A Study on the Health Education Needs-Assessment of Middle Aged Women (중년여성의 보건교육 요구도에 관한 연구)

  • This study has been attempted to measure health education needs-assessment of middle-aged women and to analyze the factors affecting health education needs-assessment. There were 618 subjects(middle-aged women, 40 to 59 years of age). The major findings of the study were as follows: 1. Seventy six point six percent of the respondents wanted health education. The remainder did not want health education because of the lack of the time. 2. For those desiring to receive health education, there was statistically significant difference in education(p〈0.001), income(p〈0.05), employment(p〈0.05), recreation(p〈0.001), knowledge on health(p〈0.001), routine medical check-ups(p〈0.01) and health education experience(p〈0.001). 3. Forty nine percent of the respondents wanted formal education as a way of education and 41.7% wanted life-long education center for the place of education. Sixty two point five percent of responded that ‘once a month’ is adequate for the health education, and 62.7% felt that ‘around one hour’ duration is appropriate. 4. The area the respondents were most interested in was disease control especially in the order of cancer and osteoporosis. 5. There was statistically significant difference between the age group of 40's and 50's in personal health care and environmental health area(p〈0.01), weight control area(p〈0.01), mental health and exercise area(p〈0.05), drinking and smoking area(p〈0.01). 6. The more knowledge on health, the higher health education needs-assessment. Middleaged women who received routine medical check-ups had more health education needs-assessments than those who didn't.

A Study on Recognition and Using behavior for Tofu(Soybean Curd) (두부에 대한 인식 및 이용실태에 관한 연구)

  • To investigate of the recognition and using behavior on the Tofu, 46.3% of male and 53.7% of female adults in Daegu and Kyungbuk area were surveyed. The main results are as follows : 96.4% of the participants usually preferred the Tofu owing to health and about 68.4% of them had purchased Tofu made of domestic soybean. In addition, the responses to 7 questions about the Tofu were also measured on 5 point Likert scale. The item "Tofu is healthy food" and "Tofu is digestive food" were totally received the high point but "Tofu prevents osteoporosis" earned the lowest. Female had generally better knowledge of Tofu than the counterparts. Most participants relatively knew and had eaten common Tofu, soft bean curd, uncurdled bean curd, fried bean curd against functional Tofu such as Tofu added with omija, surimi, spinach, etc. But they intended to eat functional Tofu such as Tofu added with green tea, black sesame, spinach, omija. Furthermore, 60.7% replied "increasing" prospects for the Tofu consumption. 40.0% of adults said the plan should be "Make of good quality domestic soybean" for the purpose of better improvement to consume.

  • Steroid 21-hydroxylase deficiency is the most prevalent form of congenital adrenal hyperplasia (CAH), accounting for approximately 95% of cases. With the advent of newborn screening and hormone replacement therapy, most children with CAH survive into adulthood. Adolescents and adults with CAH experience a number of complications, including short stature, obesity, infertility, tumor, osteoporosis, and reduced quality of life. Transition from pediatric to adult care and management of long-term complications are challenging for both patients and health-care providers. Psychosocial issues frequently affect adherence to glucocorticoid treatment. Therefore, the safe transition of adolescents to adult care requires regular follow-up of patients by a multidisciplinary team including pediatric and adult endocrinologists. The major goals for management of adults with 21-hydroxylase deficiency are to minimize the long-term complications of glucocorticoid therapy, reduce hyperandrogenism, prevent adrenal or testicular adrenal rest tumors, maintain fertility, and improve quality of life. Optimized medical or surgical treatment strategies should be developed through coordinated care, both during transition periods and throughout patients' lifetimes. This review will summarize current knowledge on the management of adults with CAH, and suggested appropriate approaches to the transition from pediatric to adult care.

Estrogenic Activity of Leguminosae Species in Korea using MCF-7 Cells (유방암 세포주를 이용한 국내 자생 콩과식물의 에스트로겐 활성검색)

  • Leguminosae plants are known for its phytoestrogen constituents which play a major role in the prevention of osteoporosis, cancer and heart disease. In this study, the estrogenic activity of 158 samples from 58 species, 3 subspecies and 10 varieties of Leguminosae plants growing in Korea was evaluated. An estrogen, 17β-estradiol was used as a reference compound, and the potency of each sample was expressed in relative efficacy (%) compared to that of the reference by a reporter gene assay using MCF-7 cells. As results, the estrogenic activity of methanolic extracts of Phaseolus vulgaris var. humilis, Sophora flavescens, Lespedeza × robusta, Indigofera pseudotinctoria, Maackia amurensis, Glycine soja, Wisteria floribunda, Robinia pseudoacacia, Astragalus sinicus, Pueraria lobata, Lespedeza maximowiczii var. tomentella, Trifolium repens and Crotalaria sessiliflora showed similar to or higher at 100 ㎍/ml than the positive control at 10 nM. These findings can be a potential evidence for developing estrogen alternatives resolving various types of menopause symptoms with information on proper harvest season and usage plant part. To the best of our knowledge, the estrogenic activity of Lespedeza × robusta, Indigofera pseudotinctoria, Wisteria floribunda, Robinia pseudoacacia and Lespedeza maximowiczii var. tomentella is reported for the first time in this study.

