The Effect of Short-term Lumbar Stabilization Exercise for Lumbar Muscle Strength and Postural Balance on Chronic LBP (단기간의 요부안정화 운동이 만성요통환자의 근력과 자세균형에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate the effect of short-term lumbar stabilization exercise to patients suffering from chronic low back pain(CLBP) on the strength of their lumbar flexor and extensor and the postural balance of them and to suggest effective treatment duration for them. METHODS: Study subjects were 20 patients with CLBP who's been reported the pain for more than 12 weeks. For these subjects, muscle strength of both extensor and flexor of lumbar and the balancing ability were measured before and after the intervention. Lumbar stabilization exercise was composed of matt, ball and sling exercise. The intervention was applied for total 12 sessions for 4 weeks (3 times a week) and each session was for 40 minutes. RESULT: For muscle strength of both extensor and flexor of lumbar, there was significant differences after the application of lumbar stabilization exercise(p<.05). For balancing ability, stabilization shows significant differences in all of the positions except PC(eyes closed on pillows)(p<.05). Distribution of weight shows no significant differences but it was generally distributed within normal range after the exercise and Fall index shows significant differences between before and after the exercise(p<.05). CONCLUSION: These results could have positive effect on deciding different periods of therapeutic intervention.

Correlation Analysis between HIVD of L-spine MRI and Digital Infrared Thermal Image (DITI) on the Patients of LBP Who Visit Korean Medicine Hospital (한방병원에 요통, 하지방사통으로 내원한 환자 236명에서 MRI상 추간판 탈출증과 적외선 체열 검사(DITI)와의 연관성 분석)

  • Objectives This study is planned to classify correlation between HIVD of L-spine MRI and Digital Infrared Thermal Image (DITI). Methods We measured the temperature of both leg whose 120 men and 116 women patients with lumbar pain in Bucheon Jaseng Korean Medicine Hospital. And We use Magnetic Resonance Imaging (MRI) for classifying the patients who has lumbar intervertebral disc or not. Results 1) There was no statistical relation between difference of both leg's temperature and gender (p>0.05). 2) There was meaningful statistical relation between difference of both leg's temperature and age (p<0.05). 3) There was meaningful statistical relation between direction of HIVD of L-spine and direction of temperature reduction. 4) There was meaningful statistical relation between the severity of HIVD of L5/S1 and degree of temperature reduction. But there was no statistical relation between the severity of HIVD of L3/4, L4/5 and degree of temperature reduction. Conclusions We can use Digital Infrared thermal image (DITI) on low back pain patients for diagnosis. But we should not use DITI alone. DITI has limit in diagnosis.

Objective Evidence for the Effectiveness of Single-session Treatment with a Spinal Thermal Massage Device: A Pilot Study (척추온열마사지기기의 1회 치료의 효과에 대한 객관적 증거: 선행 연구)

  • Individuals often report significant relief from pain and stress even after a single session of massage therapy; however, no previous studies have provided objective evidence supporting the effectiveness of a solitary massage therapy session. In the present study, we aimed to investigate the effectiveness of one-time treatment with a spinal thermal massage device reported to exert the same therapeutic effects as massage therapy in terms of pain reduction and stress relief. A man with chronic low back pain (LBP) underwent two rounds of experiments involving spinal massage treatment and bed rest, respectively. Pain was assessed using a visual analog scale, and heart rate variability (HRV) was measured in real-time to examine autonomic nervous system (ANS) activity. Blood samples were obtained at five points during each round of the experiment to examine changes in cortisol, epinephrine, and norepinephrine. Spinal massage significantly reduced pain and enhanced parasympathetic activity when compared with the bed rest condition. In addition, both epinephrine and norepinephrine levels were lower following spinal massage than following bed rest. These results are consistent with the reported effects of conventional massage therapy and support the effectiveness of one-time treatment using a spinal thermal massage device.

