Pancreatic duct lavage cytology combined with a cell-block method for patients with possible pancreatic ductal adenocarcinomas, including pancreatic carcinoma in situ

  • Background/Aims: This study aimed to clarify the efficacy and safety of pancreatic duct lavage cytology combined with a cell-block method (PLC-CB) for possible pancreatic ductal adenocarcinomas (PDACs). Methods: This study included 41 patients with suspected PDACs who underwent PLC-CB mainly because they were unfit for undergoing endoscopic ultrasonography-guided fine needle aspiration. A 6-Fr double lumen catheter was mainly used to perform PLC-CB. Final diagnoses were obtained from the findings of resected specimens or clinical outcomes during surveillance after PLC-CB. Results: Histocytological evaluations using PLC-CB were performed in 87.8% (36/41) of the patients. For 31 of the 36 patients, final diagnoses (invasive PDAC, 12; pancreatic carcinoma in situ, 5; benignancy, 14) were made, and the remaining five patients were excluded due to lack of surveillance periods after PLC-CB. For 31 patients, the sensitivity, specificity, and accuracy of PLC-CB for detecting malignancy were 94.1%, 100%, and 96.8%, respectively. In addition, they were 87.5%, 100%, and 94.1%, respectively, in 17 patients without pancreatic masses detectable using endoscopic ultrasonography. Four patients developed postprocedural pancreatitis, which improved with conservative therapy. Conclusions: PLC-CB has an excellent ability to detect malignancies in patients with possible PDACs, including pancreatic carcinoma in situ.

정맥주입한 알콜이 흰쥐의 췌장 외분비에 미치는 영향 (Effects of Intravenous Infusion of Ethanol on Exocrine Pancreatic Secretion of Rats)

  • To investigate the effect of intravenous ethanol administration on pancreatic exocrine secretion, we measured volume and protein amount in pancreatic juice and assayed amylase activity and phospholipase $A_2$ activity in pancreatic fragments and serum. Acute pancreatitis induced by obstruction of common bile-pancreatic duct (CBPD) and caerulein infusion (5 $\mu\textrm{g}$/kg/hr) showed typical characteristics, such as hyperamylasemia and pancreatic edema and increase of phospholipase $A_2$ activity in pancreatic fragments and serum. Intravenous ethanol infusion (50 mg/kg/hr) significantly stimulated pancreatic exocrine secretion, but such a stimulatory effect of ethanol disappeared at dose of 100 mg/kg/hr without typical symptoms of acute pancreatitis. In microscopic examination, there were no typical changes of edematous pancreatitis in ethanol administrated rats. These results suggest that acute ethanol administration has dual effect on exocrine pancreatic secretion: low dose of ethanol (50 mg/kg/hr) stimulates pancreatic exocrine secretion, whereas high dose of ethanol (100 mg/kg/hr) does not without typical changes of edematous pancreatitis.

Incidental double duct sign: Should we be worried? Results from a long-term follow-up study

  • Backgrounds/Aims: Double duct sign (DDS) (dilated common bile and pancreatic duct) is synonymous with pancreatic head/peri-ampullary tumor (PHPAT). There is limited evidence on whether incidental DDS (I-DDS) is associated with an increased risk of malignancy. This study aimed to evaluate 5-year outcomes of I-DDS. Methods: Patients were categorized according to their risk of malignancy. 'Low-risk' patients, including those with I-DDS between 2010 and 2015, were analyzed in this study. The primary outcome was incidence of PHPAT within five years of identification of DDS. Histology results from endoscopic ultrasound-guided biopsy were considered diagnostic. Secondary outcomes were incidence of benign causes, extent of follow-up investigations, and clinical indicators of malignancy in patients with DDS. Results: Among 103 patients with DDS, 20 had I-DDS. Subsequent follow-up of these 20 patients found no patient with PHPAT, two (10%) patients with chronic pancreatitis, and 18 (90%) patients with no cause found. The median follow-up duration for 'low-risk' patients was 7.3 years (range, 6-11 years). The mean number of follow-up investigations per patient was two (range, 0-9). Investigations included computed tomography (n = 27), magnetic resonance cholangiopancreatography (n = 23), endoscopy (n = 16), and ultrasound (n = 14). Patients with jaundice were more likely to have malignancy (p < 0.01). Those with abdominal pain were more likely to have a benign cause (p < 0.01). Hyperbilirubinemia and/or deranged liver enzymes and raised CA19-9 were more likely to be associated with PHPAT (p < 0.01). Conclusions: Patients with I-DDS have a low risk of developing PHPAT within five years.

