Mapping between Musculoskeletal Patient-Reported Outcome Measures and KCF: Physical Therapy Perspective (근육뼈대계 환자보고식 결과 측정과 KCF의 연결: 물리치료 중심으로)

  • PURPOSE: The present study was conducted to improve the understanding of the Korean Standard Functional, Disability, and Health Classification (KCF) and its ease of use in the clinical domain of the musculoskeletal system by comparing, analyzing, and linking the KCF codes with items from patient-reported outcome measures (PROMs), which are currently mainly used to evaluate patients with neck, shoulder, waist, and knee pain. METHODS: The items of the most widely used PROMs, the Neck Disability Index (NDI), Disabilities of the Arm, Shoulder, and Hand (DASH) scores, Oswestry Disability Index (ODI), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and the codes of the KCF were linked by two experts according to the linking rules. RESULTS: The concordance between the items of each of the PROMs and the KCF code linked by the two experts was NDI,86.4%86.4%, DASH 83.3%, ODI 92.0%, and WOMAC 80.7%. The NDI, DASH, and WOMAC indexes were found to comprise items corresponding to physical function, activity, and participation areas, and were linked to 22, 43, and 31 KCF codes, respectively. In addition to these two areas, the ODI included items related to environmental factors and was linked to 25 codes (duplicated codes are treated as one). CONCLUSION: This research can be used by adding the KCF code to the questions of the currently used evaluation tool. This coding can be easily applied and will contribute to the easy understanding of the KCF.

Outcomes of Meniscal Allograft Transplantation (동종 반월상 연골 이식술의 결과)

  • Treatment options are limited for young, meniscal-deficient patients with pain. This patient population is not age appropriate for total joint replacement, but the loss of the meniscus leaves them at significant risk for the development of osteoarthritis. One increasingly popular option is the use of allograft meniscal transplantation. However, many questions still surround allograft meniscus transplantation. Furthermore, most reports in the literature on the results of meniscal transplantation describe small case series using clinical outcome measures and/or incomplete direct evaluation of the meniscus. Therefore, the results of meniscal allograft transplantation have been difficult to interpret and compare due to many confounding variables. In this study, we reviewed the current research of concerns on the results of meniscal allograft transplantation.

Time-frequency Analysis of Vibroarthrographic Signals for Non-invasive Diagnosis of Articular Pathology (비침습적 관절질환 진단을 위한 관절음의 시주파수 분석)

  • Vibroarthrographic(VAG) signals, emitted by human knee joints, are non-stationary and multi-component in nature and time-frequency distributions(TFD) provide powerful means to analyze such signals. The objective of this paper is to classify VAG signals, generated during joint movement, into two groups(normal and patient group) using the characteristic parameters extracted by time-frequency transform, and to evaluate the classification accuracy. Noise within TFD was reduced by singular value decomposition and back-propagation neural network(BPNN) was used for classifying VAG signals. The characteristic parameters consist of the energy parameter, energy spread parameter, frequency parameter, frequency spread parameter by Wigner-Ville distribution and the amplitude of frequency distribution, the mean and the median frequency by fast Fourier transform. Totally 1408 segments(normal 1031, patient 377) were used for training and evaluating BPNN. As a result, the average value of the classification accuracy was 92.3(standard deviation ${\pm}0.9$)%. The proposed method was independent of clinical information, and showed good potential for non-invasive diagnosis and monitoring of joint disorders such as osteoarthritis and chondromalacia patella.

  • Although total meniscectomy had been performed as a treatment of meniscal tear, many published articles reported progression of the degeneration process in the articular cartilage and development of the osteoarthritis after meniscectomy. With the increased knowledge of the biomechanical properties of the meniscus, meniscal repair has been performed increasingly to maintain the important functions of the meniscus. Outside-in, inside-out, all-inside technique have been used as meniscal repair methods. In this review, indications, operative technique, and outcomes of the all-inside meniscal repair are discussed.

