여성 만성 경통 환자에 대한 등척성 운동과 안정화 운동치료의 효과 비교 (Clinical Effect of Therapeutic Exercise in Women Suffering From Chronic Neck Pain)

  • This study compared the effect of therapeutic exercise with conservative treatment in women with chronic neck pain. Subjects were allocated to control, neck isometric exercise, and cervical stabilization exercise groups consisting of 29, 32, and 31 patients, respectively. The conservative treatment for the control group consisted of hot packs, ultrasound, interferential current therapy, and intermittent cervical traction. The neck isometric exercise group performed a neck isometric exercise and the cervical stabilization exercise group performed a cervical stabilization exercise. To compare the three groups, the levels of perceived pain, neck disability, and psychological depression were assessed on admission, at discharge, and one month after discharge. On admission and at discharge, the pain and neck disability scores did not differ significantly among the groups. However, one month after discharge, both the neck isometric and cervical stabilization exercise groups had significantly improved perceived pain levels as compared to control (p<.05). The improvement in the degree of neck disability was most significant in the cervical stabilization exercise group (p<.05). On comparing the changes in the variables at the three assessments for each group, there was a significant difference between the control and cervical stabilization exercise groups (p<.05). The findings indicate that therapeutic exercises, especially cervical stabilization exercises, are more effective for improving pain and neck disability than conservative treatment.

일부 요통환자를 대상으로 한 OLBPDI의 신뢰도 및 타당도 (Reliability and Validity on Oswestry Low Back Pain Disability Index(OLBPDI) in Patients with Low Back Pain)

  • Purpose : his study was to analyze the reliability and validity on Oswestry Low Back Pain Disability Index (OLBPDI) in patients with low back pain. Methods : The sample consisted of 211 patients who had received treatments at the physical therapy units of 3 medical institutions from February to December 2008 in Andong city. Questionnaires on the OLBPDI were recruited by 6 physical therapists. The internal structure and reliability of the scales were evaluated by means of item-internal consistency(Cronbach's alpha coefficient:${\alpha}$), item-discriminant validity, Pearson's relation coefficient. Results : An average of patients's age was 41.1 years. The range of OLBPDI subscales were .93~.94 in Cronbach's ${\alpha}$. The internal consistency reliability of total item-each item were also internally consistent with Cronbach's ${\alpha}$ range of .94~.95(Pearson's correlation coefficient range: .62~.89). However, high correlation were obtained among 10 items(.67~.83), therefore the item-discriminant validity was a little low. Conclusion: In conclusion, the results reported here confirm the reliability of the OLBPDI scales in patients with low back pain. The collection of information on the level of disability due to low back pain using this instrument was acceptable to patients. A further prospective multi-center study will be necessary to prove the reliability and validity.

Effects of Breathing Exercise of Pilates on Dysfunction and Lumbar Flexibility in Patients with Chronic Non-Specific Low Back Pain

  • Objective: The purpose of this study was to investigate the effects of Pilates breathing exercises on functional disability and lumbar flexibility in patients with non-specific chronic low back pain. Design: A randomized controlled trial. Methods: 30 patients with non-specific low back pain were selected and classified into an experimental group (n=15) and a control group (n=15). Both groups were assessed for functional disability using the Oswestry Disability Index and flexibility of the lumbar spine using the modified-modified Schober test before and after the intervention. The intervention was applied twice a week for four weeks. The experimental group performed Pilates breathing exercises followed by spinal stabilization exercises, while the control group only performed spinal stabilization exercises. Results: Both groups showed significant improvements in functional disability and lumbar flexibility after the intervention. However, when comparing between the two groups, the experimental group showed a more significant effect than the control group. Conclusions: The combination of Pilates breathing exercises and spinal stabilization exercises can be considered an effective approach to improve low back pain and prevent chronic pain in patients with non-specific chronic low back pain.

Comparison of three different exercise trainings in patients with chronic neck pain: a randomized controlled study

  • Background: Neck pain is a common problem in the general population and second only to low back pain in musculoskeletal problems. The aim of this study is to compare three different types of exercise training in patients with chronic neck pain. Methods: This study was conducted on 45 patients with neck pain. Patients were divided into 3 groups: Group 1 (conventional treatment), Group 2 (conventional treatment plus deep cervical flexor training), and Group 3 (conventional treatment plus stabilization of the neck and core region). The exercise programs were applied for four weeks, three days a week. The demographic data, pain intensity (verbal numeric pain scale), posture (Reedco's posture scale), cervical range of motion ([ROM] goniometer), and disability (Neck Disability Index [NDI]) were evaluated. Results: In all groups, a significant improvement was found in terms of pain, posture, ROM, and NDI values in all groups (P < 0.001). Between the groups, analyses showed that the pain and posture improved more in Group 3, while the ROM and NDI improved more in Group 2. Conclusions: In addition to conventional treatment, applying core stabilization exercises or deep cervical flexor muscle training to patients with neck pain may be more effective in reducing pain and disability and increasing ROM than conventional treatment alone.

