An Investigational Study of Nursing College Student's Clinical Practice Satisfaction (3년제 간호대학생의 임상실습 만족도에 관한 조사 연구)

  • The purpose of this study was to investigate the factors and the degree of nursing college student's clinical practice satisfaction. The sample consisted of 488 nursing students who had been in the clinical practice experience from 6 junior nursing colleges in Korea. Data for this study was collected from 20 May to 20 June, 2000. Data were analysed by SPSS/PC using frequency, percentage, mean, standard deviation, T-test, one-way ANOVA, and regression. The results of this study are as follows : 1. The highest motive of selecting a nursing science came to be 'having a good prospect for getting a job' (29.3%), major place of the clinical nursing practice was university hospital(46.5%), and the degree of satisfaction about the nursing science was 'relatively satisfied' (43%). 2.The fields that the highest satisfaction of the nursing student's clinical practice were DR(3.25), NR(3.18), ICU(3.11), and OR(3.10). The highest rank of the nursing student's clinical practice satisfaction was 'feels more continuous nursing studying is required after getting a job or pass the RN exam'(4.24). 3. There was significant value between the Grade(2yr, 3yr) and the satisfaction of the clinical practice environment(p<0.001), the clinical practice interpersonal relationship (p<0.001), the clinical practice evaluation (p<0.05), and there was significant value between the satisfaction about the nursing science and the satisfaction of the clinical practice environment(p<0.001), the clinical practice interpersonal relation ship (p<0.05), the satisfation after the clinical practice(p<0.001). 4. There was significant value between 'the opinion of the clinical practice periods' and the satisfaction of the clinical practice place(p<0.05), the clinical practice environment(p<0.001), the clinical practice interpersonal relationship(p<0.001), the satisfation after the clinical practice(p<0.001). 5.The satisfaction of the clinical practice interpersonal relationship influenced on the nursing student's clinical practice satisfaction($R^{2}=.188$). From these results, it is suggested that the higher the satisfaction of the clinical practice interpersonal relationship be like to improve the nursing college student's clinical practice satisfaction.

The Climacteric Symptoms and Quality of Life in Climacteric Women according to Hormone Replacement Therapy (호르몬 대체요법에 따른 갱년기여성의 갱년증상과 삶의 질)

  • The purpose of this study is to find the difference of the climacteric symptoms and quality of life according to hormone replacement therapy (HRT) in climacteric women. The research design was a descriptive survey with questionaries. A convenience sample of 181 climacteric women aged 45-65 who had climacteric symptoms were selected in Kwangju city, Korea. Sixty-six climacteric women among those subjects were receiving HRT at K university hospital in Kwangju city. Data were collected by the interview with questionaries from Feb 3. 1999 to March 25, 1999. Climacteric symptoms were measured using scores developed by Neugarten(1965) and modified by Park(1989), and the quality of life scale related to climacteric symptoms developed by Hildtich(1996) and modified Kim(1998). Data were analyzed by $x^2$-test, t-test, ANOVA and Pearson's correlation procedures, using SAS/win PC+. The results were as follows ; 1. No statistically significant differences were found between the receiving HRT group and not receiving HRT group in demographic, obstetrical and health related characteristics. 2. There were no significant difference on the climacteric symptoms between the group of women receiving HRT (score 2.4) and the group of women who were not (score 2.6) except psychological area (t= 2.407, p= 0.017). 3. The quality of life of the group of women receiving HRT was significantly higher than that the group of women who were not (t=2.151, p=.032). 4. The quality of life of the group of women receiving HRT were high scores There were significantly high of the quality of life in the vaso-motor change area (t=2.634, p= 0.009), psycho-social change area (t=3.239, p=0.001), and physical change area (t=2.031, p=0,043) in the group of receiving HRT compared to not receiving HRT group. 5. The variables showed significantly differences on the degree of climacteric symptoms of the group of women not receiving HRT were the subjective health state (t=15.81, p=0.000), age (t=7.50, p=0.007), feeling of menopause (t=30.88, p=0,000) and climacteric periods (t=8.66, p=0.003), and receiving HRT were number of para (t=3.95, p=0.050) and feeling of menopause (t=3.94, p=0.050). 6. The variables showed significantly difference on the quality of life of the group of women not receiving HRT were the subjective health state (t=4.14, p=0.044) and feeling of menopause (t=10.86, p=0.001). 7. There were significantly positive correlations between the climacteric symptoms and the quality of life in climacteric women (r=0.512, p=0.000).

