  • Recently the Elderly with dementia is increasing in Korea with rapid aging of the population. In addition to increasing a number of nuclear family and women's advancement in society, people are less capable of supporting family with dementia. In response, the Seoul city increased welfare facilities for the elderly with dementia about two times for one year. I think that the quantitative portion of facility get to meet the requirements but it is lack of qualitative one in fact. Therefore, the space for the elderly can keep up with the falloff of mental and physical functions, dementia and geriatric diseases according to aging and careful consideration about the space considering stability and convenience is required. At this point, this study has the purpose to plan a daycare center which reflects the characteristics of the elderly to protect them within the community safely and comfortably. As for the research method, case study was conducted to understand the overall situation on the space plan of the daycare center for 3 daycare centers located in Seoul and found out problems. From the generalization of the above study, a space plan for a daycare center considering the characteristics of the elderly was provided. The composition of Dementia welfare facility should be carefully planned to fit characteristics of the patient's behaviors.

A Study on the Path Search for the Rapid Suppression of Naval Ships Casualties (함정 재해의 신속 진압을 위한 경로 탐색에 관한 연구)

  • Naval ships could be seriously damaged by enemy attacks in battle. Moreover, this damage could be spread and deteriorated into a secondary accident. Secondary accidents that have adverse effects on naval ship's survivability, such as fire, flood, smoke extension, and patient occurrence, are defined as casualties. These casualties sharply degrade the survivability of naval ships. Furthermore, naval ships could be burned-out and sunk by casualties in isolated sea. Therefore, damage control and rapid suppression of the casualties in the naval ships is essential. This study was conducted in the establishment of suppression paths according to the characteristics of each casualty so that the developed system can support the rapid suppression in an emergency and even the training situation on a regular state. To establish the suppression paths, the two-dimensional numerical map is designed by converting the three-dimensional features of the naval ships, and the well known algorithms are compared to present the appropriate one for path finding problem on the naval ships. Finally, we devised a specific routing algorithm that fits the characteristics of each casualty in accordance with the Korean Navy's doctrines and handbooks of casualty suppression.

  • This paper proposed about a load-balanced multi-agent model in mobile distribution environment to monitor moving patients and to deal with a situation of emergency. This model was designed to have a structure based on distribution framework by expanding a mobile system, and provides healthcare services based on real time situational information on moving patients. In order to overcome the limitation of middleware when we design system, we provided an abstract layer between applications and their base network infrastructure so that balance between QoS requests and network life can be maintained. In addition, clustering was used in cells for the efficient load distribution among multi-agents. By using Clustering FCM, we got optimal resources and had solve about transmission delay.

  • In Korea, current medical technology is the highest level in the world. As a result, many doctors have specialized knowledge of various disorders or diseases, and are proceeding in a better way. With such high medical technology, it is possible to increase the probability of success of surgery to provide high reliability to patients. Rehabilitation is also a form of medical treatment that reduces the side effects that occur after surgery that is done for quick cure. However, the situation in this section is slightly different. There are moves to develop rehabilitation devices and operations, but most of them are now dependent on foreign technology. Rehabilitation therapy, which we commonly know, is dominated by behavior. However, it is also a kind of rehabilitation to find out how much the patient's symptoms are improved or recovered. In this paper, we have studied the development of a program by using the Deeplearning method in order to detect the problem of the food swallowing operation by the severity.

Analysis of Korean Medical status of Acute Bronchitis, Chronic Bronchitis and Allergic Rhinitis patients (급·만성 기관지염과 알레르기성 비염의 국내 의료 이용 현황 분석)

  • Objectives: Patients with respiratory diseases are increasing as air pollution due to fine dust gets worse. Diseases that occupy a large proportion of respiratory diseases in medical institutions are acute bronchitis, chronic bronchitis and Allergic rhinitis. The number of patients with all three diseases is gradually increasing. This study was to suggest assignment of medicine policy for improving accessibility to Korean medical treatment of respiratory diseases analyzing the current treatment status of acute bronchitis, Chronic bronchitis and Allergic rhinitis patients in Western medical and Korean medical institutes in this situation. Methods: This study used 2017 National patient sample data from the Korean Health Insurance Review and Assessment Service for research. Acute bronchitis, Chronic bronchitis, Allergic rhinitis was according to KCD code. This research contains Socio-demographic analysis classified by sex and age, the number of three disease's patients. Also the number of medical treatment, the expense of recuperation cost, medical practices were analyzed compare with Western and Korean medicine. Results: The incidence of three diseases is higher among female than male. There are many patients under the age of 10 in the case of acute bronchitis and allergic rhinitis, while there are many patients over 50 years of age in chronic bronchitis. Western medical treatment take up a larger proportion than Korean medical treatment in part of the number of three disease's patients, the number of medical treatment, the expense of recuperation cost. Conclusions: Much more patients of these diseases visit in Western medical clinic and hospital than Korean medical clinic and hospital for treatment. There are many parts of Korean medical treatment that are not covered by Health insurance benefits like herbal decoction, pharmacopuncture, etc. Korean medicine need to do the efforts for expanding medical field in variety. Also it is demanded for institutional support for reduction of the cost burden and improving on accessibility of Korean medical treatment in order to treat with the increase in respiratory diseases due to fine dust.