A Clinical Study on the Effect of Korean Medical Treatments for Patients with Lumbar Disc Herniation (요추 추간판탈출증 환자에 대한 한약, 봉독약침, 침, 추나요법의 유효성 평가)

  • Objectives : To investigate therapeutic outcomes of back pain modalities in patients with disc hemiation; Korean medical treatments(Herbal medicine, chuna therapy, acupuncture, bee-venom acupuncture.). Methods : Operation(OP) recommended 20 patients with lumbar disc hemiation were treated with chuna therapy, acupuncture, bee-venom acupuncture one times a week and took herbal medicine after a meal two times daily. The patients' symptoms were assessed 1 week, 2 week, 4 week, 6 week, 8 week by Visual analogue scale(VAS), Oswestry disability index(ODI), 36-Item short-form health survey(SF-36). Results : 1. VAS(LBP and Sciatica) score was significantly improved after 8 weeks (P<0.05). 2. ODI, SF-36 score was significantly improved after 8 weeks (P<0.05). 3 There were significant changes in physical functioning(PF), bodily pain(BP), social functioning(SF), role emotinal(RE), mental health(MH) score of SF-36 after 8 weeks (P<0.05). Conclusions : This study suggests that Korean medical treatment(Herbal medicine, chuna therapy, acupuncture, bee-venom acupuncture) can be applicable to improve symptoms in patients with lumbar disc herniation.

A Psychophysical Approach on the Assessment of Lifting Loads (정신물리학적 접근방법을 이용한 들기작업의 작업하중 평가에 관한 연구)

  • Low back pain (LBP) is a major issue in modern industrialized society which is mainly caused by manual material handling (MMH) tasks. The objective of this study was to provide scientific data for establishing work standard for Korean workers throughout the laboratory experiment including some specific lifting tasks. Thirty male college students were recruited as participants. The maximum voluntary contraction (MVC), recommended weight limit (RWL), and psychophysical safety weight using Borg's CR-10 scale were studied. Results showed that the RWL was 8.4% MVC higher than the proposed psychophysical safety weight. Based on this result, it is suggested that the NIOSH lifting equation (NLE) should not be directly applied to Korean without reasonable modifications. The ratio of psychophysical safety weight to MVC was ranged from 20.1 to 26.4%. It is expected that use of the methodology in this study may provide better expectation of the work ability of Korean for reducing lower back pains caused by MMH.

Difference of Trunk Muscles Activity during Hollowing vs Bracing Contraction in Various Position (다양한 자세에 따른 복부 할로잉과 브레이싱 수축시 체간근 활성도의 차이)

  • PURPOSE: The purpose of this study was to investigate the difference of trunk muscles activity during abdominal hollowing and bracing contraction in various position. METHODS: This pilot test was carried out in a volunteer sample of normal adults(n=24) without a history of low back pain or injury. 24 subjects were randomly allocated to three groups(n=8) as a contraction method respectively. In hooklying position, trunk muscles activity of subjects was measured using EMG in various bridging position. RESULTS: Abdominal bracing contraction made to more great trunk deep and superficial muscles activity than hollowing contraction.(p<0.00) Especially, Multifidus activity was the biggest.(p<0.00) CONCLUSION: The result from this study showed that abdominal bracing contraction made to more balancing activity of trunk muscles than abdominal hollowing contraction. Thus, It will good for trunk muscles unbalanced LBP patient to improve lumbar stabilization.

Application of EMG Analysis for Department Store Female Workers (일부 서비스업 종사 여성근로자의 근육피로에 대한 EMG 분석)

  • The EMG(Electromyography) analysis was used to identify the fact the degree of inclined step was selected as dependent variable and feet muscle fatigue was selected as a independent variable. In a final result from EMG test. the shift in median frequency (MF) with 20, 25, 30, 35 degree of inclined steps indicated that 30 degree step was identified as most effective for a decrease in feet muscle fatigue. In a department store, 80% of the workers are female standing sales workers. They work at standing on average 10 hours per day. They performed heavy duty jobs such as lifting, lowering. packing and carrying heavy materials. Furthermore, even though they have work shoes, they usually use various kind of high heels. Eventually, this situation develops low-back-pain (LBP) problems for female workers. In conclusion, it is recommended that a particular branch in a department store claimed this step can effectively to circulate blood and significantly decrease feet muscle fatigue in lower extremity.

