Background: There is minimal literature on the morphology of partial distal biceps tendon (DBT) tears. We sought to investigate tear morphology by retrospectively reviewing 3-Tesla magnetic resonance imaging (3T MRI) scans of elbows with partial DBT tears and to propose a basic classification system. Methods: 3T MRI scans of elbows with partial DBT tears were retrospectively reviewed by two experienced observers. Basic demographic data were collected. Tear morphology was recorded including type, presence of retraction (>5 mm), and presence of discrete long-head and short-head tendons at the DBT insertion. Results: For analysis, 44 3T MRI scans of 44 elbows with partial DBT tears were included. There were 9 isolated long-head tears (20%), 13 isolated short-head tears (30%), 2 complete long-head tears with a partial short-head tear (5%), 5 complete short-head tears with a partial long-head tear (11%), and 15 peel-off tears (34%). Retraction was seen in 5 or 44 partial tears (11%), and 13 of the 44 DBTs were bifid tendons at the insertion (30%). Conclusions: Partial DBT tears can be classified into five sub-types: long-head isolated tears, short-head isolated tears, complete long-head tears with partial short-head involvement, complete short-head tears with partial long-head involvement, and peel-off tears. Classification of tears may have implications for operative and non-operative management. Level of evidence: III.

Background: Injection therapy around the distal biceps tendon insertion is challenging. This therapy may be indicated in patients with a partial distal biceps tendon tear, bicipitoradial bursitis and tendinopathy. The primary goal of this study was to determine the accuracy of manually performed injections without ultrasound guidance around the biceps tendon. Methods: Seven upper limb specialists, two general orthopedic specialists, and three orthopedic surgical residents manually injected a cadaver elbow with acrylic dye using an anterior and a lateral infiltration approach. After infiltration the cadaveric elbows were dissected to determine the location of the acrylic dye. Results: In total, 79% of the injections were localized near the biceps tendon. Of these injections, 20% were localized on the radius near the bicipitoradial bursa. In total, 53% of the performed infiltrations were injected by anterior and 47% by lateral approaches. Of the injections near the distal biceps (79%), 47% were injected by an anterior and 53% by a lateral approach. Of the injections on the radius (20%), 33% were injected by anterior and 67% by lateral approach. Of the inaccurate injections (21%), 75% were injected anterior and 25% lateral. Conclusions: Manual infiltration without ultrasound guidance for distal biceps pathology lacks accuracy. We therefore recommend ultrasound guidance for more accurate infiltration.

Background: The study of conservative and surgical treatment of distal biceps tendinopathy and associated biceps tendon partial rupture. Methods: Twenty-one cases with distal biceps tendonitis and partial ruptures were studied who visited Pohang Semyeong Christianity Hospital from June 2010 to August 2017. The mean age was 57.1 years (39-69 years), 14 males and 7 females. The mean duration of symptom at the time of first visit was 4.9 months (0.2-14 months). Ultrasonography and magnetic resonance imaging were performed for patients with severe symptoms. According to the severity of the symptoms, splint immobilization, oral nonsteroidal anti-inflammatory drugs, and ultrasound-guided steroid injection were performed. Surgical treatment was performed if the patient did not respond to conservative treatment for 3 to 6 months or longer. Results: There were 9 cases of partial rupture of the distal biceps tendon associated with distal biceps tendinopathy on imaging studies. Conservative treatment showed symptomatic improvement in 16 of 21 cases. In 4 cases with a relatively mild symptom, anti-inflammatory analgesics and intermittent splinting showed good result. In 12 cases, symptoms improved after ultrasonography-guided steroid injection. Surgical treatment was performed on 5 cases that did not respond to conservative treatment. Conclusions: Conservative treatment of distal biceps tendinopathy may promise good results. However, in case of partial tear of the distal biceps tendon and refractory to conservative treatment, surgical treatment may be needed.
Background: Chronic subscapularis tendon tear (SBT) is a degenerative disease and a common pathologic cause of shoulder pain. Several potential risk factors for chronic SBT have been reported. Although metabolic abnormalities are common risk factors for degenerative disease, their potential etiological roles in chronic SBT remains unclear. The purpose of this study was to investigate potential risk factors for chronic SBT, with particular attention to metabolic factors. Methods: This study evaluated single shoulders of 939 rural residents. Each subject undertook a questionnaire, physical examinations, blood tests, and simple radiographs and magnetic resonance imaging (MRI) evaluations of bilateral shoulders. Subscapularis tendon integrity was determined by MRI findings based on the thickness of the involved tendons. The association strengths of demographic, physical, social, and radiologic factors, comorbidities, severity of rotator cuff tear (RCT), and serologic parameters for SBT were evaluated using logistic regression analyses. The significance of those analyses was set at p<0.05. Results: The prevalence of SBT was 32.2% (302/939). The prevalence of partial- and full-thickness tears was 23.5% (221/939) and 8.6% (81/939), respectively. The prevalence of isolated SBT was 20.2% (190/939), SBT combined with supraspinatus or infraspinatus tendon tear was 11.9% (112/939). In multivariable logistic regression analysis, dominant side involvement (p<0.001), manual labor (p=0.002), diabetes (p<0.001), metabolic syndrome (p<0.001), retraction degree of Patte tendon (p<0.001), posterosuperior RCT (p=0.010), and biceps tendon injury (p<0.001) were significantly associated with SBT. Conclusions: Metabolic syndrome is a potential risk factor for SBT, as are these factors: overuse activity, diabetes, posterosuperior RCT, increased retraction of posterosuperior rotator cuff tendon, and biceps tendon injury.
