Effect of Laugher Therapy on Pain, Depression and Sleep with Elderly Patients in Long Term Care Facility (웃음요법이 입원 노인 환자의 통증, 우울 및 수면에 미치는 영향)

  • Purpose: This paper was aimed to investigate the effect of laugher therapy on the pain, depression and sleep disturbance in elderly patients who admitted in long term care facility. Method: This study used a nonequivalent control group pretest-posttest design. A sample of 40 elderly patients (20: experimental group, 20: control group, 65 or older) were recruited. Visual analogue scale was used to measure level of pain level, Geriatric Depression Scale Short Form Korea Version (GDSSF-K) for depression. Sleep Scale for sleep. The laugher therapy was given twice a week for four weeks (8 times). Each therapy consists of preparatory, action and finishing stage. Data were analyzed by PASW 18.0. Results: The pain score in experimental group was significantly different from that in control group (t=4.17, p<.001). The level of depression in experimental group was significantly different from that in control group (Z=4.12, p<.000). The level of sleep in experimental group was not significantly different from that in control group (Z=-1.43, p<.152). Conclusion: A laugher therapy is expected to be practical used an efficient method of a nursing intervention to elderly patients in long term care facility.

Effects of Distraction using Balloon Art on Pain during Intravenous Injections in Preschool Children (풍선 아트를 이용한 관심 전환이 학령전기 아동의 정맥 주사 삽입 시 통증에 미치는 효과)

  • Purpose: This study was done to evaluate the effects of distraction using balloon art on pain experienced by preschool children when undergoing an intravenous injection. Methods: Participants were 40 hospitalized preschool children. Children in the experimental group (20) were distracted by balloon art while undergoing an intravenous injection. Children in the control group (20) received regular care. Pain experienced by the children was measured using self-report, observation tools, heart rate and oxygen saturation. The data were analyzed using the SAS program. Results: Compared with the control group, children in the experimental group showed significant difference in pain as shown by pulse rate. However, children in the experimental group did not show a decrease in pain in the self-report or behavior observation when compared with children in the control group. Conclusion: Distraction using balloon art can be used to reduce pain for preschool children when undergoing an intravenous injection. Further nursing interventions need to be developed and provided to preschool children when undergoing painful procedures.

Effects of the Aroma Massage on Shoulder Pain, Depression, Sleep Disturbance in Hemiparesis Patients (아로마 마사지가 편마비 환자의 견통, 우울 및 수면장애에 미치는 효과)

  • Purpose: The purpose of this study is to find out the effects of the aroma massage on shoulder pain, depression, and sleep disturbance in hemiparesis patients. Method: The research design was a nonequivalent control group nonsynchronized design. The data was collected from July 1 to August 31, 2003 at D medical Center in Busan. The subjects were 37 patients that they were assigned to two groups, nineteen subjects in the experimental group and seventeen subjects in the control group. The oil made from a blend of lavendar, clarysage, bergamot at a ratio 3:1:2 used for relieving shoulder pain, depression, sleep disturbance. The instruments used for this study were Visual Analogue Scale(VAS) for shoulder pain, Zung' depression scale, 4 score's scale for sleep disturbance. The data were analyzed using SPSS and hypotheses were examined with ANCOVA. Results: Shoulder pain, depression, and sleep disturbance were significantly lower in the experimental group compared than the control group. Conclusion: The aroma massage using selected essential oils is an effective intervention for relieving shoulder pain, depression, sleep disturbance in hemiparesis patients. However differences between aroma massage and massage were not clear, further study needs to be done.

Changes in pain, swelling, and range of motion according to physical therapy intervention after total knee arthroplasty in elderly patients

  • Objective: This study was carried out to compare changes in pain, swelling and range of motion (ROM) between the two groups according to physical therapy intervention. Design: Randomized controlled trial. Methods: There were a total of 20 elderly subjects ages 65-75 years old who have been admitted to the hospital for a total knee arthroplasty. After surgery, the experimental group (n=10) carried out quadriceps femoris muscle strengthening exercise, hamstring stretching exercises and gastrocnemius stretching exercise with a physical therapist for 30 minutes and additionally, received ice pack therapy for 20 minutes once a day. The control group (n=10) carried out continuous passive motion for 30 minutes and received ice pack therapy for 20 minutes once a day. The experimental group and control group carried out each intervention program for 2 weeks 3 times a week. The Visual Analogue Scale was used to assess pain, tape measurements were taken to assess swelling, and a steel goniometer was used to assess knee joint ROM. Results: As a result, the experimental group showed a statistically significant decrease in the pain and swelling, and a significant increase in knee flexion ROM after the intervention, compared to the control group (p<0.05). There was a significant improvement in pain, edema, and knee flexion and extension ROM in all subjects after intervention (p<0.05). Conclusions: According the results of this study, exercise in the experimental group is effect on the pain, swelling and ROM for total knee arthroplasty.

