Through the development and dissemination of embedded devices, studies that may help patients are rapidly emerging. Recently, as wearable devices have become one of the ways to diagnose diseases in daily life, they are being studied as a way to assist severely ill patients to lead their daily lives. Among them, a method of detecting and giving signals to detect and solve symptoms using acceleration sensors to diagnose Parkinson's disease is being studied, and there is no study to measure and analyze various factors that can affect Parkinson's disease. To solve them, we designed and developed a wearable device, P-Band, with various sensors capable of diagnosing related symptoms, including acceleration sensors capable of diagnosing Parkinson's disease. In this paper, the overall structure of the P-Band and the description and operation method of the measurable sensors are presented. In addition, it was confirmed that the symptoms of Parkinson's patients could be determined complexly through the results measured in actual patients.
Purpose: The aims of this study was to evaluate the effects of shoulder lateral rotation and trunk extension exercises on standing posture improvement exercises using turning round in the patients with Parkinson's disease. Methods: For twenty-one patients with Parkinson's disease, shoulder lateral rotation and trunk extension exercises were performed for a total of three sets, ten times a set, three days a week for twelve weeks. To measure turning round, ink foot-print method was used. The number of steps and time were measured every week for before and after study (twelve weeks). Results: The results are as follows: Changes in steps and time on each week during twelve weeks showed significant differences, compared with before test(p<0.05). Conclusion: Standing posture improvement exercises can help perform turning round in the patients with Parkinson's disease.
Objectives : This study was designed to evaluate the effect of acupuncture with various scales on symptoms of Idiopathic Parkinson's disease. Methods: Subjects were voluntarily recruited through newspapers and internet advertisement. All the subjects were confirmed as idiopathic parkinson's disease by a neurologist. The acupuncture therapy was performed twice a week for 4 weeks by oriental medical doctor at Kyung-hee University hospital. Patients were randomly assigned to three groups : sham acupuncture group, acupuncture treatment group and Sasang-Constitution acupuncture treatment group. Acupun cture points used in acupuncture treatment group were $GB_{34}$, $LR_3$, and $ST_{36}$, which were proven to be effective in rat Parkinson's disease study. Acupuncture points used in sham treatment were non-acupoints near $GB_{34}$, $LR_3$, and $ST_{36}$. In Sasang-Constitution acupuncture treatment, we classfied Sasang constitution of patients by QSCCII. Acupuncture was applied based on theory of Korean medicine. The patient's symptoms were assessed at baseline and after 4 weeks of treatment by one evaluator with Unified Kingdom Parkinson's Disease Rating Scale (UPDRS), modified Hoehn-Yahr(H-Y) stage, and Schwab & England activ ity of daily living and freezing of gait questionnaire (FOGQ). Results : The results were as follows ; 1. UPDRS IV score differences between sham group and Sasang-Constitution acupuncture treatment were statistically significant(p=0.001). 2. UPDRS total score differences between Sasang-Constitution acupuncture treatment and acupuncture treatment group, Sasang-Constitution acupuncture treatment and sham group were statistically significant(p=0.041, p=0.014). 5. FOGQ score differences between Sasang-Constitution acupuncture treatment and acupuncture treatment group, Sasang-Constitution acupuncture treatment and sham group were statistically significant(p=0.001, p=0.001). Conclusion : The results suggest Sasang-Constitution acupuncture treatment can be applicable to improve symptoms in patients with idiopathic Parkinson's disease.
Objectives: The purpose of this study was to investigate patients' demand for music and art therapy in the treatment of Parkinson's disease and to understand how patients view the use of music and art therapy as a complementary form of treatment that can improve their quality of life and increase their self-efficacy in addition to medication. Methods: To this end, 60 Parkinson's patients visiting Daejeon Korean Medicine Hospital at Daejeon University were surveyed about music and art therapy. Results: The survey demonstrated a highly positive response to the music program, with strong belief in its efficacy for Parkinson's disease and significant interest in participation. Group music therapy was perceived as effective for enhancing mental health, respiratory function, language disorders, and overall quality of life. Although the art program was also deemed effective, patients showed slightly lower interest and willingness to participate compared to music therapy. The patients preferred 60-minute sessions once a week for about 8 weeks. Conclusions: We propose the following directions for a Parkinson's music and art therapy program. First, develop a once-a-week, 8-week music and art program for Parkinson's patients. Second, design the music program to focus on improving respiratory function and quality of life. Third, the art program focuses on creating a sense of security and comfort through emotional exchange among patients and improving self-efficacy.