목적: 회전근 개 질환 환자에서 상완 이두건의 병소에 대한 수술 전 초음파 검사의 정확성을 알아보기 위하여 본 연구를 시행하였다. 대상 및 방법: 2006년 1월부터 2007년 3월까지 회전근 개 질환으로 수술적 치료를 받은 환자 중 수술 전 초음파 검사를 시행 한 67명의 환자를 대상으로 후향적 조사를 하였다. 환자들의 연령은 평균 58세였으며, 남자 41예, 여자 26예였다. 상완 이두건의 관절경 및 초음파 소견은 '정상', '탈구', '아탈구', '건증', '부분 또는 완전 파열'등으로 기술하였으며, 관절경과 초음파 소견 간의 일치 여부를 조사하였다. 결과: 초음파 검사 67예 중 37예가 '정상'이었으며, 30예가 '비정상'이었다. '정상' 37예 중 5예는 관절경 검사상 '부분 파열'이 있었으며, '비정상' 30예 중 8예는 관절경 검사상 '정상'이었다. 초음파 검사상 3예의 '탈구' 및 4예의 '완전 파열'은 관절경 검사 결과와 일치하였다. 초음파 검사에서 '건증'이 기술된 20예는 관절경 검사에서 '부분 파열' 15예, '정상' 5예로 관찰되었다. 결론: 상완 이두건의 병소에 대한 초음파 검사는 상완 이두건의 탈구 또는 완전 파열 시에는 진단이 일치하였으나, 부분 파열에 대해서는 정확한 진단이 어려웠다.
In the patients of retracted massive rotator cuff tears, there are much of difficulty to functional recovery and pain relief. Nevertheless the development of treatment, there are still debates of the best treatments in the massive rotator cuff tears. Recenlty various of treatments are introduced; these are acromioplasty with debridement, biceps tenotomy, great tuberoplasty with biceps tenotomy, partial repair, mini-open rotator cuff repair, arthroscopic rotator cuff repair, soft tissue augmentation, tendon transfer, flap, hemiarthroplasty, and reverse total shoulder arthroplasty. That there is no difference of result for reverse total shoulder arthroplasty between patients who have massive rotator cuff tear without arthritis and patients who have cuff tear arthropathy. Reverse total shoulder arthroplasty is one of reliable and successful treatment options for massive rotator cuff tear. Especially it is more effective for patients who have a pseudoparalysis.
목적: 본 연구에서는 봉합 불가능한 광범위 회전근 개 파열에 있어서 상완 이두근 건 장두를 이용한 관절경하 부분 봉합술의 임상적 유용성 및 장기 추시 결과를 발표하고자 한다. 대상 및 방법: 봉합 불가능한 광범위 회전근 개 파열 환자 중 상완 이두근 건 장두에 50% 미만의 파열이 있는 41명의 환자를 대상으로 상완 이두근 건 장두를 이용하여 관절경하 부분 봉합술을 시행받은 1군과 파열되지 않은 상완 이두근 건 장두가 있으나 자가 이식건을 이용하지 않고 관절경하 부분 봉합술만을 시행한 2군으로 나누었다. 임상 점수는 시각통증점수(visual analogue pain scale, VAS), 운동범위(range of motion, ROM), The University of California, Los Angeles shoulder score (UCLA), American Shoulder and Elbow Surgeons Shoulder Score (ASES) 및 Korean Shoulder Scoring System (KSS) 점수로 측정하였으며 최종 추시에 측정된 점수가 사용되었다. 수술 전과 최종 추시에 촬영한 단순 방사선 사진을 이용하여 견봉상완거리(acromiohumeral interval, AHI)를 측정하였고 최종 추시에 시행한 초음파 또는 자기공명영상 검사를 이용하여 재파열 여부를 평가하였다. 결과: 환자의 평균 연령은 62.1±12.7세였으며, 평균 추시 기간은 90.3±16.8개월이었다. 1군과 2군 간에 VAS와 ROM (전방 거상, 외회전, 내회전)의 유의한 차이는 없었다(p=0.179, p=0.129, p=0.098, p=0.155). UCLA, ASES 및 KSS 점수는 2군과 비교해 1군에서 기능적 개선을 보였다(p=0.041, p=0.023, p=0.019). 최종 추시 시 AHI는 1군 9.46±0.41 mm, 2군 6.86±0.64 mm로 측정되었다(p=0.032). 재파열은 1군 21예 중 6예(28.6%), 2군 20예 중 9예(45.0%)에서 발견되었다. 1군의 경우 2군에 비하여 유의하게 낮은 재파열률을 보였다(p=0.011). 결론: 본 연구에서는 봉합이 불가능한 광범위 회전근 개 파열이 있는 환자들에게 상완 이두근 건 장두를 이용한 관절경하 부분 봉합술을 시행하여 장기 추시한 결과, 상완 이두근 건 장두를 사용하지 않은 군에 비하여 기능적 향상 및 낮은 재파열률을 보인다. 따라서 이는 봉합 불가능한 광범위 회전근 개 파열이 있는 환자에서 좋은 수술적 방법으로 생각된다.
