누리장나무의 진통 효과 (Anti-nociceptive Effects of Clerodendrum Trichotomum)

  • Clerodendrum trichotomum has been widely used as a traditional medicine for the treatment of numerous diseases, including pain management. However, there have been extremely limited pharmacological and phytochemical studies on Clerodendrum trichotomum up to now. In this study, we investigated the effects of the methanolic extract of Clerodendrum trichotomum (MCT) on nociceptive pain in mice. Our finding demonstrate that MCT treatment significantly extended the latency time in both the tail-immersion test and hot-plate test. Additionally, MCT treatment reduced acetic acid-induced writhing motions. These results suggest that MCT possesses strong anti-nociceptive activities against thermal and chemical nociception. In the formalin test, mice fed with MCT exhibited reduced licking time during both the early and late phases, thereby confirming the therapeutic potential of MCT in central and peripheral nociception. Furthermore, in combination tests using naloxone, the MCT-mediated anti-nociception was slightly reduced, indicating that MCT might act as a partial opioid receptor agonist. Based on these results, MCT may be a valuable candidate for the development of anti-nociceptive agents.

  • Background: This study investigated the effects of face-to-face and non-face-to-face McKenzie exercises and elastic band strengthening exercises on falls and fitness in older adults. Methods: A total of 44 subjects were divided into a face-to-face group (FG; n=22) and a non-face-to-face group (NFG; n=22) who met older adults. Both groups used McKenzie exercises and strength training with elastic bands The fall efficacy scale (K-FES) and glycosylated hemoglobin (HbA1c) Pro were used to compare the senior fitness test (SFT). Results: No significant differences were detected between the two groups before the intervention; however, a significant difference was noted in the K-FES scores after the intervention. The change in HbA1c after the intervention was not statistically significant in the FG or the NFG, and no significant difference was detected between the two groups. The change in lower limb muscle strength in the elderly physical fitness test showed no significant difference between the two groups before or after the intervention. The change in upper limb flexibility after the intervention was not significantly different in the FG or NFG, indicating no significant difference between the two groups. The change in the 3-meter round-trip test after the intervention was significantly different in both the FG and NFG groups, but the difference between the groups was not statistically significant. The change in the grip strength test of the dominant hand showed no significant difference after the intervention in the FG or the NFG, and the difference between the FG and NFG groups was not statistically significant. The dominant foot external standing after the intervention was significantly improved in the FG, but no significant difference was detected in the NFG, and the difference between the two groups was not statistically significant. Conclusion: The study findings revealed that FG and NFG McKenzie exercises and elastic band strengthen exercises were equally effective at preventing falls and improving fitness in older adults.

  • Objective: The purpose of this study is to investigate the effects of human motion and javelin kinematics during the energy transfer in javelin throwing on records, and to provide evidence-based training insights for athletes and coaches to enhance records. Method: Three javelin throw athletes (age: 22.67 ± 0.58 years, height: 178.33 ± 7.37 cm, weight: 83.67 ± 1.15 kg) were recruited for this study. Each athlete attempted ten maximum record trials, and the kinematic data from each performance were analyzed to determine their influence on the records. The Theia3d Markerless system was used for motion analysis. Results: Key factors were modeled and identified at each moment. In E1, main variables were COM Y (𝛽 8.162, p<.05) and COM velocity Z (𝛽 -72.489, p<.05); in E2, COM X (𝛽 -17.604, p<.05); in E3, COM X (𝛽 -18.606, p<.05), COM velocity Y (𝛽 38.694, p<.05), and COM velocity X (𝛽 66.323, p<.05). For the javelin throw dynamics in E3, key determinants were Attitude angle and Javelin velocity in the Y-axis. Conclusion: The study reveals that controlled vertical movement, center of mass management during braking, and enhanced pelvic rotation significantly improve javelin throw performance. These kinematic strategies are critical for record enhancement in javelin throwing.

