허리관절가동기법이 만성요통환자의 균형과 요통 기능장애 지수에 미치는 영향 (The Effect of Spine Mobilization Technique on Balance and the Low Back Pain Disability Index of Patients with Chronic Back Pain)

  • Purpose : The purpose of this study was to examine the effects of waist mobilization technique intervention for patients with chronic back pain on balance and the low back pain disability index. Methods : The subjects were 30 patients with chronic back pain. They were sampled and divided into a manual therapy group and a spinal decompression group. 15 subjects were randomly assigned to each group. Each training in this study participated for 6 weeks, 5 times a week, once a day, 15 minutes a day. Balance ability was measured with a balance analyzer, and the low back pain disability index was measured using the Oswestry Disability Index (ODI). Results : In the balance abilities, there was a significant difference in the manual therapy group compared to the spinal decompression group. In the low back pain disability index, there was a significant difference in the manual therapy group compared to the spinal decompression group. Conclusion : The analysis results of the effect of 6 weeks of waist mobilization technique intervention on the balance and low back pain disability index for patients with chronic back pain revealed that the manual therapy is more effective for static and dynamic balance ability and the low back pain disability index. In the future, we can promote independent life skills and expect a rapid recovery of patients with chronic back pain. Based on this study, further studies are needed on the effects of balance, the mechanical properties of muscle, and the low back pain disability index depending on various manual therapy techniques.

수면장애가 있는 성인의 통증, 목과 어깨의 기능장애수준, 두통 및 스마트폰 중독 간 상관성 연구 (Correlation between Neck and Shoulder Pain, Neck and Shoulder Disability, Headache and Smartphone Addiction in Adults with Sleep Disorders)

  • PURPOSE: To investigate the correlation between sleep quality, neck and shoulder pain, neck and shoulder disability, headache, and smartphone addiction in adults, as well as factors affecting sleep disorders in adults. METHODS: A total of 112 adults in their 20s were recruited for participation in this study. This study measured Pearson's correlation of sleep quality, neck and shoulder pain, neck and shoulder disability, headache and smartphone addiction. Logistic regression analysis (sleep disorder, without sleep disorder adults), was performed to analyze factors affecting sleep disorders. RESULTS: Sleep quality index was positively correlated with shoulder pain and disability index(SPADI)-pain score, SPADI-global score and smartphone addiction score. There was a significant difference in SPADI-pain score between the group with sleep disorders and the group without sleep disorders. Shoulder pain and disability were identified as factors affecting sleep disorders. The group with sleep disorders showed 1.088 times more shoulder pain than the group without sleep disorders, and the group with sleep disorders showed 1.006 times higher frequency of shoulder disorders than the group without sleep disorders. CONCLUSION: The results of the present study indicate that as sleep quality worsens, shoulder pain, disability, and smartphone addiction get worse. The group with sleep disorders had significantly increased shoulder pain compared to the group without sleep disorders. We also found that shoulder pain and disability were factors affecting sleep quality. Comparison between intervention method is warranted to better understand adults with sleep disorders.

우울, 신체화, 턱기능장애가 측두하악장애 환자의 만성통증척도에 미치는 영향 (Impacts of Depression, Somatization, and Jaw Disability on Graded Chronic Pain in TMD Patients)

