Antidepressant Prescription Patterns in Bipolar Disorder: a Nationwide, Register-based Study in Korea

  • Background: The role of antidepressants (ADs) in bipolar disorder is long-standing controversial issue in psychiatry. Many clinicians have used ADs as a treatment for bipolar depression, and the selection of therapeutic agents is very diverse and inconsistent. This study aimed to examine recent AD prescription patterns for patients with bipolar disorder in Korea, using the nationwide, population-based data. Methods: This study utilized the Korean nationwide, whole population-based registry data of the year 2010, 2011, and 2013. All prescription data of the ADs, antipsychotics, and mood stabilizers of the sampled patients diagnosed with bipolar disorder (n = 2,022 [in 2010]; 2,038 [in 2011]; 2,626 [in 2013]) were analyzed for each year. Results: Annual prescription rate of ADs was 27.3%-33.6% in bipolar disorder, which was gradually increasing over the 3-year period. The combination pattern of ADs and antipsychotic drugs tended to increase over 3 years. The proportion of females and the prevalence of comorbid anxiety disorder were significantly higher in AD user group in all three years. Among individual ADs, escitalopram was prescribed most frequently, and fluoxetine and bupropion were prescribed to the next many patients. The mean duration of bipolar depressive episodes was 135.90-152.53 days, of which ADs were prescribed for 115.60-121.98 days. Conclusion: Our results show prescription rate of ADs in bipolar disorder was maintained at substantial level and increased in recent 3 years. More empirical data and evidence are needed to establish practical treatment consensuses.

Analysis of the Characteristic of Railroad(level-crossing) Accident Frequency (철도 건널목 사고의 발생빈도 특성분석 연구)

  • Railroad traffic accident consists of train accident, level-crossing accident, traffic death and injury accident caused by train or vehicle, and it is showing a continuous downward trend over a long period of time. As a result of the frequency comparison of train accidents and level-crossing accidents using the railway accident statistics data of Railway Industry Information Center, the share of train accident is over 90% in the 1990s and 80% in the 2000s more than the one of level-crossing accidents. In this study, we investigated time series characteristic and short-term prediction of railroad crossing, as well as seasonal characteristic. The analysis data has been accumulated over the past 20 years by using the frequency data of level-crossing accident, and was used as a frequency data per month and year. As a result of the analysis, the frequency of accident has the characteristics of the seasonal occurrence, and it doesn't show the significant decreasing trend in a short-term.

Analysis of Addition Power for New Wearer of Progressive Addition Lenses (누진렌즈 안경 처음 착용자의 가입도 분석)

  • Purpose: To analyse the addition power of new wearer of progressive addition lenses. Methods: Data of 636 subjects who have been prescribed progressive addition lenses as the first time were used for analyse. The range of age for was between 41~78 years old and they visited the optical practice in Gwangju metropolitan city from 2001 to 2013Date of refractive state, gender and age were analysed. Results: The difference of addition by gender was 1.71 D in male and 1.67 D in women. The difference of addition by refractive error was 1.67 D in emmetropic patients and 1.74 D in myopic patients, 1.90 D in hyperopic patients. The difference of addition by age was1.26 D in 41~44 years old sge group, 1.48 D in 45~49 years old age group,1.72 D in 50~54 years old age group 1.84 D in 55~59 years old age group, 2.10 D in 60~64 years old age group and 2.43 D in over 65 years old age group. The difference of addition by diopter in myopic patients was 1.58 D in low myopic patients and 1.48 D in middle myopic patients, 1.67 D in high myopic patients. The difference of addition by axis of astigmatism was 1.80 D in with-the-rule astigmatism, 1.64 D in against-the-rule astigmatism and 1.65 D in oblique astigmatism. Conclusions: The Addition power of progressive lenses were different according to the types of refractive error, astigmatism axis and age.

Rapidly growing pediatric trampoline-related injuries in Korea: a 10-year single center retrospective study

  • Purpose: Several published policy statements have warned against the risks associated with trampoline use and recommended safety guidelines. However, few studies have focused on trampoline-related injuries in Korea. This study aimed to assess the incidence and characteristics of pediatric trampoline-related injuries presented to Ulsan University Hospital. Methods: We retrospectively reviewed the medical records of children aged <16 years with trampoline-related injuries who visited our Emergency Department between 2008 and 2017. Results: Over the 10-year period, 178 trampoline-related injuries were reported, which represented a significant increase (P=0.016). Most (87.6%) of the injuries occurred during the last 5 study years, and a rapid increase in injuries was observed in children aged <6 years. Lower extremity injuries (62.4%) were the most common, followed by injuries of the upper extremities, head and face, and trunk, including injuries to the neck and spine. Sixty-seven children (37.6%) had fractures, and proximal tibia fractures were the most common. Fractures were significantly more common in younger children (<6 years old) than in older children (P=0.026). Conclusion: In Korea, the mechanism of trampoline injury is similar to that of injuries incurred in indoor trampoline parks but is characterized by smaller spaces and multiple users. Trampoline use and the incidence of trampoline-related injuries in children aged <6 years are increasing rapidly. Prohibiting the use of trampolines for children aged <6 years, restricting simultaneous use by multiple children, and ensuring adult supervision should be strictly emphasized. Public awareness and policy guidelines are needed to reduce the incidence of trampoline-related injuries.