근무기관에 따른 정형도수물리치료사의 건강상태, 번아웃증후군에 관한 연구: 서울시 지역을 중심으로 (A Study on the Health Status and Burnout Syndrome of Orthopedic Physiotherapists According to the Working Institution: Focusing on the Seoul Metropolitan Area )

  • Background: This study compared orthopedic physiotherapists from different working institutions regarding their health status and burnout syndrome to understand the work-related factors that affect health status and burnout syndrome Methods: This study was conducted from October 28 to November 13, 2022, focusing on physiotherapists working in medical institutions located in metropolitan areas, such as Seoul, Gyeonggi, and Incheon. A total of 591 questionnaires were analyzed. The questionnaire consisted of questions related to the basics of physical therapy, general matters, types of working institutions, health conditions, and burnout syndrome. Results: No significant difference was detected in health self-awareness status whether the working institution was a clinic, nursing hospital, hospital, oriental hospital, general hospital, or university hospital. Among the burnout factors, nursing hospitals showed the highest scores for retirement demands, work unfavorability, physical fatigue, mental fatigue, and reasons for work because of salary. By contrast, university hospitals showed the highest enthusiasm and other institutions showed the lowest enthusiasm for work. In particular, in terms of enthusiasm, scores were significantly more positive in university hospitals than in other institutions. Conclusion: A management plan for burnout for orthopedic manual physiotherapists working in nursing hospitals is needed, and future research will examine the health status of orthopedic manual physiotherapists and work that may affect burnout syndrome. An exploration of management measures, such as health promotion and burnout syndrome prevention, is needed that adds other items, such as differences in environment and differences in the number and disease severity of patients

소아마비에서 4절 연쇄 장하지보조기 사례연구 (Case Study of 4-Bar Linkage KAFO in Person With Poliomyelitis)

  • The purpose of this study was to compare the ring lock type knee-ankle-foot orthosis (KAFO) with newly developed 4-bar linkage KAFO on the gait characteristics of persons with poliomyelitis clinically. This 4-bar linkage is the stance control type KAFO which provide the stability during stance phase and knee flexion during swing phase. Two subjects participated in this study voluntarily. We provided the customized 4-bar linkage KAFO then asked the subjects to walk in level surface and stairs under the two different KAFO conditions. The characteristics of gait in the persons with poliomyelitis were evaluated using a 3D motion analysis system and force plate. Additionally 6 minute walk test for physiological cost index were conducted using pulse oximeter to measure the energy consumption. In the results of this study, the differences of 4-bar linkage KAFO compared with ring lock type KAFO are as follows: (1) Walking speed, stride length, and step length on level increased in subjects, (2) The gait symmetry was improved by generated knee flexion and decreased pelvic external rotation on level and stairs walking, (3) Decreased vertical excursion of center of mass and pelvic elevation during swing phase was decreased on level, (4) Knee extension moment, hip flexion moment, hip and knee internal rotation moment of non-braced limb were decreased on level walking, (5) Walking speed in 6-minute walk test was increased and physiological cost index was decreased. These findings indicate that 4-bar linkage KAFO compared with ring lock type KAFO is effective in enhancing pattern, endurance, and energy consumption in level surface and stairs walking.

생강나무 추출물의 암전이 억제효과 (Effect of Lindera obtusiloba extract on cancer metastasis)

  • 목 적 : 본 연구에서는 생강나무 메탄올 추출물이 암전이 억제에 미치는 영향을 조사하고자 하였다. 방 법 : 생강나무 추출물의 암전이 억제능을 확인하기 위해서 B16F10 흑색종 세포를 이용하여 금속단백분해효소의 활성 및 발현을 측정하였으며, 암세포의 이동능이나 침윤능도 조사하였다. 폐전이 동물모델에서 생강나무 추출물이 미치는 영향을 조사하여 활성을 최종적으로 확인하였다. 결 과 : 1. 생강나무 추출물은 B16F10 흑색종 세포에서 뚜렷한 금속단백분해효소의 효소활성 및 발현 억제효과를 보였으며 이는 NF-${\kappa}B$의 활성 억제에서 기인한 것임을 확인하였다. 2. 흑색종 세포의 이동이나 침윤 역시 생강나무 추출물 투여에 의해 현저히 감소하였다. 3. 폐전이 동물 모델에서도 생강나무 추출물에 의해 폐로 전이되 집락의 수가 감소하였다. 결 론 : 이상의 결과로 생강나무 추출물은 뛰어난 암전이 억제효과가 있는 것을 확인할 수 있었으며, 전이성 암치료에 있어서 유용하게 사용될 수 있을 것으로 사료된다.