  • 측두하악장애 연구진단기준 (Research Diagnostic Criteria for Temporomandibular Disorders; RDC/TMD)을 이용하여 측두하악장애 환자들에서 심리학적 상태 및 턱기능장애의 정도가 통증의 정도 및 지속기간, 통증과 관련된 일상생활 장애의 정도 및 기간, 만성통증척도에 미치는 영향 등을 알아보기 위하여 RDC/TMD axis I criteria에 따라 측두하악장애로 진단받은 평균연령 30.1$\pm$12.5 세인 93 명의 환자들(남성 17명, 여성 76명)을 대상으로 RDC/TMD axis II 병력 설문지를 작성하게 한 뒤, 우울증점수, 신체화장애점수, 턱기능장애점수와 만성통증척도의 통증 강도, 통증 지속기간, 통증과 연관된 장애 점수, 통증과 연관된 장애의 기간 및 만성통증척도 사이의 연관성을 분석하여 다음과 같은 결론을 얻었다. 1. 93명의 측두하악장애 환자들에서 만성통증척도의 경도의 장애를 보이는 군 (low disability group, grade I and II)은 42.0% 이었으며, 심도의 장애를 보이는 군 (high disability group, grade III and IV)은 51.5% 이었다. 2. 우울증점수는 통증 강도, 통증 지속기간, 통증과 연관된 장애 점수, 통증과 연관된 장애의 기간, 만성통증척도 모두에 통계적으로 유의한 영향을 미치지 않았다. 3. 신체화장애점수는 통증 강도 (p<0.01), 통증과 연관된 장애의 기간 (p<0.01), 만성통증척도 (p<0.01)에 유의한 영향을 미쳤으나, 통증 지속기간이나 통증과 연관된 장애 점수에는 통계적으로 유의한 영향을 미치지 않았다. 4. 턱기능장애점수는 통증 강도 (p<0.001), 통증과 연관된 장애의 기간 (p<0.01), 만성통증척도 (p<0.001)에 유의한 영향을 미쳤으나, 통증 지속기간이나 통증과 연관된 장애 점수에는 통계적으로 유의한 영향을 미치지 않았다. 측두하악장애환자에서 신체화장애와 턱기능장애는 환자가 느끼는 통증의 강도 및 장애의 정도와 밀접하게 관련되어 나타날 수 있으며, 환자가 호소하는 정서적 스트레스에 대한 포괄적인 이해와 기능장애에 대한 적절한 관리가 측두하악장애 환자의 성공적인 치료에 매우 중요하리라 생각된다.

대학생의 허리통증에 대한 SNAGS(Sustained Natural Apophyseal Glides)기법이 허리통증과 오스웨스트리 장애지수(KODI)에 미치는 영향 (Effect of SNAGS Technique on the Lower Back Pain Korean Oswestry Disability Index(KODI) and Pain in Lower Back Pain of University Students)

  • Background: This study investigated the effects of sustained natural apophyseal glides (SNAGS) technique on lower back pain and pain disability among university students with lower back pain. Methods: A total thirty 20~30 year old university students with lower back pain were divided into two groups. A lumbar flexibility exercise was applied to the control group (n=15). The SNAGS technique group (n=15) received L1~L4 segment mobilization using SNAGS technique. Intervention was implemented for 20 minutes, 3 times a week for 3 weeks. The SNAGS were performed using belt with flexion and extension, whereas lumbar flexibility exercise included lumbar flexion and extension. Lower back disability was measured using the Korean Oswestry disability index (KODI), and pain sensitivity was measured by pain pressure threshold (PPT). The groups were assessed for lower back pain disability and pain, before and after the intervention. Results: There were significant improvements in both the KODI and PPT of the SNAGS technique group (p<.05), while the lumbar flexibility exercise group showed no significant change (p>.05). In addition, the SNAGS technique group had a statistically significant difference in KODI and PPT compared to the lumbar flexibility exercise group (p<.05). Conclusion: SNAGS technique is more effective than lumbar spine joint flexibility exercise in the improvement of lower back pain disability and pain among university students with lower back pain.

Epidemiological Study on Temporomandibular Disorders Using Research Diagnostic Criteria for TMD (RDC/TMD)