The Effects of Virtual Reality Interactive Games on the Balance Ability of Elderly Women with Knee Osteoarthritis (가상현실 상호작용 게임이 무릎 골관절염 여성 노인의 균형능력에 미치는 영향)

  • 연구목적 : 본 연구는 Xbox 360 Kinetic Sensor를 사용한 가상현실 상호작용 게임이 무릎 골관절염을 앓는 여성 노인의 균형능력에 미치는 영향을 알아보는 것이다. 연구방법 : 65세 이상의 지역사회에 거주하는 여성 노인으로 무릎 골관절염으로 진단받은 40명을 대상자로 선정하였다. 대상자는 20명씩 무작위로 실험군과 대조군에 배치되었다. 실험군은 Xbox 360 Kinetic Sensor를 사용한 가상현실 상호작용 게임을 매 30분, 주 3회, 4주간 처치하였다. 대조군은 가상현실 상호작용 게임을 적용하지 않았다. 결과 측정은 눈을 뜬 상태와 눈을 감은 상태에서 신체 중심의 이동거리로 측정하였다. 연구결과 : 실험군의 신체중심 이동거리는 눈을 뜬 상태와 눈을 감은 상태 모두 중재 전보다 유의하게 증가하였다. 또한 중재후 실험군의 신체중심 이동거리는 눈을 뜬 상태와 눈을 감은 상태 모두 대조군보다 유의하게 높게 증가하였다. 결론 : Xbox 360 Kinetic Sensor를 이용한 가상현실 상호작용 게임은 무릎 골관절염을 앓는 여성 노인의 균형 능력을 증진시키는데 효과적이었다.

Cause of Pes Anserinus Tenderness (거위발 건 압통의 원인)

  • Purpose: The purpose of this study was to know the cause of the tenderness at pes anserinus. Materials and Methods: Out of 24 patients with tenderness at pes anserinus, 23 patients were female and 1 patient was male, and their average age was 65.9 years old. We checked the tenderness at pes anserinus by history taking & physical examniation and then, checked the pes anserinus for the presence of bursitis by US in outpatient clinic. With no evidence of bursitis by US, we injected steroid-lidocaine mixture intraarticularly and checked whether the tenderness disappeared after 2~3 minutes. Results: There was no case with bursitis at pes anserinus by US. The tenderness at pes anserinus diminished in 2-3 minutes after the intraarticular injection of the steroid-lidocaine mixture. After 6 weeks follow up, 16 patients(66.7%) had little or no tenderness at pes anserinus. 8 cases had the recurrence of tenderness recurred, 4 cases underwent arthroscopic operation on the meniscal tear, and 1 case underwent total knee arthroplasty. All cases underwent any operations had the tenderness at pes anserinus disappeared. The others 3 cases did not undergo total knee arthroplasty despite of radiologic obliteration of knee joint adequate for Kellgren-Lawrence grade IV. Conclusion: Without the bursitis at Pes Anserinus, patients the osteoarthritis may have the tenderness due to the referred pain.

Effects of Laughter Therapy on Joint Pain, Functional Status of Knee Joint and Depression in Elderly with Degenerative Knee Arthritis (웃음요법이 퇴행성 슬관절염 노인의 관절통증, 관절기능상태, 우울에 미치는 효과)

  • This study examined the effects of laughter therapy in the elderly with degenerative arthritis who had joint pain, joint dysfunction, and depression. Through a single-blind, randomized assignment, an experimental group of 26 subjects and a control group of 26 subjects were recruited among the elderly with degenerative knee arthritis, who were over 65 years of age. From January 8 to March 11 of 2019, a total of 16 laughter therapies were administered two times a week, 60 minutes per session, in eight weeks. The pain threshold (p=.012) and joint function status (p<.001), which represents the pain, joint stiffness, and physical function, and depression (p<.001), in the experimental group improved after the eight weeks laughter therapy compared to the control group. Laughter therapy is a therapeutic intervention that can be delivered through physical or intellectual activities. This therapy is a non-invasive procedure and has the advantage that nurses can practice easily with a little training. It can be used as an intervention within community and clinical settings to relieve pain and improve depression in the elderly with arthritis.

