파킨슨병에서 $^{18}F-FDG$ PET의 임상이용 (Clinical Application of $^{18}F-FDG$ PET in Parkinson's Disease)

  • Parkinson's disease is the second most common neurodegenerative disorder. It is slowly progressive disease that affects a small area of cells in the mid brain known as the substantia nigra. Gradual degeneration of these cells causes a reduction in a vital chemical known as dopamine. In the diagnosis of Parkinson's disease, it has difficulty in biopsy and limits in radiologic modalities. $^{18}F-FDG$ PET shows various findings from normal to diffuse decrement of FDG uptake. $^{18}F-FDG$ PET is expected to be a evaluation tool in the treatment of Parkinson's disease.

  • Parkinson's disease is representative degenerative diseases of the nervous system, which is from deficiency of dopamine neurons to pass in which the gradual degeneration of the body. In this paper, open UCI repository data of Parkinson's patients is used for experiments. The classification based on correlation analysis is examined. In addition, the relationship between groups is differentiated by cluster analysis based on patients with Parkinson's disease by apriori algorithm and correlation analysis. It is used to find the properties that distinguish cluster analysis. Though the disease is the same in the basic structure, each group is compared as each gender group with the most distinctive part of the characteristics.

  • Parkinson's disease is characterized by motor disturbances and dopaminergic neurodegeneration. parkin and PINK1, two most critical Parkinson's disease-associated genes, have been intensively studied to address the underlying molecular pathogenesis of the disease, but our understanding still remains unclear. Through generation and characterization of Drosophila mutants for PINK1, we show that PINK1 is required for mitochondrial integrity and function in both indirect flight muscles and dopaminergic neurons. Surprisingly, we find that PINK1 mutants share striking phenotypic similarities with parkin mutants. Indeed, transgenic expression of parkin dramatically ameliorates all PINK1 loss-of-function phenotypes, but not vice versa, implicating that Parkin acts downstream of PINK1 in maintaining mitochondrial integrity and function in both muscles and dopaminergic neurons. With the establishment of the PINK1-Parkin pathway, we are trying to further investigate the detailed molecular relationship between PINK1 and Parkin using both mammalian dopaminergic neuronal cells for biochemical analysis and Drosophila model animal for genetic analysis. We believe that elucidating the molecular function of Parkinson's disease-associated genes will be of big help for the ultimate understanding of the pathogenic mechanism of this disease and also for the development of effective drugs for Parkinson's disease.

열다한소탕가감방(熱多寒少湯加減方)으로 호전된 파킨슨병 환자 1례 (A Case Study of Parkinson's Disease Patient Treated with Yuldahanso-tang gagam)

  • Objectives This study is about a "Taeumin" parkinson's disease patient with bradykinesia, tremor, weakness and dysautonomia etc. In this case, we evaluated the effect of Yuldahanso-tang gagam for parkinson's disease. Methods This patient was treated by Sasang constitutional herbal medication based on "Donguisusebowon". Acupuncture and west medications were used together. We evaluated the clinical progress through two methods, DITI(Digital Infrared Thermal Imaging) and VAS(Visual Analogue Scale). Results During 11months treatment, chief complaints and symptoms of this patient were improved. Conclusions This study shows that Yuldahanso-tang gagam is an effective treatment for "Taeumin" parkinson's disease patients.

파킨슨병의 침 치료 효과에 대한 최신 연구 동향 고찰 - 최근 5년간의 임상 연구를 중심으로 - (Review on the effect of acupuncture on Parkinson's disease over the last 5 years)

  • Objectives: The objective of this study was to summarize clinical studies conducted over the last five years that investigated the effect of acupuncture on Parkinson's disease and to propose a better process of study. Methods: Research Information Sharing Service (RISS), Korea Studies Information Service (KISS), Cochrane Central Register of Controlled Trials (CENTRAL), PubMed, Embase, and China National Knowledge Infrastructure (CNKI) were systemically searched for clinical trials that had investigated the effect of acupuncture on the course of Parkinson's disease from May 2016 to April 2021. Results: A total of 23 studies met all the inclusion criteria. In most reports, acupuncture had significant positive effects on the course of Parkinson's disease. Furthermore, there were no serious adverse events associated with acupuncture in any of the studies. In addition to the acupuncture methods that showed effectiveness in previous studies, various types of acupuncture have been used to treat sub-symptoms of Parkinson's disease. The outcome measures were subdivided through individual symptom evaluation and mechanical analysis. Follow-up assessments were also performed to analyze the continuous effect. Conclusion: In the clinical studies conducted over the last five years, many studies investigated the various types of acupuncture used to treat Parkinson's disease and the segmentation and diversification of outcome measures focusing on individual symptoms, and a new approach for excluding placebo effects through follow-up studies has been made. Further attempts like these are needed to overcome methodological flaws in studies on the effects of acupuncture on Parkinson's disease.

