  • The cytologic distinction of carcinoma cells from reactive mesothelial cells can be difficult, especially in specimens containing abundant reactive mesotheilal cells and inflammatory cells with scant carcinoma cells. This study evaluates the usefulness of mucin and immunocytochemistry for discrimination between reactive mesotheilal cells and carcinoma cells, and sensitivity and specificity of these stains for the detection of metastatic carcinoma in serous effusions. Immunocytochemical panel including mucin cytochemistry with the periodic acid-Schiff(PAS) reaction after or without diastase digestion was undertaken on 127 serous effusion specimens with histologically confirmed diagnoses. The specimens including cell smears and cell blocks were stained with PAS and antibodies to carcinoembryonic antigen(CEA), epithelial membrane antigen(EMA), cytokeratln(CK), and vimentin. The sensitivities of these stains for metastatic carcinoma(127 cases) were 49%(46/94) in PAS, 48%(60/124) in CEA, 89%(97/109) in EMA, 88%(93/106) in CK, and 25%(20/81) in vimentin. The sensitivities of stains for reactive mesothelial cells(36 cases) were 19%(7/36) in EMA, 78%(28/36) in CK, and 75%(27/36) in vimentin. The PAS and CEA stains were not reacted with all cases of benign reactive serous effusions containing abundant reactive mesothelial cells. The specificities of stains for metastatic carcinoma(127 cases) were 100% in PAS, 100% in CEA, 81% in EMA, 22% in CK, and 25% in vimentin. The optimal combination of stains for use in a panel was PAS and CEA. Combined results from these two stains yielded an advanced sensitivity of 8% in PAS and 4% in CEA for metastatic carcinoma. EMA was also cosiderably useful for identification of carcinoma cells. CK and vimentin were not suitable for distinguishing between reactive mesothelial cells and carcinoma cells.

  • Background: The posterior movement of mandible was known as the main cause of the changes in the pharyngeal airway space (PAS) and the postoperative obstructive sleep apnea (OSA). The purpose of this study was to know the changes of PAS and position of hyoid bone. Methods: Lateral cephalographies of 13 patients who had undergone sagittal split ramus osteotomy (SSRO) setback surgery were taken preoperatively (T1), postoperatively within 2 months (T2), and follow-up after 6 months or more (T3). On the basis of F-H plane, diameters of nasopharynx, oropharynx, and hypopharynx were measured. The movements of the soft palate, tongue, and hyoid bone were also measured. Results: The amount of mandible setback was $7.5{\pm}3.8mm$. In the measurements of PAS, there was a statistically significant decrease of $2.8{\pm}2.5mm$ in nasopharynx (P < 0.01), and $1.7{\pm}2.4mm$ in oropharynx (P < 0.01) were observed after surgery. The hypopharynx decreased $1.0{\pm}2.1mm$ after surgery and continuously decreased $1.0{\pm}2.8mm$ at follow-up. The changes in hyoid bone position showed the posterior movement only after surgery and posteroinferior movement at follow-up. Conclusions: The PAS such as nasopharynx, oropharynx, and hypopharynx showed relatively high correlation with the amount of mandibular setback. The change of resistance in upper airway may be important for the prevention of OSA after mandibular setback surgery.

  • Diagnosis of acute appendicitis in children is sometimes difficult. The aim of this study is to validate a clinical scoring system and ultrasonography for the early diagnosis and treatment of appendicitis in childhood. This is a prospective study on 59 children admitted with abdominal pain at St. Mary's Hospital, the Catholic University of Korea from July 2002 to August 2003. We applied Madan Samuel's Pediatric Appendicitis Score (PAS) based on preoperative history, physical examination, laboratory finding and ultrasonography. This study was designed as follows: patients with score 5 or less were observed regardless of the positive ultrasonographic finding, patients with score 6 and 7 were decided according to the ultrasonogram and patients above score 8 were operated in spite of negative ultrasonographic finding. The patients were divided into two groups, appendicitis (group A) and non-appendicitis groups (group B). Group A consisted of 36 cases and Group B, 23 cases. Mean score of group A was 8.75 and group B was 6.13 (p<0.001). Comparing the diagnostic methods in acute appendicitis by surveying sensitivity, specificity, positive predictive value, negative predictive value, and accuracy, PAS gave 1.0000, 0.3043, 0.6923, 1.0000, and 0.7288, and ultrasonography gave 0.7778, 0.9130, 0.9333, 0.7241, and 0.8300 while the combined test gave 1.0000, 0.8696, 0.9231, 1.0000, and 0.9490, respectively. Negative laparotomy rate was 3 %. In conclusion, the combination of PAS and ultrasonography is a more accurate diagnostic tool than either PAS or ultrasonography.

