflected that physical restraint provided a sense of security to patients. On the basis of core category, a model of the experience process of restrained patients in ICU was developed. The experience process were categorized into four stages: resistance, fear, resignation, and agreement. Stages of these proceeds appeared to have been influenced by the nurses' attitude and caring behavior such as the frequency of nurse-patient interaction, repetition of explanation, and empathetic understanding. Conclusion: These findings indicate that patients have mixed feelings towards restraint use, although negative feelings were stronger than positive ones. The result of this study will help nurses make effective nursing intervention.

Does the Impairment of Functional Life Increase the Probability of Suicide in Cancer Patients?

  • Background: Cancer affects patients in many ways including physical, social, emotional, psychological and economic and restricts the functional lives. Psychiatric problems seen among cancer patients may increase the suicide probability and patients perceive suicide as a peaceful death type. The aim of this study was to examine the correlation between functional life and suicide probability among cancer patients. Materials and Methods: This study was conducted with 105 cancer patients as descriptive. The Functional Living Index_Cancer (FLIC)," suicide probability scale" (SPS) and personal information form were used as data collecting tools. Data were evaluated by descriptive analysis, and Pearson's correlation. Results: It was determined that 34.3% of patients thought of suicide. Significant negative correlation was found between functional life and suicide probability (r=-.641, p=0.000), increase being evident in those with poor functional life. Conclusions: It is recommended that cancer patients should be supported for improving their functional lives with help in coping processes for illness and treatment symptoms. Evaluation of the patient mental status to prevent the suicide among this group is an important role for nurses.

Licensing strategies and tasks for medical devices utilizing 3D printing technology in dentistry (치의학분야 3D 프린팅 기술이 적용된 의료기기의 인·허가전략과 과제)

  • 3D printing technology supporting the specific patient medical services is actively being implemented in dentistry. The purpose of this study is to introduce the legal and institutional considerations to the medical practitioners in dentistry who must observe when they manufacture medical devices using 3D printers, and to provide a ways to activate and enhance their utilization in the domestic approval point of view for medical devices. Through the public data of government agencies and related organizations, the statutory system and compliance matters related to the manufacture of 3D printing medical devices have been examined and reviewed for the government's improvement efforts. Through the study, the government has been actively improving the system and making policy, but the active interest and participation of medical professionals and related workers are continually required to solve the problems which are scattered. 3D printing technology is expected to be more frequently utilized in the field of dentistry in near future. Therefore, it is essential to establish measures to improve the regulation through continuous cooperation with the related ministries with the long-term point of view enhancing smooth entry to the market for the medical devices by taking data from the continued research.

Comparison of ADL and Symptoms in Elders with Dementia according to Family Resiliency (가족 복원력이 높은 집단과 낮은 집단간 치매노인의 일상활동능력 및 치매증상 비교)

  • Purpose: The purpose of this study was to identify family resiliency, ADL in elders with dementia, and symptom for families with an elder with dementia. Method: The participants were 102 people who were caregivers to elders with dementia and who visited the out-patient department at S. hospital for follow up care. Data were collected from August to October 2003 using a questionnaire. The collected data were analyzed using descriptive statistics and t-test aided by the SPSS/PC. Result: Family resiliency for the whole sample was within normal limits but when the sample was quarter-divided by good and poor family resiliency, based on established cut-off scores, there were differences for some characteristics according to good/poor family resiliency. These differences were for perceived health state of caregivers (t=-2.78, p=.008), economic state of family (t=-3.34, p=.002), and ADL (t=-2.56, p=.014). Conclusion: Future research should focus on the way in which family resiliency can be enhanced and on the effects of interventions for those caregivers who report poor family resiliency.

Analysis of Awareness and Requirement of Home Care Nursing in Cancer Patients by Experienced Symptoms and Nursing Items (암 환자의 증상경험과 가정간호항목별 가정간호 인식도 및 가정간호 요구도)

  • Purpose: This study sought to provide basic information for the establishment of home care nursing system in cancer patients. Methods: Data were collected by the descriptive questionnaires consisting of 42 articles from five fields of nursing services. Patient's symptoms were investigated by the Korean version of EORTC QLQ-C30. Data were analysed using SPSS-PC 12.0. Results: Among the cancer patients (n=182), 40.1% had serious limitations on their usual lives. 74.7% had serious economic burdens. 79.7% agreed strongly with the necessity of home care nursing system, 74.2% were willing to use home care nursing, and 91.2% felt that home care nursing should be mandatory in cancer centers. There was no correlation between the frequency of symptoms or nursing items and the degree of home care nursing requirements. Digestive symptoms, symptoms requiring procedures, and symptoms to meet educational help displayed a high degree of requirement. Conclusion: Home care nursing should be activated for cancer patients as a bridge between hospital-based acute care and community-based chronic care which could increase the quality of care and reduce insurance related payments.
