Background: The aim of this study was to investigate the association between neuroticism, pain catastrophizing, and experimentally induced pain threshold and pain tolerance in a healthy adult sample from two regions of the country of Croatia: the island of Korcula and city of Split. Methods: A total of 1,322 participants were enrolled from the Island of Korcula (n = 824) and the city of Split (n = 498). Participants completed a self-reported personality measure Eysenck Personality Questionnaire (EPQ) and pain catastrophizing questionnaire Pain Catastrophizing Scale (PCS), followed by a mechanical pain pressure threshold and tolerance test. We have explored the mediating role of catastrophizing in the relationship between neuroticism and pain intensity. Results: The results showed that pain catastrophizing partially mediated the relationship between neuroticism and pain intensity, suggesting the importance of pain catastrophizing in increasing vulnerability to pain. The results also indicated gender-related differences, marked by the higher pain threshold and tolerance in men. Conclusions: This study adds to the understanding of the complex interplay between personality and pain, by providing a better understanding of such mechanisms in healthy adults.
Objectives : This study was designed to estimate the effects of Spiral Taping theraphy on low back pain or neck pain patients by using pressure pain threshold, visual analogue scale(VAS), and range of motion(ROM). Methods : The patients were allocated into two groups by randomisation. Spiral Taping theraphy group consisted of 11 patients and acupuncture theraphy group consisted of 10 patients. The degree of improvement of neck pain was evaluated by pressure pain threshold, VAS, and ROM before treatment and after treatment. The patients were commonly treated with acupuncture theraphy. After acupuncture Spiral Taping theraphy was peformed in Spiral Taping theraphy group. Results : Pressure pain threshold and VAS were significantly different between two groups. In Spiral Taping theraphy group the pressure pain threshold of the all points was significantly increased and VAS was significantly decreased than control group. But there was no significant difference between the two groups in ROM. Conclusion : The effectiveness of Spiral Taping theraphy on low back pain and neck pain patients was shown through pressure algometer and VAS. These imply that Spiral Taping theraphy may be useful for on low back pain or neck pain. Further study is needed about Spiral Taping theraphy.
Purphose. This present study examines the effect of brief, intense transcutaneous electrical nerve stimulation(BTENS) on sensory nerve conduction, electrical pain threshold, and two-point discrimination measured at the superficial radial nevre distribution in 20 healthy subjects. Subjects. Twenty volunteercs, (10 females and 10 males(age range : 20-38 years : $mean{\pm}SD\;:\;27.00{\pm}5.12$), only subjects without prior traumatological and pathological were eligible to participated in this study. Methods. Nerve conduction were determined for the right superficial radial nerve. Electrical pain threshold were determined for the right wrist ipsilateral to the site of BTENS. Small disc electrodes were attached to the surface of the skin stradding the end of the radius. Square wave electrical pulses were delivered from an isolated stimulator through a constant current device at a frequency of 2 Hz(5 ms pulse width). Two-point discrimination, measured on the sensory distribution of superficial radial nerve. BTENS was delivered using a Max-SD( Medical design co.) portable battery powered stimulator. A cicular Ag/AgCl electrode in contact with hypertonic saline gel was attached to the lateral(radial side) surface of the forearm. Results. No significant effects were observed between stimulation methods in the prestimulation cycle(multi-way ANOVA repeated measures : distal latency ; F1.14=0.332. amplitude ; F 0.80=0.445, pain threshold ; F0.06=0.940.2 point discrimination ; F1.50=0.236). Highly significant effects were observed time with the pretreatment and 6 posttreatment cycles(p<0.01). Mighty significants differences in nerve conduction and pain threshold were found using un multi-way ANOVA repeated measures among stimulation methods for each cycles(p<0.01). Conclusion and Discussion The authors concludes that both nerve conduction and pain threshold changes are associated with therapy (stimulation) level of BTENS.
Objectives : The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on anterior talofibular ligament injury induced by acute ankle sprain. Methods : From October 31, 2008 to July 21, 2009, the 11 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental medicine hospital, Dae-Jeon university with acute ankle sprain were performed heating-conduction acupuncture therapy on anterior talofibular ligament injury. To evaluate the efficiency of heating-conduction acupuncture therapy, pain threshold with Pressure algometer and visual analogue scale(VAS) were applied before treatment and after 1st, 2nd treatment. Results : 1. The pain threshold score and the VAS score showed statistical significant improvement after 1st treatment. 2. The pain threshold score and the VAS score showed improvement but had no statistical significance after 2nd treatment. 3. The difference between left and right were not statistical significant in pain threshold score and VAS score. Conclusions : Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with anterior talofibular ligament injury induced by acute ankle sprain.
