A Clinical Study: Pain Intensity Before and After Balneotherapy in Participants with Low Back Pain (5주간의 온천요법 전후 요통의 통증변화에 관한 임상연구)

  • Objectives The aim of this clinical study was to find out the changes of self-reported pain intensity before and after balneothrapy in participants with low back pain. Methods In this study 20 participants with low back pain went through 5 weeks of balneothrapy course, once in a week. Each week visual analogue scale (VAS) and pain threshold were measured before and after balneothrapy, which consisted of 30 minutes of high pressure underwater shower ($32{\sim}36^{\circ}C$) and another 30 minutes of whole body bathing ($32{\sim}40^{\circ}C$). Results 1. The average of VAS significantly decreased from $64.00{\pm}16.35$ to $34.00{\pm}15.69$ after 5 weeks of balneothreapy (p<0.001). 2. The average of pain threshold significantly increased from $8.31{\pm}2.52$ lbf to $9.53{\pm}3.11$ lbf after 5 weeks of balneothreapy (p<0.05). Conclusions The results showed that 5 weeks of balneotherapy had significant effect on alleviating pain in low back pain participants, which means balneothreapy may be used as a complementary treatment on musculoskeletal diseases and other chronic diseases. Further studies are anticipated in the future to find out other various effects of balneothrapy.

  • This case was report of intra-articular bee venom pharmacopuncture injection on the patient with Acute Traumatic Partial tear of meniscus. We used intra-articular bee venom pharmacopuncture injection to Acute Traumatic Partial tear of meniscus diagnosed by symptoms and MR imaging. Be under treatment if necessary we prescribed herbal medication and physiotherapy. The state of patient was measured by Visual Analog Scale(VAS) and Walking time and Western Ontario and McMaster Universities(WOMAC) Index score. After several times of treatments, noticeable reduction of pain was measured and increased time of walking on floor and decreased WOMAC score. This results suggest that intra-articular bee venom pharmacopuncture injection are effective to treatments of Acute Traumatic Partial tear of meniscus.

Korean Medical Therapy for Knee Pain after Anterior Cruciate Ligament Reconstruction

  • Objectives : The aim of this study was to report the effect of Korean medical therapy on pain and dysfunction after anterior cruciate ligament (ACL) reconstruction. Methods : A 25-year-old man experienced severe pain after right ACL reconstruction surgery. He received Korean medical treatments such as acupuncture, herbal medicine, and physiotherapy from July 10, 2014 to August 2, 2014. Results : After the treatments, his visual analogue scale scores generally decreased and the range of motion of the right knee improved from $0^{\circ}$ to $90^{\circ}$. Furthermore, the Knee Infury and Osteoarthritis Outcome Score increased from 99 to 142. Conclusion : The findings suggest that Korean medical treatments might be effectively used to treat pain and dysfunction after soft-tissue surgeries such as ACL reconstruction. Nevertheless, further research is warranted because of the limited sample size of this study.

A Literatural Study on the Evidence of Using Thermotherapy and Cryotherapy of Meridian Muscle in Korean Medical Physiotherapy - Focusing on 『Hwangjenaegyeong』 - (경근(經筋)에 대한 온열(溫熱)·한냉요법(寒冷療法)의 적용에 대한 고찰(考察) - 『황제내경(黃帝內經)』을 중심으로 -)

  • Objectives : The purpose of this paper is to establish theoretical bases upon which to complement the clinical evidence in representative literature (Hwangjenaegyeong;黃帝內經)of Korean medicine. Methods : We searched applicable paragraphs about thermotherapy and cryotherapy in Hwangjenaegyeong(黃帝內經) and defined them as historical and theoretical bases of thermotherapy and cryotherapy in korean medicine. Results : The meridian muscle treatment originated from Young-Chu(靈樞) was developed for many centuries. Five paragraphs about thermotherapy and one paragraph about cryotherapy are in Hwangjenaegyeong(黃帝內經). These records to treat the disease of the muscle meridian are enough to explain historical and theoretical evidences. Conclusions : Thermotherapy and cryotherapy of meridian muscle are treatments which are used widely in Korean medicine. They mean that thermotherapy and cryotherapy has been explained under the system of Korean medicine. It is necessary for more literatural study and clinical trials to be carried out to secure the evidence of physical therapies in Korean medicine.

