Purpose: This study was done to evaluate effectiveness of home healthcare services (HHCS) specialized for elders who received spinal surgeries. Methods: A non-equivalent control group pre-post test quasi-experimental study was performed. HHCS was developed based on the Rice model of dynamic self-determination for self-care. For data collection, a control group (n=23) and an experimental group (n=23) were selected by matching age, BMI, pain, general characteristics and type of spine surgery. Measurement tools to evaluate uncertainty and knowledge were developed by the authors. The Numeric Rating Scale (NRS) and Japanese Orthopedic Association Back Pain Evaluation Questionnaire (JOABPEQ) were used to evaluate pain levels. Muscular strength in the legs was measured using a digital muscle tester and tape ruler. Questionnaires were used to evaluate disability in performing ADL and psychological distress levels. Results: The experimental group showed significant decrease in uncertainty (p=.028), increased knowledge (p=.038), and partially decreased pain (p=.003-.331). Partial muscle strength increased significantly (p=.021-.644). Disability in performing ADL and psychological distress in the experimental group decreased significantly compared to control group (p=.002, p=.004). Conclusion: Results indicate HHCS is an efficient home care nursing program for these elders. Further experimental studies with larger samples are required to confirm effects of HHCS.
Purpose: The study was to analyze clinical outcomes and risk factors of for complications associated with the hip fracture surgery in the elderly before and after interdisciplinary treatment. Methods: A retrospective method was used to investigate the general and therapeutic characteristics, frequency of complications and clinical outcomes. The subjects of the study were 553 patients who underwent hip fracture surgery from January, 2009 to December, 2014. Results: The interdisciplinary group was older and less likely to walk independently even before the fracture than a usual care group. The incidence of complications was higher in the interdisciplinary group than the usual care group. The prevalence of complications in both groups was 66.5%. Multivariate logistic regression analysis showsed that the risk factors for complications of hip fracture surgery were as follows: advanced age, stroke, Parkinson disease, time interval from emergency room to operation, pre & post ambulatory status, American Society of Anesthesiologists (ASA) classification quality of postop intensive care unit (ICU) care and foley indwelling. Conclusion: This study has implications in that it recognized the necessity for interdisciplinary treatment and provided the basic base data for nursing intervention of the elderly patients who underwent hip fracture surgery.
Purpose: The purpose of this study was to examine the effects of relaxation therapy to reduce anxiety level among spinal anesthesia surgery patients. Methods: The study employed a quasi-experimental pre-and-posttest design with nonequivalent control group. The experimental group was provided with the 15-min muscle relaxation therapy on the day before the surgery and an hour before the spinal anesthesia. Data were collected from 44 patients (22 in experimental and 22 in control group) who were planned to have spinal anesthesia surgery from an orthopedic surgery department at a general hospital in Seoul from January 13 to March 21, 2006. Results: The experimental group reported lower level of state anxiety after the surgery (23.18 vs. 33.64) than did control group but was not statistically significant. There were no significant differences in blood pressure or pulse rate between experimental and control groups after surgery. Conclusions: The further research should be conducted to polish the muscle relaxation therapy for various types of patients and to create more comfortable environment setting for application of the therapy. The muscle relaxation therapy may be a convenient and useful nursing intervention to reduce anxiety among spinal anesthesia surgery patients if more studies prove its effectiveness.
Purpose: Postoperative nausea and vomiting (PONV) is very common among postoperative orthopedic surgical patients with patient-controlled analgesia (PCA), especially for narcotics. Therefore, the purpose of this study was to investigate the effects of an evidence-based PONV management protocol on nursing and patient outcomes. Methods: A methodological study was conducted to develop PONV protocol and a quasi-experimental study to evaluate the effectiveness of protocol. The preliminary PONV protocol was drawn by conducting a systematic review and by reviewing clinical guidelines and best practice recommendations. Validation of the content was done by expert clinicians, and the clinical applicability was evaluated by staff nurses and patients. The effect was evaluated in clinical outcomes associated with PONV and nursing outcomes. Results: In the experimental group, the occurrence of vomiting (z= 2.147) was significantly decreased, the maintenance PCA (${\chi}^2=4.212$) and the satisfaction of patients (z= 5.007) were significantly higher. In the outcomes of nurses, the PONV knowledge of nursing care (z = 3.791), awareness (z = 2.982) and self-efficacy (z= 2.745) were higher in the experimental group. The attitude towards evidence-based nursing practice (z= 2.446) was significantly positive. Conclusion: The results show that an evidence-based approach to the implementation of PONV care is effective in improving patient clinical outcomes and quality of care.
