농양으로 오인된 하퇴의 루푸스 지방층염: 증례 보고 (Lupus Panniculitis of the Lower Leg Misdiagnosed as an Abscess: A Case Report)

  • Lupus panniculitis is a rare disease that may present with similar clinical manifestations to an infection, such as cellulitis or abscess. This paper reports a case of a 25-year-old woman with systemic lupus erythematosus who underwent surgical intervention initially with the suspicion of an abscess but who was eventually diagnosed with lupus panniculitis. Despite the low incidence of lower leg involvement of panniculitis, clinicians should be aware of the possibility of an etiology other than infection when encountering patients with inflammatory diseases. Moreover, a skin biopsy and magnetic resonance imaging with a diffusion-weight image may be helpful.

Mesenteric Panniculitis in a Thirteen-Year-Old Korean Boy Treated with Prednisolone: A Case Report

  • Pediatric mesenteric panniculitis is an extremely rare disease of unknown etiology characterized by chronic inflammation, fat necrosis, and fibrosis in the mesenteric adipose tissue. A previously healthy 13-year-old boy was admitted because of right upper abdominal pain. An abdominal computed tomography scan revealed increased attenuation and enhancement in the left upper abdominal omental fat and anterior peritoneal wall thickening. A laparoscopic biopsy showed mesenteric panniculitis with chronic inflammation, adiponecrosis, and septal fibrosis. Serological tests for autoimmune diseases, nested polymerase chain reaction for Mycobacterium tuberculosis, and special immunohistochemical stains for malignancy were all negative. Symptomatic improvement and improved abnormal findings were achieved after an 8-month treatment with prednisolone according to a follow-up abdominal computed tomography scan. Here, we report a case of pediatric mesenteric panniculitis treated with prednisolone.

소아에서 발생한 Subcutaneous Panniculitis-like T Cell Lymphoma 1례 (A Case of Subcutaneous Panniculitis-like T Cell Lymphoma in Childhood)

  • Subcutaneous panniculitis-like T cell lymphoma는 흔하지 않은 피하 림프종이다. 이 질환은 사지와 몸통을 침범하는 다수의 종괴나 판 등의 피부소견을 보이며 발열, 불쾌감, 피로, 근육통, 오한 그리고 체중 감소 같은 증상을 나타낸다. 조직학적 소견은 피하지방층염과 유사하며 크고 작은 비정형의 림프구들이 지방세포들 사이에 침윤되어 있는 양상을 보인다. 이 질환은 세포독성 T 림프구로부터 유래한, 특징적인 임상병리학적 소견을 가지며 피하지방층을 침범하는 다른 양성 그리고 악성 림프종과 감별을 요한다. 이 질환의 치료는 아직 정립된 것이 없으며, 다른 진행된 림프종에서 사용되어 온 복합화학요법으로 치료를 하여도 예후가 그다지 좋지 않은 것으로 보고되고 있다. 좋지 않은 예후를 시사하는 소견으로는 혈구탐식을 나타내는 소견들로 빈혈, 백혈구감소증, 간비종대, 전신림프절종대, 그리고 응고장애 등이 있다. 이 질환으로 인한 사망 원인은 림프종의 전신적인 침범에 의한 장기부전보다는 혈구탐식증후군과 연관된 혈구감소의 합병증에 의한 것이다. 저자들은 발열과 다수의 피하결절을 주소로 내원한 12세 남자 환아에서 subcutaneous panniculitis-like T cell lymphoma로 진단 받고 치료 중인 환아 1례를 문헌 고찰과 함께 보고하는 바이다.

닥스훈트 개의 특발성 다발성 결절 지방층염 증례 (Idiopathic Multiple Nodular Panniculitis in a Dachshund Dog)

  • A 2-year-old intact female dachshund dog was presented with recurrent subcutaneous nodules and fistulations on the neck, back and hip. The patient was diagnosed as a sterile nodular panniculitis based on the cytology, histopathology, and cultures for bacteria and fungus. The fistulas were surgically removed and methylprednisolone was administrated 2 mg/kg twice daily per oral. The lesions were all disappeared, but the relapses were happened when the dosage of the drug was tapered off. The repeated treatments with methylprednisolone or azathioprine were performed, consequently, the patient was controlled with relatively low-dosage methylprednisolone (0.25 mg/kg, every third day).