Case Report of a Breast Cancer Patient Treated with Adriamycin/5-Fluriuracil Regimen in Combination with Traditional Korean Medicine for Mitigating Side Effects (Adriamycin/5-Flurouracil 치료 중인 유방암 환자의 한.양방 병용치료를 통한 부작용 경감에 대한 증례보고)

  • Objectives : To observe the mitigating effects of a Traditional Korean Medicine treatment program, called Wheel Balanced Cancer Therapy (WBCT), with Adriamycin and 5-FU chemotherapy on a breast cancer patient. Methods : A 26 year old female patient diagnosed with stage IIb breast cancer was admitted to the East-West Cancer Center (EWCC) in February of 2010. She received Adriamycin and 5-FU from February 22nd, to July 20th, 2010 followed by WBCT consisting of herbal medicine, acupuncture, moxibustion and physiotherapy for 5 months. Her symptoms were measured by Common Terminology Criteria for Adverse Events (CTCAE) and her quality of life was measured by Eastern Cooperative Oncology Group (ECOG). Results : WBCT significantly alleviated chemotherapy-induced nausea, oral dryness, and peripheral neuropathy. Quality of life also significantly improved. Conclusions : This case study potentiates WBCT's significant efficacy in aiding breast cancer patients suffering from Adriamycin plus 5-FU chemotherapy induced adverse effects.

A statistical report of combined treatment of Oriental and Western medicine for inpatients (한·양방 협진(協診) 및 협치(協治)에 관한 통계 보고 -입원 환자를 중심으로-)

  • After 394 inpatients examined as to the result of combined treatment of Oriental and Western medicine on Dongguk Incheon oriental medicine hospital and Dongincheon Gil hospital from july/21/1994 to june/21/1995 for 11 months, the results were obtained as follows : 1. Distribution of sex ; male 187(47.5%), female 207(52.5%) 2. Distribution of age ; 60-69 years 110(28%), 50-59 years 88(22.3%), 70-79 years 84(21.3%), 40-49 years 39(9.9%), 30-39 years 34(8.6%) and the others 39(9.9%) 3. Distribution of disease ; cardiovascular disease 234(59.5%), musculoskeletal disease 62(15.7%) and the others 98(24.3%) 4. Distribution of consult from Oriental medicine to Western medicine ; internal medicine 373(87.4%), Orthopedic 32(7.5%) and the others 22(5.1%) 5. Distribution of Western medicine treatment ; fluid therapy 163(35.5%), antihypertensiv 100(21.8%), antibiotics 53(11.6%), oral hypoglycemic agents and insulin preperations 46(10.1%) and the others 95(20.8%) 7. Effects of Moxibustion on the Hemiplegic Upper Extremity After Stroke (중풍환자의 상지마비에 대한 뜸치료의 유효성 검토)

  • Background and Purpose : Hemiplegic upper extremity is a problem frequently encountered in the rehabilitation of patients with stroke. In Korean traditional medicine, moxibustion has been used clinically in treatment of stroke patients with hemiplegia. So far, its efficacy has not been proven clinically. The purpose of this study was to evaluate the efficacy of the moxibustion in treating hemiplegic upper extremity in stroke patients. Design : Randomized Control Trial. Subjects and Methods : Forty hemiplegic stroke patients admitted to Kyunghee oriental medicine hospital were randomized into the treatment with standard physiotheraphy combined with Moxibustion-group or Control-group with standard physiotherapy alone. It took them 2-5 weeks from the onset to start this study. Moxibustion was applied at LI4(合谷), LI11(曲池), TE3(中渚), TE5(外關) in hemiplegic hand, once a day for 2weeks. The effect of treatment on hemiplegic upper extremity was assessed using Fugl-Myer motor scale, Motricity Index and Modified Barthel Index(drinking/feeding, dressing upper body, grooming) Results : These 2 groups had comparable clinical characteristics; sex, age, plegic side (Rt., Lt.), pretreatment impairment. After two weeks, patients in the moxibustion group perfomed better on Fugl-Myer test and Motricity index test. The differences were significant.(P=0.038, 0.002) But Results on the Modified Barthel Index revealed no effect.(P=0.348) Conclusion : This results suggest that moxibustion is an effective treatment for improvement of motor function of hemiplegic upper extremity.