Correlation Analysis of the Lumbar Spine and Femur Neck BMD using Dual Energy X-ray Absorptiometry in Rehabilitated Patients (재활치료 환자에서 DXA를 이용한 요추부와 대퇴경부 골밀도 검사의 상관관계)

  • Average life expectancy is getting longer due to medical developments and improvements in living standards. So much so that the elderly have an increased risk of developing osteoporosis. Therefore, it is important to prevent, diagnose, and treat the senile disease at an early stage through a bone density test. Bone density is measured by dual energy X-ray absorption (DXA). In this study, while using DXA, in cases when the measurements for both the lumbar and the femur could not be taken simultaneously, the correlation between both measurements were known, and the measurement of one area was used to make a clinical inference for the value of the other. Measurements were taken using Lunar Prodigy Advance (GE) for 43 participant with clinically significant fractures. Statistical calculations were produced and analysed regarding bone density. In case of T-score, lumbar spine produced a statistical result of $-2.112{\pm}1.836$ and femur neck was $-1.716{\pm}1.565$. In case of Z-score lumbar spine produced a statistical result of $-0.151{\pm}1.513$, and femur neck $-0.026{\pm}1.283$. It is indicated that the pearson correlation coefficient of T-score between lumbar spine and femur neck is high at 0.699, and the pearson correlation coefficient of Z-score is considered relatively high at 0.503. The correlation of bone density between lumbar spine and femur neck is shown to be statistically meaningful in T-score's p-value at 0.000 and Z-score's p-value at 0.001. In conclusion, it seems to have clinical usefulness that we can infer the result of one measurement through that of the other part tested, based on the knowledge of the correlation coefficients between lumbar spine and femur neck.

Evaluation of Consumer Nutrition Education Program to Reduce Sodium Intake Based on Social Cognitive Theory (사회인지론에 근거한 나트륨 섭취 줄이기 소비자 영양교육 프로그램의 효과 평가)

  • Objectives: This study was performed to evaluate the consumer education program for reducing sodium intake based on social cognitive theory (SCT) and investigate consumer perceptions of environmental, cognitive and behavioral factors. Methods: Consumers (n=4,439) were recruited nationwide in Korea to participate in a nutrition education program for reducing sodium intake which was targeted on senior housewives (SH), parents (P), and office workers (OW). Questions regarding main factors of SCT were asked both before and after the education program. Results: SH and P recognized external social efforts and information to reduce sodium including nutrition labeling more than OW. The main barriers to practice reducing sodium intake were limited choice of low sodium food and menu, interference with social relationship when dining with others, and limited information, knowledge and skills. SH had lower barriers to practice reducing sodium intake and OW perceived 'preference to soup or stew' and 'preference to Kimchi, salted fish and fermented sauces' as barriers more than other groups at the baseline. Less than 50% of participants knew the relationship between sodium and salt, sodium in nutrition labeling, and recommended sodium intake. In addition, OW had little knowledge for capability to reduce sodium intake and lower self-efficacy to practice compared with SH and P. After education, positive outcome expectations such as lowering blood pressure, prevention of cardiovascular disease and osteoporosis were increased and barriers to practice reducing sodium intake were decreased in all groups (p < 0.05). The knowledge for behavioral capability and self-efficacy to reduce sodium intake were also improved but OW had still lower scores compared with other groups. Conclusions: These results suggested that nutrition education programs could be an effective tool to impact general population by facilitating awareness and increased capability to reduce sodium intake.

Straight-Forward versus Bicortical Fixation Penetrating Endplate in Lumbosacral Fixation-A Biomechanical Study

  • Objective : Many lumbosacral fixation techniques have been described to offer a more screw-bone purchase. The forward anatomical fixation parallel to the endplate is still the most preferred method. Literature revealed little knowledge regarding the mechanical stability of lumbosacral trans-endplate fixation compared to the traditional trans-pedicular screw fixation method. The aim of this study is to assess the pull-out strength of lumbosacral screws penetrating the end plate and comparing it to the conventional trans-pedicular screw insertion method. Methods : Eight lumbar and eight sacral vertebrae, with average age 69.4 years, Left pedicles of the 5th lumbar vertebrae were used for trans-endplate screw fixation, group 1A, right pedicles were used for anatomical trans-pedicular screw fixation, group 1B. In the sacral vertebrae, the right side S1 pedicles were used for trans-endplate fixation, group 2A, left side pedicles were used for anatomical trans-pedicular screw fixation, group 2B. The biomechanical tests were performed using the axial compression testing machine. All tests were applied using 2 mm/min traction speed. Results : The average pull-out strength values of groups 1A and 1B were $403.78{\pm}11.71N$ and $306.26{\pm}17.55N$, respectively. A statistical significance was detected with p=0.012. The average pull-out strength values of groups 2A and 2B were $388.73{\pm}17.03N$ and $299.84{\pm}17.52N$, respectively. A statistical significance was detected with p=0.012. Conclusion : The trans-endplate lumbosacral fixation method is a trustable fixation method with a stronger screw-bone purchase and offer a good alternative for surgeons specially in patients with osteoporosis.

