Complications of Calcaneal Fracture (종골 골절의 합병증)

  • Quite high prevalence of acute and chronic complications of calcaneal fractures has been reported. Acute complications include blisters, wound necrosis or infection. Late complications include subtalar arthritis, calcaneal malunion, lateral subfibular impingement, tendon problems, sural nerve complications. There are many surgical or nonsurgical treatment modalities to manage those complications. However strategic initial surgical approach with gentle soft tissue handling accompanied by comprehensive understanding about numerous complications might be the best tool to achieve pain free and functional heel after treating calcaneal fractures.

The Prevalence, Subtypes and Risk Factors of Irritable Bowel Syndrome by ROME III among Korean University Students (ROME III에 의한 국내 대학생의 과민성 장 증후군의 유병률, 증상유형 및 위험요인)

  • Purpose: The purpose of the present study was to investigate the prevalence, subtypes and risk factors of irritable bowel syndrome by ROME-III among Korean university students. Methods: This study was descriptive survey research. The sampls were 796 and variables were measured by structured questionaire. Rome-III criteria was used for diagnosis of IBS. The gathered data were analyzed with %, $x^2$-test, t-test, logistic regression by SPSS win 17.0. Results: The students with IBS were 61 (7.7%) and the most of the subtype was IBS-M (42.6%). Meal (times/day), breakfast, stress, quality of sleep, neuroticism, bodily pain, general health, social function, role emotional restriction, mental health, somatization, obcessive-compulsive state, depression, anxiety, hostility, global severity index, positive symptom distress index, positive symptom were significantly different between IBS group and non-IBS group. The prevalence of IBS was low in the higher score of role emotional in general health state. There were more 2 times students who had score of the obsessive-compulsive in psychological health over the 50 than below the 50 in IBS group. Conclusion: 7.7% of students were diagnosed by Rome-III criteria and the most of the sybtype was IBS-M. The risk factors of IBS were role emotional restriction, obsessive-compulsive state.

A Survey on the Prevalence of Cumulative Trauma Disorders and Related Factors among Seafares (선원들의 누적외상성질환 발생과 관련요인)

  • The purpose of this study is to provide basic data for the prevention and management of Cumulative Trauma Disorders in Korea seamen. 569 seamen's questionnaire had been acquired at Korea Marine Training and Research Institute. The prevalence in Cumulative Trauma Disorders within recent 12 months was 68.5%. The more working hours(p<0.01), services on the vessel(p<0.01), and the less job satisfaction(p<0.05), the higher prevalence of Cumulative Trauma Disorders. The distribution of Cumulative Trauma Disorders by the part of the body was turned out as Back ; 43.6%, knee ; 23.9%, shoulder ; 19.3%. The officers had more frequency of symptom around the neck than the ratings(p<0.01) and the seamen who work at the engine room were more frequent in elbow than those at deck department(p<0.05). In the duration of pain, 55.4% were less than one week, 20.6% less than 30 days, and 24.0% 30 days and more. the cause of symptom was turned out as 34.5% by excessive hard work and 30.1 % was unknown.

Prevalence of rotator cuff diseases in adults older than 40 years in or near Chuncheon city, Korea

  • Background: To determine the prevalence of rotator cuff diseases in a population older than 40 years in or nearby Chuncheon city, Republic of Korea. Methods: Sixty shoulders of 30 people older than 40 years who participated in a health lecture were examined for free by an orthopedic surgeon. Visual analog scale of pain and American Shoulder and Elbow Surgeons scores were assigned, and routine physical examination was performed. Ultrasonography was performed on the shoulder. Results: On ultrasonographic examination, there were one shoulder with full thickness rotator cuff tear, 20 of 60 (33%) with partial thickness rotator cuff tear, five of 60 (8%) with calcific tendinitis, one of 60 (2%) with tear of the long head of the biceps, and five of 60 (8%) with tendinitis of the long head of the biceps. Participants older than 60 years showed significantly high proportions of lesion of the long head of the biceps and rotator cuff diseases (P=0.019 and P=0.015, respectively). Participants who performed physical labor had high proportions of rotator cuff tear and rotator cuff disease (P=0.001 and P<0.001, respectively). Conclusions: Rotator cuff diseases showed a high prevalence in aged persons and resulted in a decrease in shoulder function.

