The effect of balance training with plantar flexor stretching on range of motion, balance, and gait in stroke patients: a randomized controlled pilot trial

  • Objective: The aim of this study was to investigate the effect of balance training with plantar flexor stretching on ankle dorsi flexion range of motion (ROM), balance, and gait ability in stroke patients. Design: A randomized controlled pilot trial. Methods: Thirty stroke patients volunteered to participate in this study. The subjects were randomly allocated to two groups: the experimental group (n=15) received the neurodevelopment therapy plus balance training with plantar flexor stretching for 20 minutes in one session. The control group (n=15) received the same neurodevelopment therapy plus plantar flexor static stretching for 20 minutes in one session. Both groups underwent sessions four times a week, for a total of 4 weeks. Measurements included passive range of motion (PROM), active range of motion (AROM) of ankle dorsiflexion using a goniometer, timed up and go (TUG), the functional reaching test (FRT), and the 10 m walk test (10 MWT). Results: There were significant improvements in AROM and PROM of ankle dorsiflexion, TUG, and FRT scores after the intervention in the experimental group (p<0.05). However, the control group showed no statistically significant differences except for PROM of ankle dorsiflexion. The experimental group showed a significant improvement in PROM, TUG, and FRT scores compared to the control group (p<0.05). Conclusions: Balance training with plantar flexor stretching improves ankle dorsiflexion ROM and balance ability in patients with stroke. Therefore, this therapeutic intervention will be effective for rehabilitation of stroke patients in the clinical setting.

테이핑요법과 수동적 관절범위운동이 노인의 견관절과 손의 민첩성에 미치는 영향 (Effects of Taping Therapy and Passive Range of Motion Exercises on Shoulder joint, Hand dexterity in the Elderly)

  • 본 연구는 견관절 통증과 관절가동범위에 제한을 가지고 있는 65세 이상의 노인을 대상으로 하여 테이핑요법과 수동적 관절가동범위 운동이 견관절의 통증과 관절 가동력에 미치는 효과를 확인하기 위하여 실시하였다. 이 연구는 비동등성 전후 실험 연구이다. 2009년 9월에 B시의 복지관을 이용하는 65세 이상의 노인들 40명으로 견관절의통증과 관절가동범위, 손의 민첩성을 조사하였다. 수집된 자료는 SPSS Win 12.0을 이용하여 분석하였다. 결과, 테이핑군과 수동적 관절가동범위 운동군에서 노인의 견관절 통증은 감소하였다. 견관절 굴곡, 신전, 외전, 수평외전 범위와 손의 민첩성은 증가하였다. 노인의 견관절 통증의 감소정도와 손의 민첩성 정도는 테이핑군과 관절범위운동군 간에 차이가 나타나지 않았다. 테이핑군에서 관절범위운동군 보다 견관절 신전의 범위가 증가하는 정도가 더 큰 것으로 나타났다. 따라서, 테이핑요법과 수동적 관절범위운동은 노인의 견관절 통증을 감소시키고, 관절가동범위, 손의 민첩성에 효과가 있다고 할 수 있겠다.

  • 최근, Microprocessor가 염가로 입수가능함에 따라 광범위한 분야에 걸쳐서 사용되고 있다. microprocessor를 유효하게 사용하면 컴퓨터 시스템의 설계가 용이해진다. 이 논문에서는 MMI-6701을 사용하여 NOVA의 Emulator를 설계 하였으며 연구의 성과로서는 NOVA를 Microprocessor로 대치하므로서 IC의 수가 약 3분의 1로 대치할 수 있었고, 또 Emulator의 효율화를 위하여 PROM의 Micro명령은 32bit로서 구성하여 4종류의 명령형식으로 설계 되였다. In recent years, Microprocessor have the use of extended wide fiexd because of obtainable to low price. Design of computer system be easy to do by this microprocessor apply validly. This Papers: NOVA Emulator designed by use of MMI-6701. As a result of this studies, quantity of IC changed with about a third part by NOVA exchanged with Microprocessor. Micro Instruction of PROM consist of 32bit that designed Instruction Format of four kinds.

Effect of Joint Mobilization and Kinesio Taping on Pain, Range of Motion, and Knee Function in Patients with Knee Osteoarthritis

  • Purpose: This study was conducted to investigate the effects of joint mobilization and kinesio taping on pain, range of motion and knee function in patients with knee osteoarthritis. Methods: Thirty subjects were selected and divided into three groups: group 1 was treated with joint mobilization, group 2 was treated with kinesio taping and group 3 was treated with joint mobilization and kinesio taping. Joint mobilization was performed for 20 minutes three times a week for a period of 4 weeks, after which tape was applied for the same period of time and it was not exceeded 24 hours. Pain, range of motion and knee function were then assessed to identify the effectiveness. A visual analog scale (VAS) was used for pain assessment, while active and passive range of motion (AROM, PROM) were assessed using smart phones application, and knee injury and osteoarthritis outcome score (KOOS) was used to assess knee function. Results: After intervention, the joint mobilization group and kinesio taping group showed significant improvements in VAS, AROM, PROM and KOOS (Symptom, Pain, ADL, QOL), whereas no significant difference was found in sport/recreation. The joint mobilization with kinesio taping group showed significant improvements in all items, and a significant increase was found in AROM and PROM compared to the other two groups. Conclusion: We found that joint mobilization and kinesio taping effectively improved pain, range of motion and knee function in patients with knee osteoarthritis, but that application of joint mobilization with kinesio taping was most effective.

