한방간호중재에 관한 필요성, 지식 및 실천에 관한 연구 (A Study on Oriental Nursing Intervention of Necessity, Knowledge and Practice)

  • Purpose: The purpose of the study is to analyse the current situation of oriental nursing intervention in local university oriental medicine hospital to provide qualified oriental nursing. Its specific purposes are as follows. 1) To analyse the objective's necessity to the oriental nursing intervention, knowledge and practice. 2) To analyse the correlation between the oriental nursing intervention, knowledge and practice. Method: The design of the study is a narrative investigation-research. The objectives of the study composed of 288 RNs having more than a three-year-experience, working for the university oriental medicine hospital. The tool of the study comprised of 194 questionnaires with the reliability of Cronbach's ${\alpha}=.95$. The data were collected from Dec. 11. 2002 to Feb. 20, 2003 and computerized by SPSS program. Result: The necessity of oriental nursing intervention of nurses working in the oriental hospital showed 3.60 measured by 5-scale-measurement, 2.69 in knowledge and .24 in practice measured by 1-scale-measurement. Conclusion: The oriental nursing intervention on site showed high necessity, moderate knowledge and low practice recommending a continuous education comprised of regular nursing education and in-service program.

한방간호연구 논문분석 (An Analysis of Research Trend about Oriental Nursing Published in Korea)

  • The study attempts to analyze the trends of study related with oriental nursing. This article reviewed 81 researches on oriental nursing from 1985 to July 2000, by examining them according to the articles' characteristics, sources by publication period, research design, nursing intervention effect of experimental study and concept, and content of literature review. The findings of the analysis can be summarized as follows; 1. The number of studies related with oriental nursing has progressively increased by year from 1985, and that of articles was the highest during the period between 1995 to 1999. Since the 1990s, the number of articles including thesis and dissertation increased, and a third of subject articles were thesis among them. 2. The majority of research design were survey and correlational studies, and most of them were published in the 1990s. Literature review articles also occupied a certain portion, and most of them were published after 1995. In addition, the research design of dissertation revealed in various types. 3. Nursing intervention used in the experimental study was Dan Jeon Breathing, San-Yin-Jiao (SP-6) Pressure, Percutanous Electrical Nerve Stimulation, Guided Imagery, Diet for Sasang Constitution, Yoga Exercise, Qigong Exercise, Meridian Massage, Bed Exercise Program, Health Contract and Structured Rehabilitation Education. Among the study subjects, patients with chronic illness were at the highest rate, and the most nursing intervention effects presented physiological effect, activities of daily living and relieving pain. In most studies, the affirmative effect proved. 4. The key concept of literature review articles is energy, oriental nursing, health promotion, oriental medicine management of illness and etc. Being important concepts to understand oriental nursing, they confirm similar perspective of oriental medicine and nursing and show simultaneously the importance of nursing considered into the cultural perspective. The summary of the above results shows a tendency that the thesis and dissertation have been relatively considered with various research designs, while the others have focused on literature review. Especially, experimental study proved affirmative effect through various experimental treatment of nursing intervention related with oriental medicine. Besides in the literature review, inquiry of theories and practical perspective, which is necessary for the establishment of oriental nursing, was introduced. However, for the establishment of oriental nursing, more studies are to be required along with development of nursing intervention related with health promotion.

간호중재분류(NIC)에 근거한 간호중재수행분석 II -한방병동과 일반병동 간호사를 중심으로- (Nursing Interventions Classification(NIC) Use in Korea : Oriental Medicine Hospitals and General Hospitals)