The Effect of Hand Massage Program on Anxiety in Patients with Ischemic Heart Diease (손마사지가 허혈성 심질환 환자의 불안에 미치는 효과)

  • The purpose of this study was to investigate the effect of a hand massage program on anxiety, vital sign and pain in clients with ischemic heart disease. The design utilized for this study was quasi-experimental with a nonequivalent control group non-synchronized design The subjects were fifty-four patients, twenty-eight for the experimental and twenty-six for the control group, who were admitted with ischemic heart diease at a cardiac intensive care unit in K medical center of K university. This study was carried out from May, 1999 to March, 2000. The level of anxiety and pain measured by Visual Analogue Scale, systolic blood pressure, diastolic blood pressure and pulse rate were measured before and after hand massage, the state of Anxiety was measured by the Spielberger (1970) scale at admission and after hand massage for three days. The collected data were processed by using the SPSS PC program and analyzed using $\chi^2$-test and t-test. The result of this study are as follows : 1. The scores of VAS anxiety and State of anxiety of the experimental group were lower than those of the control group. 2. The degree of systolic blood pressure, diastolic blood pressure and pulse rate of the experimental group were lower than that of the control group. 3. The score of VAS pain of the experimental group was lower than that of the control group. The results suggested that hand massage can decrease VAS anxiety, State of anxiety, vital signs and VAS pain of patients who were admitted with ischemic heart disease at cardiac intensive care unit Therefore, it is proposed that hand massage is an appropriate nursing intervention to relieve anxiety of the patients who were admitted with ischemic heart disease at a cardiac intensive care unit.

A Study on the Clinical Characteristics of 4-Constitution Groups in 8-Constitution Medicine (8체질의학에서 목$\cdot$$\cdot$$\cdot$수 4체질군의 임상적 특징에 대한 연구)

  • Backgrounds : In recent years there has teen renewed interest in the importance of 8-constitution medicine as evidenced by growth of research in this field. Although diagnosis by 8-constitution depends on pulse types, it still is subjective and thus could be insignificant. Therefore, diagnosis based on objective characteristics is essential. Objectives : The purpose of this study was to find objective characteristics of the 4-constitution groups {木(Mok; hepatonia, cholecystonia), {土(To; pancreotonia, gastrotonia), {金(Gum; pulmotonia, colonotonia), {水(Soo; renotonia, vesicotonia)} in 8-constitution for further diagnosis and therapy. Also, some significant clinical tendencies of 4-constitution groups were studied. Methods : From Nov. 2001 to Apr. 2004, data were collected through a questionnaire given to 720 patients who were classified into one of 8 constitutions according to their pulse types and their responses to constitution-based acupuncture therapy. The questionnaire was composed of 80 items which were about personality, physical shape, habit, physiological and pathological symptoms, response to drugs, favorites and responses to various foods. The data were statistically analyzed by cross-tabulation and one-way analysis of variance, and SPSS V10.0.7PC was used. Results : The results are summarized as follows: 1) Among participants, proportions of hepatotonia, pancreotonia, and cholecystotonia were higher than those of vesicotonia, pulmotonia, colonotonia, renotonia, grstrotonia. 2) 52 items of 80 variances showed significant differences by gender, 3) 13 items were highly correlated with 4-constitution groups in males, and 26 items in fremales. 4) 22 items showed significant group differences among 4-constitution groups in males, and 41 items in females. Also, for each item 4-constitution groups were sorted according to their means, so that the constitutional tendencies were objectively figured out by personality, shape, habit, physiological and pathological symptoms, response to drugs, favorites and responses to foods. Conclusions : Most clinical characteristics of 4-constitution groups found in this study coincided with those from the previous ones in this fold. Thus, classifying patients into one of the 4-constitution groups based on significant objective characteristics is of great benefit to diagnosis and therapy. further study should be followed to improve proper therapy for each constitution.

  • Nerve growth factor(NGF) has a critical role in peripheral nerve regeneration. The aim of this study is to construct a well-functioning hNGF-$\beta$ recombinat adenovirus for the ultimate development of improved method to promote peripheral nerve regeneration with adenovirus mediated hNGF-$\beta$ gene transfection into Schwann cells. First PCR associated cloning of GFP-tagged hNGF-$\beta$ which was ligated into E1/E3 deleted adenoviral vector was performed and tranfected into E. coli to construct hNGF-$\beta$ recombinant adenovirus. After production of recombinat adenovirus in a large scale, its transfection efficiency, expression, and function were evaluated using cell lines or primarily cultured cells of HEK293 cells, Schwann cells, fibroblast(NIH3T3) and myocyte(CRH cells). GFP expression was observed in 90% of infected cells compared to uninfected cells. Total mRNA isolated from hNGF-$\beta$ recombinat adenoviru infected cells showed strong RT-PCR band, however, LacZ recombinant adenovirus infected or uninfected cells did not. NGF quantification by ELISA showed a maximal release of 18.865 +/- 0.31ng/mL at 4th day. PC-12 cells exposed to media with hNGF-$\beta$ recombinant adenovirus infected Schwann cell demonstrated higher levels of differentiation compared with controls. We generated hNGF-$\beta$ recombinant adenovirus and induced over expression of NGF successfully in nonneuronal and neuronal cells. Following these result, it is expected to develop an improved treatment strategy peripheral nerve regeneration using the hNGF-$\beta$ gene transfected cells.

