Evaluation of bone formation and membrane degradation in guided bone regeneration using a 4-hexylresorcinol-incorporated silk fabric membrane

  • Background: The aims of present study were (1) to evaluate new bone formation among the 4-hexylresorcinol (4HR)-incorporated silk fabric membrane (SFM), conventional SFM, and uncovered control groups and (2) to compare the amount of residual membrane between the 4HR-incorporated SFM and conventional SFM in a rabbit parietal defect model. Methods: Nine New Zealand white rabbits were used for this animal study. After the formation of a bilateral parietal bone defect (diameter 8.0 mm), either 4HR-incorporated SFM or conventional SFM was grafted into the defect. The defect in the control was left uncovered. New bone formation and the amount of residual membrane were evaluated by histomorphometry at 8 weeks after the operation. Results: The total amount of new bone was $37.84{\pm}8.30%$ in the control, $56.64{\pm}15.74%$ in the 4HR-incorporated SFM group, and $53.35{\pm}10.52%$ in the conventional SFM group 8 weeks after the operation. The differences were significant between the control and 4HR-incorporated SFM group (P = 0.016) and between the control and conventional SFM group (P = 0.040). The residual membrane was $75.08{\pm}10.52%$ in the 4HR-incorporated SFM group and $92.23{\pm}5.46%$ in the conventional SFM group 8 weeks after the operation. The difference was significant (P = 0.039). Conclusions: The 4HR-incorporated SFM and conventional SFM groups showed more bone regeneration than the control group. The incorporated 4HR accelerated the partial degradation of the silk fabric membrane in a rabbit parietal defect model 8 weeks after the operation.

  • 목 적: 본 저자들은 VGB과 시야결손과의 관련성을 알기 위해 본 연구를 시행하였다. 방 법: 간질로 진단 받고 VGB을 추가 복용으로 치료를 시행한 환아 중 발달 연령이나 인지 연령이 8세 이상인 환아 78명을 대상으로 하였다. 시야검사는 Humphrey 자동 시야측정계로 시행한 후 결과가 의심이 되면 Goldman 시야측정계로 검사를 즉시 다시 시행하였으며, 이들은 다시 6개월 후에 재검을 실시하였다. 결 과 : 본 연구에서는 78명 중 5명에게 의증이 있었으나 재검시 다시 정상으로 나와 시야결손이 있는 환아는 없었다. 결 론: 본 조사에서 VGB은 시야결손을 일으키는 약물로 나타나지는 않았다. VGB의 시야결손으로 사용에 제한을 두기보다 다른 약으로 조절이 되지 않는 부분발작과 영아연축의 치료에 초기 선택 약물로 조심스럽게 사용하면 무방할 것으로 생각되어진다. 이와 함께 시야결손의 선별검사가 추천되며, VGB을 복용하는 소아에게 있어 정기적인 검사가 필요하겠다.

Reconstructive Trends in Post-Ablation Patients with Esophagus and Hypopharynx Defect

  • The main challenge in pharyngoesophageal reconstruction is the restoration of swallow and speech functions. The aim of this paper is to review the reconstructive options and associated complications for patients with head and neck cancer. A literature review was performed for pharynoesophagus reconstruction after ablative surgery of head and neck cancer for studies published between January 1980 to July 2015 and listed in the PubMed database. Search queries were made using a combination of 'esophagus' and 'free flap', 'microsurgical', or 'free tissue transfer'. The search query resulted in 123 studies, of which 33 studies were full text publications that met inclusion criteria. Further review into the reference of these 33 studies resulted in 15 additional studies to be included. The pharyngoesophagus reconstruction should be individualized for each patient and clinical context. Fasciocutaneous free flap and pedicled flap are effective for partial phayngoesophageal defect. Fasciocutaneous free flap and jejunal free flap are effective for circumferential defect. Pedicled flaps remain a safe option in the context of high surgical risk patients, presence of fistula. Among free flaps, anterolateral thigh free flap and jejunal free flap were associated with superior outcomes, when compared with radial forearm free flap. Speech function is reported to be better for the fasciocutaneous free flap than for the jejunal free flap.

  • Currently, different kinds of diagnosis and inspection technologies are applied to prevent the internal mechanical transformation of transformers. For example, examination of internal Partial Discharge of transformer, analysis of transformer oil gas, and measurement Frequency Response Analyzer(FRA) are used to diagnose defect. Especially, diagnosis technique through Frequency Response Analyzer(FRA) has been used and developed from 1960, when it was first introduced, till now to become an important tool to examine presence of defect and to prove quality of machines for the most electric machine producers electric power company in the world. However, diagnosis through FRA is still in introduction level in Korea and the application method for FRA is not established yet. For that reason, study about the application of domestic electric installation according to the FRA is needed. It is expected that the study play an important part in the prevention of defect due to the internal transformation of transformer by introducing measurement theory, providing measurement method, and analyzing application cases.

