Orthostatic Intolerance: Postural Tachycardia Syndrome (기립성 못견딤증: 기립성 빈맥 증후군)

  • Orthostatic intolerance is defined as the development of various symptoms during standing that are relieved by recumbency. Postural tachycardia syndrome (POTS) is another nomenclature of orthostatic intolerance. POTS characterized by a heart rate increase ${\geq}30$ bpm from supine to standing or >120 bpm at standing without orthostatic hypotension. POTS is a heterogenous in presentation with various pathophysiologic mechanisms. Important mechanisms are hypovolemia, denervation, hyperadrenergic and deconditioning state. There are presented as lightheadness or dizziness, palpitations, presyncope, sense of weakness, tremulousness, shortness of breath. POTS are classified under 3 groups that are neuropathic, hyperadrenergic, and deconditioning POTS. Most patients can be improved from a pathophysiologically based regimen of management.

  • Orthostatic proteinuria is documented as a benign condition and the most common cause of isolated proteinuria. The etiology and pathogenesis of orthostatic proteinuria is not clear yet. Recently there were a few report that nutcracker syndrome seemed to cause orthostatic proteinuria. We experienced a case of a twelve-yeat-old female patient with incidently discovered orthostatic proteinuria who was suspected to have nutcracker phenomenon by doppler sonography. We confirmed this patient as nutcracker syndrome by renal venography.

  • Orthostatic hypotension (OH) is commonly associated with autonomic failure in the peripheral nervous system. Less often it is related to central lesions in brainstem and cerebellum. We describe a patient with OH associated with tuberculosis meningoencephalitis involving the brainstem including rostral ventrolateral medulla. This is the first case of OH resulting from focal lesions in the dorsal medulla in a patient with meningoencephalitis.

  • In this paper, the autonomic function analysis system was designed to noninvasively assess the autonomic function of cardiovascular system. Orthostatic stress protocol was designed to stimulate the autonomic nervous system. designed protocol and analysis algorithm were evaluated by experiments for 25 normal subjects and 22 hemiplegia patients. Data were processed by usign the power spectral analysis. Nwe indexes of autonomic function, LF$_{N}$ and HF$_{N}$, were proposed and were compared with LF/HF ratio. New indexes of the sympathetic and parasympathetic activity, respectively. The IST and the DPT are balanced and have positive value for normal subjects during orthostatic stress but not for hempilegia patients. This result suggest that the IST and the DPT are used as new criteria of normal autonomic function during orthostatic stress.ess.

A Case of Orthostatic Proteinuria Progressed to Persistent Proteinuria Associated with Renal Pathology

  • Orthostatic or postural proteinuria is the most common cause of asymptomatic proteinuria in children. As orthostatic proteinuria (OP) is a benign disease with relatively good prognosis, it has no specific management, and patients only need to be observed. However, if OP shows a persistently high level of proteinuria, in theory, glomerular changes can occur. An 11-year-old girl was referred to the hospital due to asymptomatic proteinuria and was diagnosed as having OP based on the results of clinical and laboratory examinations, urinalysis, and protein/creatinine (TP/Cr) ratio at both supine and erect positions. During follow-up observation, the 24-hour TP/Cr ratio was persistently higher than 1.5 mg/mg for 2 years. We performed renal biopsy, which showed mesangial proliferative glomerular lesions with focal effacement of the podocyte foot processes, but without immune depositions. OP can be accompanied by glomerular lesions if moderate to severe proteinuria persists.

  • Postural orthostatic tachycardia syndrome (POTS) refers to the presence of orthostatic intolerance with a heart rate (HR) increment of 30 beats per minute (bpm) or an absolute HR of 120 bpm or more. There are sporadic reports of the autonomic nervous system dysfunction in migraine and fibromyalgia. We report a case of POTS associated with migraine and fibromyalgia. The patient was managed with multidisciplinary therapies involving medication, education, and exercise which resulted in symptomatic improvement. We also review the literature on the association between POTS, migraine, and fibromyalgia.

A study of association between Cold Hypersensitivity and Orthostatic Hypotension(OH) (냉증과 기립성 저혈압의 연관성에 관한 연구)

  • Purpose The purpose of this study was to verify the association between Cold Hypersensitivity and Orthostatic Hypotension(OH). Method This study was carried out on 39 women who visited outpatient department of Dept. of Gynecology. Bundang CHA oriental Hospital. College of Medicine. Pochun CHA univ. from December 2001 to November 2002. We divided them into two groups (Orthostatic Hypotension Group 24, Normal BP Group 15) and investigated general characteristics, weight, height, past history. family history through out the patient's note. OH was assessed 1 minute after the patients rose from a supine position by using tilting table. For the diagnosis of cold hypersensitivity, thermographic measurements were performed on two pairs of areas(palm-upper arm and back of hand-upper arm. And for that of the feet, other two pairs of areas(anterior thigh-top of the feet and posterior thigh-heel). And then by which verify the association between Cold Hypersensitivity and Orthostatic Hypotension(OH) Results As a results, 16 out of 24 patients, the OH group had cold hypersensitivity and 4 out of 15 Patients the normal BP group had cold hypersensitivity. It means that there was association between Cold Hypersensitivity and Orthostatic Hypotension(OH). Conclusion Above these results, we can reach the conclusion that OH is considered one of the causes of cold hypersensitivity.

  • Postural tachycardia syndrome (POTS) is the most common form of orthostatic intolerance in young people. However, it is still considered an underrecognized disorder and so deserves more attention from clinicians. This review covers the diagnostic challenges, correlations between the symptoms, evidence of autoimmune involvement in the pathogenesis, and treatment strategies in POTS.

  • The nutcracker syndrome is the congestion of left renal vein due to the compression of left renal vein by the aorta and the superior mesenteric artery and has been known as tile cause of hematuria with or without left renal flank pain, mild to moderate proteinuria and orthostatic proteinuria. We present here one case of 13.5 year of girl has severe typical nutcracker syndrome with orthostatic protinuria and idiopathic chronic fatigue. (J. Korean Soc Pediatr Nephrol 5 . 64- 8, 2001)

Effects of Tailored Water Drinking on the Orthostatic Hypotension in the Elderly (맞춤식 수분섭취가 노인의 체위성저혈압 발생에 미치는 영향)

  • Purpose: The purpose of the study was to examine preventive effects of tailored water drinking on orthostatic hypotension and heart rate variability in the elderly. Methods: A non-equivalent control group pre and post time-series design was adapted. Among a total of 64 elderly people admitted to two nursing homes, 35 elderly were assigned to the experimental group and 29 were assigned to the control group. As for the elderly people in the experimental group, tailed water drink was provided according to the scheduled time for six weeks. Blood pressure and heart rate were measured twice before the intervention in both groups. The data were analyzed with SPSS program using t-test, ${\chi}^2$-test, and repeated measure of ANOVA. Results: There were significant differences in blood pressure and heart rate variability between the two groups. Conclusion: Tailored water drinking had preventive effects on decreasing blood pressure fall as well as prevalence of orthostatic hypotension in the elderly people.