Surgical Experience of Infratentorial Meningiomas : Clinical Series at a Single Institution during the 20-Year Period

  • Objective : Based on surgical outcomes of patients with infratentorial meningiomas surgically treated at our institution, we analyzed the predictors for surgical resection, recurrence, complication, and survival. Methods : Of surgically treated 782 patients with intracranial meningioma, 158 (20.2%) consecutive cases of infratentorial location operated on between April 1993 and May 2013 at out institute were reviewed retrospectively. The patients had a median age of 57.1 years (range, 16-77 years), a female predominance of 79.7%, and a mean follow-up duration of 48.4 months (range, 0.8-242.2 months). Results : Gross total resection (Simpson's grade I & II) was achieved in 81.6% (129/158) of patients. Non-skull base location was an independent factor for complete resection. The recurrence rate was 13.3% (21/158) and the 5-, 10-, and 15-year recurrence rates were 8.2%, 12.0%, and 13.3%, respectively. Benign pathology, postoperative KPS over than 90, low peritumoral edema, and complete resection were significantly associated with longer recurrence-free survival rate. The 5-, 10-, and 15-year survival rates were 96.2%, 94.9%, and 94.9%, respectively. Benign pathology, postoperative KPS over than 90 and complete resection were significantly associated with a longer survival rate. The permanent complication rate was 13% (21/158). Skull base location and postoperative KPS less than 90 were independent factors for the occurrence of permanent complication. Conclusion : Our experience shows that infratentorial meningiomas represent a continuing challenge for contemporary neurosurgeons. Various factors are related with resection degree, complications, recurrence and survival.

The Relationship Between Self-Perceived Benefits From Singing and Quality of Life in Older Adults Aged Over 75 Years (75세 이상 노인이 지각하는 노래부르기의 혜택과 삶의 질)

  • The purpose of this study was to examine the relationship between self-perceived benefits from singing and the quality of life of older adults aged 75 years and older. A total of 231 older adults participated in this study with 141 of them living alone and 90 of them living with others. Each participant was instructed to complete the investigator-constructed questionnaire and the Quality of Life scale. The questionnaire consisted of 49 items on demographic information, involvement in leisure activities and singing-related experiences, and perceived benefits from singing. The results showed that the amount of involvement in leisure activities and the measured quality of life were found to differ between the two subgroups of older adults living with and without others. The number of older adults who had singing-related experiences was also differed between the groups. A positive correlation was found between the amount of self-perceived benefit from singing and quality of life. These results indicate that self-perceived benefits from singing could be an important factor for the quality of life of older adults. This study supports the potential benefits of incorporating singing as an active leisure activity into interventions for older adults, expanding the research that has been extensively based on listening-based interventions. Also, it presents clinical implications for how singing interventions and programs could be designed and suggested to support this population to better cope with their life issues and enhance the quality of life.

Research on the Hematological Changes in Accordance with Radiation Dose and Radiation Exposure period of the Medical Radiation Workers (의료 방사선 종사자의 피폭기간 및 피폭선량과 혈액성분 변화에 대한 조사)

  • In this study, we analyzed the effects of radiation exposure, as compared to the hematological parameters change of medical radiation workers and the public. The mean value of all hematological parameters were in the normal range. Eosin mean value of the radiation workers($2.52{\pm}1.79%$) showed that a significantly lower than the control group($2.92{\pm}1.39%$). In the comparison of the results depending on the occupation period, it showed high value that the mean of the radiation workers group WBC, platelet, Lymph, Mono, Baso. Over 20 years of radiation workers WBC, Mono showed low values and less than 10 years of radiation workers mean value of Baso showed low values, there was no statistical significance. In the comparison of the results depending on the 4 years cumulative radiation dose, Over 5.0 mSv of Radiation works RBC($4.61{\pm}0.53$ vs $4.91{\pm}0.38$), Hct($41.51{\pm}4.07$ vs $43.97{\pm}3.40$), Eosin($1.74{\pm}1.14$ vs $2.92{\pm}1.39$) showed low value, it was statistical significance. 0.5~1.0 mSv radiation exposure workers Hb ($13.93{\pm}1.75$) showed a significantly lower value than that of the control group ($14.90{\pm}1.29$).