A Study on the Circadian Blood Pressure Rhythm of Diabetic Patients (당뇨병 환자의 혈압 일주기 리듬에 관한 조사연구)

  • This study was performed to investigate the relationship between reversed circadian blood pressure and risk factors of peripheral vascular disease in non-insulin-dependent diabetes mellitus (NIDDM) subjects. The subjects in this study were 18 NIDDM patients who were hospitalized in a medical unit of an university medical center located in Incheon, Korea, between November, 1998 and March, 1999. Blood pressure was measured with a mercury sphygmomanometer by 2 trained examiners every 2 hours during 24 hours. NIDDM subjects were divided into a dipper group and non-dipper group. Dippers are defined as those who show a mean nighttime blood pressure(BP) drop of more than 10% compared with daytime BP. Non-dippers are defined as those who show a mean nighttime BP drop of less than 10%, or an elevation in BP compared with daytime BP. Daytime BP included values obtained between 6 a.m. and 10 p.m. Night time BP included values obtained between 10 p.m. and 6 a.m. Data was analyzed by SPSS/PC package. Chi-square( $^2$) test was used for the comparison of sex between The dipper group and non-dipper group. Mann-Whitney test was used for comparisons of values of the risk factors of peripheral vascular disease and the frequency of complications of diabetes between the dipper group and non-dipper group. The results are as follows. There were no significant differences in daytime systolic, diastolic, and mean blood pressures between the dipper group and non-dipper group. However, night time systolic, diastolic, and mean blood pressures in the non-dipper group were significantly nigher than those in the dipper group (p=.021). There were no differences in sex, age, body, weight, duration of diabetes, serum lipid levels, BUN and HbA1c between the two groups. On the contrary, 87.5% of non-dipper group subjects showed having hypertension, 30% of dipper group subjects showed having hypertension and this difference was statistically significant (p=.018). All of the non-dipper group subjects (N=8) showed having at least one diabetic complication. However, 40% of the dipper group subjects (N=10) showed having no diabetic complication at all and this difference was also statistically significant (p=.049). There were no significant differences in frequency of nephropathy, neuropathy and retinopathy between the dipper group and non-dipper group.

Study on the Development of Modularized Instruction and the Effect of Its Application - Focused on the Asepsis and Elimination Practice - (기본 간호학 실습교육에서 모듈 학습자료 개발과 그 효과 연구 - 감염과 배설에 관한 실습 교육을 중심으로)

  • This study was done to develop self - directed learning modules related to asepsis and elimination including urine and stool for Fundamentals in Nursing practice education contents and to measure the effectiveness of those modules. The subjects of this study were 96 sophomore students in the nursing college. Self-directed learning modules were developed by the researcher on the basis of the Lippincott Learning System of Kruger (1986) and Modules for Basic Nursing Care of Ellis (1992). Videotape was editted by using videotape made by the Lippincott Company and Film strip made by the Trainex Company with Korean dubbing. Self-directed learning was done for one week with the asepsis module and two weeks with the elimination modules after confirming the requiered level of knowledge acquisition through pre-test. For measuring proficiency in self-directed learning, a written test for cognitive domain, a sufficiency test for psychomotor domain, and a confidnece examination for affective domain were given. The data were analyzed using descritive statistics, and Pearson correlation coefficient using a SPSS-PC program. The results are summarized as follows: 1. Sufficiency test and confidence examination grades showed high levels in both asepsis and elimination. 2. Written test grades showed a high level in asepsis and elimination of urine but showed a medium level in eliminationin of stool. 3. Grades of sufficiency and confidence in asepsis and elimination practice were statistically significant with a moderate positive correlation (r=0.4- 0.5, p<0.001). 4. Grades of sufficiency and written tests in asepsis and elimination practice also were statistically significant with a moderate positive correlation (r=0.5-0.7, p<0.001). 5. Students showed relatively high contentment with the self-directed learning modules themselves but revealed relatively low contentment with video program and the self-directed learning process. In conclusion, this study disclosed that proficiency levels in cognitive, affective, and psychomotor domains were high when asepsis and elimination modules were applied. Also students showed high satisfaction with the modules themselves, but didn't show high contentment with the video programs. In considering low contentment with the self-directed learning process, it is estimated the students had experienced some difficulties about using self-directed learning modules because this was their first exposure to the self-directed learning module and they were already accustomed to the demonstration-practice method.

