Current Status of Pediatric Critical Care in Korea: Results of 2015 National Survey

  • Background: The aim of this study was to describe the structure, organization, management, and staffing of pediatric critical care (PCC) in Korea. Methods: We directed a questionnaire survey for all Upper Grade General Hospitals (n = 43) in Korea in 2015. The first questionnaire was mainly about structure, organization, and staffing and responses were obtained from 32 hospitals. The second questionnaire was mainly about patients and management. Responses to second questionnaire were obtained from 18 hospitals. Results: Twelve from 32 Upper Grade General Hospitals had pediatric intensive care units (PICUs) and 11 of them had the PICU which was exclusive for children. Total number of PICU beds in Korea was 113. The ratio of the number of PICU beds to the number of children was 1:77,460 in Korea and this ratio is lower than that of other developed countries. The mean number of beds in the PICUs was $9.4{\pm}9.3$ (range, 2-30). There were 16 medical doctors who were assigned for PCC and only 5 of them were full time pediatric intensivists. In the 18 Upper Grade General Hospitals that responded to the second questionnaire survey, there were 97 patients in the PICUs with an average number of $5.7{\pm}7.2$ (range, 0-22) on the survey day. The mean age of the patients was $3.4{\pm}5.6$ years. The mean length of hospital stay was $82{\pm}271days$. The mean Pediatric Risk of Mortality score III was $9.4{\pm}7.8$ at the time of admission to the PICUs. Conclusion: There is a considerable shortage of PICU beds compared to those in developed countries. In addition, the proportion of PICUs with PCC specialists is much lower than those in the US and European countries.

느티나무 보호수의 내부 단층 진단을 통한 건강 분석 (The Health Analysis of Protected Tree 'Zelkova serrata' Using an Ultrasonic Tomograph)

  • This study was conducted to evaluate defects in Zelkova serrata trees using an ultrasonic tomograph (PICUS) and suggest a management plan to maintain it as a natural and cultural resource. In previous study, health information of 16 protected trees were obtained by surveying the following categories; tree appearance, crown, bark, root exposure, water and nutrition, deterioration, pest, vitality, ground status, and pollutant. The average score of health was 16.5 in 16 trees, which means that general monitor is needed for management. In this study, the evaluation result of tree's inside defects using an ultrasonic tomograph showed that 43.8% of decay rate has been found in 16 trees. In fact, some trees look to be good even though they actually have a defects causing broken by natural disaster such as strong wind. Therefore, it is urgent to put some support to the trees and come up with a protective plan. The results of this study would be useful as basic data in developing a guideline for the efficient conservation and management of big and old trees.

A Potent Anti-diabetic Agent from Kalopanax pictus

  • To search for the anti-diabetic principle from the stem bark of Kalopanax pictus, seven kinds of chemical constituents including hederagenin glycosides and phenolic glycosides wre isolated. The anti-diabetic evaluation of these isoltes in the streptozotocin-induced diabetic rats exhibited that kalopanaxsaponin A has a potent anti-diabetic activity in contrast to a mild activity of hederagenin. In addition, significant hypocholesterolemic and hypolipidemic activities of kalopanaxsaponin A and hederagenin were observed. The structure-activity relationship of kalopanaxsaponin A was also investigated in the present work.

소아중환자실 부모의 돌봄참여에 대한 간호사의 인식: 내용 분석 연구 (Nurses' Perceptions toward Parent Participation in Pediatric Intensive Care Unit: A Content Analysis)

  • 본 연구의 목적은 부모의 돌봄참여에 대한 소아중환자실 간호사의 인식을 파악하는 것이다. 연구대상은 서울에 위치한 상급종합병원 2곳의 소아중환자실에 근무하고 있는 간호사 5명이며, 일대일 심층 면담을 수행하였다. 자료수집 기간은 2016년 1월부터 2월까지였고 전통적 내용분석을 이용해 자료를 분석하였다. 연구 결과 소아중환자실 부모의 돌봄참여에 대한 간호사의 인식은 5개 범주와 43개 하위범주가 도출되었으며, 5개 범주는 필요성, 의미, 효과, 장애 요인과 촉진방안으로 나타났다. 간호사는 소아중환자실에서 집중 간호를 필요로 하는 자녀의 중증 상태로 인해 부모가 정서적 부담을 경험하며, 면회가 제한된 병원 정책이 이러한 부담을 악화시킨다는 점에서 소아중환자실에서 부모 돌봄참여의 필요성과 그 효과를 인식하고 있었다. 주요한 장애 요인은 부모의 돌봄참여에 대한 간호사의 지식과 역량의 부족, 과중한 업무 부담, 소아중환자실과 병원 내 정책이나 지침의 부족으로 나타났다. 촉진방안은 간호사 대상 부모 돌봄참여의 의미와 실천방안에 대한 교육 제공, 간호사뿐 아니라 부모를 포함하여 인식개선 활동, 전담 전문가를 지원하는 방안으로 나타났다. 본 연구의 결과를 기반으로 향후 소아중환자실에서 부모의 돌봄참여를 통합적으로 도입하기 위한 간호 중재의 개발과 효과 검증을 위한 후속 연구가 필요할 것이다.