Effect of Cytokines and bFGF on the Osteoclast Differentiation Induced by $1\;{\alpha},25-(OH)_2D_3$ in Primary Murine Bone Marrow Cultures

  • Bone is a complex tissue in which resorption and formation continue throughout life. The bone tissue contains various types of cells, of which the bone forming osteoblasts and bone resorbing osteoclasts are mainly responsible for bone remodeling. Periodontal disease represents example of abnormal bone remodeling. Osteoclasts are multinucleated cells present only in bone. It is believed that osteoclast progenitors are hematopoietic origin, and they are recruited from hematopoietic tissues such as bone marrow and circulating blood to bone. Cells present in the osteoclast microenvironment include marrow stromal cells, osteoblasts, macrophages, T-lymphocytes, and marrow cells. These cells produce cytokines that can affect osteoclast formation. In vitro model systems using bone marrow cultures have demonstrated that $IL-l{\beta},\;IL-3,\;TNF-{\alpha},$ bFGF can stimulate the formation of osteoclasts. In contrast, IL-4 inhibits osteoclast formation. Knowledge of cytokines and bFGF that affect osteoclast formation and their capacity to modulate the bone-resorbing process should provide critical insights into normal calcium homeostasis and disorders of bone turnover such as periodontal disease, osteoporosis and Paget's disease.

The Effect of Nutrition Education Program in Physical Health, Nutritional Status and Health-Related Quality of Life of the Elderly in Seoul (영양교육 프로그램이 서울 지역 노인의 체력, 영양상태, 건강관련 삶의 질에 미치는 영향)

  • This study was performed to investigate the effects oi nutrition education program in physical health, nutritional status and health-related quality of life (HRQoL) of the Elderly in Seoul. Nutrition education program was consisted of healthy eating, prevention and diet therapy of obesity, diabetes, cardiovascular disease, and osteoporosis. Seventy eight free-living elderly people (13 male, 65 female), aged ${\geq}60 $ years participated in this program. Before and after nutrition education program, we surveyed the general characteristics, physical health, general health, nutrition status, and health-related quality of life to the subjects. All the subjects were divided into program completers (N=47) and noncompleters (N=31). All the data were analyzed by student t-test, chi-square test, paired t-test, and marginal homogeneity test using SPSS 9.0 version at p<0.05. After nutrition education program, physical activity and ADL maintained, however IADL improved in program noncompleters. In eating habits, 'slow eating' significantly improved in program completers in program completers. Nutrition knowledge and recognition scores were significantly increased in both groups, and accuracy score was significantly increased in program completers. However, nutrient-intakes of %RDA were not significantly changed in both groups, and it seemed to be more influenced by other factors such as 'family income' or 'family type' than by the nutrition education program. In HRQoL, social functioning was improved after nutrition education program in both groups (p<0.05). The nutrition education program has more effects on the program completers than on the noncompleters, and it is also needed social supports for the Elderly to fulfill their nutrient requirements.

Evaluation of Public Health Nutrition Education Program for High School Girls (여고생 대상 가임여성 보건 영양교육 프로그램 평가)

  • Impact and process evaluations were performed in order to verify the effectiveness of a public health nutrition program developed for child-bearing aged women in Korea. Participants included 58 high school girls who were divided into two groups. Each group received four 50 - 60 minute nutrition education lectures regarding healthy eating, osteoporosis, constipation and nutrition labeling in every two weeks. Each session took 50- 60 minutes. Regarding nutrition knowledge, there was a significant increase of degree of perception (p = 0.0004) , but no change in degree of accuracy after implementation (p = 0.9522) . Nutrition education was also effective in attitude change, showing more participants were ready to change their eating behaviors in terms of meal regularity (p = 0.0455) and less processed food intake (p =0.0143) . After implementing nutrition education, effective behavioral changes were observed in milk consumption (p =0.0037) and meal regularity (p = 0.0882) as well as daily activity such as stair use (p = 0.0701) . However, nutrition education had no effect on body mass index and perceived health status. In process evaluation conducted by a 9 item questionnaire, grand mean score was $4.17 \pm$0.72 out of 5. Proportion of items with scores higher than 4 ranged $68-91\%$. These results suggest that the nutrition education program used in this study was effective and useful. For a wider use of this program, more research was recommend for a strategy development of program diffuse. (Korean J Nutrition 38(10): 873$\sim$879,2005)