  • Hardware dedicated off-the-shelf maritime differential GPS RSIM lacks the open architecture to meet all the minimum maritime user requirements and to include future GNSS improvements after recapitalization. This paper carries out a study to replace existing hardware dedicated differential GPS RSIM with software differential GPS RSIM in order to make up the weak point of hardware dedicated off-the-shelf maritime differential GPS RSIM. In this paper, the architecture of software RSIM is proposed for maritime DGPS recapitalization. And the feasibility analysis of the proposed software differential GPS RSIM is performed as the first phase to realize the proposed architecture. For the feasibility analysis, the prototype RF module and DSP module are implemented with properties as wide RF bandwidth, high sampling frequency, and high speed transmission interface. This paper shows that the proposed architecture has the possibility of real time operation of software RSIM functionality onto the PC-based platform through the analysis of computation time. Finally, this paper verifies that the L1/L2 dual frequency software differential RSIM designed according to the proposed method satisfies the performance specifications set out in RTCM paper 221-2006-SC104-STD.

  • Advances of Internet and rC accelerate distribution and sharing of information, which make P2P(Peer-to-Peer) computing paradigm appear P2P computing Paradigm is the computing paradigm that shares computing resources and services between users directly. A fundamental problem that confronts Peer-to-Peer applications is the efficient location of the node that stoles a desired item. P2P systems treat the majority of their components as equivalent. This purist philosophy is useful from an academic standpoint, since it simplifies algorithmic analysis. In reality, however, some peers are more equal than others. We propose the P2P protocol considering differences of capabilities of computers, which is ignored in previous researches. And we examine the possibility and applications of the protocol. Simulating the Magic Square, we estimate the performances of the protocol with the number of hop and network round time. Finally, we analyze the performance of the protocol with the numerical formula. We call our p2p protocol the Magic Square. Although the numbers that magic square contains have no meaning, the sum of the numbers in magic square is same in each row, column, and main diagonal. The design goals of our p2p protocol are similar query response time and query path length between request peer and response peer, although the network information stored in each peer is not important.

Application of Gamma Ray Densitometry in Powder Metallurgy

  • The most important industrial application of gamma radiation in characterizing green compacts is the determination of the density. Examples are given where this method is applied in manufacturing technical components in powder metallurgy. The requirements imposed by modern quality management systems and operation by the workforce in industrial production are described. The accuracy of measurement achieved with this method is demonstrated and a comparison is given with other test methods to measure the density. The advantages and limitations of gamma ray densitometry are outlined. The gamma ray densitometer measures the attenuation of gamma radiation penetrating the test parts (Fig. 1). As the capability of compacts to absorb this type of radiation depends on their density, the attenuation of gamma radiation can serve as a measure of the density. The volume of the part being tested is defined by the size of the aperture screeniing out the radiation. It is a channel with the cross section of the aperture whose length is the height of the test part. The intensity of the radiation identified by the detector is the quantity used to determine the material density. Gamma ray densitometry can equally be performed on green compacts as well as on sintered components. Neither special preparation of test parts nor skilled personnel is required to perform the measurement; neither liquids nor other harmful substances are involved. When parts are exhibiting local density variations, which is normally the case in powder compaction, sectional densities can be determined in different parts of the sample without cutting it into pieces. The test is non-destructive, i.e. the parts can still be used after the measurement and do not have to be scrapped. The measurement is controlled by a special PC based software. All results are available for further processing by in-house quality documentation and supervision of measurements. Tool setting for multi-level components can be much improved by using this test method. When a densitometer is installed on the press shop floor, it can be operated by the tool setter himself. Then he can return to the press and immediately implement the corrections. Transfer of sample parts to the lab for density testing can be eliminated and results for the correction of tool settings are more readily available. This helps to reduce the time required for tool setting and clearly improves the productivity of powder presses. The range of materials where this method can be successfully applied covers almost the entire periodic system of the elements. It reaches from the light elements such as graphite via light metals (AI, Mg, Li, Ti) and their alloys, ceramics ($AI_20_3$, SiC, Si_3N_4, $Zr0_2$, ...), magnetic materials (hard and soft ferrites, AlNiCo, Nd-Fe-B, ...), metals including iron and alloy steels, Cu, Ni and Co based alloys to refractory and heavy metals (W, Mo, ...) as well as hardmetals. The gamma radiation required for the measurement is generated by radioactive sources which are produced by nuclear technology. These nuclear materials are safely encapsulated in stainless steel capsules so that no radioactive material can escape from the protective shielding container. The gamma ray densitometer is subject to the strict regulations for the use of radioactive materials. The radiation shield is so effective that there is no elevation of the natural radiation level outside the instrument. Personal dosimetry by the operating personnel is not required. Even in case of malfunction, loss of power and incorrect operation, the escape of gamma radiation from the instrument is positively prevented.