Status of Oral Care according to the Type of Surgery for Inpatients at Dental Hospitals

  • Background: The aim of this study was to understand the importance of professional oral care for inpatients by examining the type and frequency of surgery and hospital discharge period at dental hospitals, and identify the types of professional oral care actually in progress. Methods: In this study, the type of surgery and length of hospitalization were investigated among patients admitted to the dental hospital for oral and maxillofacial surgery, and the professional oral care status of inpatients who had difficulty self-managing their oral care was retrospectively identified by collecting data on oral care before and after surgery, including the type and frequency. Results: The majority of inpatients at dental hospitals were male (57.6%), elderly patients over 60 years accounted for 20% of patients, and the average length of hospitalization was 4 days. In the 20s (aged 20~29 y), the number of orthognathic surgery patients (73.1%) was high, and the incidence of cysts was high in middle-aged patients. Regarding the oral care of hospitalized patients, scaling was performed once by a dentist before surgery. After surgery, surgical dressings using H2O2 balls were applied and oral care education was introduced before discharge. Conclusion: Based on the results of this study, professional oral care is essential to prevent infection and complications caused by oral bacteria among inpatients at dental hospitals. It is necessary to use various oral hygiene aids for inpatients and to conduct effective oral care instruction according to each patient's situation. In addition, it is necessary to raise awareness and the role of dental hygienists in professional oral care.

Barriers to the practice of fall prevention by nurses and nurse assistants at geriatric hospitals (요양병원 간호사 및 간호조무사의 낙상예방활동 실천의 방해요인)

  • Older adults are at a high risk of falling, causing severe injuries and increased hospital stays and treatment costs. This can be a burden not only on the family but also on the national economy. Thus, fall prevention is very important in nurses' and nursing assistants' work. This study intended to grasp the real situation faced by nurses and nursing-assistants working at geriatric hospitals in J province, Korea. The researcher interviewed thirty-six nurses and nursing-assistants in four geriatric hospitals and performed four focus group interviews. Data were analyzed through an inductive content analysis based on Elo & Kyngäs's approach. Three categories and five subcategories were identified. Categories were patients, environmental, and personal factors. Patient factors included patients' impaired cognitive function. Environmental factors included lack of personnel support and assignment of inpatients without regard to fall risk. Personal factors included hopelessness regarding improvement and fear of being blamed. Promotion of fall prevention practice needs a gradual increase in nursing staff at geriatric hospitals, a system for assigning hospital room according to fall risk, and education and support system to reduce nursing burden.

  • The recent medical treatment guidelines and the development of information technology make hospitals reduce the expense in surrounding environment and it requires improving the quality of medical treatment of the hospital. That is, with the new guidelines and technology, hospital business escapes simple fee calculation and insurance claim center. Moreover, MIS(Medical Information System), PACS(Picture Archiving and Communications System), OCS(Order Communicating System), EMR(Electronic Medical Record), DSS(Decision Support System) are also developing. Medical Information System is evolved toward integration of medical IT and situation si changing with increasing high speed in the ICT convergence. These changes and development of ubiquitous environment require fundamental change of medical information system. Mobile medical information system refers to construct wireless system of hospital which has constructed in existing environment. Through RFID development in existing system, anyone can log on easily to Internet whenever and wherever. RFID is one of the technologies for Automatic Identification and Data Capture(AIDC). It is the core technology to implement Automatic processing system. This paper provides a comprehensive basic review of RFID model in Korea and suggests the evolution direction for further advanced RFID application services. In addition, designed and implemented DB server's agent program and Client program of Mobile application that recognized RFID tag and patient data in the ubiquitous environments. This system implemented medical information system that performed patient data based EMR, HIS, PACS DB environments, and so reduced delay time of requisition, medical treatment, lab.

Identify the status of pretreatment on antithrombotic agents in practice at the dentist's office (치과에서 시행되고 있는 시술 전 항혈전제 관련 처치에 대한 실태 파악)

  • The purpose of this study is to understand using of antithrombotic agents in clinical settings. The number of patients receiving antithrombotic therapy is increasing in the long term due to the increased prevalence of cardiovascular disease in the elderly. There is a guideline for discontinuation of antithrombotic treatment before dental treatment, but the present study shows that there is a difference from the actual clinical situation. We used an online survey methodology to involve 1000 participants. The questionnaires were asked about general items and Clinical experience such as history of antithrombotic history, experience of complications due to antithrombotics, discontinuation of use, reasons for discontinuing use. The result is as follows. The incidence of delayed bleeding was found to be 55.1%, and discontinuation of antithrombotic therapy before withdrawal was 87.8%. Among respondents 98.3% responded that they stopped antithrombotics because of delayed bleeding and 57.4% responded that they would stop for three to five days. Korean dentists maybe cope with delayed bleeding considering the emotional problem of the patient, the relationship with the patient's physician, and the additional socioeconomic problems.

Necessity of revision of the mandatory medication guidance regulation under the Pharmaceutical Affairs Act (약사법상 복약지도의무 규정의 개정 필요성)

  • The Pharmaceutical Affairs Act stipulates medication counseling as an obligatory requirement in the case of preparation of medicine. In fact, there are many cases where pharmacists only tell patients the dose and time and do not properly guide them on taking medications. However, in light of the current situation where non-face-to-face treatment is being attempted, there is a high possibility of drug-taking accidents due to insufficient medication guidance. In addition, as an aging society progresses, the need for explanations on pharmaceuticals is increasing. If a pharmacist causes damage to a patient by failing to give appropriate medication guidance, the patient can claim compensation for damages. In addition, if a drug accident occurs due to a conflict between the pharmacist's duty to guide medication and the doctor's duty to explain, a joint tort is established between the pharmacist and the doctor. Nevertheless, there are cases in which only doctors are judged to bear the tort liability. However, the Pharmaceutical Affairs Act includes providing information for the selection of over-the-counter drugs in the medication guidance as part of the medication guidance obligation. Therefore, in order to reconsider the importance of the medication-taking guidance duty, it is necessary to define the medicationaking information provision method and the medication-taking guidance duty as separate concepts. In addition, it is necessary to amend related regulations centered on patients so that medication guidance, such as side effects of medicines and interactions with concomitant medications, can be made in detail.