연구목적 본 연구는 치매 군과 비치매 군의 통증 역치 및 통증 경험의 특성을 비교 하고자 한다. 방 법 2010년 강원 조기치매 검진사업의 일환으로 시행된 것으로, 연구 대상은 지역사회 거주중인 65세 이상 노인 8,302명이었다. 이 중, 한국판 간이정신상태검사(MMSE-KC) 점수가 저하 된 1259명을 선별했고, 이중 365명이 한국형 치매 진단평가 도구(CERAD-K)를 이용한 정밀검진을 받았다. 검사 결과 및 전문의의 판단 하에 정상, 경도인지장애, 치매 군으로 진단 후, 최종 비치매 군 90명, 치매 군 57명이(경도-중등도 알츠하이머형 치매) 분석되었다. 압통각계(Pressure algometer)를 이용하여 실험적으로 통증 역치를 측정했고, 자가보고식 검사인 Brief pain inventory(BPI)를 이용하여 통증 경험(통증 심도, 통증 방해도)을 조사했다. 집단 간 차이 분석은 Pearson Chi-Square와, 혼란변인(성별, 나이, 교육수준, GDS, 당뇨)을 보정한 ANCOVA검정을 시행했다. 결 과 인구학적 특성상 치매 환자 군에서 여성이 많고, 연령이 높고, 교육 연수가 낮았다. 통증 역치는 두 군에서 통계적으로 의미 있는 차이가 발견되지 않았다. BPI결과에서 비치매 군이 어깨 통증, 전체 통증 개수, '통상적 일'의 통증으로 인한 방해를 의미 있게 더 많이 호소했다(p<0.05). 그러나 두 군에서 통증 유병률 및 치료는 차이가 없었다. 결 론 치매 군은 비치매 군보다 통증을 더 적게 경험했다. 이는 치매 환자의 통증 역치는 보존되었으나, 내성이 증가한다는 선행 연구를 뒷받침 하는 증거로 여겨진다. 따라서, 치매 환자를 위한 적극적인 통증 사정 및 치료를 제안한다.
편광 근적외선 조사가 실험적 통증역치에 영향을 미치는지를 규명하기 위해서 건강한 성인의 합곡혈에 여러 단계의 출력량으로 편광 근적외선을 30초간 조사하고 시간 경과에 따라 실험적 통증역치를 측정하여 분석하였다. 실험 결과 출력량 0 %인 대조군과 10 % 및 20 %의 출력량에서는 실험적 통증역치의 유의한 변화가 없었다. 그러나 출력량 40 %군의 실험적 통증역치는 편광 근적외선 조사전 $3.85{\pm}0.51mA$에서 조사 직후 $4.77{\pm}0.87mA$, 조사 후 30분 $5.57{\pm}0.98mA$, 조사 후 1시간 $5.68{\pm}1.06mA$, 조사 후 2시간 $5.34{\pm}0.96mA$로 증가되었고, 출력량 80 %군에서도 실험적 통증역치가 조사전 $3.87{\pm}0.92mA$에서 조사 직후 $4.45{\pm}0.62mA$, 조사 후 30분 $4.91{\pm}0.51mA$, 조사 후 1시간 $4.93{\pm}0.62mA$, 조사 후 2시간 $5.55{\pm}1.11mA$로 증가하였다. 실험적 통증역치는 편광 근적외선 조사 직후부터 2시간까지 유의하게 증가하였다. 40 % 출력량과 80 % 출력량의 실험적 통증역치는 유의한 차이가 없었고, 출력량과 시간의 상호작용효과는 없었다. 건강인의 실험적 통증역치를 대상으로 한 본 연구 결과를 통증이 있는 환자에게 직접 적용하기는 어렵다고 생각하며 앞으로 환자를 대상으로한 연구가 수행되기를 기대한다.
Background: A correlation between a T-type voltage activated calcium channel (VACC) and pain mechanism has not yet been established. The purpose of this study is to find out the effect of ethosuximide and mibefradil, representative selective T-type VACC blockers on postoperative pain using an incisional pain model of rats. Methods: After performing a plantar incision, rats were stabilized on plastic mesh for 2 hours. Then, the rats were injected with ethosuximide or mibefradil, intraperitoneally and intrathecally. The level of withdrawal threshold to the von Frey filament near the incision site was determined and the dose response curves were obtained. Results: After an intraperitoneal ethosuximide or mibefradil injection, the dose-response curve showed a dose-dependent increase of the threshold in a withdrawal reaction. After an intrathecal injection of ethosuximide, the threshold of a withdrawal reaction to mechanical stimulation increased and the increase was dose-dependent. After an intrathecal injection of mibefradil, no change occurred in either the threshold of a withdrawal reaction to mechanical stimulation or a dose-response curve. Conclusions: The T-type VACC blockers in a rat model of postoperative pain showed the antihyperalgesic effect. This effect might be due to blockade of T-type VACC, which was distributed in the peripheral nociceptors or at the supraspinal level. Further studies of the effect of T-type VACC on a pain transmission mechanism at the spinal cord level would be needed.