MR 영상에서의 PACS Image Viewer를 이용한 대퇴골 전염각 측정 (MR Imaging Measurement of the Femoral Anteversional Angle as a PACS Image Viewer)

  • 대퇴골 전염각의 정확한 측정은 정형외과 분야에서 중요한 의미를 가지며, 원위 대퇴골의 양과부를 횡단하는 면에 대하여 대퇴골 경부의 장축이 전방으로 기울어진 축면상의 각도이다. 대퇴골부와 슬개골부를 전산화단층촬영한 영상을 측정하고 있는 기존의 CT 영상의 대퇴골 측정법을 대신하여 다양한 Pulse sequence를 이용한 자기공명영상으로 PACS Image viewer에서 성인 남자 $28{\sim}36$세를 대상으로 대퇴골 전염각을 측정하였다. 오른쪽 대퇴골 전염각의 T2강조영상은 $18.5{\sim}45.4^{\circ}$, T1강조영상 $18.9{\sim}25.6^{\circ}$, 경사에코 $18.7{\sim}26.0^{\circ}$, 지방억제기법 $21.3{\sim}24.9^{\circ}$, 왼쪽 대퇴골 전염각의 T2강조영상은 $19.2{\sim}24.3^{\circ}$, T1강조영상 $18.8{\sim}24.5^{\circ}$, 경사에코 $20.1{\sim}25.6^{\circ}$, 지방억제기법 $20.0{\sim}25.3^{\circ}$로 측정되었고, 오른쪽과 왼쪽 대퇴골 전염각의 이원배치 분산분석에서 모두 유의한 차이가 있었다(p<0.05). 수 작업 방법의 대퇴골 전염각 측정에 비해 Image Viewer 프로그램에서는 마우스를 이용한 측정으로 편리하였고, MR 영상은 CT 영상에 비해 다양한 Pulse sequence를 선택적으로 적절한 촬영기법을 사용하여 대퇴골 전염각 측정에 유용하였다.

양적 결과지식의 종류가 요추의 고유수용성감각 훈련에 미치는 영향 (The Influence of Different Quantitative Knowledge of Results on Performance Error During Lumbar Proprioceptive Sensation Training)

  • This study is aimed at investigating the influence of different quantitative knowledge of results on the measurement error during lumbar proprioceptive sensation training. Twenty-eight healthy adult men participated and subjects were randomly assigned into four different feedback groups(100% relative frequency with an angle feedback, 50% relative frequency with an angle feedback, 100% relative frequency with a length feedback, 50% relative frequency with a length feedback). An electrogoniometer was used to determine performance error in an angle, and the Schober test with measurement tape was used to determine performance error in a length. Each subject was asked to maintain an upright position with both eyes closed and both upper limbs stabilized on their pelvis. Lumbar vertebrae flexion was maintained at $30^{\circ}$ for three seconds. Different verbal knowledge of results was provided in four groups. After lumbar flexion was performed, knowledge of results was offered immediately. The resting period between the sessions per block was five seconds. Training consisted of 6 blocks, 10 sessions per one block, with a resting period of one minute. A resting period of five minutes was provided between 3 blocks and 4 blocks. A retention test was performed between 10 minutes and 24 hours later following the training block without providing knowledge of results. To determine the training effects, a two-way analysis of variance and a one-way analysis of variance were used with SPSS Ver. 10.0. A level of significance was set at .05. A significant block effect was shown for the acquisition phase (p<.05), and a significant feedback effect was shown in the immediate retention phase (p>.05). There was a significant feedback effect in the delayed retention phase (p<.05), and a significant block effect in the first acquisition phase and the last retention phase (p<.05). In conclusion, it is determined that a 50% relative frequency with a length feedback is the most efficient feedback among different feedback types.