Purpose: This study was conducted to develop a useful evidence-based guideline for preventing venous thromboembolism(VTE) in Korea adapting previously developed VTE guidelines. Methods: The guideline adaptation process was performed using 24 steps according to the nursing practice guideline adaptation manual developed by Hospital Nurses Association in 2012. Results: The newly developed VTE prevention guideline was consisted of 16 domains and 163 recommendations. The number of recommendations in each domain were: 4 general issues, 4 risk factors, 2 intervention at occurrence of VTE, 14 mechanical interventions, 30 pharmacological interventions, 19 VTE prevention for medical patient, 10 stroke patient, 16 cancer patient, 14 pregnancy, 6 for long distance traveller, 5 for abdominal surgery, 10 thoractic surgery, 10 orthopedic surgery, 5 neurosurgery, 4 other surgical patient, 2 urological surgery, 1 ENT surgery, 1 plastic surgery, 3 day surgery, 3 education of VTE prevention. Fourteen point three percent, 61.1%, and 24.6% of the recommendations were graded A, B, and C, respectively. Conclusion: The findings suggest that the new VTE prevention guideline can be more efficiently used to prevent VTE in hospital settings.
본 연구는 로봇시스템화를 통한 병동 간호업무를 위한 방향에 대해 전망해 보고자 시도되었다. 지금까지 개발된 병동간호업무에 사용가능한 로봇시스템들은 작동의 어려움이나, 공간적 제약, 높은 가격으로 보편화되지 못했다. 병동 간호업무의 로봇시스템화는 가급적 환자와 접촉하는 시간 이외의 업무를 줄여주는데 단순하고 반복적인 업무의 로봇시스템화에 집중해야 할 것이다. 2인 인터뷰를 통해 활력징후측정에 대한 로봇시스템화와, 침상교환을 돕는 로봇시스템화에 대한 아이디어는 공학분야와 함께 논의해볼만 하다. 이에 병동내 간호업무의 로봇시스템화를 설계시 제한점들이 충분히 고려되어야 할 것이다. 그리고 성공적인 간호와 타학제간 융합을 위해 간호업무에 대한 세심한 직무분석이 필요하고, 융합의 시행착오 극복을 위해 간호학과 타학제간 융합학회를 설립할 필요가 있다. 성공적인 병동 간호업무의 로봇시스템화는 병원의 수익 창출 뿐 아니라 간호사의 위상을 높이고, 간호업무의 혁신을 일으키는 발단이 될 것이다.
Purpose: This study aims to establish a theoretical framework for the fall prevention behavior improvement program by verifying the associations between fracture risk, fall risk perception, and fall prevention behaviors in women with osteoporosis. Methods: A survey was conducted in 122 women who were diagnosed as osteoporosis by having T-score of the femur bone density below -2.5 standard deviation from the bone density examination performed in 2 orthopedic clinics located in B metropolitan city between July 2014 and September 2014. The risk of fracture, level of fall risk perception, and fall prevention behaviors were measured. Results: Fall risk perception had a complete mediating effect on the relationship between the fracture risk and fall prevention behaviors in women with osteoporosis. Conclusion: The perception of fall risk is important to enhance fall prevention behaviors in women with osteoporosis, and the development of various education programs to improve awareness of fall risk is needed.
