Direct Immunofluorescence in Clinically Diagnosed Oral Lichen Planus

  • Purpose: Oral lichen planus (OLP) is relatively common mucosal disease in clinical dentistry. OLP is intractable and regarded having malignant potential. Until now, there is some debate on how far OLP can be malignant, and which characteristics can be a risk factor for malignant transformation. Clinician need to know some differences between OLP and lesions similar to OLP to manage properly and suppose prognosis correctly. Therefore, the aim of this study was to divide clinical OLP into two groups and to compare the results of direct immunofluorescence (DIF) between two groups. Methods: This study was conducted on outpatients who visited at the department of Oral Medicine in Chonbuk National University Hospital from January 2007 to November 2015. Patients with DIF result were retrospectively reviewed. The selected patients were classified 'clinical typical of OLP' (CTO) or 'clinical compatible with OLP' (CCW) by modified World Health Organization diagnostic criteria of OLP and oral lichenoid lesion. Results: DIF were classified by deposition intensity or pattern of anti-human antibody and fibrinogen. The classification of fluorescence pattern in each specimen was graded as positive, possibly positive or negative. Conclusions: Both CTO and CCW had positive and possibly positive pattern. Prevalence of positive pattern was 68.8% in CTO and 52.6% in CCW and that of possibly positive pattern was 9.4% in CTO and 5.3% in CCW. Prevalence of negative was 21.8% in CTO and 42.1% in CCW.

Prevalence of Low Back Symptom and Impact of Job Stress among Working Women as Clinical Nurses in University Hospitals

  • Purpose: This study was aimed to investigate the impact of job stress on low back symptoms among Clinical nurses (CNs) in university hospital. Methods: A total of 322 CNs employed in a hospital in Seoul were interviewed by a well-trained interviewer using the structured questionnaire. Data collected for this study includes demographics, social and work characteristics, low back symptoms, and job stress. To test the impact of job stress on low back symptoms, we used multiple logistic regression analysis. Results: The prevalence of low back symptoms was 25.8% in this study. Low back symptoms differed significantly by factors, such as physical work burden, past history of injury and work duration. Also low back symptoms differed significantly by organizational system among independent variables of job stress. In a multiple logistic regression analysis, the odds ratio of organizational system to low back symptoms was 2.07 after an adjustment. Conclusion: This study showed that organizational system among job stress was a significant variable in explaining low back symptoms. We suggest the improvement of organizational system category as a preventive strategy for low back pain in CNs working in hospital.

물리치료사의 직무관련 근골격계 통증의 유병률과 발생요인에 관한 연구 (The Study of the Prevalence and Causes of Occurrence on Work-Related Musculoskeletal Disorders(WMSDs) in Physical Therapist)

  • Purpose : The purpose of the present study was to describe the prevalence and causes of occurrence on work-related musculoskeletal disorders(WMSDs) in physical therapist. Methods : Self-administered questionnaires were distributed by mail to 100 physical therapists in Daegu city. 85 of the questionnaires were returned and 80 of them except 5 unclearly answered ones were analyzed. Results : They felt the pain at shoulders(80%), lower back(74%), wrists(71%), neck(70%), elbow(48%), finger(40%). The major causes of the pain were "repeating the same job constantly(93%)," and "bending wrists too much and doing things that require excessive strength(90%)." It was significant different more Exercise therapist than modality therapist. Conclusion : We suggest that physical therapist for exercise therapy need the development and applies of various program for prevention of work-related musculoskeletal disorders.