보행 시 신발 굴 높이어 따른 발/발목 복합체의 Roll-Over특성 (Foot/Ankle Roll-Over Characteristics According to Different Heel Heights of Shoe during Walking)

  • 본 연구에서는 여러 굽 높이의 신발을 신고 보행하는 동안 발/발목 복합체의 roll-over 모양을 분석하였다. 본 연구를 위해서 근골격계 및 발 질환이 없는 15명의 건강한 성인 여성들을 대상으로 보행분석을 수행하였고, 실험에는 굽 높이가 다른 4종의 신발을 사용하였다. 보행분석 시 roll-over의 특성을 구하기 위해 초기접지기부터 반대발 초기접지기까지의 발목 및 무릎의 반사마커 궤적과 지면반발력의 압력중심 데이터를 이용하였다. 연구결과로 굽 높이가 증가할수록 roll-over의 모양은 원위방향으로 이동하고 그 특성은 굽 높이에 상관없이 일정한 형태로 관찰되었다. 또한, 6cm이하의 신발에서는 roll-over의 곡률중심이 수직방향으로 이동함으로써 일정한 roll-over 특성을 유지하도록 발/발목 복합체에서 자동으로 적응함을 알 수 있었다. 그러나 그 이상의 굽 높이의 신발에서는 발목관절만으로 충분한 균형을 보장할 수 없으므로 무릎 및 엉덩관절의 보상작용이 동시에 일어남을 알 수 있었다. Roll-over 특성에 대한 본 연구결과는 인간 보행의 보다 간단하고 넓은 이해를 제공할 것이고, 나아가 의지 또는 보조기와 같은 재활기기의 보다 넓은 이해를 제공할 수 있을 것으로 기대된다.

운목향 70% 에탄올 추출물의 항염증 및 면역조절에 대한 효과 (Anti-inflammatory and Immune Regulatory Effects of Aucklandia lappa Decne 70% Ethanol Extract)

  • Background: This present study was conducted to evaluate the anti-inflammatory and immune regulatory effects of Aucklandia lappa Decne (AL). Methods and Results: We measured cytotoxicity, nitric oxide (NO) content, mRNA expression (iNOS, IL-1${\alpha}$, IL-$1{\beta}$, and TNF-${\alpha}$), protein expression (iNOS, COX-2, and $I{\kappa}B$) and phagocytic activity in RAW264.7 cells. Male BALB/c mice were fed 100 mg/kg AL (Aucklandia lappa Decneon 70% ethanol extract) and 250 mg/kg AL for 4 weeks; thereafter, we observed B/T or $CD4^+/CD8^+$ lymphocyte subpopulation change, and expression patterns of $CD4^+$ and $CD8^+$ lymphocytes by immunohistochemical staining in mouse splenocytes and/or thymocytes. To determine the experimental concentration of AL, cell viability was measured by MTT assay and tested at $12.5{\mu}g/m{\ell}$ or less. AL inhibited the levels of NO, lymphokine production (IL-$1{\beta}$, and TNF-${\alpha}$), and mRNA (iNOS, IL-1${\alpha}$, IL-$1{\beta}$, and TNF-${\alpha}$) and protein (iNOS, and COX-2) expression. Additionally, the levels of $I{\kappa}B$, phagocytic activity, and splenic and thymic T lymphocytes, especially $T_H$ and $T_C$ cells were significantly increased in AL administered mice. The immuno-reactive density of $CD4^+$ and $CD8^+$ lymphocytes was stronger in AL groups than in the normal group. AL stimulated NO, iNOS, and COX-2, and regulated IL-1${\alpha}$, IL-$1{\beta}$, TNF-${\alpha}$, and $I{\kappa}B$ in macrophages treated with LPS (lipopolysaccharide). In addition, AL increased the phagocytic activity of macrophages and the immunity of mouse T ($T_H$, and $T_C$) cells. Conclusions: These results suggested that AL might show anti-inflammatory activity via the suppression of various inflammatory markers and immuno-regulatory activity.