서울시 강서구 조성녹지축의 야생조류 서식처 특성 연구 (Characteristics of Wildbirds Habitat of Artificial Green Corridor in Gangseo-gu, Seoul)

  • This study was to examine the characteristics of wirdbirds habitat for improvement plan in green corridor. The target site, Gangseo-gu artificial green corridor was set up with the structure in which small scale of core green space with Goongsan and Yeomchang neighborhood parks in urbanized city was connected with the artificial green space with Gongamnaru, Hwanggeumnae neighborhood parks with 28~42.5 m in width. Wild birds six~eleven species; Dendrocopos spp, Paradoxornis webbiana, Parus major, Phasianus colchicus, etc. were observed in core green, but wild birds of two~five species: Columba livuia, Passer montanus, Pica pica, Hypsipetes amaurotis, etc. were observed in artificial green space. Thus wild birds of artificial and generalist species only moved in artificial green space. The artificial green space where vegetation structure was consisted of single-layer with poorness chose target species laying stress on generalist species and edge species of Parus major, P. palustris, Paradoxornis webbiana etc. for short-term and interior species of Dendrocopos major, Picus canus, etc. for long-term. The result suggested enhancement methods for target species's habitat in green corridor: to secure at least a corridor 30 meters in artificial corridor, to secure ecological pond, to offer the various shelterer and environment of prey-resources through the multi-layer structure.

한국산 씬벵이과 어류 1미기록종 Antennarius pictus (First Record of the Frogfishes Antennarius pictus (Antennariidae, Lophiiformes) from Korea)

  • 씬벵이과 (Antennariidae)에 속하는 Antennarius picus 2개체가 제주도 연안에서 처음으로 채집되었다. 이 종은 뒷지느러미 연조가 7개, 가슴지느러미 연조 10개, 등지느러미 연조 127개, 꼬리지느러미 연조 9개를 가지며, 제2등지느러미 가시의 말단은 기부보다 좁고 뒷방향으로 굽어져 있는 특징을 갖는다. 그리고 어류의 머리, 몸통, 지느러미 부위에 다양한 크기의 검은 점들을 가지고 있기 때문에 본종의 신한국명을 "흑점박씬벵이"로 명명하였다.

생태네트워크 구축을 위한 목표종 선정에 관한 연구 - 판교신도시를 사례로 - (Selecting Target Species for Urban Ecological Network Construction - Focus on Pangyo New Town -)

  • With recent emphasis on the creation of environment-friendly new towns, introduction of ecological facilities for habitation and migration of wild animal's species is requested when developing new towns. In order to introduce such facilities, building an eco-network within the site based on the connectivity of the source area and habitats is essential in new town development. Therefore, this study mainly aimed at selecting species targeted for building an eco-network in Pangyo new town, which is intended to be an environment-friendly city. Therefore, criteria for selecting target species were generated. Then, species observed within the site through field surveys and literature review was evaluated based on the selection criteria and items. By totaling the score, a list of appropriate targeted species was finalized. Among species surveyed and observed in the site, appropriate target species that may be selected for Pangyo new town's eco-network include Falco tinnunculus interstinctus, Accipiter soloensis, Picus canus, Paradoxornis webbiana, Parus palustris, Parus ater, Parus major and Passer montanus Egretta in birds, Asiatic chipmunk, Nyctereutes procyonoides in mammals. For Pangyo new town, it is essential to create living environment and build a network for major wild animal species within the site based on target species. This will play a crucial role in building a reasonable ecological network enabling harmonious co-existence between mankind and nature. In order to build an ecological network successfully, follow-up studies need to be conducted on restoration technology and methods required for creating habitats appropriate to target species.

오대산 국립공원 오대산 지역의 야생조류상 및 관리 방안 (Management Guideline and Avifauca of Odaesan area in Odaesan National Park)

  • 오대산 국립공원내 야생조류상을 파악하고 관리방안을 제시하기 위하여 2개 조사구역에 대해 1995년 6월 중순부터 1996년 12월 초순까지 선조사법에 의하여 4회 조사하였다. 총 9목 22과 52종이 관찰되었으며, 이들은 텃새 25종과 여름철새 16종, 나그네새 8종 및 겨울철새 3종으로 구성되어 있다. 천연기념물 제 242호인 까막딱다구리, 제323호인 황조롱이와 붉은 배새매, 제 324호인 소쩍새와 올빼미 등 5종이 관찰되었다. 종 수 및 개체수는 제 2조사구역에서 많았고, 종 구성에서 상당한 차이를 보였다. 번식조류군집의 영소길드(nesting guild)는 두 조사구역 모두 둥지자원을 관목(bush), 수동(hole), 수관층(canopy) 순으로 높게 이용하는 것으로 나타났다. 관목 영소길드(bush-nesting guild)의 종 수가 높게 나타난 것은 제 1 조사구역의 임연부에 형성된 관목층과 제 2조사구역의 정상부근 능선의 관목층과 관련이 있을 것으로 생각된다. 수동 영소길드(bush-nesting guild) 중 오색딱다구리, 까막딱다구리, 청딱다구리 등 3종류는 제 2조사구역에서만 관찰되어 제 2조사구역이 제 1조사구역보다 등산로에 의한 단편화(fragmentation) 정도가 적을 뿐만 아니라 조사면적 내에 대경급 임목이 산재하여 있는 것과 관련이 높을 것으로 판단된다. 월정사에서 상원사에 이르는 계류 지역은 노랑할미새와 검은댕기해오라기, 물까마귀 등과 같은 조류의 번식을 위해 등산로 주변의 관목층을 보호해야 할 것이다. 적설량이 많은 겨울철에 이 지역의 야생조류에게 인공먹이를 공급하여 다음해의 넉넉한 번식을 꾀하여야 할 것이며, 까막딱다구리 및 올빼미 등 대면적의 행동권을 요구하는 종을 위해서는 서식지의 단편화(fragmentation)를 막아야 할 것이다. 한편, 상원사의 집비둘기 개체군은 둥지자원을 철저히 제거하여 더 이상 귀중한 문화재가 훼손되지 않도록 해야할 것이다.