Ultrasonographic Measurement of Articular Cartilage Thickness of Medial Femoral Condyle in Knee Osteoarthritis (슬관절 골관절염에서 초음파를 이용한 대퇴 내과 관절 연골의 두께 측정)

  • Purpose: The purpose of this study was to analyze the correlation between the medial joint space on weight bearing simple X-ray and the ultrasonographic articular cartilage thickness. Materials and Methods: The articular cartilage thickness of 91 osteoarthritic knees of 73 patients were measured with weight bearing simple X-ray and ultrasonography between June 2010 and September 2010. Male were 13 and female were 60. Right knees were 35, left knees were 20 and bilateral involvements were 18. Medial joint spaces on X-ray were measured in full extension view and $45^{\circ}$ PA(Rosenberg) view. Femoral cartilage thicknesses at and those at the point between the middle 1/3 and posterior 1/3 area were measured by ultrasonography, that were scanned coronally at $30^{\circ}$ flexion and $130^{\circ}$ flexion respectively. The results were analyzed statistically by Pearson correlation test and Bland-Altman plot test. Results: Medial joint spaces measured in full extension view and femoral articular cartilage thicknesses at the point between the anterior 1/3 and middle 1/3 area measured with sonography were statistically correlated and medial joint spaces measured in $45^{\circ}$ PA view and those at the point between the middle 1/3 and posterior 1/3 area measured with sonography were also correlated (P<0.05, $SD{\pm}2$). Conclusion: The measurement of articular cartilage thicknesses by the ultrasonography was easy even in the patient who are unable to take weight bearing view and was thought to be a useful diagnostic and follow up method to examine the degenerative change in addition to simple radiographic examination in the knee osteoarthritis.

Effect of Boswellia serrata Extracts on Degenerative Osteoarthritis in vitro and in vivo Models (보스웰리아 추출물의 골관절염 억제 효과 연구)

  • The inhibitory effects of Boswellia serrata (BW) extracts on degenerative osteoarthritis were investigated in primary-cultured rat cartilage cells and a monosodium-iodoacetate (MIA)-induced osteoarthritis rat model. To identify the protective effects of BW extract against $H_2O_2$ ($800{\mu}M$, 2 hr) in vitro, cell survival was measured by MTT assay. Cell survival after $H_2O_2$ treatment was elevated by BW extract at a concentration of $20{\mu}g/mL$. In addition, BW extract treatment significantly reduced and normalized the productions of pro-inflammatory factors, nuclear transcription factor ${\kappa}B$, cyclooxygenase-2, tumor necrosis factor-${\alpha}$, and interleukin-6 at a concentration of $20{\mu}g/mL$. Treatment of chondrocytes with BW extract significantly reduced 5-lipoxygenase activity and production of prostaglandin E2, especially at a concentration of $10{\sim}20{\mu}g/mL$. For the in vivo animal study, osteoarthritis was induced by intra-articular injection of MIA into knee joints of rats. Consumption of a diet containing BW extract (100 and 200 mg/kg) for 35 days significantly inhibited the development and severity of osteoarthritis in rats. To determine the genetic expression of arthritic factors in articular cartilage, real-time PCR was applied to measure matrix metalloproteinases (MMP-3, MMP-9, and MMP-13), collagen type I, collagen type II, and aggrecan, and BW extract had protective effects at a concentration of 200 mg/kg. In conclusion, BW extract was able to inhibit articular cartilage degeneration by preventing extracellular matrix degradation and chondrocyte injury. One can consider that BW extract may be a potential therapeutic treatment for degenerative osteoarthritis.