본 연구에서는 만성 파킨슨병 환자의 동적탄력튜빙 보행 훈련 (I, II)이 자세정렬, 보행, 삶의 질에 미치는 영향을 조사하였다. 총 3명의 만성 파킨슨병 환자를 모집하여 사례 연구를 시행하였다(Hoehn 및 Yahr 1-3단계 각 1명). 동적탄력튜빙 보행 훈련 (I 및 II)은 파킨슨병 환자에게 하루 30분, 주 5일, 5주 동안 25세션 적용되었다. 본 연구의 효과를 알아보기 위해, 자세정렬검사, 근 활성도검사, 보행분석 및 파킨슨병 환자의 삶의 질을 평가하였다. Dynamic tubing 보행 훈련(I, II) 중재 후 몸통 굽힘은 감소되었다. 또한 초기 접촉기(IC)에서 중간 디딤기(Mst)까지 보행 시 넙다리네갈래근, 뒤넙다리근, 앞정강근(TA)의 근 활성도가 증가하고 장딴지근의 근활성도가 감소하였다. 척추세움근(ES T12, L3)의 근활성도는 H&Y I, III 단계에서 증가하고 H&Y II 단계에서 감소하였다. 압력 중심(COP) 매개변수인 보행선의 길이, 한 발 지지선, 앞/뒤 위치, 좌우대칭 값이 개선되었다. 시공간 보행 매개변수인 보폭, 보폭, 의 속도는 증가하고 분당걸음수는 감소되었다. 나아가 파킨슨 병 환자의 삶의 질도 개선되었다. 이러한 연구 결과를 바탕으로 동적탄력튜빙 보행 훈련 (I 및 II)은 약물 내성이 절반으로 감소된 5년 이상된 만성 파킨슨병 환자의 자세, 보행, 삶의 질을 개선하기 위한 새로운 접근 방식으로 적용될 수 있을 것이다.
Purpose: This study was to explore the effects of trunk exercise using PNF combined with treadmill training on balance and walking ability in patients with Parkinson's disease. Methods: This study included 16 patients with Parkinson's disease. Participants were randomly assigned to 2 groups: an experimental group (n=8) and a control group (n=8). All participants underwent treadmill training for 30 minutes. In addition, the experimental group (trunk exercise using PNF) and control group (conventional training) participated in a 30-minute exercise program. Both groups performed the training 5 times per week for 4 weeks. Disease severity (determined using the unified Parkinson's disease rating scale motor subscale, UPDRS-3), balance (determined using the Berg balance scale, BBS), walking speed (determined using the 10-meter walking test, 10MWT), and walking endurance (determined using the 6-minute walking test, 6MWT) were measured at baseline and after 4 weeks. Results: Pre- to post-intervention improvement was noted on all outcome measures for both groups (p<0.05). Post-intervention, there was a significant improvement in the experimental group as compared to the control group for the following measured outcomes (p<0.05): UPDRS-3 (p=0.03; 95% CI, -5.52 to -0.24), BBS (p=0.04; 95% CI, 0.59 to 6.45), 10MWT (p=0.01; 95% CI, -2.19 to -0.42), and 6MWT (p=0.04; 95% CI, 1.81 to 96.72) Conclusion: The results of this study revealed that trunk exercise using PNF plus treadmill training improves balance and walking ability as compared to conventional training plus treadmill training in patients with Parkinson's disease.
Objective: The purpose of this study was to investigate the effect of cinnamon on the treatment of Parkinson's disease (PD) and to introduce its use in Korea. Method: We searched the experimental studies in electronic databases (PubMed, CNKI, Wanfang, CiNii, J-STAGE, Science ON, and OASIS) using the key search terms "cinnamic acid", "cinnamon", "cinnamomum", "Parkinson's disease", "Parkinson disease", "Parkinsonism", and "dopamine". This study only involved experimental studies (in vivo and in vitro) that adopted cinnamon as a single administration and measured indicators relating to Parkinson's disease, including parkin, tyrosine hydroxylase (TH), and dopamine. Results: A Total of 11 literature studies were selected, and they all showed that treatment with cinnamon has a neuroprotective effect. Cinnamon activated neuroprotective factors and restored neurotransmitters and it reduced the rate of oxidative stress and inflammation in neurons. As a result, cell viability was upregulated, while cell apoptosis and neurodegeneration were downregulated. Five in vivo studies, through behavioral tests, also confirmed that cinnamon recovers locomotor function in PD models. Conclusion: We identified that cinnamon is an effective neural protector and improves motor performance in behavioral testing in the experimental PD studies.