The Research of Headache Characteristics According to Sasang Constitution (사상체질에 따른 두통 특성 조사)

  • Objectives The study aimed to investigate the prevalence and characteristics of headaches according to Sasang constitution among 5,764 participants residing in Ansung or Ansan in Korea. Methods The Korea Sasang Constitutional Diagnostic Questionnaire was used to classify the constitution, and a self-administered questionnaire was used to collect headache data. The presence of headache, locations and patterns between groups were analyzed using the chi-square test and logistic regression adjusted for age, marital status, education, alcohol consumption, and smoking. Results Results showed that 25.6% of participants had headaches, with a significantly higher prevalence in Soeumin (30.8%) compared to Soyangin (24.9%) and Taeeumin (24.3%). Soeumin had significantly higher odds ratios for headaches than Taeeumin in both crude (1.387) and adjusted (1.319) models. Headache sites showed high frequency in the order of occipital (8.1%), temporal (8.0%), forehead (5.4%), and global regions (5.4%), and Soeumin showed significantly higher odds ratios of forehead (crude: 1.616, adjusted: 1.543) and occipital pain (crude: 1.366 adjusted: 1.379) compared to Taeeumin. No significant differences in headache patterns were observed among the groups. Conclusions This study suggests that Soeumin has a higher prevalence of headaches, particularly in forehead and occipital regions, compared to other Sasang constitutions.

Risk factors of chronic subscapularis tendon tear

  • Background: Chronic subscapularis tendon tear (SBT) is a degenerative disease and a common pathologic cause of shoulder pain. Several potential risk factors for chronic SBT have been reported. Although metabolic abnormalities are common risk factors for degenerative disease, their potential etiological roles in chronic SBT remains unclear. The purpose of this study was to investigate potential risk factors for chronic SBT, with particular attention to metabolic factors. Methods: This study evaluated single shoulders of 939 rural residents. Each subject undertook a questionnaire, physical examinations, blood tests, and simple radiographs and magnetic resonance imaging (MRI) evaluations of bilateral shoulders. Subscapularis tendon integrity was determined by MRI findings based on the thickness of the involved tendons. The association strengths of demographic, physical, social, and radiologic factors, comorbidities, severity of rotator cuff tear (RCT), and serologic parameters for SBT were evaluated using logistic regression analyses. The significance of those analyses was set at p<0.05. Results: The prevalence of SBT was 32.2% (302/939). The prevalence of partial- and full-thickness tears was 23.5% (221/939) and 8.6% (81/939), respectively. The prevalence of isolated SBT was 20.2% (190/939), SBT combined with supraspinatus or infraspinatus tendon tear was 11.9% (112/939). In multivariable logistic regression analysis, dominant side involvement (p<0.001), manual labor (p=0.002), diabetes (p<0.001), metabolic syndrome (p<0.001), retraction degree of Patte tendon (p<0.001), posterosuperior RCT (p=0.010), and biceps tendon injury (p<0.001) were significantly associated with SBT. Conclusions: Metabolic syndrome is a potential risk factor for SBT, as are these factors: overuse activity, diabetes, posterosuperior RCT, increased retraction of posterosuperior rotator cuff tendon, and biceps tendon injury.