서론: 관절경하 회전근개 봉합 시 상완 이두 근 장두를 회전근 개 건 결손부위에 포함한 봉합법의 임상적 결과에 대하여 평가하는 것이다. 재료 및 방법: 2005년 1월부터 2007년 1월까지 대파열 또는 거대파열의 회전근개 완전 파열을 가진 환자 총 21명을 대상으로 상완 이두 근 장두를 포함하여 관절경하 회전근 개 봉합술을 시행하였다. 남자 9명, 여자 12명이었으며 평균 나이는 60.3세(47~73세)이었다. 평균 추시 기간은 23개월(6~48)이었으며, 추시 된 모든 환자에게 University of California Los Angeles (UCLA), the shoulder index of the American Shoulder and Elbow Surgeons (ASES)와 simple shoulder test (SST)를 이용하여 술 전과 마지막 추시 시 점수를 비교하였다. 결과: 마지막 추시(평균 23개월)에서 VAS 점수, ASES, UCLA점수와 SST 점수는 통계적으로 의미 있게 향상되었다(p<0.05). 초음파와 MRI를 이용하여 추시 한 9예 중에서 2예 에서는 부분 파열의 소견을 보이며 건의 연결성이 관찰되었으나 나머지 7예에서는 모두 완전 파열된 소견이 관찰되었다. 결론: 회전근 개 대파열이나 광범위 파열 환자에서 상완 이두 근 장두를 관절와에서 분리시키지 않고, 회전근 개 봉합술을 시행하는 방법은 권장할 만한 술식으로 사료된다.
목적: 광범위 회전근 개 파열은 파열단의 내측 퇴축, 근위축 및 지방 변성과 함께 주변 조직과의 유착 등으로 해부학적 봉합이 불가능한 경우가 많다. 본 종설에서는 광범위 회전근 개 파열의 여러 가지 치료 방법 중 봉합술에 대하여 문헌 고찰과 함께 임상 결과 향상과 재파열율을 감소시키고 치유력을 높일 수 있는 방법을 알아보고자 한다. 대상 및 방법: 광범위 회전근 개 파열 봉합술의 선택은 환자의 나이, 동반 질환, 통증, 운동 범위 감소, 근력 약화 등의 임상 증상 및 재활 의지 등 환자와 관련된 요인과, 회전근 개 파열 기간, 크기, 퇴축 및 지방 변성 정도 등은 회전근 개와 관련된 요인을 고려하여 선택해야 한다. 결과: 조직의 가동성이 떨어져 해부학적 봉합이 어려운 광범위 회전근 개 파열은 주변 조직 유리술과 간격 활주 방법으로 가동성을 증가시켜 봉합할 수 있다. 주변 조직 유리술을 시행하고 회전근 개의 가동성을 증가시켜도 해부학적 봉합이 불가능한 광범위 회전근 개 파열은 부분 봉합, 변연 수렴 술식 및 상완 이두건 절제 및 고정술 등의 대체 술식을 고려할 수 있다. 광범위 회전근개 파열 환자에서 견봉하 감압술 및 회전근 개 봉합술에 관한 여러 보고는 통증 감소, 견관절 기능 및 근력 회복 등 만족할만한 임상 결과를 보고하고 있으나 장기 추시 결과 높은 재파열율이 관찰되고 있다. 결론: 광범위 회전근 개 파열의 치료는 아직까지 높은 재파열 발생율로 해결해야 할 과제가 많으나 광범위 파열의 병인, 진행 과정 및 임상 양상에 대한 이해와 재파열 예후 인자 분석 등을 통해 향상된 치료 성적을 얻을 수 있을 것으로 사료된다.
