Development of Hospice Care Service Program about Advanced Cencer Patient I - The Effect of Hospice Education Programs on the Death Orentation - (말기 암환자 호스피스 간호에 대한 봉사프로그램 개발 I)

  • The purpose of this study was to investigate what effect providing the hospice care team with hospice education programs had on the death orientation. The subjects of study were 28 volunteers. 14 nurses. 30 clergies who registered on hospice education programs from Aug. 6th. 1996 to May 20th. 1997. The data were analysed by descriptive analysis. ANOVA. Duncan test. paired t-test. The results of the study can be summarized as follows ; 1. The degrees of death orientation were 85.70 in volunteers group. 84.31 in nurses group. and 73.00 in clergies group. So. clergies group has more positive death orientation than others(F=6.33. p=. 000). The degree of death orientation showed significant differences between age groups(F=5.78. p =.002). and religiosity(t=3.92. p=.000). There were no significant differences between the degree of death orientation and the others general characteristics of subjects. 2. The mean of death orientation was 80.04 before hospice education programs. but was 75.56 after hospice education programs(t= 3.92. p= .000). In conclusion. the subjects who received the hospice education programs showed the positive change in the degree of the death orientation. Therefore. it has been judged that education programs has been prerequisite in positive death orientation for hospice care. Furthermore. all of the hospice care members those who complete the hospice education program. will be performed efficient hospice care intervention for dying patients and their families.

The effect of calcium ion concentrations in the medium and the treatment of caffeine and Ca-ionophore A23187 on in vitro capacitation of bull spermatozoa (배양액중의 calcium 이온 농도 및 caffeine과 Ca-ionophore A23187 처리가 소정자의 수정능획득에 미치는 영향)

  • The present study was performed to investigate the effect of Ca ion concentration on sperm viability and acrosome reaction rate and to evaluate the effect of treatments using caffeine and Ca-ionophore A23187 on acrosome reaction rate in frozen-thawed bull spermatozoa. Viabilities of in vitro capacitated bull spermatozoa at 0, 2.25 and 4.5 mM Ca ion concenrations were 21.00, 26.00 and 22.59%, respectively and significantly higher in Ca ion 2.25mM added group than Ca ion free group (p<0.05) and acrosome reaction rates of in vitro capacitated bull spermatozoa were 17.09, 52.15 and 47.92%, respectively and significantly high in Ca ion added groups(p<0.05). Viabilities in vitro capacitation by caffeine and Ca-ionophore A23187 in control, caffeine treated group, Ca-ionophore A23187 treated group and caffeine+Ca-ionophore A23187 treated group were 37.91, 27.67, 22.33 and 25.59%, respectively and significantly higher in control than treated groups(p<0.05), there were no significant differences among the treated groups, and acrosome reaction rates were 10.33, 37.92, 48.09 and 57.17%, respectively and there were significant differences among the groups(p<0.05), especially higher in caffeine+Ca-ionophore A23187 treated group than others.

Convergent Factors Affecting Problem Behaviors in Out-of-school Adolescents: A Focus on Gender Difference (학교 밖 청소년의 문제행동 관련 융복합적 요인: 성별차이를 중심으로)

  • The objective of this study was to investigate the problem behavior and its convergent factors in out-of-school adolescents, with a focus on gender differences. This study was a secondary data analysis study using out-of-school adolescents research data at Busan women and family development institute. The study was conducted in a total of 499 out-of-school adolescents (337 males, 162 females). The type of the 8 problem behaviors (run away from home, drop out, prostitution, violence, internet game addiction, theft, drug addiction, and smoking) were identified. The collected data were analyzed with multiple logistic regression. Among the problem behaviors of the participants, internet game addiction and theft were more significantly high in male out-of-school adolescents than female out-of-school adolescents. In internet game addiction, male out-of-school adolescents were 1.90 times higher than female out-of-school adolescents (p=.008, 95% CI=1.18-3.06). In theft, male out-of-school adolescents were 1.92 times higher than female out-of-school adolescents (p=.006, 95% CI=1.21-3.03). When the social measures were provided for those adolescents, a distinguished approach is required depending on the problem behavior and gender.