Purpose: This study is to grasp factors influencing the quality of life in older adults after total knee replacement. Methods: This study was conducted with 165 older adults who had TKR at four orthopedic hospitals in D city. Data were analyzed using one-way ANOVA, independent t-test, Pearson's correlation, and stepwise multiple linear regression with SPSS 19.0 software. Results: Pain and depression were negatively correlated with range of motion, social support, while sense of coherence was positively correlated with quality of life. Sense of coherence (43%, ${\beta}=.40$), pain (8%, ${\beta}=-.30$), and depression (3%, ${\beta}=-.20$) on the Physical Component Summary in the quality of life have significant explanatory power of 54%. Sense of coherence (49%, ${\beta}=.44$), social support (6%, ${\beta}=.25$), and depression (3%, ${\beta}=-.22$) on the Mental Component Summary in the quality of life have significant explanatory power of 58%. Conclusion: This study suggests developing a program to improve the quality of life in older adults who had TKR, considering factors such as sense of coherence.
This study was performed from september 26th to October 7th 1981 to investigate the contamination problems of Nurse's hands characterized by var-ious nursing functions. A total of 50 nurse's hands were sampled from 5 different wards of H. University Hospital. The samples were cultured for isolation of microorganisms. The results were as follows: 1. Of 50 Nurses 23 were found to be contaminated by 9 species of bacilli such as Non-fermentative gram negative Bacilli, Gram negative bacilli, Oxidase positive, Enterobacter, Escherichia coli, Pseudomonas aeruginosa, Acinetobacter, Staphylococcus epidermidis, Gaffkya tetragens, Bacillus subtilis. 2. The contaminate rates by wards where they have been serving are; 7 (87.5%) of 8 nurses from Intensive care unit, 7(70%) of 10 nurses from general surgery ward, 3(50%) of 6 nurses from neurosurgery ward, 2(20%) of 10 nurses from orthopedic surgery ward, 4(25%) of 10 nurses from medical ward. 3. The contamination rates by the types of clinical service offered are 6(85.7%) of 7 nurses after wound dressing assist 6 (55.3%) of 13 nurses after vital sign check. 4. The role and function of nursing care nowadays, tend to acquire sophisicated technology because specialization has expended due to increase of the medical population and the improvement of national health standards. To implement nursing care independently as a professional nurse, the apprehension of specific knowledge and skill should be acquired during basic nursing education. So it is important for nursing education not only to include theory and actual techniques, but also to strengthen the practical training in the actual clinical setting. This study was carried out with the following objectives; 1. To survey the detailed content and frequency of actual nursing students display during their clinical training. 2. To investigate the detailed content and frequency of actual nursing behavior which students display in each clinical a area. 3. To identify the motive for selection of nursing as their major and to determine the degree of self confidence, extent of knowledge and recognition of nursing responsibility. 4. To observe the relationship between actual nursing behavior and each of the following; 1) Motive for selecting nursing as a major 2) Self confidence 3) Knowledge of nursing care 4) Recognition of nursing responsibility The conclusions of this study were as follows; 1. Among the detailed nursing behavior which junior nursing college students carry out in clinical training; taking respiration's showed the highest frequency, and taking body temperatures, blood pressures, and pulses and making beds were next in frequency in this order. 2. In detailed nursing behaviors according to clinical area; taking vital signs showed the highest frequency in the emergency room, pediatric ward, orthopedic ward, general surgical ward and internal medicine ward. However, in the operating room, assisting with endotracheal tube insertion and sterile techniques were showen to have the highest frequencies. In nursery, umbilical cord care and the measurement of body weight were the highest in frequency In neurosurgical ward, the measurement of vital signs, changing position and tracheostomy care were the highest in frequency. In obstetric and gynecological ward and in the delivery room, checking duration, intensity and frequency of contractions was the highest in frequency. 3. In regard to the motive for majoring in nursing, the aptitude and interest of the student had the highest percentage(32.86%), and self-confidence in nursing activities (M=3.36), knowledge in nursing activities.(M=3. 09), and the recognition of the nursing activity (M= 3.76) wire in the middle range. 4. When the detailed nursing behaviors were compared with motive, self confidence, knowledge and recognition, it was found that when the nursing behavior was difficult and regarding much endeavor although the motive was high, the frequency of the nursing behavior was rather low. But in the cases in which there was much self confidence and a high level of skill was required, nursing behavior was carried more frequently. When there was muck self confidence and skill was not required, the frequency of nursing behavior was rather low. In the cases of a high level of knowledge, the frequency of nursing behavior was low and when recognition for nursing behavior was given the frequency of nursing behavior was low.
