The Evaluation for the Usefulness of Arthroscopic Miniopen Repair which Related with Large and Massive Sized Full Thickness Rotator Cuff Tear and Clinical Results (대범위 이상의 회전근 개 파열에 대한 관절경 감시하 소절개 봉합술의 임상 결과 및 수술의 유효성)

  • Purpose: To evaluate the usefulness of arthroscopic decompression and miniopen repair that was related with large and massive sized full thickness rotator cuff tear and assess clinical result. Materials and Methods: Twenthy-nine cases of miniopen repaired full thickness tear of rotator cuffs that arthroscopically decompressed were studied. From October 1998 to December 2004 we have analysed 29 repairs of large and massive sized FTRCT, the average age 44 ($32{\sim}71$) years old, mean follow-up was 34 ($12{\sim}84$) months. We analyzed the results statistically by paired t-test. Results: Postoperative VAS of pain improved average 7.0 to 1.7, UCLA score improved 13.7 to 31.9, ADL improved 11.3 to 25.3 respectively (all, P=0.000). Twenty five cases(82.8%) of the patients showed excellent & good results at the final follow-up. The satisfied rate was 26 cases(89.7%). Conclusions: Arthroscopic decompression and miniopen repair in large and massive sized full thickness rotator cuff tears are effective surgical methods.

A Clinical Analysis of Primary Hyperparathyroidism -A Report of II Cases- (원발성 부갑상선 기능항진증 -11예 보고-)

  • Primary hyperparathyroidism is still uncommomn disease in Korea. However the frequency of this disease has been slowly increased with routine measurement of serum calcium and increasing awareness of hyperparathyroidism in recent years. The diagnosis is established by a persistent elevation of serum calcium and parathyroid hormone and by clinical evaluation. This is a report of eleven patients with primary hyperparathyroidism treated with surgical operation during a period from 1983 to 1997 at Department of Hospital. Authors analyzed the cases to evaluate clinical characteristics and outcome of surgical treatment retrospectively. The result was as follows. 1) In sex distribution, female patients were eight and three were male, the age distribution ranged from 18 to 67 years. 2) The presenting clinical manifestations were renal and urinary stone in eight, bone pain or fracture in six, muscle weakness in four, neurologic symptoms in four, neck mass in three, hypertension in two, and G-I symptoms in one. 3) All patients showed hypercalcemia and elevated serum parathyroid hormone level. 4) Preoperative localization study was performed with computerized tomography, ultrasonography, MRI, arteriography and thyroid scaning. 5) The tumor locations were left lower in eight, left upper in one, right lower in one, and right upper location was one case. 6) Histopathologic findings disclosed adenoma in all cases. 7) All patients were treated by surgical excision and postoperatively transient hypocalcemia occurred in six patients, but no other complication was developed.

A Methodological Approach on the Evaluation of Patient Satisfaction: Focused on the Importance Performance Analysis(IPA) (환자만족도 평가에 대한 방법론적 접근: IPA기법을 중심으로)

  • The measurement and management of patient satisfaction has become one of the key issues in the last two decades. Hospitals must thoroughly understand the needs of their customers and design products and health services that meet and exceed their expectations. The importance-performance analysis(IPA) is a widely used analytical technique that yields strategies for managing customer satisfaction in a variety of applications. IP A is a two-dimensional grid based on customer-perceived importance of quality attributes and attribute performance. Depending on the interplay of these two dimensions, four strategies can be derived. The aim of this study is to develop the management strategies for improving patient satisfaction in university hospitals using the I-P analysis. The attributes on inpatient service quality in 4 university hospitals was investigated using the Martilla and James(l977)' s a mean adjusted I-P grid where the axes of the grid cross at the average rating point of all items. The patient satisfaction questionnaires were completed by 600 hospital inpatients. The main statistical methods are path analysis and IPA with SPSS 12.0 and AMOS 4.0 statistical softwares. The two attributes, physician and medical service, administrative staff kindness attributes position in first quadrant(Keep Up the Good domain). The nurse and nursing service attributes position in second quadrant(Possible Overkill domain). The two attributes, convenience of check-in service, facilities and physical environment position in third quadrant(Low Priority domain). Finally the quality of inpatient service(food etc.) attributes position in fourth quadrant(Concentrate Here domain). These findings show various implications on the development of strategies in university hospitals in the future. It was determined that quality of inpatient service(food etc.) need to concentrate more on investments. These investments include a taste, price, proper provision of food service and quick response of pain management. A low priority was given to investment in streamlining the check-in process of inpatient and hospital facilities and physical environment in the long run.

