Residual Chlorinated Hydrocarbon Pesticides in Semisulcospira spp. (Gastropoda: Pleuroceridae) Collected at the Endemic and Non-endemic Areas of Paragonimiasis in Korea (한국산 다슬기(Semisulcospira spp.)의 흡충류 유미유충 감염실태 및 농약의 잔류성에 관한 조사연구)

  • One of the most medically important snail species of Korea is Semisulcospira spp., a member of the freshwater prosobranch family pleuroceridae. The parasites that these snails transmit to humans are mainly Paragonimus westermani and Metagonimus yokogawoi. On the other hand, Semisulcospira snails are edible as a foodstuff in Korea. The present study was first aimed to detect out residual pesticides in the snails collected at the endemic and non-endemic areas of paragonimiasis in Korea and to make relationship to the detectability of larval trematodes shed from the snails. In addition, water specimens from the habitats where the snails were collected were sampled and analysed to elucidate environmental conditions of the habitats. This malaco-ecological survey was done at 7 endemic and 3 non-endemic areas of paragonimiasis for about more than 2 years, May, 1984 through October, 1986. All the water smaples both from the endemic and non-endemic areas of paragonimiasis showed normal ranges of the criteria of upper freshwater streams, with lower B.O.D. (0.4~2.0 ppm) and higher D.O. values (9.5~11.0 ppm) than those in the large river systems Infection rate of digenetic trematodes in total snails collected was 6.7%, but no Paragonimus cercaria was detected out at all even in the endemic areas of paragonimiasis. The ecological factors affecting Paragonimus larvae in the snails were also discussed. A total of 9 chlorinated hydrocarbon pesticieds was targeted to detect out from the tissues of snails. Out of 9 pesticides, aldrin, beta-BHC, p, p-DDT, dieldrin, DDE, lindane, heptachlor and heptachlor epoxide were eluted from the snails. The highest levels of the pesticides were mostly detected out in the snails collected from the Donghae-Chun area of Haman and the Kangwha area of Kyongki-Do, where the lowest infection rates of trematode cercariae (0~0.5%) were revealed. The possibilities that the residual pesticides in the freshwater animals including Semisulcospira snails might be transferable to humans were also discussed.

  • Paragonimus westermani is a common fluke in Uorea. The present study aimed to observe the cell mediated immune response in experimental paragonimiasis of mice. The mouse (BALB/c) was orally inoculated with 40 metacercariae of P. westermani from Cambaroides similis. During the infection (1, 2, 4, 6 weeks) of mouse, blastogenic response of splenic Iymphocytes to P. westermani adult antigen, metacercaria antigen, and PHA were observed. Sera from infected and noninfected mice added to normal mouse splenic Lymphocytes with or without PHA. The blastogenic response of splenic Lymphocytes to PHA was reduced after 1 week of infection. However after 6 weeks of infection, the response was restored to the control level. The blastogenic response of splenic Iymphocytes to P. westermani adult or metacercaria antigen increased significantly on 1 week after infection, and maintained up to 6 weeks after infection. The response of non-infected mice was suppressed by addition of the infected mouse serum. The present results suggested that cellular immunity was involved in P. westermani infected mice and that P. westermani anti.serum inhibited proliferation of T Iymphocytes.

Demonstration of species-specific and cross reactive components of Paragonimus tvestermani crude worm antigen by EITB (Immunoblot를 이용한 폐흡충 비항원의 특이 항원대의 증명)

  • Enzyme-linked immunoelectrotransfer blot (EITB) using crude worm antigen of adult Paragonimus westermani was performed for human patients sera to identify the species-specific components. Crude antigen was obtained by homogenizing and centrifuging 24-week old adult worms at 10,000 rpm for 60 minutes in phosphate buffered saline (PBS, PH 7.2) containing: Phenyl methyl sulfonyl auoride (PMSF). Gradient sodium dodecyl sulfate polyacrylamide gel electrophoresis(SDS-PAGE) was performed and blotted electrophoretically onto a sheet of nitrocellulose paper. The sheet was cut into strips and exposed to sera diluted 1 : 200 with PBS. SDS-PAGE showed 26 protein bands ranging 229 to 10 kDa. Of them 229, 91, 60, 50, 35∼31, 27, 25, 21, 17, 11 and 10 kDa components showed positive reaction with serum antibody of patients with p. westermani. Sera of patients infected with Clcnorchis sinensis reacted with 35∼31, 19, and 11 kDa bands. Human sera from cysticercosis and diphyllobothriasis cases showed non-specific cross reactions with 229, 35∼31, 27, 25 and 17kDa bands. Protein bands of 91, 60, 21 and 10kDa showed strong positive reaction without cross reactions with sera from other helminthic infections.

  • A 23-year-old medical student showed a positive reaction on a skin test for Paragonimus westermani, and two Tarsonemus floricolus mites were subsequently found by sputum examination and identified morphologically. Our report is the first human case of Tarsonemus floricofus in Korea.

  • Asian countries including Korea, Japan, China, Taiwan, and Philippines are endemic area of Paragonimiasis as people who live those countries have special diet habbit to ingest raw fish and meat. Of the Paragonimus westermani infestation, mediastinal paragonimiasis is a rare condition because of its life cycle. We experienced a case of mediastinal parasitic cyst resected under video-thoracoscopy, which was confirmed as paragonimiasis granuloma postoperativley.