  • Purpose : This epidemiological research was conducted to investigate the relationship between the groups of TMD and the behavioral, psychological, and physical symptoms through RDC/TMD. Subjects and Methods : The subjects of this research were the 286 patients who had visited Seoul National University Bundang Hospital; their common chief complaint was temporomandibular discomfort. The mean age of the patients was 32.9 from 11 to 85, and the number of men and women was 67 and 219, respectively. The patients were examined through clinical and radiological methods and diagnosed by 1 investigator. They were divided into 3 groups: myogenous group (group 1), arthrogenous group (group 2), and combined group (group 3). The behavioral, psychological, and physical symptoms were evaluated through questionnaires on RDC/TMD. Specific items were selected to calculate the graded chronic pain (characteristic pain intensity, disability points), jaw disability, depression, and non-specific physical symptoms (pain items included/excluded) in the questionnaire. One-way ANOVA, Kruskal-Wallis test, and chi-square test were applied as statistical methods. Results : As a result of classifying temporomandibular disorder in this study, the patients were distributed as follows: 9.1% of group 1, 79.7% of group 2, and 11.2% of group 3. In the analysis of graded chronic pain (characteristic pain intensity, disability points), jaw disability, and non-specific physical symptoms (pain items included/excluded), group 3 had the highest score, and the difference was significant (p<0.001). Moreover, the depression score of group 3 was significantly higher than groups 1 and 2 (p<0.05). Note that that the second order of jaw disability score was group 2, on the other hand, those of the other groups were group 1. Conclusion : Myofascial pain could be assumed to be related closely to the behavioral, psychological, and physical symptoms except jaw disability compared to joint pain through RDC/TMD.

요부안정화운동과 병행한 둔근운동이 요부불안정성을 가진 만성요통환자의 요부안정성에 미치는 영향 (The Effects of Gluteal Muscle Exercises Combined Lumbar Stabilization on Lumbar Stability in Chronic Low Back Pain Patients with Lumbar Instability)

  • PURPOSE: The purpose of this study was to determine if the program focusing on gluteal muscle exercise combined lumbar stabilization had an effects on pain, functional disability and lumbar instability in chronic low back pain patients who had lumbar instability. METHODS: Thirty four patients were recruited in this study. The participants were randomly allocated stabilization exercise group (SEG) (n=17) and gluteal muscle exercise group (GEG) (n=17). The gluteal muscle exercise group conducted gluteal muscle exercises combined lumbar stabilization and stabilization exercise group did only lumbar stabilization exercise for 30 minutes three times a week for six weeks. Pre-exercise assessment tools, visual analogue scale (VAS) and Korean version Oswestry Disability Index level (KODI) were used for pain and functional disability each. Also, these were scored by the tool of lumbar instability. The pain, functional disability levels, and lumbar instability after six weeks with this exercise were re-evaluated. RESULTS: First, SEG showed a significant decrease in the lumbar pain, functional disability levels and lumbar instability. Second, GEG showed a significant decrease in the lumbar pain, functional disability levels and lumbar instability. The third, GEG showed even more significant decrease in the lumbar pain levels, functional disability levels and lumbar instability than SEG. CONCLUSION: Gluteal muscle exercises combined lumbar stabilization are more effective than only lumbar stabilization exercise in the pain levels, functional disability levels and lumbar instability for the chronic low back pain patients with lumbar instability.

요통(low back pain) 환자의 통증강도와 Oswestry의 요통 장애 지수, Hendler의 10분 판별검사간 상관성에 관한 연구 (The correlation of Oswestry Disability Index and Hendler 10-Minute Screening Test in the patient of low back pain about pain strength)

  • This study is about the correlation of pain strength between Oswestry Disability Index and Hendler 10-Minute Screening Test in the patient of low back pain. The subject were 66, in and out patient who had recieved physical therapy on low back pain in several hospitals at Seoul and Kyungkido. during July 5.-August 27. 1999 1. The average score of Oswestry Disability Index was $24.06{\pm}8.16$ and that of Hendler 10-Minute Screening Test was $24.82{\pm}4.31$. 2. The Pearson's R score between each questionnaires of Oswestry Disability Index and pain strength was .5692. p<0.0001. So it is very high. But, that between each questionnaires of Hendler 10-Minute Screening Test and pain strength was not significant.. 2261 p<0.05 3. The Pearson's R score between Pain strength of Oswestry and person care was .3391, sex life was .3756, social life was .4637 (p<0.005) and that between Pain factor of Hendler, pain area of Hendler and sleeping(4358), sex(.6198), position change(2767), (p<0.005). So it is significant. 4. The categories where we could see the correlation between Pain strenth of Oswestry and Hendler each questionnaires were sleeping(.3222), sex(5524), position change(4291). (p<0.005).