  • Background: Accessory infraorbital foramen (AIOF) can change the normal course of emerging branches of the infraorbital nerve and blood vessels exiting the infraorbital foramen (IOF). This study aimed to examine the AIOF, number of foramina, and their position in relation to IOF using cone-beam computed tomography (CBCT). Methods: We performed a retrospective CBCT assessment of hospital records between January 2018 and August 2022. The CBCT of 507 patients were examined to extract information on the prevalence, number, position, linear distance from the IOF, and diameter of AIOF in relation to demographic factors. Descriptive statistics were used to evaluate the prevalence of AIOF. Mean and standard deviation were used to calculate the linear distance and diameter of the AIOF, respectively. The AIOFs, its distribution, and number were compared between sexes and sides using the chi-square test. The independent t-test and Mann-Mann-Whitney test were used to compare the mean difference between the sexes and sides. Statistical significance was set at P < 0.05. Results: In this current study, the prevalence of AIOF was 7.1% (36 of the 507 patients). Additionally, the current study examined the number of foramina using a single foramen on each side and double foramina located bilaterally at a distance from the AIOF to the IOF. The mean AIOF diameter was also studied, and the AIOF position with respect to the IOF on CBCT was superomedial or inferomedial. There were no statistically significant associations between any of the parameters assessed in this study when comparing sex and sides. Conclusions: A greater number of patients with AIOF presented with a single foramen and unilateral occurrence, without a statistically significant difference. The AIOF was most commonly located superomedial to the IOF.

  • The studys purpose were the effects of work posture, treatment method and prevention for Video Display Terminal(VDT) workers in Work-Related Musculoskeletal Disorders(WMSD) The results were as follows: 1. The prevalence rate of VDT workers in WMSD was $20{\sim}40%$. The complaint was mostly shoulder, neck, and hack area pain 2. VDT worker used to forward flexed posture and then affect of increase of muscle fatigue and pain 3. When exercise therapy PT and ADL training, were used workers decrease in pain, muscle strength, balance training, endurance strength and relief of psychiatricIn conclusion, VDT worker need good health and posture to rest and exercise with time space and treatment. It is best to prevent WMSD. VDT workers need health management by itself and systemic rehabilitation program by speciality therapist.

Occupational Diseases and Injuries among Korean Nurses (한국 간호사의 직업관련성 건강문제)

  • Purpose: This study was done to learn the status of occupational injuries and illnesses among Korean nurses. Methods: Thirty four articles published between 1990 and 2007 were selected using keywords as nurse and back pain, musculoskeletal symptom, violence, needle stick injuries and latex allergy from the database. The 609 RN cases among the workers' compensation data from 2005 to 2007 were analyzed by descriptive statistics. Results: Based on the analysis of the articles, the most frequent work-related illness and injuries were the musculoskeletal symptoms including back pain. The prevalence of back pain was ranged from 36.5% to 98.5%. Tuberculosis cases were the most frequent and the musculoskeletal cases were the second rank in the workers' compensation data. In the case of violence, there has been an increasing tendency since 2000 in the articles and workers' compensation data. The relatively high percentage of compensation cases occurred among the ones working in small institutions and with less than one year experience. Conclusion: The further research on the nurses' work-related illness and injuries needs to consider the measurement tool selection and longitudinal design. The various types of contribution to the occupational health in nursing is suggested to improve the nurses' health and safety.

Clinical Characteristics of Pediatric Constipation in South Jordan

  • Purpose: Constipation is a common pediatric problem worldwide. This study aims to describe the clinical characteristics of pediatric constipation in south Jordan according to gender and age group. Methods: All patients with constipation managed at our pediatric gastroenterology service between September 2009 and December 2012 were included. Hospital charts were reviewed. Demographic data, clinical characteristics, and final diagnosis were recorded. Data were analyzed according to gender and the following age groups: infants, pre-school, school age, and adolescents. Results: During the study period, 126 patients were enrolled. The number (percentage) of patients according to age were the following infants: 43 (34.1%), pre-school: 55 (43.7%), school age: 25 (19.8%), and adolescents: 3 (2.4%). Males made up 54.8% of the study population. There were no statistical gender differences in any age group. The most common symptom in all age groups was dry, hard stool. Infrequent defecation was found in almost one-half of the patients. Fecal incontinence was more common in school-aged children compared to pre-school-aged children and adolescents. Abdominal pain was seen in almost 40% of the constipated children. Abdominal pain was more prevalent in girls and older children. Fecal mass in the rectum was the most common physical finding, with constipated boys exhibiting higher rates. Functional constipation was the most common etiology. Conclusion: Clinical characteristics of constipation in children vary according to age group and gender. Older children had less frequent bowel motions, a longer duration of symptoms, and a higher prevalence of long-standing constipation compilations (fecal incontinence and abdominal pain).
