Mandibular Advancement Devices for Treating Snoring and Obstructive Sleep Apnea

  • Many therapeutic modalities including continuous positive airway pressure, surgery, and oral appliances are used to treat patients with sleep-disordered breathing. However, there are no definitive treatment modalities for individual patients due to various causes of sleep-disordered breathing. Clinicians should have select best options for individual patients and it is quite challenging process. Oral appliances attracted clinical attention for its convenience and safety. Several designs of oral appliances are introduces such as soft palate lifter, tongue retaining device, and various appliances which aimed to mandibular advancement. Among these oral appliances, mandibular advancement devices (MADs) are considered the most excellent based on their effectiveness and patient tolerance. Although MADs are not guarantee dramatic outcome and less consistent than continuous positive airway pressure, they offer several advantages over continuous positive airway pressure and surgical methods, including non-invasiveness, silence, portability, and tolerability, simplicity. Therefore, general dental practitioner who had passed sleep dental curriculum or coursework can treat the patients with sleep problems. This article reviews the history, clinical indications, suggested mechanism of actions, various positive effects and several side effects, factors predicting a favorable outcome, determining amounts of mandibular advancement, compliance and long-term efficacy of MADs use.

Condylar Resorption : Case Reports and Review (하악과두 흡수(Condylar Resorption)에 대한 고찰)

  • Condylar resroption, or condylysis can be defined as progressive alteration of condylar shape and decrease in mass. Although the cause is unknown, condylar resorption has been assocated with rheumatoid arthritis, systemic sclerosis, systemic erythematous, steroid usage, orthodontic treatment and orthognathic surgery. In most case, however, there is no identifiable precipitating event. Hence the term is idiopathic condylar resorption. With condylar resorption, the bone loss is resulting in a loss of posterior support in the involved condyle and the mandible can then shift. As a result, the most patients exhibit occlusal change, openbite, retrognathism, and a decrease in posterior face height. This article reports cases of condylar resorption. And the author will review factors capable of changes of condyle, pathogensis and management of conylar resorption.

Effects of Motivational Interviewing Intervention for Patients with Total Knee Replacement (슬관절 치환술 환자 대상 동기상담 적용 운동 프로그램의 효과)

  • Purpose: The study was conducted to examine the effects of motivational interviewing intervention on exercise performance and maintenance, exercise and pain self-efficacy, functional status, and quality of life in the patients with total knee replacement. Methods: This study used a nonequivalent control group non-synchronized design. A total of 100 patients with total knee replacement (33 patients in motivational interviewing group (MG), 33 patients in exercise group (EG), 34 patients in comparison group (CG)) were participated in this study. MG and EG had 7 scheduled sessions for 4 weeks. Data were analyzed with repeated measures ANOVAs by using IBM SPSS Statistics 20. Results: MG showed better exercise maintenance, higher exercise and pain self-efficacy, higher muscle strength, and mental component of quality of life than the other groups. MG and EG showed better knee flexion, less knee extension, longer walking distance for 6 minutes, and better functioning than CG. Conclusion: The results showed that motivational interviewing intervention is an effective intervention for elderly patients with total knee replacement. Future study examining long term effects of motivation interviewing intervention is necessary.

Assessment of the Thickness of the Roof of the Glenoid Fossa Using Cone Beam Computed Tomography in Orthognathic Surgery Patients: A Preliminary Study

  • Purpose: The aim of this study was to assess the change in thickness of the roof of the glenoid fossa (RGF) in patients undergoing orthognathic surgery using cone-beam computed tomography (CBCT) images. Methods: This retrospective study measured the thickness of the RGF in 19 patients (10 males, 9 females) who underwent orthognathic surgery at Chosun University Dental Hospital. The thickness of the RGF was measured perpendicularly between the 'glenoid fossa line' and 'middle cranial fossa line' on parasagittal and paracoronal reconstructions. Results: The mean RGF thickness increased from 0.83±0.44 mm to 0.86±0.46 mm after surgery. The average change in thickness of the RGF was 0.17±0.18 mm. The thickness of the RGF in the temporomandibular joint (TMJ) showed no significant difference by sex, and the change in thickness of the TMJ did not vary by surgical method. Conclusions: We found that the thickness of the RGF increased after orthognathic surgery, as revealed by CBCT. Further studies including larger numbers of subjects and long-term follow-up are needed to confirm the results of this study.

  • Pelvic Inflammatory Disease(PID) is a spectrum of inflammatory disorders of the female genital tract involving at least the endrometrium and may include the fallopian tubes, ovaries, and pelvic cavity. Women with PID acutely experience pain and are at risk for sepsis; however, the long-term complications such as chronic pelvic pain, organic changes like chronic salpingitis, adhesions, etc., and the significant increases in ectopic pregnancy and infertility are more important and hard to control. This study is about one patient who has chronic PID with inflammatory pelvic mass. The patient was treated with retention enema therapy which use herbs effective for reducing inflammation, resolution abdominal mass, promoting blood circulation. Also, treated with herbal medication of Danchisoyosan(丹梔逍遙散) and Moxibution at Zhongwan(中脘), Guanyuan(關元). After total treatment for three months, the patient's symptoms are improved and the size of inflammatory pelvic mass is reduced. This results indicates that the combination treatment, especially the retention enema therapy is effective on reducing symptoms of PID and the size of secondary inflammatory organic changes.