Relationship of Pain, Disability and Quality of Life in Individual with Neck and Shoulder Pain

  • Purpose: This study was to investigate the correlation between pain, disability and quality of life among adolescents and office workers with neck and shoulder pain. Methods: Twenty-four subjects (mean age=24.92 years, SD=3.94, range=20-37) with neck and shoulder pain participated in this study. The outcome measures of the study were visual analogue scale (VAS), neck disability index (NDI), shoulder pain and disability index (SPADI), short form-36 (SF-36) for neck and shoulder pain and disability, quality of life. Pearson's correlation and Speaman's rank correlation were used to measure the association between VAS and NDI, SPADI, SF-36. Results: The VAS showed a positive correlation between weak to moderate with the NDI and SPADI, but no significantly correlated (${\rho}=0.34$, ${\rho}=0.25$). The moderate positive correlation and significant correlation were observed between NDI and SPADI (${\rho}=0.43$, p<0.05). The PCS of SF-36 was significantly negative correlated with the VAS (${\rho}=-0.24$), NDI and SPADI (${\rho}=-0.63$, ${\rho}=-0.59$, p<0.05). Conclusion: The pain and disability of neck and shoulder has closely relevance. And, the pain and disability of neck and shoulder has a negative impact on the quality of life.

만성 요통환자에 대한 PNF와 체간운동프로그램이 통증, 기능장애 및 균형에 미치는 효과 (The Effects of PNF Technique Versus Trunk Exercise Program on the Pain, Disability and Balance in Chronic LBP Patients)

  • 본 연구는 만성요통환자 30명을 대상으로 PNF와 체간 운동프로그램을 적용한 다음 환자들의 통증, 기능장애 그리고 정적균형과 동적균형에 미치는 효과에 대해 알아보기 위해 실시하였다. 각 군의 운동효과를 측정하기 위해 모든 대상자는 통증과 함께 Oswestry 장애지수와 외다리기립검사를 통해 정적균형을 측정하였고, Balance 측정기를 이용하여 동적균형을 측정하였다. 그 결과 통증, Oswestry 장애지수, 외다리 기립검사 및 동적균형 모두 체간운동군에 비해 PNF 군이 유의한 차이가 있었다(p<0.05). 따라서 이번 연구결과를 통해 알 수 있는 것은 PNF의 내려치기와 들어올리기 패턴이 만성 요통환자에게 효과적이라는 것을 알 수 있다.

Effects of Interferential Current Treatment on Pain, Disability, and Balance in Patients with Chronic Low Back Pain: A Randomized Controlled Study

  • PURPOSE: This study investigated the efficacy of interferential current (IFC) treatment on the improvement of pain, disability, and balance in patients with chronic nonspecific low back pain. METHODS: A double-blind randomized clinical trial was conducted with 40 patients with chronic nonspecific low back pain. The patients were randomly allocated into two groups: the IFC treatment group (n = 20) and the placebo treatment group (n = 20). The IFC group received 30 minutes of IFC treatment on the lumbar region, while the placebo group received IFC treatment without real electrical stimulation. The intervention was administered five days a week for two weeks. RESULTS: The primary outcomes of resting pain and pain during functional movement were measured by a visual analogue scale. The secondary measurements included the Oswestry disability index (ODI) for low back pain and postural sway. The measurements were performed before and after the two-week intervention period. Compared to the placebo treatment group, the IFC treatment group showed significantly greater improvement in pain during anterior trunk flexion in the standing position (p = .029), ODI (p = .039), and postural sway when subjects stood with their eyes closed (p = .010) at the end of the intervention. CONCLUSION: Our findings show that IFC treatment can improve pain, disability, and postural sway, thus, highlighting the benefits of somatosensory stimulation from IFC.