만성통증을 가진 여성노인의 삶의 질 영향 요인 (Factors Influencing on Quality of Life in Aged Women with Chronic Pain)

  • This study was conducted to identify the pain characteristics, family support and physical functioning and to determine predictors of the quality of life in aged women with chronic pain. Method: The questionnaires were collected through direct interview by a trained research assistant from July 2 to August 24, 2001. Subjects were 108 women clients with chronic pain over 65 years of age. Data analyzed frequency, percentage, mean, Pearson's correlation, ANOVA and stepwise multiple regression by SAS. Result: Care providers were mostly spouses and daughters in law. Care providers who took care of elderly for a few hours a day had the highest percentile. Aged women had persistently had chronic pain of moderate intensity and was moderately satisfied with pain management. The mean score of disability due to pain was 3 on a 10 point scale. The mean scores of physical function and quality of life were moderate and there were negative correlations between pain characteristics, physical functioning, and quality of life at the range from r=-.46 to r=-.83. Satisfaction with care, duration of pain, disability due to pain, and physical functioning accounted for 56% of the variance in perceived quality of life for aged women with chronic pain. Disability due to pain was the most predictable variable of quality of life and physical function was the second .Conclusion: The results suggest that care by family, education in pain control, prevention of disability, and maintenance of physical function are important to improve and maintain quality of life in aged women with chronic pain. Therefore, there is a need for program development that enhance family support and nursing intervention that focuses on active pain control.

요통완화프로그램이 만성 요통호소 여교사의 배근력, 통증정도, 기능장애에 미치는 효과 (An Effect of Low Back Pain Relieving Program on the Back Muscle Strenght, Intensity of Pain, Disability Level in Elementary School Women Teacher)

  • The purpose of this study was to examine the effect of low back pain relieving program on back muscle strength, intensity of pain, low back disability level in elementary school teachers who have low back pain. subjects were elementary school women teachers who worked at eight elementary school located in Seoul. Intended subjects size were seventy consist of thirty-four experimental group(three schools) and thirty-six control group(five schools), but actual subjects size was forty-four. Among the forty-four patients subjects, twenty-three were experimental group receiving health education about right postures, etiologies of low back pain, diagnosis of low back pain and exercise program composed of muscle strengthening exercise, stretching exercises and twenty-one were control group. During the 8 weeks program, the subjects were received two times education and six times group exercise practices in 1st week and three times per week group exercise practices, two times education in other 7 weeks. This study as carried out from April 1, 1999 to June 30, 1999. Back muscle strength was measured by back muscle strength measuring machine and the intensity of pain were measured by the Visual Analogue Scale(VAS), and level of disability was measured by Oswestry low back pain disability scale. Study measurements were taken before and after 8 week exercise program. Data were analyzed using paired t-test, and ANCOVA. The results were summarized as follows. 1. After low back relieving program, back muscle strength was increased significantly(p=0.000) and there was significant difference in back muscle strength change between experimental group and control group(p=0.002). 2. After low back pain relieving program, pain on anterior bending, pain on posterior bending were decreased significantly than measurements before the program(p=0.000 p=0.000) and there was significant difference in pain on anterior bending and posterior bending change between experimental group and control group(p=0.000, p=0.000). 3. After low back pain relieving program, Oswestry disability scale scores were decreased significantly(p=0.000, p=0.000) but there was no significant difference in Oswestry disability score change between experimental group and control group.

상부등뼈도수교정과 목뼈부위안정화운동이 만성 목뼈부위 기계학적 통증 환자의 관절가동범위와 장애지수에 미치는 효과 비교 (Comparison of between Upper Thoracic Manipulation and Cervical Stability Training on Range of Motion and Neck Disability in Patients with Chronic Mechanical Neck Pain)

  • PURPOSE: The aim of this study is to investigate how upper thoracic manipulation and cervical stability training affects cervical range of motion and neck disability index of patients with chronic mechanical neck pain. METHODS: 30 patients with chronic mechanical neck pain, and randomly divided into the upper thoracic manipulation and the cervical stability training group. Upper thoracic manipulation group was conducted to the upper thoracic manipulation, and cervical stability training was conducted to the cervical stability training. Intervention period was 6 weeks, and 3 sessions, each of which was run for 5~10 minutes. The subjects were measured neck range of motion before and after intervention by electro-goniometer. Neck disability index was used to measure neck disability index Korean version. RESULTS: Comparison within groups, there were significant difference in neck range of motion before and after intervention, and Neck disability index significantly reduced in the cervical stability training group. The comparison between groups, there were no significant difference in neck range of motion and neck disability index. CONCLUSION: Upper thoracic manipulation and cervical stability training to the patients with chronic neck pain was helpful to improve neck range of motion and cervical stability training was helpful to improve neck disability index.