The Effects of Team-Based Learning on Outcome based Nursing Education (성과기반 간호교육에서 팀기반학습 적용의 효과)

  • This study was performed to develop and evaluate the effects of Team-Based Learning(TBL) on cardiac problem in adult nursing. The study measured problem-solving ability, critical thinking disposition, academic achievement and, class satisfaction for learning outcomes of course. Pretest-posttest design with nonequivalent control group was utilized to analyze the effects of TBL. Experimental group and control group each consisting of 52, 54 participants was picked from the junior students. The experimental group was given the TBL for 6 hours and control group was given a lecture and case-based announcement class. In results, significant improvements were found in problem-solving ability(F=5.92, p=.017), and class satisfaction(F=5.80, p=.018). Critical thinking disposition and academic achievement of experimental group were improved than those of control group, but there were not statistically significant difference.

HIV Knowledge and Attitude and Its Related Factors of Cambodian Adolescents (캄보디아 청소년의 HIV 지식 및 태도와 영향요인)

  • This describes the level of knowledge and attitudes about HIV and their related factors among Cambodian adolescents. A cross-sectional design was used to examine the knowledge and attitude of 463 Cambodian high school students using HIV-KQ-18 (HIV-Knowledge Questionnaire-18) and HIV/AIDS Behavior Surveillance Survey Index (measuring attitude about HIV). The majority of the adolescents had a relatively low level of overall HIV knowledge (6.70 ± 3.66 (range: 0-16)) and held a very negative attitude (1.92 ± 0.87 points (range: 0-4)) toward the disease. Using multiple regression analysis, being male (β = 0.28, p < 0.001) and using YouTube as a social network service (β = 0.33, p = 0.035) were found to be independent factors associated with higher level of HIV knowledge. Study findings suggest the importance of informing policymakers and school nurses about the need to develop a and require a culturally sensitive specific health education program on HIV for Cambodian adolescents.

Validation of Critical Care Non-verbal Pain Scale for Critically Ill Patients (중환자 통증사정 도구의 타당성 평가)

  • Purpose: This study was done to examine predictive validity of Critical Care Non-verbal Pain Scale (CNPS) and develop criteria for pain assessment using CNPS with critically ill patients who have communication problems. Methods: Data were collected from intensive care units at three major general hospitals in Seoul and Kyunggi province. During each observation, a nurse assessed pain severity using CNPS ratings (range 0-9) at four treatment stages: at rest, during central catheter dressing change (nonpainful procedure), position change and suctioning (routine painful procedures). Patients also assessed their pain using a self-report 4-point VRS-4. Results: There were significant differences between the four treatment stages except between "at rest" and "nonpainful procedure". Strong correlations were found between CNPS and VRS-4 for "at rest" (r=.552, p<.001), central catheter dressing change (r=.505, p<.001), position change (r=.709, p<.001), and suctioning (r=.662, p<.001). ROC curve analysis of CNPS based on 3 point on VRS-4 showed the cutoff point was 3 for CNPS, the starting point for pain management with 73% sensitivity, 92.2% specificity, 73% positive predictive value, and 92.8% negative predictive value. Conclusion: Results indicate that CNPS is a valid tool for measuring pain in critically ill patients with communication problems and 3 point should be the standardized pain treatment point.