Clinical and Structural Outcomes of Arthroscopic Intraarticular Knotless Fixation for Upper Subscapularis Tendon Tears: A Preliminary Report

  • Background: A novel technique for the repair of tears of the upper subscapularis tendon-intraarticular knotless fixation-has been introduced recently. The purpose of this study was to evaluate the clinical and structural outcomes of arthroscopic intraarticular knotless fixation for the treatment of upper subscapularis tendon tears. Methods: We retrospectively analyzed the clinical and radiological outcomes of 27 patients who underwent arthroscopic intraarticular knotless fixation for upper subscapularis tendon tears. Finally, a total of 10 patients who could participate in at least a 6 month follow-up of magnetic resonance imaging evaluation and in a least 1-year follow-up on an outpatient basis were enrolled in our study. The mean age at the time of operation was 60.7 years, and the mean duration of follow-up was 14.7 months. Two patients had concomitant tears of the supraspinatus tendon and 8 patients had concomitant tears of the supraspinatus and the infraspinatus tendons. Results: The clinical and radiological outcomes improved after the patients had undertaken arthroscopic intraarticular knotless fixation. The mean visual analogue scale score for pain during motion improved from 6.7 preoperatively to 1.4 at the final follow-up (p<0.001). The mean Constant score improved from 59.3 preoperatively to 79.6 at the final follow-up, and the mean the University of California at Los Angeles score, from 21.7 to 30.2, respectively (p<0.001 and p<0.001). The upper subscapularis tendon tear was healed in every patient except one (90%), for the patient had suffered from a postoperative trauma that resulted in a retear. Conclusions: We found that arthroscopic intraarticular knotless fixation gives good clinical and structural outcomes for the repair of upper subscapularis tendon tears. Arthroscopic intraarticular knotless fixation provided such a reliable and efficient restoration of the subscapularis tendon footprint that we anticipate it will become a widely-used procedure for upper subscapularis tendon tears.

Abdominal Injury by Falls from a Height in Children (소아에서 추락사고에 의한 복부손상)

  • Falls from a height are the leading cause of injury and death among urban children. This study describes the incidence, clinical characteristics, and treatment results of children under 15 year of age who fell from a height of more than one meter and were admitted for abdominal injury. The medical records of 585 consecutive patients treated between January 1997 and December 2003 at Ewha Womans University Mokdong Hospital were analyzed retrospectively. The falling heights were 1 to 31.2 meters, and 28 patients(4.8 %) suffered from blunt abdominal trauma. The male to female ratio was 2.1: 1. The median age of the victims was 5.5 years, and the median height fallen was 3 meters. Fifteen patients (53.6 %) were injured during the summer and seventy-nine percent of the falls occurred between noon and 9 pm. Eighteen (64.3 %) of falls occurred in residential place and 19(67.8 %) of patients arrived at the emergency department within 30 minutes of the accident. Only 16 patients (57.1 %) complained of abdominal pain. Liver injuries were found in 12(42.9 %), spleen injuries in 5(17.9 %), kidney injuries 3(10.7 %), pancreatic injuries in 1(3.6 %) and nonspecific abdominal injuries in 9(32.1 %) cases. Increased SGOT and SGPT were found in 23(82.2 %) and 18(64.3 %) cases. Eleven patients (39.3 %) had associated head injuries. Limb injuries were present in 17.9% and thoracic injuries in 7.1%. Twenty-five patients (89.3 %) recovered without operation. The median length of hospital stay was 6 days (2 -20 days). Despite the absence of abdominal symptoms or shock, falls from a height in children may carry significant intra-abdominal organ injuries. The height falling could not predict the degree of the abdominal injury. For the evaluation of potential abdominal injuries, CT scan should be utilized.