어깨뼈 다이나믹 테이핑이 어깨 수술 환자의 통증과 기능장애 수준, 상지 자세와 관절가동범위에 미치는 영향 (The Effect of Scapular Dynamic Taping on Pain, Disability, Upper Body Posture and Range of Motion in the Postoperative Shoulder)

  • PURPOSE: The aim of this study was to investigate the effect of scapular dynamic taping on pain, disability, upper body posture, and range of motion (ROM) in the postoperative shoulder. METHODS: Twenty-two patients who underwent acromioplasty and rotator cuff repair surgery volunteered for this study. The subjects were randomly divided into an experimental group (EG, n=11) and a control group (CG, n=11). For the EG, exercise therapy, manual therapy, and scapular dynamic taping were applied for 6 weeks; for the CG, only exercise therapy and manual therapy were applied for 6 weeks. Shoulder pain, disability, upper body posture, and ROM were evaluated at baseline, after 3 weeks of intervention, and after 6 weeks of intervention. Assessment tools included quadruple visual analog scale (QVAS) for level of pain; shoulder pain and disability index (SPADI) for functional disability level; forward head angle (FHA), forward shoulder angle (FSA), and pectoralis minor index (PMI) for upper body posture; and ROM testing. RESULTS: Significant differences were observed between the EG and CG in SPADI total scores; internal rotation and external rotation ROM of the glenohumeral joint ; FSA ; and PMI. All groups showed statistically significant improvement in QVAS; SPADI; flexion; abduction; external rotation and internal rotation ROM of the glenohumeral joint; FSA; and PMI. CONCLUSION: These results suggest that, for patients who have undergone acromioplasty and rotator cuff repair surgery, the addition of scapular dynamic taping during therapy is effective for improvement of shoulder disability level, ROM, and upper body posture.

Foot analyzer를 이용한 만성 요통 환자들의 족저 압력 분석 (Foot Pressure Analysis of Chronic Low Back Pain Patients by Foot Analyzer)

  • Objectives : The aim of this study was to evaluate whether the foot pressure distribution correlates with the lumbo-sacral curvature, and the Oswestry Disability Index in chronic low back pain patients. Methods : We measured the fore foot pressure and the rare foot pressure using the foot analyzer in 28 women subjects with chronic low back pain. The lumbo-sacral curvature and the Oswestry Disability Index(ODI) were also measured. Results : 1. Subjects with higher ODI(%) had significantly lower Fore foot pressure/Rare foot pressure ratio(F/R ratio) (p<0.01). 2. Lumbar lordotic angle and Ferguson angle were inversely related to ODI(%) (p<0.05, p<0.01). 3. Lumbar lordotic angle and Ferguson angle were positively related to F/R ratio (all p<0.05). Conclusions : Using the Foot Analyzer(FA-48S, Tech storm Inc.) we have shown that F/R ratio has significant correlation with the lumbo-sacral curvature and the Oswestry Disability Index. These result suggest that the Foot analyzer may be used in assessing back pain in chronic low back pain patients.

경통 환자 평가를 위한 Neck Disability Index의 신뢰도와 타당도 (Reliability and Validity of the Neck Disability Index in Neck Pain Patients)

  • The purpose of this study was to determine reliability and validity of the Neck Disability Index (NDI) now in use for the first time in neck pain patients of Korea. Fifty subjects (26 males and24 female) with neck pain enrolled in the study. They completed a standardized self-administered questionnaire that include pain intensity, personal care, lifting, reading, headaches, concentration, work, driving, sleeping and recreation. Reliability was determined by intra-class correlation coefficient (ICC) and Cronbach's alpha by internal consistency. Validity was examined by correlating the NDI scores to the Visual Analog Scale (VAS) score. The Test-retest reliability of the translated versions of the NDI was good ICC (2,1) = .90 (95%CI .85 .95). Cronbach's alpha value for NDI was found to be .95 and this was statistically significant (p<.05). The criterion-related validity coefficients was .72 (p<.01). We conclude that the Korean version of NDI has shown to be a reliable and valid instrument for the assessment of neck pain.