Vegetation Structure Analysis and Ecological Distance of Pinus densiflora Community in Chayang-chon Area, Soraksan National Park (설악산국립공원 자양천지역 소나무림 군집구조 및 생육거리)

  • To study vegetation structure and distance between trees of Pinus densiflora forest in Chayang-chon area, Soraksan National Park, thirty-nine plots(10m$\times$10m) were established. They were classified into two communities by TWINSPAN and DCA technique. In the Community I, P. densiflora(DBH 35~75cm, height 9~16m, age 60~80) dominated in canopy and were predicted to be changed by deciduous broad-leaved trees also. In the Community II, P. densiflora(DBH 9~26.5cm, height 9~16m, age 30~50m) dominated in canopy. It seemed that the community II would maintain P. densiflora community for a long time since P. densiflora dominated in both canopy and understory. Shonnon's diversity index was higher in the community II(1.4247) than in the community I(1.2978). The distantes between canopy trees were 4.92$\pm $2.14m in the community I and 2.41$\pm $0.97m in the community II. The regression between DBH in canpy and ecological distance was Y(Distance) = 0.06355$\times $(DBH)+1.51613.

Role Expectation, Role Performance, and Role Conflict among Nurses Working in Social Welfare Facilities (사회복지시설 간호사의 역할기대, 역할수행, 역할갈등에 관한 연구)

  • Purpose: The aim of this study was to investigate the perceptions of nurses regarding their roles in social welfare facilities, including role expectation, role performance, and role conflict and its influencing factors. Methods: Data were collected by administering a structured questionnaire to 92 nurses working in 5 types of social welfare facilities. Descriptive statistics, paired t test, Pearson correlation coefficient, and multiple linear regression analyses were performed using the SPSS Win 18.0 program. Results: The mean score of role expectation ($4.44{\pm}0.41$) was significantly higher(t =17.50, p<.001) than that of role performance ($3.46{\pm}0.005$). The biggest mean difference between role expectation and role performance was found in "research activities" ($2.92{\pm}0.81$). The mean score of role conflict was $2.89{\pm}0.66$, with the highest mean score found in "conflict caused when one nurse takes up two or more roles" ($3.31{\pm}0.69$). The influencing factors on role conflict were the difference between role expectation and role performance(${\beta}=.45$, p<.001), and facility size (${\beta}=-.37$, p<.001), which accounts for 51.5% incidence of role conflict controlling nurses' age, career, position, and working periods in social welfare facilities (F=17.13, p<.001). Conclusion: The nurses working in the social welfare facilities perceived some restrictions on their role performance compared with their role expectation, this difference being a major factor influencing their role conflict. Therefore, future studies need to investigate interventions to minimize this effect.

A Study on the Characteristics Analysis and Design of High Sensitivity Silicon Photodiode for Laser Detector (레이저 검출용 고감도 실리콘 포토다이오드 제조 및 특성 분석에 관한 연구)

  • In order to improve spectrum sensitivity of photodiode for detection of the laser wavelength at 850 nm ~ 1000 nm of near-infrared band, this study has produced silicon-based photodiode whose area is $5000{\mu}m{\times}2000{\mu}m$, and the thickness is $280{\mu}m$. It was packed by the TO-5 type. The electrical properties of the dark currents have valued of approximately 0.1 nA for 5 V reverse bias, while the capacitance showed 32.5 pF at frequency range of 1 kHz and about 32.4 pF at the range of 200 kHz for 0 V. In addition, the rising time of output signal was as fast response as 20.92 ns for 10V. For the optical properties, the best spectrum sensitivity was 0.57 A/W for 890 nm, while it was relatively excellent value of 0.37 A/W for 1,000 nm. Over all, there were good spectrum sensitivity for this diode over the range of 870 ~ 920 nm.

  • Among 74 patients who came to pain clinic to treat their backache, 37 patient were randomly allocated to experimental(finger-pressure therapy)group and another 37 patients were allocated to control(transcutaneous electrical nerve stimulator)group. The extent of backache was compared before and after experiment by ANCOVA test at both groups(p<0.005). The reduction of pain level by visual analogue scale at experimental group was statistically significantly bigger than control group when motion with front flexion(1.92), extension after front flexion(2.22), right flexion(1.23), and left flexion(1.21)(p<0.05).