둔부천공지피판의 둔부 인접 부위로의 적용: 둔부천공지피판의 자유로운 작도 (Reconstruction of Defect Adjacent to the Buttock with Gluteal Perforator Flap: Free Style Flap Design)

  • Purpose: Gluteal perforator flap has evolved to one of the standard tools for coverage of pressure sore. We used this flap to cover the defect adjacent to the buttock. Methods: From September 2004 to August 2006, gluteal perforator flaps were performed in 3 patients with sore and 9 patients with tumor. We made the rule for free style design of the flap. First, the defect should be covered fully regardless of the shape or area. Second, the location of perforators was decided to maximize flap mobility. Third, the donor-site should be closed directly. Results: Successful reconstruction was fulfilled. In 2 cases, initial flap congestion was observed but medical leech was applied and it was resolved. Partial flap loss occurred in one case. Infection was observed in one case. But there were no major complications. Conclusion: Gluteal perforator flap is very good option for the reconstruction of the defects adjacent to the buttock.

하지에서의 역행성 지방근막 피판술: 유리피판술을 대치할 수 있는가? (Reversed Adipofascial Flap in Lower Leg: Can It Replace the Free Flap?)

  • A soft tissue defect of the lower leg or foot presents a challenging problem. Reconstructive surgeon should be armed at all points of wound site, tendon and bone exposure, injury of major vessel and so on in the lower limb. We reconstructed the defects of lower legs and feet of 25 patients between February, 1997 and December, 2003. Applying reversed adipofascial flap with skin graft on a soft tissue defect of the lower leg or foot is challenging. We did a comparative study of 25 reversed adipofascial flaps with 51 free flaps. All 25 cases of reverse adpofascial flap reconstruction were successful except for a partial loss of skin graft in 3 occasions. The reversed adipofascial flap had a merit of a short operation time and hositalization, a high success rate and minimum complications. Besides major vessels in the lower leg are better preserved and donor morbidity is minimal. However, the flap is unmerited in reconstructing a hug hallowed defect and in the leg with poor blood circulation and once previous surgery. The operators may consider the feasible substitution of reversed adipofascial flap for free flap before applying in the lower leg.

  • 순수한 Rutile에서 점결함의 형태와 전기적 특성을 연구하기 위해 $1~10{-23}$atm의 산소분압 범위 및 $700~1300^{\circ}C$의 온도 범위에서 전기전도도를 측정하였다. 전기전도도의 산소분압 의존성($log\sigma/logPo_2$)으로부터 산소분압 및 온도 변화에 따라 다음과 같은 지배적인 결함들을 제안한다. 1) $Ti_nO_{2n-1}$, 2)침입형 Ti 이온 3)2가로 하전된 산소빈자리 4)불순물에 의해 형성된 2가로 하전된 산소빈자리 5) n-p전이 및 p형 전도 또한, 고유범위의 실험결과로부터 계산한 Ti와 Vo의 결함형성 엔탈피는 각각, 10.2eV와 4.92eV였다.

A New Image Enhancement Algorithm Based on Bidirectional Diffusion

  • To solve the edge ringing or block effect caused by the partial differential diffusion in image enhancement domain, a new image enhancement algorithm based on bidirectional diffusion, which smooths the flat region or isolated noise region and sharpens the edge region in different types of defect images on aviation composites, is presented. Taking the image pixel's neighborhood intensity and spatial characteristics as the attribute descriptor, the presented bidirectional diffusion model adaptively chooses different diffusion criteria in different defect image regions, which are elaborated are as follows. The forward diffusion is adopted to denoise along the pixel's gradient direction and edge direction in the pixel's smoothing area while the backward diffusion is used to sharpen along the pixel's gradient direction and the forward diffusion is used to smooth along the pixel's edge direction in the pixel's edge region. The comparison experiments were implemented in the delamination, inclusion, channel, shrinkage, blowhole and crack defect images, and the comparison results indicate that our algorithm not only preserves the image feature better but also improves the image contrast more obviously.

활선중 절연유 분석을 통한 유입변압기 건전성 평가 방법에 관한 연구 (A Study on the Health Evaluation Method of Oil-immersed Transformer through Analysis of Insulating Oil)

  • The health state of the oil transformer is evaluated by the age of use and the state of internal defects. Mineral Oil, used as an insulator for oil transformers, creates specific gases and compounds through chemical reactions caused by heat, moisture, and partial discharge inside the transformer. It is possible to determine the aging and defect of the transformer through these gases and compounds. So, it is an important indicator to evaluate the health of a transformer. In this study, factors for assessing the health of transformers were hierarchically categorized, and key factors for each hierarchy were selected for design weighting. These weights were determined through surveys conducted with experts in the fields of transformer design, operation, and quality. For the health of a transformer, defect-related factors are approximately three times more important than factors related to aging. Additionally, defect-related factors showed a higher weighting for gases generated at high temperatures. Furthermore, Furan was determined to have a high weight, directly associated with insulating paper aging. Based on these findings, a health index was proposed, and a comparative analysis was conducted by categorizing 40 operational transformers into normal and comparison groups to evaluate and validate it.

모의 GIS 내부에 유전체 파티클 존재시 방전진전에 따른 방사전자파 특성 (Characteristics of Radiated Electromagnetic Waves with discharge propagation in Model GIS being Insulation Particle)

  • In this paper the imitated electric defect was simulated by insulation particle in the model GIS. This paper studied the distribution of frequency spectrum of the radiated electromagnetic waves using antenna (30-2,000[MHz]) and spectrum analyzer. From results of this study, a new method was introduced for measurement and analysis of the radiated electromagnetic waves in accordance with discharge progress of each defect in the model GIS. It was confirmed that detecting partial discharge and estimating discharge progress can be possible in the model GIS.