슬관절 전치환술 후 수동 및 능동 보조 관절가동범위운동이 슬관절 가동범위 증가에 미치는 영향 (The Effect of PROM and AAROM Exercise After TKA on Increasing the Knee Range of Motion)

  • Background: The purpose of this study was to find out the real truth of the effect of PROM (Passive range of motion) and AAROM (Active assistive range of motion) exercise on increasing the knee flexion. This randomized, controlled trial examined whether or not the incorporation of PROME(passive range of motion exercise) to a postoperative rehabilitation protocol would offer a better clinical outcome after TKA (Total knee arthroplasty) Method: The subject (n=36) measured range of motion (ROM).18 consecutive patients who underwent TKAs at SNUH dept. OS received PROME for knee by a physical therapist during the physiotherapy sessions and not for the other 18 patients who underwent TKAs received No-PROME (AAROME) for knee by herself. Result: There were significant differences in the PROME group, No-PROME group ROM progress width And there were significant differences in the PROME group between No-PROME group ROM improvement width. Conclusion: This study demonstrates that the incorporation of PROME does offer additional clinical benefits to the patients after TKA. Our findings may suggest that encouraging patients to perform PROM exercises would be a better option and that physiotherapy session by a physical therapist holds good even now.

Successful Treatment of Ulna Fracture in a Small Raptor

  • A wild Falco tinnunculus, which is a small raptor, was rescued and presented due to immobilization. On physical examination, a left ulnar fracture was detected, which was reduced with an intramedullary (IM) pin and soft bandaging. Passive range of motion (PROM) exercises were then conducted to prevent complications such as ankylosis. The raptor was released into the wild four weeks postoperatively. We suggest that IM pinning with bandaging and appropriate PROM exercises may effectively treat ulnar fractures in small raptors.

고관절 원형인대 파열 환자에 대한 한의학적 치료: 증례보고 (A Clinical Case of the Korean Medical Treatment for the Patient with Ligamentum Teres Tear of the Hip)

  • The purpose of this clinical case is to evaluate the efficacy of the Korean medical treatment on a ligamentum teres tear of the hip. One patient, admitted to Daejeon Jaseng Hospital of Korean Medicine was diagnosed with a ligamentum teres tear of the hip by hip magnetic resonance imaging. We treated the patient using the Korean medical treatments, including acupunture, pharmacopuncture, herbal medication, and cupping. We measured the numerical rating scale (NRS), passive range of motion (PROM) and the changes of symptoms. After 13 days, NRS decreased from 8 to 1 respectively, PROM of hip joint has recovered, and the symptoms of the patient improved. In conclusion, this study shows that Korean medical treatment might be effective in treating the patient with a ligamentum teres tear of the hip.

족부족관절 분야의 환자 자가 보고 결과 평가법 (Patient-Reported Outcome Measures of the Foot and Ankle)

  • The patient-reported outcome measures (PROMs) are important in assessing the patient's overall health, lesion-specific outcomes, and condition-specific outcomes. PROMs are also known as "scoring systems" and are usually in questionnaires. There are almost a hundred different PROMs available in foot and ankle surgery. Each PROM has its merits, demerits, and validity. Selecting an appropriate PROM is important for adequately evaluating a patient's health status. This article summarizes the most frequently used PROMs in the literature on foot and ankle surgery and presents the authors' recommendations.

  • The purpose of this study was to determine the effects of weight shift training with joint mobilization on the ankle joint passive range of motion (PROM), balance capacity and gait velocity in hemiplegic patients. Fourteen subjects were randomly assigned to either the experimental group (EG) or the control group (CG), with seven subjects in each group. The EG received weight shift training with joint mobilization in the paretic leg's subtalar joint in order to increase ankle dorsiflexion. The CG received general physical therapy training. Both groups received training five times a week over a period of two consecutive weeks. The figures for PROM of ankle dorsiflexion on the paretic leg, the functional reach test (FRT), the timed up and go (TUG) test, and gait velocity were recorded both before and after the training sessions for both groups. The EG's results in gait velocity, the FRT and the TUG test improved after training (p<.05). The PROM of ankle dorsiflexion improved both in the EG and the CG (p<.05), the EG demonstrated a significantly higher increase (p<.05) than that of the CG. The results of this study suggest that increased joint mobilization positively affects balance and gait velocity of hemiplegic patients. Further studies with a greater sample size are necessary in order further prove the accuracy of the results of this study.