발생단계에 따른 한국재래산양 췌장에서 pancreatic polypeptide 면역반응세포에 대한 면역조직화학적 연구 (Immunohistochemical study of pancreatic poly-peptide-immunoreactive cells in the pancreas of the Korean native goat (Capra hircus) during developmental stages)

  • 태아기, 신생아기, 생후 1개월령, 6개월령 및 성체의 한국재래산양 췌장에서 pancreatic poluypeptide(PP) 면역반응세포의 분포 및 출현빈도를 PAP 법에 의해 검색하였다. 외분비부에서는 모든 연령에서 관찰되었으며 췌장세포 사이에 분포하였다. PP 면역반응세포는 신생아가기에서 생후 1개월에 증가한 후 점차 감소하였다. 췌도관에서는 PP 면역반응세포가 생후 1개월부터 관찰되면 그후 점차 감소하였다. 췌도관에서는 PP 면역반응세포가 생후 1개월부터 관찰되면 그후 점차 감소하였다. 이들 면역반응세포는 생후 1개월령 및 6개월령의 췌도관 상피하 결합조직에서 소수 관찰되었다. 그러나 성체에서는 췌도관 상피세포 사이에서 극소수 관찰되었다. 그러나 성체에서는 췌도관 상피세포 사이에서 극소수 관찰되었다. 내분비부(췌장섬)에서 PP 면역반응세포는 신생아기부터 관찰되며, 생후 1개월령부터의 PP면역세포의 분포는 췌장성 주변부에 중등도 또는 소수가 산재하는 것과 다수의 PP 면역반응세포가 췌도 전체 (생후 1개월령)또는 췌도 중앙부(생후 6개월령)에 산재하는 것 등 두가지 유형을 나타내었다. 결론적으로 한국 재래산양의 췌장에 있어서 PP 면역반응세포의 분포와 출현빈도는 발생단계에 따라 상이하였다. 이는 발생단계 동안의 식이성 및 생리학적 조건변화가 이와같은 상이함을 유발하는 요인이 된 것으로 사료된다.

  • 췌장 손상은 소아에서는 흔하지 않으며 임상 증상과 이학적 소견이 뚜렷하지 않아 진단이 어려운 경우가 많은데 손상 정도와 위치에 대한 정확한 평가를 바탕으로 치료가 지연되지 않도록 하여 합병증 발생을 막도록 해야 할 것이다. 저자들은 8세 남아에서 가성 낭종, 늑막 삼출액을 동반한 췌장 절단 및 췌관 손상을 진단하고 경피적 배액술을 통하여 효과적으로 치료하였기에 문헌 고찰과 함께 보고하는 바이다.

  • 두툴상어의 췌장 내분비세포를 면역조직화학적으로 관찰하였던바, 5종의 면역반응세포들이 동정되었다. glucagon 면역반응세포는 도관의 상피간 및 췌장외분비선의 선포주위에서 한개 또는 다수의 집단으로 출현하였으며, insulin 면역반응세포는 islet의 주변부 또는 도관의 상피간에 각각 관찰되었다. somatostatin 면역반응세포는 도관의 상피간 또는 췌장외분비선의 주위에 한개 또는 집단으로 분포하였으며, 소수의 5-HT 연역반응세포는 islet의 주변부 및 췌장 외분비선의 선세포간에서 관찰되었다. BPP 면역반응세포는 극소수로 관찰되었으며, GAS/CCK외 chromogranin 면역반응세포들은 관찰되지 않았다.

  • 췌공장문합술 후 췌관 내 스텐트를 삽입하는 것은 수술 후에 췌장루나 췌관 협착의 발생을 예방하기 위해 널리 시행하는 술기이다. 그러나 삽입한 스텐트가 막히거나 이동하는 등 스텐트에 의해서도 다양한 합병증이 발생할 수 있다. 저자들은 췌관에 삽입한 플라스틱 스텐트가 이동하면서 발생된 매우 드문 합병증의 예를 보고하고자 한다. 유문부 보존 췌두부십이지장 절제술 시행 후 췌장 내 삽입한 스텐트는 공장으로 이동하여 스텐트-돌 복합체의 근원으로 작용하여 공장 폐색을 일으켰다. 스텐트-돌 복합체는 탐색적 개복술로 제거되었다.

  • Therapeutic endoscopic ultrasonography (EUS) procedures using the forward-viewing convex EUS (FV-EUS) have been reviewed based on the articles reported to date. The earliest reported procedure is the drainage of pancreatic pseudocysts using FV-EUS. However, the study on drainage of pancreatic pseudocysts focused on showing that drainage is possible with FV-EUS rather than leveraging its features. Subsequently, studies describing the characteristics of FV-EUS have been reported. By using FV-EUS in EUS-guided choledochoduodenostomy, double punctures in the gastrointestinal tract can be avoided. In postoperative modified anatomical cases, using the endoscopic function of FV-EUS, procedures such as bile duct drainage from anastomosis, pancreatic duct drainage from the afferent limb, and abscess drainage from the digestive tract have been reported. When a perpendicular puncture to the gastrointestinal tract is required or when there is a need to insert the endoscope deep into the gastrointestinal tract, FV-EUS is considered among the options.

  • A portion of duodenum laid pancreatic duct opening were perfused continuously with physiological saline under urethane anesthesia in rats. The pancreatic amylase secretory response to caerulein was studied with autonomic blockers, such as phenoxybenzamine, dibenamine, phentolamine, hexamethonium, propranolol, atropine, and cyproheptadine. The pancreatic amylase output to caerulein, 7.5ng/kg i.v., was markedly increased and the value was approximately three times greater than control. The caerulein-stimulated pancreatic amylase secretion was significantly decreased by i.v. phenoxybenzamine and propranolol treatment, but not by phentolamine or dibenamine. Secretory response of pancreatic amylase to caerulein was not affected by i.v. atropine, hexamethonium or cyproheptadine. These result lead to the conclusion that phenoxybenzamine may inherently inhibit the secretory response of pancreatic amylase to caerulein, and this effect was not related with ${\alpha}-adrenergic$ receptor blocking action.