노인 요통에 관련된 요인조사 (A Survey on Factor Related to Rate of Low Back Pain in Eldery Person)

  • This study was conducted to fine out factors related with the complains low back pain in eldery persons. The Questionnaires was done for 273 eldery persons who live in Pusan, Anyang, Ansan region from May 1996 ro April 1997. The results were as follows : 1. The prevalence rate were 49.5% and among them 20.74% were male was while 68.12% which shows high prevalance of female 2. There were no statistical significance except sex difference(P>0.05) 3. There statistical different with some factor fators which includes general health condition, the frequency of treatment, sleeping bed style, condition for future life (P<0.05). But showed no statistical significance with the other factors. 4. When compared between rural and urban area, there were some statistical difference among some factor such as ; where to treat, moring exercise frequency, walking duration, sleeping bed style, agricultural work involement, prepartory condition for future life.(P<0.05)

Considerations in the Diagnosis and Treatment of Temporomandibular Disorders in Children and Adolescents: A Review

  • Temporomandibular disorders (TMDs) are a group of musculoskeletal disorders that encompass symptoms caused by abnormalities of the craniofacial structures of the temporomandibular joint (TMJ), muscles involved in the masticatory system, and other related tissues or structures. Although TMDs can occur at any age, research on the prevalence, epidemiology, and treatment strategies of TMDs has been conducted in all age groups, but primarily in adults. Unlike adults, children and adolescents are in a period of cognitive and physical development. Because of this growth potential, children respond better to TMD treatment than adults do. However, clinicians must remember that chronic pain and growth abnormalities can occur if the patient's symptoms and signs are not accurately diagnosed and treated. This article reviews the growth and development of the craniofacial region, including the TMJ, and discusses considerations when diagnosing and treating TMDs in children and adolescents.

Orofacial Manifestations of Hyperparathyroidism: A Dental Perspective

  • Hyperparathyroidism (HPT) is a significant condition marked by the overproduction of parathyroid hormones, affecting both systemic health and orofacial regions. Predominantly, secondary HPT associated with chronic kidney disease (CKD) is critical because of its link to widespread conditions such as diabetes and hypertension. This short article highlights the vital role of dental professionals in identifying HPT through panoramic radiography, which can reveal critical orofacial signs such as brown tumors, altered dental development, and specific bone changes. With the CKD prevalence expected to increase alongside an aging population, the importance of early detection of HPT and its manifestations in dental settings cannot be overstated. Dental practitioners play a crucial role in the early detection of HPT, emphasizing the importance of being knowledgeable about its orofacial manifestations.

체성분 분석 변수와 요부 등속성 근력 측정을 근거로 한 비만과 요통의 관계 (The Study on Relation of Obesity and Low Back Pain Based on Body Composition Using Segmental Bioelectrical Impedance Analysis and Isokinetic Trunk Muscle Strength)

  • Objectives : In order to investigate the relation of body composition analysis and isokinetic trunk muscle strength for the screening test of low back pain. Methods : This study was carried out with the data from comprehensive medical testing. 75 subject aged 20-59 performed the segmental bioelectrical impedance analysis, isokinetic trunk muscle strength test and questionnaire. Then we analyzed the relationship of data. Results : Low back pain(LBP) prevalence in high obesity index(Body mass index(BMI), Percentage of Body Fat(PBF), Waist Hip Ratio(WHR)) group was higher than LBP prevalence in normal obesity index group(p<0.001). In LBP group, Extension Peak Torque(Ext.PT), Extension Peak Torque per Body Weight(Ext.PT/BW) were significantly lower than Non-LBP group(p<0.001). And 90% of LBP group indicated abnormal Extension-Flexion Ratio(E/F ratio)(1.0 < Normal E/F ratio <1.6). When it comes to analyze relation between obesity index and muscle strength, Ext.PT/BW was significantly decreased according to PBF, WHR score. And correlation coefficient in Flex.PT, Flex.PT/BW, Ext.PT, Ext.PT/BW and PBF showed decreasing function. Conclusions : Results from this investigation showed positive correlation between obesity and LBP prevalence. Decreased muscle strength and inbalanced E/F ratio were shown in LBP. Trunk muscle strength was changed according to body mass composition parameters. This results are expected to contribute to prevent and diagnose LBP by application the clinical index of body composition analysis.

