Sjögren Syndrome after Radioiodine Therapy in Thyroid Cancer Patients

  • Salivary and lacrimal gland dysfunction is relatively frequent after radioiodine therapy. In most cases this is a transient side effect, but in some patients it may persist for a long period or appear late. Radioiodine ($^{131}I$) therapy is often administered to patients following total thyroidectomy to treat well-differentiated follicular cell-derived thyroid cancer. In addition to the thyroid, $^{131}I$ accumulates in the salivary glands, giving rise to transient or permanent salivary gland damage. Salivary gland dysfunction following radioiodine therapy can be caused by radiation damage. But, it also may be associated with $Sj{\ddot{o}}gren$ syndrome (SS) developed after radioiodine therapy. It would be recommended that the evaluation for SS including anti-SSA/Ro and anti-SSB/La should be considered before and after radioiodine therapy.

Simple Qualitative Sensory Assessment of Patients with Orofacial Sensory Dysfunction

  • Purpose: Oral and facial sensation is affected by various factors, including trauma and disease. This study assessed the clinical profile of patients diagnosed with sensory dysfunction and investigated their sensory perception using simple qualitative sensory tests. Methods: Based on a retrospective review of the medical records, we analyzed a total of 68 trigeminal nerve branches associated with sensory dysfunction in 52 subjects. We analyzed the frequency and etiology of sensory dysfunction, and the frequency of different types of sensory perception in response to qualitative sensory testing using tactile and pin-prick stimuli. Results: The inferior alveolar nerve branch was the most frequently involved in sensory dysfunction (88.5%). Third molar extraction (36.5%) and implant surgery (36.5%) were the most frequent etiological factors associated with sensory dysfunction. Hypoesthesia was the most frequent sensory response to tactile stimuli (60.3%). Pin-prick stimuli elicited hyperalgesia, hypoalgesia, and analgesia in 32.4%, 27.9%, and 36.8%, respectively. A significant association was found between the two kinds of stimuli (p=0.260). Conclusions: Sensory dysfunction frequently occurs in the branches of the trigeminal nerve, including the inferior alveolar nerve, mainly due to trauma associated with dental treatment. Simple qualitative sensory testing can be conveniently used to screen sensory dysfunction in patients with altered sensation involving oral and facial regions.

  • Neuropathic orofacial pain (NOP) is initiated or caused by a priomary lesion or dysfunction in the peripheral nervous system or the central nervous system. NOP is divided into paroxysmal and continuous by duration of pain. NOP is important for dentists, because they can occur after dental procedures, such as extractions, endodontic treatment, and dental implant insertion. Dentists should have concern about NOP, because it is difficult to diagnose and treat.

  • Vestibular schwannoma, also known as acoustic neuroma, is a rare benign brainstem tumor surrounding the vestibular division of the 8th cranial nerve. The presenting symptoms are hearing loss, tinnitus, and dizziness. Unabated growth can compress 5th (trigeminal nerve) and 7th (facial nerve) cranial nerve, which can cause nerve dysfunction such as orofacial pain, sensory abnormalities, or trigeminal neuralgia. We report a 51-year-old woman who presented with orofacial dysesthesia on her left side of the face with abnormal findings on 5th cranial nerve and 8th (vestibulocochlear nerve) cranial nerve examination. Brain magnetic resonance imaging scan revealed cerebellopontine angle tumor. She was referred to a neurosurgeon and diagnosed with vestibular schwannoma.

THE ORAL HEALTH STATUS OF DISABLED PEOPLE WITH CEREBRAL PALSY (뇌성마비 장애인의 구강보건 실태)

  • The objective of this study was to assess oral health status such as prevalence of dental caries, gingivitis and other combined disorder, orofacial dysfunction in patients with cerebral palsy(CP), as well as the state of tooth brushing, possibility of dental treatment. Sixty-six subjects with CP aged 9 to 37 years were included in the study. Clinical caries status(DMFT index) and other data were evaluated and statistically analyzed using SPSS program (SPSS 17.0). The results were as follows: The DMFT index and prevalence of gingivitis of study subjects were 2.82(male), 3.33(female) and 89%, respectively. DMFT index classified into four groups according to age as follows: DMFT index were 1.14(ages 6-11), 1.40(ages 12-14), 2.16(ages 15-24), 4.15(ages 25-37). In addition to the physical disorder, speech difficulty(86%), epilepsy(35%) and visual impairment(14%) were associated and the epilepsy medication was the most common medication. And orofacial dysfunction such as the eating difficulties(79%), drooling(36%), swallowing disorder(30%), breathing difficulty(15%), bruxism(30%) and snoring(33%) was shown. Most people with cerebral palsy can't brush alone and didn't use oral care adjunctive supplies. Moreover, Fifty-nine percent of them were noncooperative to dental treatment.