A Study on the Characteristics of Facial Shape in Adult Women by Sasang Constitution Using Hyungsang Classification (형상분류를 이용한 성인여성의 체질별 안면형태 특징에 관한 연구)

  • Objectives This study was aimed to analyze characteristics of facial shapes in adult women by sasang constitution using hyungsang classification. Methods Using a digital camera, we took a picture of 1,011 women who participated in clinical study on menstrual pain and acquired their 3D facial images with a face-only scanner. They filled out SSCQ-P(sasang constitution questionnaire for patient) for the diagnosis of sasang constitution. Based on the above photographs and 3D images, one of the hyungsang medicine specialist diagnosed according to five diagnostic criteria. The sasang constitution was diagnosed by referring to questionnaires and photographs. Frequency analysis was performed using the statistical analysis system version 9.4 and chi-square test was performed for validity evaluation. Results In taeeumin, the wide face shape(n=261, 74.36%) was much more than the narrow shape(n=90, 25.64%) and the convex face profile(n=164, 85.86%) was much more than the concave profile(n=27, 14.14%). Regardless of sasang constitution, angular face shape(n=501, 50%) was the most, followed by oval shape(n=317, 31.64%). Subjects with big ears(n=291, 29.19%) were the most, while big eyes(n=104, 10.43%) were the least. Subjects with eyes and nose tip upward(n=615, 78.05%) were the most, while eyes and nose tip downward(n=22, 2.79%) were the least. Conclusions Most Korean adult women have angular face, such as square or diamond, with slanted eyes and upturned nose. Taeeumin women have wide facial shape and convex profile.

The Review on the Study of Bee Venom in the Journals of Korean Medicine (국내 봉독 관련 연구에 대한 고찰)

  • Objectives : The focus of the analysis was laid on changes in research pertaining to bee venom in regards to time progression. Methods : We collected 365 articles on Bee venom study result from OASIS system using the Keyword 'bee venom, apitoxin, apitherapy, bee sting'. We figured out number and percentage of theses according to year, study method, journal, subject. Results : Bee venom papers published in the journal of korean medicine from 1976. The classification of papers associated with bee venom, clinical studies outnumbered the other study types by a ratio of 1.3 to 1, followed by 138 for experimental papers, and 22 for literature studies. Proportion of Experimental Papers Classified According to the Theme, 16 for pain-killing, 14 papers concentrated on apoptosis anticancer, 13 for anti inflammatory, 11 for arthritis, and other disorders were followed. Type analysis of papers associated with bee venom in clinical trials, lumbar disorders comprised 38 out of 205 papers, 35 papers concentrated on upper limb disorders, 34 papers concentrated on systemic disease, followed by the effect on body. Conclusions : Bee venom is a treatment method based on the unique theory of Korean traditional medicine. Its effort and academical approach on bee venom are expected to receive positive evaluation through numerous research works.