암 환자의 통증 외 증상의 빈도 및 고통 지수 조사 - "표준형 성인 암성 통증평가 도구"개발을 위해 - (The Frequency and Distress Score of Symptom of Cancer Patients - for the development of Korean Cancer Pain Assessment Tool (K-CPAT))

  • 목적 : 본 연구는 한국의 암 환자에서 나타나는 증상 발병율과 고통과의 관계를 파악하여 암환자의 통증조절을 위한 초기 평가 도구를 개발하기 위해서 실시하였다. 방법 : 2003년 1월 6일부터 20일 사이에 국내 5개 종합병원에 내원한 암 환자 1%명을 대상으로 하였다. 이들 환자들은 MSAS (Memorial symptom assessment scale)을 이용하여 나타난 증상에 대해 기록하였다. 결과 : 가장 흔한 증상은 무기력(82.1%), 구갈(73.5%), 통증(73.5%) 등이었고 흔한 증상 24가지 중에서 고통 지수가 높은 증상은 구갈(2.11), 가려움증(2.07), 통증(2.03) 등이었다. 통증과 정신 심리학적 증상을 제외하고 빈도와 고통 지수가 높았던 10가지 증상을 평가 항목으로 선정하였다. 결론 : 이들 10가지 증상은 한국의 암 환자에서 발병율과 고통 지수가 높은 증상들이다. 따라서 이들 증상들에 대한 평가가 암 환자에서 통증 조절을 위한 통증 초기 평가 도구를 개발하는데 도움이 될 것으로 생각된다.

Age Differences in Signs and Symptoms of Patients with Temporomandibular Disorders

  • Purpose: The aims of this study were to evaluate the differences in subjective symptoms, clinical characteristics, distribution according to Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) subgroup, psychological profile of TMD patients, and to identify the prevalence and trend according to age. Methods: A total of 1,052 patients (261 men and 791 women; mean age, $34.40{\pm}15.73$ years) who visited the Orofacial Pain Clinic of the Department of Oral Medicine, Seoul National University Dental Hospital complaining of TMD symptoms of were evaluated. All patients were questioned for medical history, clinical symptoms and contributing factors. Clinical examination and patient grouping based on RDC/TMD was conducted. Radiographies were taken. The Korean version of RDC/TMD axis II and Symptom Checklist-90-Revision (SCL-90-R) were administered to evaluate pain-related disability level and psychological status of the patients. Results: Prevalence peaked in the 20-year-old age group. There were more women than men in all groups. The highest T-score among SCL-90-R dimensions was somatization in each group, except for teenagers who showed the highest T-score in interpersonal sensitivity. The 30-year-old age group showed the highest distribution of high disability based on the graded chronic pain scale. Age was positively associated with pain intensity (r=0.100), number of positive muscles on palpation (r=0.137) and negatively associated with maximum mouth opening (r=-0.168). Conclusions: Subjective symptoms and clinical characteristics of TMD patients show distinct tendencies according to different age groups. Treatment should be customized and personalized according to age for efficient symptom resolution and patient satisfaction.

Crowned dens syndrome as a rare cause of anterior neck pain after transurethral resection of the prostate: a case report

  • We describe the case of a 79-year-old man who presented with progressive aggravation of severe axial neck pain and fever 3 days after transurethral resection of the prostate (TURP), despite maintaining neutral neck posture during surgery. Laboratory examination revealed markedly elevated C-reactive protein levels and erythrocyte sedimentation rates. Computed tomography revealed crown-like calcifications surrounding the odontoid process. We diagnosed crowned dens syndrome (CDS) as the cause of acute-onset neck pain after TURP. The patient was treated with nonsteroidal anti-inflammatory drugs for 5 days, and his symptoms resolved completely. CDS is a rare disease characterized by calcific deposits around the odontoid process with acute onset of severe neck pain and restricted motion. Evidence of inflammation on serological testing and fever are typical of CDS. However, the prevalence and pathophysiology of CDS remain unclear. We hypothesized that systemic inflammation after prostate surgery may have induced a local inflammatory response involving calcification around the odontoid process.