A Study of Bite Force of the Male Patients with TMJ Dysfunction (악관절기능장애환자(남성)의 교합력에 관한 연구)

  • A kinematical study of bite force during voluntary isometric contraction was investigated in 20 Korean men with TMJ dysfunction and 30 Korean men as normal subject, ranging from 20 to 27 years old. The author observed maximum bite force, slope of bite force graph, curve index and duration of maximum bite force with the use of the foil strain gauge (MPM-3000) and RS Dymograph (Beckman). The obtained results were as follows : 1. Maximum bite forces were 29kg and 29kg for left and right side of normal subject (p>0.05) and 19kg and 29kg for affected and non-affected side of TMJ dysfunction patient. 2. The slopes of bite force graph were $68^{\circ}$ and $68^{\circ}$ for left and right side of normal subject (p>0.05) and $59^{\circ}$ and $71^{\circ}$ for affected and non-affected side of TMJ dysfunction patient. (p<0.01) 3. The curve indices were 0.93 and 0.90 for left and right side of normal subject and 1.10 and 0.90 for affected and non-affected side of TMJ dysfunction patient. (p>0.05) 4. The durations of maximum bite force were 424 msec and 413 msec for left and right side of normal subject and 337 msec and 334 msec for affected and non-affected side of TMJ dysfunction patient.   • The author examined 206 temporomandibular disorders patients, who visited department of Oral Diagnosis and Oral Medicine in Seoul National University Hospital during the period from May 1st 1987 to April 30th 1988, clinically and radiologically and with prepared questionnaire. By clinical and radiological examination, the patients were divided into 2 groups(the patients with temporomandibular joint internal derangement and myofacial pain dysfunction syndrome or with myofacial pain dysfunction syndrome: the patient with temporomandibular joint internal derangement only). The symptom severity index and contributing factors were examined with the questionnaire, and differences in 2 groups were studied. The conclusions are followings : 1. The symptom severity index presenting the severity of symptom was higher and the scope of symptom was wider in the patients with temporomandibular joint internal derangement and myofacial pain dysfunction syndrome or with myofacial pain dysfunction compared to the patients with temporomandibular joint internal derangement only. 2. Emotions, behaviors, cognitions, and social factors were more undesirable in the patients with temporomandibular joint internal derangement and myofacial pain dysfunction syndrome or with myofacial pain dysfunction syndrome than the patients with temporomandibular joint internal derangement only.

Helkimos Indices and Frictions Craniomandibular Index in Korean Young Population

  • To evaluate Helhimo's Anamnestic Dysfunction Index(AI), Helkimo's Clinical Dysfunction Index(HDI), and Friction's Craniomandibular Index(CMI) in Korean Young population, clinical examinations were performed in 207 dental college students who were healthy and had no evidence of the craniomandibular disorders. The obtained results were as follows; 1. The mean values of the Helkimo's Anamnestic Index(AI), Clinical Dysfunction Index(HDI), Fricton's Palpation Index(PI), Dysfunction Index(DI), and Craniomandibular Index(CMI) were 0.35, 0.71, 0.03, 0.05, and 0.04 in male subjects. 2. The mean values of the Helkimo's Anamnestic Index(AI), Clinical Dysfunction Index(HDI), Fricton's Palpation Index(PT), Dysfunction Index(DI), and Fricton's Craniomandibular Index(CMI) were 0.42, 0.72, 0.02, 0.04, and 0.03 in female subjects. 3. There was no statistically significant difference between male and female subjects in each parameter. 4. The palpation index(PI) observed in Korean Young Population was lower than that in American population.

Clinical Features of the TMD Patients with Degenerative Joint Disease (퇴행성 악관절장애환자의 임상양태에 관한 연구)

  • An Epidemiologic study was carried out on 77 TMD patients with degenerative joint disease who had visited the Orofacial Pain Clinic in Pusan National University Hospital. Al subjects were interviewed and examined clinically and radiologically using a standardized examination form. As related to gender and duration, subjective and objective sysmptoms in DJD patients were studied. The obtained results were as follows : 1. There were much more patients in the twenties or thirties, women and histories such as chronic duration and microtrauma. 2. Most patients responded positively more often to the questions of jaw function, unilateral chewing in habits, poot appetite and depression in behavioral response and shoulder pain in worsening prognosis 3. While the most common reasons for treatment were pain, noise, and limitation of opening, the associated symptoms such as headache, neckache, earache, jaw dysfunction, neck dysfunction, acute bite change and dizziness, ringing or fullness in the ears as secondary CNS excitatory effects were complained. 4. Opening the mouth in 25 to 40mm, soft end feel and deflective incisal pathway were seen and more tenderness to lateral or dorsal capsule of joint than intra or extra oral muscles were complained. 5. While there appeared no click, crepitus and single click in acute group, in chronic group, crepitus, single click and no click appeared in order of sequence. 6. Tomogram or bone scan revealed more bony changes than panorama and transcranial view.

Electromyographic Observation of Silent Period of the Masseter Muscle (교근의 휴지기에 관한 근전도학적 관찰)

  • The author has studied the duration of silent period of the masseter muscle using electromyography in order to obtain the information that is necessary to diagnose the patients with TMJ dysfunction syndrome objectively. The 55 normal subjects and the 40 patients with TMJ dysfunction syndrome were selected for this study. The results were as follows : Duration of silent period of the masseter muscles in normal subjects were 24.4 msec in males, 24.0 msec in females, and mean of $24.2\pm3.19$ msec. Duration of silent periods of the masseter muscles in patients with TMJ dysfunction syndrome were 39.4 msec in males, 41.3 msec in females, and mean of $40.5\pm7.01$ msec. There was no significant difference of the duration of silent periods between in the right and the left side of the masseter muscles, and between in the males and the females (p>0.05). Duration of silent periods of the masseter muscles in patients with TMJ dysfunction syndrome were significantly more prolonged than those in normal subjects (p<0.01). Borderline of silent periods of the masseter muscles between in normal and patients with TMJ dysfunction syndrome was 31.5 msec.