The Comparative Study of Improvement of Patients Who Were Diagnosed with HIVD of L-Spine with Iliopsoas Muscles Tenderness by MST(Motion Style Treatment) on Iliopsoas Muscles: A Randomized, Controlled, Trial (장요근 압통을 동반한 요추 추간판 탈출증 환자에 대한 장요근 MST 호전도 비교 연구: 무작위 대조 연구)

  • Objectives : This research was to evaluate the MST effectiveness on iliopsoas muscle of herniated inter-vertebral disc in lumbar spine. Methods : This research was carried out with the 58 inpatients who received treatment for their HIVD in lumbar spine from January 1 to April 12, 2014 in Daejeon Jaseng hospital of oriental medicine. We randomly divided into two groups: Group A=common treatment on HIVD of L-spine without MST(acupuncture, pharmacopuncture, herb medication, chuna treating and physiotherapy) and Group B=common treatment on HIVD of L-spine with MST. To verify the MST effect, tenderness(checked by algometer pressure), NRS(numeric rating scale), and ODI(Oswestry disability index) were measured before and after treatment. The statistically significance was evaluated by SPSS 18.0 for Windows. Results : In the Group B, the tenderness was significantly decreased compared with Group A. However, on the NRS and ODI, it did not produce the meaningful results as these two values decreased in all groups. Conclusions : From this research, when patients who were diagnosed HIVD of L-spine with iliopsoas muscles tenderness receive MST additionally, the satisfaction of patients as well as physician confidence will increase.

The Retrospective Review of 410 Shoulder Pain Inpatients in Korean Medicine Hospital (어깨 통증으로 한방병원에 입원한 환자 410명을 대상으로 한 후향적 연구)

  • Objectives The purpose of this study is to investigate the characteristics of inpatients who were admitted to Korean medicine hospital due to shoulder pain. Methods Characteristics of four hundred ten shoulder pain inpatients of single institution from 2011 to 2013 were retrospectively reviewed through medical charts. Results Patients showed different characteristics in sex, age, motives, interval between onset and admission day, first occurrence or not, related medical history before admission, usage of west medication, admission days, treatment results, radiological examinations by shoulder pain disease groups. Accordingly, Korean medical treatment such as acupuncture, moxibustion, external treatment, cupping therapy, physiotherapy, herb-medication varied with the characteristics of shoulder pain disease groups. Conclusions Through retrospective medical charts review, characteristics of 410 single institution shoulder pain inpatients showed distinctive features and versatile Korean medicine treatments by disease groups. It is expected that this study would accelerate multi-institutional and large scale characteristic review of shoulder pain patients, which would raise reconsideration and expand boundary of Korean medicine.

A Comparative Study of Clinical Papers about Pharmacopuncture in Korea and China - Focusing on Papers about Musculoskeletal and Nervous Diseases - (중국과 국내에서의 약침 관련 임상논문 비교 연구 - 근골격계 및 신경계 질환을 위주로 -)

  • Objectives : To research the trend of the study related to pharmacopuncture about musculoskeletal and nervous diseases in China and to make a comparison with clinical application on pharmacopuncture about musculoskeletal and nervous diseases in China and Korea. Methods : I reviewed and analyzed four Chinese Journals relating pharmacopucture from 2004 to 2008 and two Korean Journals relating pharmacopuncture from 1999 to 2008. Results : The following results were obtained in this study. 1. The pharmacopuncture was often used for diseases of spines and joints. 2. In the medicine of pharmacopuncture, Salviae Miltiorrhizae Radix, Angelicae Gigantis Radix were mainly used in China, and Bee-venom in Korea. 3. As a method of needling, local acupoint and Ashi-point were mainly ueed in both China and Korea. 4. In combined treatments, acupuncture, electronic acupuncture, Chuna, heating acupuncture were mainly used in China, and acupuncture, herb medicine, physiotherapy in Korea. 5. In the case of control group study, compared with other treatments pharmacopuncture was not verified the superiority of effectiveness in China, but in Korea, the effectiveness of a pharmacopuncture could be compared with that of others and the effectivenss of different pharmacopuncture could be compared. Conclusions : As seen above, in Korea, clinician should carry out clinical application about musculoskeletal and nervous diseases with various herbal injections besides Bee-venom. And I think that it will be starting point to use Salviae Miltiorrhizae Radix and Angelicae Gigantis Radix that were often used in China.