  • The ultrastructure of the integumental epidermis of Korean planaria (Dugesia japonica) is studied by light microscope, scanning electron microscope and transmission electron microscope. The planaria has mono-layered integumental epidermis in which most of cells exhibit irregularly columnar shape. The epidermal cells of the integument are classified into six types on the basis of cytochemical and ultrastructural characteristics. 1) Ciliated epithelial cells: These cells have cilia in their free surfaces. The axonemes of cilia exhibits fundamental 9+2 microtubular pattern. 2) Eosinophilic cells: These cells contain a few large eosinophilic granules. The core of eosinophilic granule is consisted of sparsely dispersed fibrillar structures in relatively electron-lucent ground material. 3) Mucous cells: These cells are filled with irregularly shaped, PAS-positive mucous granules which have an average size of $0.8\\times0.3 \\muM$. 4) Rhabdite-forming cells: These cells possess a few strongly-eosinophilic large rhabdite granules. The rhabdite granules are synthesized either in the rhabdite-forming cells which constitute integumental epidermis or in the corresponding cells which are developed in the parenchyma and later transferred to epidermal cells of integumental epidermis through basement membrane. 5) A-type of basophilic granule cells: These granule cells possess round or irregularly-shaped granules which are strongly stained with Alcian blue. These electron-dense granules have an average size of $1.5\\times1.0 \\muM$. This type of cells is derived from parenchymal tissue. 6) B-type of basophilic granule cells: These basophilic granule cells with PAS-positive granules, are found in the epidermis of lateral body wall. The granules, which are about $0.7\\times0.4 \\muM$ in size, occupying most part of this cell type are originated from the parenchyma.

  • Background: Chlorophenols (CPs) and related phenoxyacetic acids (PAs) are pesticide groups contaminated with highly toxic 2, 3, 7, 8-tetrachlorodibenzo-p-dioxin (TCDD) during production. PAs and CPs exposure is associated with risk of cancer, but the situation regarding lung cancer has not been clearly defined. We proposed a meta-analysis of published researches to evaluate relationship between chronic exposure to PAs and CPs in pesticide production workplaces and the risk of lung cancer. Materials and Methods: After searching PubMed, Scopus, Scholar Google, Web of Sciences until August 2013, the association between chronic PAs and CPs exposure in production workplace and lung cancer was studied in 15 cohort studies. The standardized mortality rate (SMR) and 95% confidence intervals (CI) were collected from the papers. We used random or fixed-effects models, Egger test, funnel plot and meta regression in our analysis. Results: Five papers with six reports were included in the final analysis. The standardized mortality rate for lung cancer from the random model was 1.18 (95% CI: 1.03-1.35, p=0.014) with moderate heterogeneity. Publication bias was not found for included studies in meta-analysis (p=0.9). Conclusions: Our findings has strengthen the evidence of lung cancer from chronic exposure to chlorophenol related compounds (PAs, CPs).

  • The morphological study on the parenchymal cells in the adult planaria performed to observe their cytochemical and ultrastructural characteristics. The results are as follows. Nine types of cells are found in parenchyma. 1. Free parenchymal cell: These cells contain several chromatoid bodies around the nucleus. Heterochromatins are evenly dispersed in large nucleus. These cells are abundant in free ribosomes. 2. Fixed parenchymal cells: These cells have well-developed granular endoplasmic reticulum, mitochondria and Golgi complex but they contain the cytosols exhibiting electron-lucencies. 3. Rhabdite-forming cells: These cells contain the electron-dense rhabdite granules of up to about 0.3 x 0.9 $\mu$m in size. Rhabdite-forming cells have well-developed cell organelles, granular endolplasmic reticulum, mitochondria and Golgy complex. 4. A-type of basophilic granule cells: These cells contain irregularly-shaped granules exhibiting alcianophilia. These granules surrounded by a limited membrane, approximately 1.4 x 0.7 $\mu$m in size, are accumulated in the cytoplasm. 5. C-type of basophilic granule cells: These cells contain electron-dense granules of less than 0.2 $\mu$m in size, which exhibit PAS- positive reaction. This type of granule is also found in the muscle layer of parenchyma. 6. D-type of basophilic granule cells: This type of granule cell occurs only in the parenchyma around reproductive organ. The granules have cytochemical characteristics that they exhibit strongly positive reaction with PAS and weakly eosinophilic property. These electron-dense granules, which are 0.2 to 0.6 $\mu$m in length, have oval shapes. 7. E-type of basophilic granule cells: These cells are found only in the parenchyma around re productive organ. The granules contained in a small number in the cell, exhibit PAS-positive reaction and have an average size of 0. 2pm. 8. Eosinophilic granule cells: These cells contain a large number of eosinophilic granules which have relatively diverse sizes from 0.3 x 0.2 to 0.8 x 0.4 $\mu$m. Most of granules are round or irregularly-shaped and highly electrondense. These cells have an array of well-developed granular endoplasmic reticulum of which cisternae are distened. 9. Transparent granule cells contain electron-lucent granules which exhibit negative reactions with three kinds of cytochemical methods used in this experiment.

  • Just after hatching the embryo has a yolk sac and straighted digestive tract. Just after parturition in 5.0mm TL the mouth and anus were opened. In the larval stage of 5.2mm TL, digestive tract could be divided into pharynx, esophagus, stomach, pyloric caeca and intestine of the early differentiated state. The esophagus of larvae in 5.3mm TL has a PAS positive mucous cell. From over 5.7mm TL, PAS positive goblet cell recognized in the intestine. PAS positive absorptive cell began to appear in the intestine from 5.9mm TL. Yolky materials were absorbed completely in 6.0mm TL. In the larvae of B.5mm TL, gastric glands were observed in the mucosa, but the surface epithelium did not have PAS positive granules. From over 9.0mm TL, the histological structure of esophagus showed similar to adult. In the juvenile stage from 13.0mm to 15.0mm TL, histological structure of the stomach, pyloric caeca and intestine showed similar to adult. From the ultrastructural and histochemical study, it is concluded that the functional digestive tract is present in the juvenile stage from 18.0mm to 20.0mm TL.