  • Purpose: The purpose of this study was to examine the effects of Tai-Chi self-help program for osteoarthritis patients. Method: This study was performed from 1th August. to 30th Dec. 2006. 27 osteoarthritis patients were participated in the Tai-Chi self-help program. Outcome measures were pain, flexibility and balance, self-efficacy. Data were analysed by SPSS WIN 12.0. Result: There were statistically significant in balance and knee flexibility. but no significant differences were found in pain and self-efficacy. Conclusion: In conclusion, Tai-Chi self-help program was proved to be an effective nursing intervention to increase the balance and flexibility. Also 12 forms of Tai-Chi self-help program has been found safely applicable to the patients with osteoarthritis for 8 weeks. A rigorous research design should be further considered to identify the long-term benefit for this population.

Effects of Aroma Therapy on Exam Syndromes and Fatigue in Grade 12 Students Preparing for College Entrance Exams (아로마 흡입이 고3수험생의 고3증후와 피로에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effect of aroma therapy on exam syndromes and temporary fatigue of senior high school students who are preparing to take college entrance exams. Method: This study was a pre-experimental research using a one-group pre-post test design and was conducted between April and July 2003 with 35 high school students. A pre-post test was used to measure long-term exam syndromes and temporary fatigue during the first period of school. Paired t-test was used. Aromas were given using an aroma electrical lamp, aroma necklet with basic oils of rosemary and lemon. According to the condition of the students, one or two oils were added: peppermint, pine, eucalyptus and Clary-Sage. Results: After two months (May, June), the result of administering aroma therapy to senior high school students showed in June there was a decrease in principal exam syndromes such as eye fatigue, headache, shoulder pain, neck stiffness, back pain, and common cold, and there was an increase Un study concentration during the two months. However, in May, there were no significant effects. The reason may have been a strong extraneous factor from midterm exams that would affect their college entrance. Conclusion: According to these results, if we use aroma oil carefully, with evidence-based applications, aroma therapy can have a positive effect on senior high school students who are preparing for the college entrance exams.

Effects of Low Incident Energy Levels of Infrared Laser Irradiation on the Proliferation of Candida Albicans Part I : A Long Term Study according to Pulse Type

  • This experiment was performed to confirm the hypothesis that LLLT had biostimulation effect for all kinds of cells and there would be differences in the growth of cells among different types of pulsed laser. 360 samples were used in this study. The samples were randomly divided in 6 groups according to the pulse type : quasi continuous type (CW), pulse 1(P1), pulse 7(P7), pulse 9(P9), pulse 15(P15) and shame-irradiated control(Co) groups. Energy fluences of all experimental groups, P1, P7, P9, P15 and CW were 2.12, 2.12, 6.37, 57.32 and 31.85 mj/cm2 respectively. All samples were irradiated for every 1 minute at 0, 12, 24, 36, 48 and 60 hours. Ten samples of each group were sacrificed at 0 and every 12 hours and then the optical density of all samples was measured with the spectrophotometer. As a result, some types of pulses showed significant differences among groups. The increase of cells were markedly stimulated with laser irradiation in P7 and P9 groups, while inhibited in CW, P1, and P15 groups compared with control group. It is therefore, suggested that specific laser pulse should be recommanded to have the biostimulation effects on the specific tissue or cells, although the biostimulation effect is does dependant.

Impact of Korean Workers' Experience of Exposure to the Physical Work Factors on Absence

  • In this paper, we propose to enhance a Physical working conditions to decrease absence from work. we demonstrated the correlation of worker's absence with various environmental factors in workplace by surveying 6,962 workers. As result, first, concerning exposure to physical work factors, most of them complained of vibration, and concerning exposure to improper working posture, most of them complained of repetitive arm and hand motions. Second, the absence experience had correlations with age, monthly income, smoking, number of employees, long term working, and shift work, and of the physical factors, hand vibration, noise, high temperature, low temperature and improper working posture had correlations with physical pain posture, movement of people and carrying heavy materials. Third, experience of exposure to hand vibration, noise and low temperature of the physical factors had impact on absence, and of the improper working posture, physical pain posture and carrying heavy stuff had impacts on absence. Through this study, it was found that of the work factors of Korean workers, physical factors and improper working posture had impacts on absence. The results of this study confirmed that physical factors and inappropriate working posture among work factors influenced the absenteeism. Therefore, it is required to improve the work environment regarding physical risk factors and prepare a systematic management plan.

Acute Osteomyelitis of the Mandible by Extended-Spectrum β-Lactamase Producing Klebsiella Pneumoniae: A Case Report

  • Acute osteomyelitis caused by Klebsiella pneumoniae is rare in the oral and maxillofacial region. Klebsiella pneumoniae is a Gram-negative bacillus and the normal flora of the human body, but it can cause pneumonia, urinary tract infection, meningitis, and osteomyelitis in patient with compromised immune systems. These infections are mainly caused by nosocomial infection. Microbacterial osteomyelitis was developed by clinical cause such as tooth extraction, fracture, and surgical history, which requires long-term antibiotic administration and surgical treatment. This report describes that a 56-year-old male patient with acute osteomyelitis caused by Klebsiella pneumoniae infection after implant placement was treated with intravenous administration of ertapenem without open surgery treatment. Through this case, we report that antibiotic susceptibility test is essential for the treatment of acute osteomyelitis caused by a bacterial infection resistant to empirical antibiotics, and early administration of appropriate antibiotics can reduce the possibility of extensive bone destruction or additional open surgery.