Effect of Acupuncture on 6-Hydroxydopamine-induced Nigrostriatal Dopaminergic Neuronal Cell Death in Rats

  • Objectives: Acupuncture treatment has been clinically used for functional recovery in Parkinson's disease. In the present study, we investigated the effect of acupuncture at Zusanli (ST36) on nigrostriatal dopaminergic neuronal cell death in rats. Methods: A Parkinson's disease model was induced by the unilateral injection of 6-hydroxydopamine (6-OHDA) into the striatum. Acupuncture treatment was performed at Zusanli (ST36) and at the hip, as a non-acupoint, once a day for 14 days. Two weeks after 6-0HDA injection, an apomorphine-induced rotational behavior test showed significant rotational asymmetry in rats with Parkinson's disease. Immunostaining for tyrosine hydroxylase demonstrated a dopaminergic neuronal loss in the substantia nigra and dopaminergic fiber loss in the striatum. Results: Acupuncture at the ST36 acupoint significantly inhibited rotational asymmetry in rats with Parkinson's disease, and also protected against 6-OHDA-induced nigrostriatal dopaminergic neuronal loss. These effects of acupuncture were not observed for non-acupoint acupuncture. Conclusions: The present study shows that acupuncture treatment, especially at the ST36 acupoint, can be used as a useful strategy for the treatment of Parkinson's disease.

Effects of Aerobic Exercise on Disease Severity and Walking Ability in Patients with Parkinson's Disease

  • Purpose: This study was conducted to explore the effects of aerobic exercise on the severity of disease and walking ability in patients with Parkinson's disease. Methods: Twelve patients with Parkinson's disease participated in the study. Participants were randomly assigned to either an aerobic exercise group (n=6) or a self-exercise group (n=6). All participants underwent treadmill training for 30 minutes. In addition, the experimental group (aerobic exercise group) and control group (self-exercise group) participated in a 30 minutes exercise program. In both groups, exercise was performed five times a week for four weeks. Outcome including disease severity (Unified Parkinson's Disease Rating Scale) and walking ability (10 m walking speed test, 6 minutes walking test, timed up-and-go test) were measured at baseline and after 4-weeks. Results: Significant differences in disease severity and walking ability were observed between the pre- and post-exercise groups (p<0.05). The improvement of disease severity and walking endurance was significantly higher in the experimental group than in the control group (p<0.05). Conclusion: These findings demonstrate that aerobic exercise is effective at improving disease severity and walking endurance in patients with Parkinson's disease.

기혈양허로 변증한 파킨슨병 환자 증례 보고 (Case Report of Parkinson's Disease Diagnosed as Deficiency of Qi and Blood(氣血兩虛))

  • Parkinson's disease is a slowly progressive degenerative disorder of the central nervous system. It is characterized by tremor when muscles are at rest, increased muscle tone, slowness of voluntary movements, and difficulty maintaining balance. In oriental medicine, these symptoms are diagnosed as yin(陰)-deficiency of liver and kidney, deficiency of qi(氣) and blood, retention of phlegm(痰), qi-stagnation and blood stasis. In this case, we diagnosed patients as deficiency of qi(氣) and blood type according to symptoms and treated by herbs that strengthen yang and benefiting yin for two weeks, while maintaining existing parkinson's western medication. After treatment, clinical symptoms were improved, while UPDRS (Unified Parkinson's Disease Rating Scale) score was decreased. These cases suggest that oriental medicine therapy maybe effective in the treatment of Parkinson's disease.

  • Objectives: This study aimed to examine factors that affect chronic illness anticipated stigma among patient living with Parkinson's disease. The attributes of stigma measured in this study were labeling, expectation of prejudice, negative stereotyping, and discrimination. Chronic illness anticipated stigma was correlated with measures of mental and physical health as well as certain health behaviors. Methods: The sample included 250 adults with idiopathic Parkinson's disease that attended D university hospital in B metropolitan city from June to August 2014. Results: The chronic illness anticipated stigma were significantly correlated to depression, anxiety, Schwab & England ADL, Hoehn & Yahr stage, and age. The significant predictors of chronic illness anticipated stigma were anxiety, depression, and age. These factors accounted for 47% of variance in chronic illness anticipated stigma. Conclusions: In order to reduce the chronic illness anticipated stigma, nurse involved in the treatment and care of patients with Parkinson's disease should encourage patients to attend programs of depression and anxiety management in Parkinson's disease and systemic education programs on stigma.

강도 및 음도 조절을 이용한 훈련이 파킨슨병 환자의 음성 및 발화명료도 개선에 미치는 효과: 사례연구 (The Effects of Voice and Speech Intelligibility Improvements in Parkinson Disease by Training Loudness and Pitch: A Case Study)

  • The purpose of this study was to examine the effects of manipulating loudness and pitch in terms of speech intelligibility and voice of a patient with Parkinson's Disease. The subject, who was diagnosed as a patient with Parkinson's disease 11 years ago, demonstrated a severely breath voice with low intensity. The accuracy of articulation in consonants was intelligible only at the single word level, and the overall intelligibility in continuous speech was low. The results showed that the subject's articulation accuracy and speech intelligibility was significantly improved after having loudness and pitch training. Habitual Fo, Jitter, Shimmer, Fo tremor, Amp tremor were decreased after training. In addition, the value of HNR also increased after training. It was shown that the changes of these acoustic parameters were closely related to the decrease of breathiness in Parkinson's voice, and this decrease of breathiness affected speech intelligibility considerably. Based on the experimental results, it was claimed that the vocal training by manipulating the loudness and pitch could be highly effective in improving the voice quality and speech intelligibility in Parkinson's Disease.