Background: The treatment of pain in the iliotibial band friction syndrome has been difficult to determine, according to studies to date. However, recent studies have suggested that flexibility in the subacute stage of pain in the iliotibial band friction syndrome may help reduce pain. Objective: To investigate the immediate effect on pressure pain threshold and flexibility of the tensor fascia latae and iliotibial band by applying static and dynamic myofascial release foam rolling and self-stretching to adults with shortening iliotibial band. Design: Randomized controlled trial Methods: In this study, 50 subjects who were selected in advance as a randomized controlled trial were randomly allocated using a R Studio program. The included subjects were randomly allocated to three intervention groups. The static self-myofascial release 18 people, dynamic self-myofascial release group 16 people separated the self-stretching group 16 people and conducted a homogeneity check in advance. Before the start of the experiment, after of the experiment, 5 minutes after the end of the experiment, the pressure pain threshold and flexibility change for each part were measured. Results: The results of this study showed that the static self-myofascial release showed a significant difference in the pressure pain threshold in the tensor fascia latae and middle, lower part of the iliotibial band, compared with the other intervention groups (p<.05). In change of flexibility, the static self-myofascial release was significantly different than the other intervention groups (p<.05). Conclusion: The result of this study suggest that static self-myofascial release using foam roller may help to improve the pain and flexibility of the iliotibial band and to apply it as a more discerning intervention.
Objective: Patients with scoliosis complain of various symptoms such as muscle imbalance, dysfunction, back pain, abnormal posture and gait abnormality. The most basic treatment for scoliosis is to observe the progress based on conservative treatment. Therefore, in this case report, the effect of cytoskeletal manual therapy (CMT), a soft tissue mobilization technique, on pain intensity, muscle thickness, and pressure pain threshold (PPT) in a patient with scoliosis was investigated. Design: A case report Methods: A 25-year-old male diagnosed with scoliosis visited the Neuromusculoskeletal Science Laboratory with chronic back pain. In the laboratory, scoliosis was confirmed through the X-ray image used for his diagnosis, and it was confirmed again through Adam's forward bending test. Pain, pressure pain threshold and muscle thickness were measured to compare the immediate effects of CMT applied in the laboratory for 40 minutes. Treatments were visited two weeks after the first visit and outcome measures were assessed after a total of two visits. Results: After receiving CMT up to the second session, the pain intensity decreased by 4 points and the screening angle decreased by 15 degrees. Muscle thickness decreased in all but 10 mm on the dominant side of the thoracic spine. All of the PPTs increased, and the greatest increase was 3.1 lb on the dominant side of the thoracic spine. Conclusions: CMT showed positive improvement in pain during trunk flexion, spinal curvature, muscle imbalance, and pressure pain, which is considered as an ancillary treatment option for scoliosis management.
Objective : This study was performed to confirm the effects of acupuncture on myofascial pain syndrome(MPS) through the change of visual analogue scale(VAS) and pain pressure threshold(PPT) and the usefulness of pressure algometer on the evaluation of pain. Methods : We perfomed this study with 20 outpatients complaining of upper back pain. Before acupuncture therapy(AT), immediately after AT and 2-3 days after AT, we respectively checked visual analogue scale(VAS) and pain pressure threshold(PPT) through pressure algometer, with patients seated and relaxed. The PPT was checked at major trigger point of upper trapezius, levator scapulae, supraspinatus, infraspinatus, rhomboideus minor. and the patients were needled at the same points and maintained for 15 minutes. Results : VAS of immediately after AT was mild higher than that of before AT, but not significantly different. and VAS of 2-3 days after AT was significantly lower than before AT and immediately after AT. PPT of immediately after AT was lower than before PT, but not significantly different. PPT of 2-3 days after AT was significantly higher than that of before AT and immediately after AT. Also PPT was significantly correlated with VAS. Conclusion : PPT of omen was signicantly lower than that of men. and there was no significant difference by age. PPT was increased according to pain duration. Effectiveness of acupuncture on myofascial pain syndrome through PPT and VAS is showed at 2-3 days after AT rather than immediatly after AT. and pressure algometer is useful for the evaluation of Acupuncture therapy on myofascial pain syndrome.