  • Paragonimiasis typically results from the consumption of raw or improperly cooked crustacea, especially crabs and crayfish. Although previously endemic in Korea, the prevalence of this disease decreased in the early 1970s because of educational campaigns and fewer intermediate hosts as a result of ecological changes. Recently, we were presented with a family where all members were infected with Paragonimus after ingestion of Kejang (= drunken crab). The mother was hospitalized for general myalgia and weakness first, followed by the father, who was hospitalized for dyspnea 2 month later. After the parents were diagnosed with paragonimiasis, we recommended their daughter to visit our hospital for a checkup, because they all had eaten freshwater crabs soaked in soybean sauce. She complained of generalized myalgia, fever, and pleuritic pain, and was also diagnosed with paragonimiasis. Peripheral blood of the 3 patients revealed hypereosinophilia, and computed tomography (CT) scans of their chests showed pleural effusion. The results of antibody tests by ELISA were positive for paragonimiasis. We report here the case series of familial paragonimiasis in a modern urban city, rather than in a typical endemic area.

A Case of Delayed Diagnosis of Pulmonary Paragonimiasis due to Improvement after Anti-tuberculosis Therapy

  • Here, we report a case of pulmonary paragonimiasis that was improved with initial anti-tuberculosis (TB) therapy but confused with reactivated pulmonary TB. A 53-year-old Chinese female presented with a persistent productive cough with foul smelling phlegm and blood streaked sputum. Radiologic findings showed subpleural cavitary consolidation in the right upper lobe (RUL). Bronchoscopic and cytological examination showed no remarkable medical feature. She was diagnosed with smear-negative TB, and her radiologic findings improved after receiving a 6-month anti-TB therapy. The chest CT scans, however, obtained at 4 months after completion of anti-TB therapy showed a newly developed subpleural consolidation in the RUL. She refused pathologic confirmation and was re-treated with anti-TB medication. Nevertheless, her chest CT scans revealed newly developed cavitary nodules at 5 months after re-treatment. She underwent thoracoscopic wedge resection; the pathological examination reported that granuloma caused by Paragonimus westermani. Paragonimiasis should also be considered in patients assessed with smear-negative pulmonary TB.

Protein composition and antigenicity of the tegument from Paragonimus westermani (폐흡충 총체표피의 단백질 조성 및 항원성)

  • To ascertain that tegument of Pnragonimus westermoni has specific antigenic proteins, the tegumental fraction was isolated from 10-month-old worms by 0.1% digitonin solution, and subjected to SDS-PAGE and immunoblot. Component proteins of tegumental syncytium comprised of 94, 74 (76-66), 62, 54, 44, 42, 38, 28, 26, 25, 24, 17, 15.5 and 13.5 kDa proteins. Of them, the 94, 44 and 42kDa proteins were more specific to tegument, especially the 94 kDa protein was the most prevailing one. In immunoblot, antigens of the 94, 90, 78, 76, 74, 68, 65, 63, 60, 59 and 54 kDa proteins were commonly detected by 7 sera of 10 human paragonimlasis, but none of them reacted with 5 sera of clonorchiasis. In conclusion, the 94 kDa protein was the major tegumental protein, as well as the specific antigen. The 76 and 66 kDa proteins were the minor components of tegument, which were also specific antigens of R westermcni.

Persisting antibody reaction in paragonimiasis after praziquantel treatment is elicited mainly by egg antigens

  • Antibody responses in serum and cerebrospinal fluid (CSF) samples from patients with active and chronic paragonimiasis and in sera from patients on whom follow-up studies were done after praziquantel treatment were analyzed using antigens of Paragonimus westermani prepared from eggs, metacercariae, juveniles of 4-and 7-week old, adult worms and recombinant protein of 28 kDa cruzipain-like cysteine protease (rPw28CCP). The patient sera/CSFs of active and chronic paragonimiasis revealed strong antibody reactions against the crude extracts of 4-and 7-week old juveniles as well as against those from egg and adult. rPw28CCP also showed specific reaction to the sera with active paragonimiasis. After the treatment, levels of specific antibodies in the sera gradually decreased to negative range in most patients. In some cases with persisting high antibody levels, however, the reactions at 27 kDa egg Protein were sustained throughout the observation period of 34 months. The reactions at 35 and 32 kDa in adult extract and rPw28CCP disappeared rapidly after the treatment. Persistent antibody reactions even after successful treatment are provoked by continuous antigenic challenge from eggs which were not resolved by treatment.

Ultrastructure of the Integument of Adult Paragonimus westermani (폐흡충 표피의 미세구조)

  • The present study was performed to observe the ultrastructure of the integument of adult Paragonimus westermani. Dogs experimentally infected with 60 metacercariae of F. westermani were autopsied 4 months after the infection. Adult p. westermani were extracted from the dogs and the fine structure was studied by means of scanning and transmission electron microscope. The findings are as follows: 1. Scanning electron microscopic findings showed that the spines and the papillae are distributed at whole body surface but the well developed spines or papaillae are shown around the oral sucker and ventral sucker. 2. At the end of the body, excretory pore was found, the shape was irregular. 3. Transmission electron microscopic findings showed that plasma membrane, tegument, basal lamina, connective tissue, circular muscle layer, longitudinal muscle layer. nerve axon and tegumental cell were observed. 4. In higher magnification, plasma membrance and bar-shaped granules were found at the outer surface of the tegument.