  • The purposes of this research were to identify nursing interventions performed by Korean nurses and to compare the interventions performed by nurses working in the oriental medicine hospitals and with those performed by nurses working in the general hospitals. The samples consisted of 144 Korean nurses working in three hospitals, 70 nurses working in the oriental medicine hospitals and 74 nurses working in the general hospitals. The Nursing Interventions Classification (NIC) Use Questionnaire developed by the Iowa Intervention Project team was translated to Korean and verified using the method of back-translation. The questionnaire consists of 433 intervention labels and definition. Thirteen interventions were used at least daily by nurses working in the oriental medicine hospitals, while twenty-one interventions were used at least daily by nurses working in the general hospitals. The most frequently used interventions by nurses working in the oriental medicine hospitals were Documentation, Shift Report Vital Signs Monitoring, Pressure Ulcer Prevention, Positioning, Fall Prevention, Exercise Promotion, Intravenous (IV) Therapy, Pressure Ulcer care, and Bed Rest Care in that crder. For nurses working in the general hospitals the most frequent intervention was Analgesic Administration, followed by the interventions of Medication Administration : Parenteral and Intravenous Therapy (IV) Therapy, Documentation, Intravenous(IV) Insertion, Shift Report, Fall Prevention, Vital Signs Monitoring, Medication Adnninistraction : and, Fluid Monitoring, and Medication Maragement in that order. The interventions performed least often by nurses working in the oriental medicine hospitals were Hemodialysis Therapy and Bleeding Reduction : Antepartum Uterus, while the interventions performed least often by nurses working in the general hospitals were Rape Trauma Treatment and Contact Lens Care. The nurses working in the oriental medicine hospitals performed the interventions in the Physiological : Complex domain significantly more often than the nurses working in the general hospitals, while the nurses working in the general hospitals performed the intervention in the Behavior domain significantly more often than the nurses working in the oriental medicine hospitals. This study suggests that further study will be needed to developed and validate more interventions sensitive to Korean culture.

  • The auricular acupuncture therapy is stimulus treat method that acupuncts or moxibusts at the reaction blood vessel point of various diseases in the upper portion of ear. And this effects from the reaction blood vessel point of an ear to the meridian of whole body system through blood stream. The auricular acupuncture has cured and cared signs and symptoms of many diseases and pathologic problems. And this treatment heels abnormal reactions, nursing focuses so it was examined through literature review that nurses use many portions of the auricular acupuncture in their clinical cares of oriental medicine. This study was proved that the Mae-Chim acupuncture method among various methods of the auricular acupuncture is easy and simple so this method can be used by nurses. And the ear acupuncture site which controls diarrhea, insomnia, migraine, stress vomiting, dyspepsia, high fever, insufficient lactation, bowel dysfunction, menoxenia, anxiety, pruritus and can be found easily and is few the number of acupuncture so the oriental medicine nurses can apply easily to their clients. Therefore the independent oriental medicine nursing intervention can be developed by application of the auricular acupuncture therapy on the oriental medicine clinical nursing and home care in our community.

Diet for Sasang Constitution etc. 4. The key concept of literature review articles is ki(energy), oriental nursing, health promotion, oriental medicine management of illness etc. Conclusion: The summary of the above results shows a tendency that the study has increased by year. Especially, experimental study proved affirmative effect. Besides in the literature review, inquiry of theories and practical perspective, which is necessary for the establishment of oriental nursing was introduced. However, for the establishment of oriental nursing, more experimental and qualitative studies are to be required.

  • Purpose: To identify the agreed opinion of experts on oriental health promotion services and nursing in Korea. Method: The study is based on the 3-round Delphi method. As a Delphi panel, a total of 28 people with at least three years of experience in the area of oriental medicine consisted of this. Results: The experts suggested that Yangsaeng, lectures on prevention of CVA, correct understanding of the misuse/abuse of oriental medicine, daily health care, meridian/acupoints massage, and Kigong therapy should be included in the promoting oriental health nursing servies. Conclusion: The agreed opinions by the experts on the oriental health promotion services and nursing, can help enhance the understanding of oriental health promotion. The curriculum should be developed further and tested with the development of the Korean oriental nursing intervention methods which is appropriate to Korean culture.