Intranasal Phototherapy for Allergic Rhinitis : a systematic review (알레르기 비염의 비강 내 광 치료 : 체계적 문헌고찰)

  • Objectives : We investigated the effectiveness and safety of intranasal phototherapy for allergic rhinitis (AR). Methods : We searched 8 electronic databases (PubMed, Cochrane Library, CNKI, CiNii Articles, OASIS, NDSL, KISS, KMbase) to identify randomized controlled trials (RCTs) that reported the use of intranasal phototherapy for AR from their inception until May 30, 2020. Two investigators independently searched, collected, and screened the RCTs. We performed data extraction and evaluation for risk of bias using the Cochrane risk-of-bias tool. Results : This study included 12 RCTs; six studies compared intranasal and sham phototherapy, of which four studies reported a significant inter group difference and two studies reported a significant difference partially. No significant changes in symptoms were observed between the phototherapy and conventional therapy groups. The phototherapy and concurrent acupuncture treatment group showed a significantly higher effectiveness rate compared with the group that received only acupuncture. Both the phototherapy and laser acupuncture group showed significant improvement in the symptom severity scale scores. Six studies reported mild adverse effects, such as dryness and nasal pain in the intranasal phototherapy group; however, no severe adverse effects were reported. Conclusions : This study confirmed the safety and effectiveness of intranasal phototherapy for symptom relief and improved quality of life in patients with AR. However, further studies are needed on this topic in order to demonstrate it clearly.

Ergonomic Evaluation of Workload in Imbalanced Lower Limbs Postures

  • Objective: The purpose of this study is to compare the workload level at each lower limbs posture and suggest the ergonomic workstation guideline for working period by evaluating the imbalanced lower limbs postures from the physiological and psychophysical points of view. Background: Many workers like welders are working in various imbalanced lower limbs postures either due to the narrow working conditions or other environmental conditions. Method: Ten male subjects participated in this experiment. Subjects were asked to maintain 3 different lower limbs postures(standing, squatting and bending) with 3 different working conditions(balanced floor with no scaffold, imbalanced floor with 10cm height of scaffold, and imbalanced floor with 20cm height of scaffold). EMG data for the 4 muscle groups(Retus Femoris, Vastus Lateralis, Tibialis Anterior, Gastrocnemius) from each lower limbs posture were collected for 20 seconds every 2 minutes during the 8 minutes sustaining task. Subjects were also asked to report their discomfort ratings of body parts such as waist, upper legs, lower legs, and ankle. Results: The ANOVA results showed that the EMG root mean square(RMS) values and the discomfort ratings(CR-10 Rating Scale) were significantly affected by lower limbs postures and working time(p<0.05). The correlation was analyzed between the EMG data and the discomfort ratings. Also, prediction models for the discomfort rating for each posture were developed using physical condition, working time, and scaffold height. Conclusion: We strongly recommend that one should not work more than 6 minutes in a standing or squatting postures and should not work more than 4 minutes in a bending posture. Application: The results of this study could be used to design and assess working environments and methods. Furthermore, these results could be used to suggest ergonomic guidelines for the lower limbs postures such as squatting and bending in the working fields in order to prevent fatigue and pain in the lower limbs body.

Participation of IL-1β in temporomandibular nociception in rats with CFA-induced inflammation

  • The aim of the present study was to develop an animal model for evaluation of temporomandibular (TMJ) nociception under TMJ inflammation. We also investigated the participation of $IL-1{\beta}$ in inflammation-induced TMJ nociception. Experiments were carried out using male Sprague-Dawley rats. Intra-articular injection of 3% formalin was administered to evaluate hyperalgesia 3 days after CFA injection. Intra-articular injection of 3% formalin did not produce nociceptive behavior in normal rats. Although intra-articular injection of 3 doses of CFA produced TMJ inflammation, only 1:3 diluted CFA produced hyperalgesia when formalin was injected intra-articularly 3 days after CFA injection. Co-administration of IL-1 receptor inhibitor with formalin into the TMJ cavity 3 days after CFA injection was performed. Co-administration of IL-1 receptor inhibitor significantly inhibited formalin-induced hyperalgesia in rats with CFA-induced TMJ inflammation. These results suggested that intra-articular injection of formalin produced hyperalgesia under chronic TMJ inflammation. Moreover, $IL-1{\beta}$ plays an important role in TMJ hyperalgesia under chronic inflammation and blockade of $IL-1{\beta}$ is a potential therapeutic target for inflammatory TMJ pain.