Effect of treatment with S.O.T block on musculoskeletal pain caused by Traffic Accident (교통사고 환자를 대상으로 한 S.O.T block의 치료 효과)

  • Objectives : The purpose of this study was to investigate the effect of treatment with Sacro-Occipital Technique block on musculoskeletal pain caused by traffic accident by analysis of the Visual Analogue Scale(VAS), Neck Disability Index(NDI), Pain Disability Index(PDI), Oswestry Low back Pain Disability Index(ODI) and Short Form - McGill Pain Qusetionnaire (SF-MPQ). Methods : This study carried out on 18 patients who have received hospital treatment in Daejeon Univ. Dun-San Oriental Hospital. Control group got acupunture-therapy, herbal medication, physical therapy and Experimental group got all the therapies and treatment with Sacro-Occipital Technique block. We measured VAS, NDI, PDI, ODI and SF-MPQ on 1st day and 7 days later. Results : After being treated by our methods, Both group were improved in VAS, NDI, PDI, ODI, and SF-MPQ. Especially, Experimental group was significantly meaningful improved in VAS, PDI, and ODI. Control group was significantly meaningful improved in VAS and SF-MPQ. But, differences between control and experimental group were nonsignificant. Conclusions : The results suggest that treatment with Sacro-Occipital Technique block is not significantly meaningful but gives a positive impact on musculoskeletal pain caused by traffic accident. But further long term study in a large scale is needed.

Effects of SP-6 Seoam Pellet Therapy on Menstrual pain, Menstrual distress and Difficulty of ADL in Female University Students (삼음교(SP-6) 압봉요법이 여대생의 월경통, 월경불편감, 월경관련 일상생활수행정도의 어려움에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of SP-6 Seoam pellet therapy given to female university students in terms of menstrual pain, menstrual distress, and ADL difficulties. Methods: This study used a pre-test, post-test design with a nonequivalent control group. The participants were 68 female students in two universities. The experimental group(n=34) received Seoam pellet therapy at Sanyinjiao while the control group(n=34) received sham therapy. Data were collected from Sep. 16 to Nov. 30, 2012. Severity of menstrual pain, menstrual distress, and difficulty of ADL were assessed prior to and following treatment. Independent t-test, $x^2$ test, and repeated measures ANOVA were used for examination of group differences using the SPSS/WIN program. Results: Severity of menstrual pain (F=41.09, p<.001), menstrual distress (F=56.31, p<.001), and ADL difficulties (F=46.64, p<.001) showed a significant decrease in the experimental group compared to the control group. Conclusion: Results indicate that this Seoam pellet therapy is effective in reducing menstrual pain, menstrual distress, and difficulty of ADL. Therefore, use of this method is recommended for relief of menstrual pain and menstrual distress through self-care.

Identification of Nursing Diagnosis-Outcome-Intervention (NANDA-NOC-NIC) Linkages in Surgical Nursing Unit (일반외과 입원 환자에 적용되는 간호진단-간호결과-간호중재 연계 확인)

  • Purpose: This study was to identify nursing diagnosis-outcome-intervention (NANDA- NOC-NIC: NNN) linkages applied to inpatients in general surgical nursing units. Methods: We developed the NNN linkage computerized nursing process program, which consisted of the 107 nursing outcomes and the 190 nursing interventions linked to the 39 nursing diagnoses. This program was applied to 324 patients who admitted to those nursing units from July, 2004 to February, 2005. Results: First, nursing outcomes of each nursing diagnosis were identified as follows: for 'acute pain', pain control, pain level, and comfort level; for 'risk for infection', wound healing: primary intention, wound healing: secondary intention, and infection status; for 'nausea', nutritional status: food & fluid intake, comfort level, symptom severity and hydration. Second, major nursing interventions for each nursing outcome were analyzed as follows: for pain control or comfort level, pain management and medication management; for pain level, pain management and analgesic administration; for wound healing: primary intention, incision site care and wound care; for Wound healing: secondary intention or infection status, infection control; for nutritional status: food & fluid intake, fluid monitoring; for comfort level, nausea management; for symptom severity, nausea management and vomiting management; for hydration, fluid/electrolyte management. Conclusion: This identified NNN linkages will facilitate the use of nursing process in surgical nursing practice and documentation systems.