  • F-18 FDG PET is a metabolic imaging modality that is efficacious in staging and assessment of treatment response for variety of lymphomas. We report usefulness of F-18 FOG PET/Cl in evaluating severity of the disease and response to therapy in a patient with subcutaneous panniculitis- like T-cell lymphoma (SPTCL). Here we describe a case of SPTCL in 24-year-old man who had wide spread firm and tender nodular lesions with increased F-18 FOG uptake. After chemotherapy follow up F-18 FDG PET/CT image shows disseminated malignancy and then the patient died with hemophagocytic syndrome. This report suggests that F-18 FDG PET/CT may be useful in determining disease activity at the time of initial diagnosis, after treatment, and evaluating a suspected outcome of SPTCL.

Autologous Fat Transfer in Lupus Panniculitis Facial Lipoatrophy

  • Lupus panniculitis (LP) often presents with tender nodules and intermittent ulcers that then heal with scarring and lipoatrophy. The current mainstay of treatment is medical treatment. Research regarding the treatment of lipoatrophy from LP with autologous fat grafting is limited. We would like to share our experience in this rare case, which was treated with autologous fat transfer. A 48-year-old female presented with erythematous plaque, tender nodules, and ulcers following by a depression of the lesion at the left temporal area. The patient also had indurated erythematous plaque at her left cheek. Both lesions were aggravated by sunlight exposure. After several investigations, she was diagnosed as LP with secondary lipoatrophy and tumid lupus erythematosus at her left temporal and left cheek, respectively. She received antimalarial drug and topical steroids. The patient underwent two sessions of autologous fat transfer. She was satisfied with the volume and contour improvement in the scar following the injection of 8 and 3.7 mL of fat. Furthermore, the patient reported the remission of tender nodules and ulcers since the first fat graft injection. In conclusion, the autologous fat transfer is a simple and effective treatment for lipoatrophy and scar secondary to LP with promising results.

푸들개에서 발생한 특발성 결절성 피부지방염의 발생 예 (Idiopathic Nodular Panniculitis in a Poodle Dog)

  • A seven-year-old male poodle was presented with multiple non-pruritic draining skin lesions. On physical examination the dog was depressed and had three draining nodules 3.0 to 4.0 cm in diameter on the neck, flank and back. The multifocal sparing of fat lobules, macrophages, and numerous neutrophils were observed in aspiration cytologic examination of nodule. Haematological examination demonstrated a moderate leukocytosis and neutrophilia. An aerobic bacterial culture was undertaken from draining lesions on the flank. But there was no bacterial growth in aerobic culture. Therapy was initiated with oral cephalexin at a dose of 20 mg/kg, body weight three times daily and prednisolone at 1.0 mg/kg twice daily. The (lug returned to normal within 24 hours of treatment and its general attitute and appetite were improved. However, within 1 month new nodules had appeared on the dorsum over the rump, coalescing into a mass of draining nodules and tracts.

  • Erythema nodosum is a nodular erythematous eruption predominently limited to the pretibial area but occasionally involving the arms or other areas. That is clinical entity defined easily but there are many different opinions about histopathologic findings, Recently erythema nodosum is characterized histopathologically by a septal panniculitis in which the fibrous septa of subcutaneous fat become inflamed. Erythema nodosum has been known to be frequently associated with some kinds of drugs, infections with streptococci, mycobacteria but in $60\%$, no cause is found, Treatment of erythema nodosum consists of supportive care and elimination of underlying causes. Because spontaneous resolution of the lesions can occur in 3 to 6 weeks. Although there are trial treatments are applied to erythema nodosum, ego corticosteroids, NSAIDs, potassium iodine, there isn't any definite. So We report a case of erythema nodosum which was not relieved by NSAIDS but by experimental acupuncture treatment.