Correlation between Trunk Stabilization Muscle Activation and Gait Parameters (몸통 안정화 근육과 보행요소의 상관관계)

  • Purpose: This study aimed to investigate the correlation between trunk stabilization muscle activation and the parameters of gait analysis in healthy individuals. Methods: Thirty healthy adults (15 male, 15 female) with no history of lower back pain (LBP) or current musculoskeletal and neurological injuries were studied. Trunk stabilization muscle activation (e.g., external oblique, internal oblique, transverse abdominis, erector spinae) were assessed using surface electromyography. To analyze gait, we measured temporal parameters (e.g., gait velocity, single support phase, double support phase, swing phase, and stance phase) and a spatial parameter (e.g., H-H base of support). Results: A statistically significant correlation was found between the internal oblique, transverse abdominis, and erector spinae muscle activity and gait velocity, single support phase, double support phase, swing phase, and stance phase. No statistically significant correlation was found between the external oblique muscle activity and the gait velocity, single support phase, double support phase, swing phase, and stance phase. No statistically significant correlation was found between the external oblique, internal oblique, transverse abdominis, and erector spinae muscle activity and the spatial parameter. Conclusion: This study demonstrated that a relationship exists between trunk stabilization muscle activation and temporal parameter (i.e., gait velocity, single support phase, double support phase, swing phase, and stance phase) during gait analysis. Therefore, the trunk's stabilizer muscles play an important role in the gait of healthy individuals.

A Study on Sports Injuries in Korean National Sitting Volleyball Players (국가대표 좌식배구 선수들의 운동상해 조사연구)

  • PURPOSE: This study examined sports injuries among national sitting volleyball players and to provide baseline data for the development of programs to prevent injuries and enhance performance. METHODS: The study surveyed 21 national team athletes (12 males and nine females) participating in the 4th Hangzhou Asian Para Games. The questionnaire consisted of 17 items, including general information, type of disability, sites and types of sports injuries and their causes, the current state of sports injuries and the treatment and management of injuries. RESULTS: The survey results suggested that the most common injury sites were the finger, shoulder, and waist. The most frequent types of injuries were sprain, muscle cramp, and LBP. The causes were insufficient warm-up, playing unhealed and carelessness. Injuries were most prevalent during morning training and in the winter. Most injuries occurred in practice, and the actions most likely to cause injuries were blocking, spike and sitting movements. Ice and spray were the most common treatments, usually administered by the athletes themselves. Physical therapy was the most common post-injury management, and the most common sequelae involved continuing to use despite pain. CONCLUSION: Based on these results, systematic and individualized training and therapeutic support tailored to the characteristics of sitting volleyball and the types of disabilities are necessary to prevent and manage sports injuries among national players. Continuous injury management by medical staff, particularly physical therapists, is essential to maintain the health of disabled athletes.

The Study on Effectiveness of Oriental Medicine Treatment for Lumbar Disc Herniation Inpatients on 208 Cases (요추 추간판 탈출증 입원환자 208례를 통한 한방치료의 효과 연구)

  • This study was introduced the short-term effectiveness of Oriental medicine treatment for lumbar disc herniation inpatients. We selected 208 inpatients with a magnetic resonance imaging(MRI)-confirmed lumbar disc herniation since June 2012 until August 2012. Their demographics characterisitics, numeric rating scale(NRS) and Oswestry back-related disability index(ODI) for low or leg pain, lumbar flexion and extension angle were assessed at baseline and when discharged from the hospital. When it measured the straight leg raisingscale(SLR), the low side of the angle was examined. Treatment every patient has received as follow; herbal medicine, bee venom, acupuncture and physical examination, spinal manipulation. NRS of low back pain and leg pain and ODI is $5.6{\pm}2.3$, $4.9{\pm}2.8$ and $45.5{\pm}20.0$, respectively at baseline and $2.8{\pm}1.8$, $2.7{\pm}1.9$ and $27.2{\pm}14.1$, respectively when discharged from the hospital. range of motion(ROM) of lumbar flexion and extension angle and SLR is $70.0{\pm}27.1$, $15.4{\pm}7.2$ and $61.3{\pm}23.0$ respectively at baseline and $80.5{\pm}16.9$, $18.25{\pm}4.1$ and $73.2{\pm}14.0$, respectively when discharged from the hospital. It shows that statistically significant improved(P<0.001). Furthermore, regarding patient satisfaction with the treatment, excellent(33.7%), good(55.4%), normal(10.4%), poor(0.5%), it suggests that most patients(89%) satisfied with the treatment.