Effects of Distant Reiki On Pain, Anxiety and Fatigue in Oncology Patients in Turkey: A Pilot Study

  • Background: Fatigue, stress and pain are common symptoms among cancer patients, affecting the quality of life. The purpose of the present study was to determine the effect of distant Reiki on pain, anxiety and fatigue in oncology patients. Materials and Methods: Participants in the control group received usual medical and nursing care during their stay. The intervention group received usual care plus five distant Reiki sessions, one each night for 30 min. A face to face interview was performed and patient personal and illness related characteristics were evaluated using the Patient Characteristics form. Pain, stress and fatigue were evaluated according to a numeric rating scale. Results: The experimental group was predominantly composed of women (71.4%), married individuals (40%), and primary school graduates (40%). The control group was predominantly male (72.7%), married (60%), and primary school graduates (60%). The control group demonstrated greater levels of pain (p=0.002), stress (p=0.001) and fatigue (p=0.001). The Reiki group pain score (p <0.0001), stress score (p <0.001) and fatigue score were also significantly lower. Conclusions: The results of this study indicate that Reiki may d ecreasepain, anxiety and fatigue in oncology patients.

High Spinal Block following Epidural Block for Postoperative Pain Control -A case report- (수술후 통증조절을 위한 경막외 차단후 발생한 고위척추차단 -증례 보고-)

  • High spinal block is a rare complication during epidural block, but it may result in serious events. 56-year-old man with gall stones was scheduled for cholecystectomy under general anesthesia. After operation, lumbar epidural catheterization was done at $T_{8-9}$ interspace for postoperative pain control. At the recovery room, initial bolus drug (0.1% bupivacaine 10 ml containing fentanyl $100{\mu}g$) was administered via epidural catheter and observed carefully. 15 minutes later, hypotension and bradycardia ouccurred. Hartman' solution was administered rapidly and ephedrine 5 mg was injected. 30 minutes after drug administration, loss of consciousness and respiratory arrest developed. Tracheal intubation was done immediately. Cardiovascular and respiratory functions were monitored continuously. The location of intrathecal catheter was confirmed by cerebrospinal fluid (CSF) seen in syringe after aspiration of catheter. The patient recovered gradually and was placed in the ward 4 hours after drug administration, without any problems. He was discharged 1 week later in good health.

Effect of Intravenous Patient Controlled Analgesia for Postoperative Pain in Adult Tonsillectomy (성인에서 편도적출술후 정맥내 동통자가조절법에 의한 동통조절 효과)

  • Postoperative pain following tonsillectomy remains a significant obstacle to speedy recovery and smooth convalescence. Inadequate analgesia causes poor oral intake and influences the length of hospital stay and ability to return to normal activity. Patient Controlled Analgesia (PCA) is a method of analgesia adminstration that consists of a computer driven pump with a button that the patient may press to adminster a small dose of analgesic drug. The aim of this study was to examine whether Intravenous Patient Controlled Analgesia (IV-PCA) can reduce postoperative pain after tonsillectomy. The 100 patients undergoing tonsillectomy with general anesthesia were divided into two groups. The PCA group patients (n=80) received a mixture of nalbuphine and ketorolac by Walkmed PCA infusor during first 48 postoperative hours. In control group (n=20), the patients received oral acetoaminophen (Tyrenol) regularly and tiaprofenic acid (Surgam) intramuscularly on a p.r.n basis. Analgesic efficacy was evaluated with visual linear analogue scale (VAS) and the adverse effects were evaluated with 4 point scale. The patients of PCA group had less pain than those of control group. The adverse effects in the PCA group were nausea and vomiting. This study suggests that IV-PCA may be safe and effective method of pain control after adult tonsillectomy and is better accepted than oral or intramuscular pain medications.