본 연구는 지역사회에 거주하는 파킨슨병환자의 운동기능, 우울, 낙상공포의 관계를 파악하고 낙상공포에 미치는 영향요인을 규명하기 위한 것이다. 대상자는 편의표집에 의해 선정된 파킨슨병환자 180명이다. 수집된 자료는 IBM SPSS Statistics 20.0을 이용하여 기술통계, t-test, ANOVA, Pearson correlation coefficient와 multiple linear regression으로 분석하였다. 그 결과, 파킨슨병환자는 여성, 직업이 없는 경우. 보행보조기구 사용자, 2회 이상 낙상 경험자, 파킨슨병 4단계 환자, 유병기간이 9년을 초과한 경우, 항고혈압제 복용 환자, 독립적 운동수행력 하위군, 우울이 있는 경우 낙상공포가 더 높게 나타났다. 낙상공포는 운동기능 및 우울과 유의한 순상관관계를 나타냈다. 파킨슨병환자의 낙상공포의 영향요인은 우울, 낙상횟수, 파킨슨병 단계, 성별, 항고혈압제, 운동기능으로 변량의 36.0%의 설명력을 나타냈으며 영향요인 중 우울이 가장 강력한 요인으로 나타났다. 따라서 지역사회 거주하는 파킨슨병환자의 낙상공포를 예방하기 위해서는 영향요인에 대한 체계적 간호사정과 함께 우울을 완화할 수 있는 중재전략이 필요함을 제시해 주었다.
파킨슨병은 신체 움직임을 담당하는 도파민의 부족으로 신체 기능의 장애로 기본적인 일상생활까지 영향을 미치는 퇴행성 신경질환이다. 현재 의술로는 완치가 힘들어 병의 지연과 예방 차원으로 운동치료에 관심을 모으고 있다. 이에 본 연구에서는 펠든크라이스 기법®을 파킨슨병 환자에게 적용하여 심신의 상태를 스스로 돌볼 수 있는 신체 움직임 프로그램을 설계하고 보급하는데 목적을 갖는다. 펠든크라이스 기법®은 신체 움직임을 이용한 심신 자각 학습 방법으로 신경가소성의 기능인 뇌와 행동을 연결하여 신경계를 재교육하는 방법론이다. 본 연구에서는 연구자가 개발하고 검증한 신체 움직임 프로그램을 펠든크라이스 기법®의 자각(⾃覺)에 초점을 맞추어 수정·보완하였다. 24회기의 신체 움직임 프로그램은 파킨슨병 환자의 자기관리능력을 향상하기 위해 5단계로 구성하였다. 첫 번째 단계는 자기 인식이고 두 번째 단계는 자기 관찰이다. 셋째, 자기조직화, 넷째, 자기 통제. 그리고 다섯 번째 단계는 자기 관리다. 전반적인 변화는 자신의 상태를 인식하고 내적 감각과 외적 환경의 변화를 감지하는 능력을 향상시킨다. 결론적으로 파킨슨병 환자에게 심신 기능의 향상과 자기관리가 가능한 신체 움직임 프로그램을 펠든크라이스 기법을 적용하여 설계하였다. 앞으로 설계된 프로그램을 현장 적용 가능성 여부는 후속과제로 남긴다. 나아가 노년층의 웰니스를 위한 움직임의 참고자료로 메타버스를 적용한 과학분야와의 융합적 협력을 통해 보다 폭넓게 확산될 수 있도록 체계적 구조를 구축할 필요가 있다.
파킨슨병은 떨림, 느려진 움직임, 근육의 경직, 자세 불안정성, 보행장애 등의 특징을 나타내는 신경변성 질환으로 도파민과 같은 신경전달 물질이 고갈되어 생긴다. 이러한 파킨슨병의 특성으로 인해 환자들은 협조적으로 치과 치료를 받는 것이 어렵다. 본 증례에서는 만성 치주염이 있는 47세의 파킨슨병 환자에게 고정성 보철 및 임플란트 식립을 통한 전악구강수복을 시행하였고 기능적, 심미적으로 만족할만한 결과를 얻을 수 있었다. 손 조작능력의 저하와 같은 파킨슨병의 특성을 고려하여 짧은 간격의 주기적인 내원을 통한 관리가 필요할 것이다.