Ecological Characteristics of Tachanovsky's Gudgeon, Ladislabia taczanowskii in Songcheon Stream, Korea (송천에 서식하는 새미(Ladislabia taczanowskii)의 생태 특성)

  • This study surveyed the ecological characteristics of Ladislabia taczanowskii at Songcheon Stream from March to November 2019. The species inhabited upstream, where the riverbed structure was mostly covered with boulders and cobbles. The water depth was 31-148 cm, and the stream velocity was fast at 0.94±0.23 (0.51-1.39) m/sec. The gender ratio of females to males was 1 : 0.89. The age according to the total length frequency distribution indicated that the group with 38-70 mm below in total length was one year old, the group with 70-100 mm was two years old, the group with 100-120 mm was three years old, and the group over 120-128 mm was over four years old. The total length of sexually mature fishes was 70 mm for females and 75 mm or more for males. The spawning season was from May to August, and the water temperature was 15.5-20.1℃ during the period. The prosperous spawning season was from June to July, and the water temperature was 15.8-17.2℃ during the period. The spawning ground was where the riffle began in the pool, and its bottom was formed of sand and gravel. The width was about 150 cm, and the water depth was 20 to 50 cm. The average number of eggs in the ovaries was 821 (401-1,314) per matured female, and the matured eggs were yellowish and spherical with a mean diameter of 1.62±0.02 (1.43-2.01) mm. The live foods of L. taczanowskii Cyanophyta, Chlorophyta, Bacillariophyta, Ephemeroptera, and Diptera. The feeding habit of L. taczanowskii is omnivorous, but more than 90% of the stomach content was attached algae.

The Association between Resting Heart Rate and Colorectal Cancer Prevalence in Korean adults: The Korea National Health and Nutrition Examination Survey (KNHANES) 2005-2014 (한국 성인의 안정시 심박수와 대장암 유병율의 관계: 국민건강 영양조사 2005년부터 2014년 자료를 바탕으로)

  • The purpose of this study was to examine the association between resting heart rate (RHR) and the prevalence of colorectal cancer in Korean adults. A cross sectional analysis was performed using the Korea National Health and Nutrition Examination Survey (2005 to 2014). Total number of subjects were 10,564 adults aged over 19 years. The results showed that individuals in the highest quartile of RHR had a significantly higher prevalence of colorectal cancer (Odds ratio (OR) 2.27, 95% confidence interval (CI): 1.34-3.85 to OR 3.00, 95%CI: 1.30-6.92) compared to those in the lowest quartile. Futhermore, every 10 beat per minutes (bpm) increase in RHR was associated with 27% (95%CI: 1.03-1.58) increased prevalence of colorectal cancer. In addition, after stratification by age and gender, the result showed that older people (>65 years) in the highest quartile of RHR had a significantly higher prevalence of colorectal cancer compared to those who were in the lowest quartile of RHR. (male OR 3.19, 95%CI: 1.10-9.24 to OR 3.38, 95%CI: 1.18-9.73; female OR 2.90, 95%CI: 1.13-7.42 to OR 5.59, 95%CI: 1.20-25.99). In this study, we examined the feasibility of RHR as a predictive factor of colorectal cancer prevalence. Moreover, we found that RHR was closely related to the prevalence of colorectal cancer in the age of over 65 years.

Risk Factor Analysis of Morbidity and 90-Day Mortality of Curative Resection in Patients with Stage IIIA-N2 Non-Small Cell Lung Cancer after Induction Concurrent Chemoradiation Therapy

  • Background: Major pulmonary resection after neoadjuvant concurrent chemoradiation therapy (nCCRT) is associated with a substantial risk of postoperative complications. This study investigated postoperative complications and associated risk factors to facilitate the selection of suitable surgical candidates following nCCRT in stage IIIA-N2 non-small cell lung cancer (NSCLC). Methods: We conducted a retrospective analysis of patients diagnosed with clinical stage IIIA-N2 NSCLC who underwent surgical resection following nCCRT between 1997 and 2013. Perioperative characteristics and clinical factors associated with morbidity and mortality were analyzed using univariable and multivariable logistic regression. Results: A total of 574 patients underwent major lung resection after induction CCRT. Thirty-day and 90-day postoperative mortality occurred in 8 patients (1.4%) and 41 patients (7.1%), respectively. Acute respiratory distress syndrome (n=6, 4.5%) was the primary cause of in-hospital mortality. Morbidity occurred in 199 patients (34.7%). Multivariable analysis identified significant predictors of morbidity, including patient age exceeding 70 years (odds ratio [OR], 1.8; p=0.04), low body mass index (OR, 2.6; p=0.02), and pneumonectomy (OR, 1.8; p=0.03). Patient age over 70 years (OR, 1.8; p=0.02) and pneumonectomy (OR, 3.26; p<0.01) were independent predictors of mortality in the multivariable analysis. Conclusion: In conclusion, the surgical outcomes following nCCRT are less favorable for individuals aged over 70 years or those undergoing pneumonectomy. Special attention is warranted for these patients due to their heightened risks of respiratory complications. In high-risk patients, such as elderly patients with decreased lung function, alternative treatment options like definitive CCRT should be considered instead of surgical resection.