안면경락마사지 간호중재가 뇌졸중 환자의 안면마비 회복에 미치는 영향 (The Effect of Meridian Points Massage Nursing Intervention on the Recovery of Facial Paralysis)

  • The purpose of this study was to develop on east-west nursing intervention program. This study was conducted to identify the effectiveness of using facial meridian points for massage nursing intervention on the recovery of facial paralysis for 46 patients (22 for experimental group). All had suffered from strokes, and were also admitted to the division of oriental medicine in K University Hospital. Method: Data was collected from February 1st to December 31st of 2000. This study used a quasi experimental, non-equivalent control group pre-test and post-test design. The recovery of facial paralysis was measured by the difference between left and right facial length, observation and self report on a seven point scale, and facial discomfort by a facial scale after 6-7 facial massages in a two week period. Data was analyzed using the SPSS package program with x2 and t test. Result: The result of this study are as follows; The experimental group who received the facial meridian massage showed higher scores in recovery of facial paralysis (t=2.72, p=.009), and a smaller difference between left and right facial strength than those in control group (t=2.26, p= .29). The discomfort of the facial area in the experimental and control group showed no significant differences. Conclusion: These findings indicate that a facial meridian points massage could be an effective nursing intervention to the recovery of facial paralysis. This study contributes in developing an east-west nursing intervention with the oriental meridian theory and western massage therapy.

사상체질의학을 적용한 국내 간호연구 동향 분석 (Trends in Nursing Research using Sasang Constitutional Theory)

  • Purpose: This research aimed to acquire the trends of Korean nursing research on Sasang constitutional theory. Methods: Total of 25 research articles selected from 17 nursing journals published in Korea since 1990. Data were analyzed by descriptive statistics. Results: Experimental studies preferred to non-experimental research design. The subjects were mostly adult or geriatric population. Most frequently used intervention was diet based on Sasang constitutional theory, and health-related factors were most frequently used as variables. Conclusion: The Sasang constitutional theory considers personal differences important, so it may be suitable for individually tailored nursing care. Therefore, continuous nursing studies using interventions based on Sasang constitutional theory are needed to establish it as one of evidence-based nursing interventions.

발반사요법이 뇌졸중환자의 일상생활동작과 피로에 미치는 효과 (The Effects of Foot Reflexology on ADL and Fatigue in Stroke Patients)

  • Purpose: This study was to examine the effects of foot reflexology on ADL and fatigue in stroke patients. Method: The subjects were 31 stroke patients hospitalized in the Oriental Medicine Hospital of D University from June to November, 2002. Foot reflexology was applied to the experimental group twice a week for 6 weeks, 40 minutes each. For the data analysis, $x^2$-test was conducted to verify the homogeneity of general characteristics and clinical characteristics, and t-test was done to verify the homogeneity of ADL and fatigue. To examine the relative efficacy of the intervention, ANOVA and ANCOVA were conducted. Results: After foot reflexology, the subjects in the experimental group showed significant improvement in ADL. They also had less physical, psychological, and neurosensory fatigue, which are three areas of fatigue. Conclusion: The results suggest that foot reflexology is an effective intervention that helps the body work efficiently, eases stress and strain, and enhances the homeostasis of the body through stimulating the reflex zone of internal organs in the body. Therefore, it is necessary to develop foot reflexology as an unique nursing intervention.

한방근무 간호사의 업무분석 (An Analysis on Nursing Activity in Oriental Hospital)

  • The need of oriental nursing role is recently increasing. The implication of this trend is enormous not only for the need of independent of independent oriental nursing intervention. Hence, this study was designed to describe the characteristics of oriental nursing activity and identify the meaning of nursing activity in oriental hospital. A total of 24 nurses were selected by convenience sampling from eight units at one oriental hospital. A total of 168 data were collected using minute-by-minute recording during nurse s work shift. The data were analyzed using nova and scheffe method as post-hoc test The results of this study are as follows; 1. the category of nursing activity in oriental hospital 74.4% of nursing activity was the patient-oriented nursing care. The nursing activity included others (18.28%), the function-oriented nursing care (7.07%), and staff-oriented care (0.2%). 29.1 % of the patient-oriented nursing care was the direct care, and 24. 5 % of the patient-oriented nursing care was the indirect care. There was a lack of patient education and the communication patient. 2. Nursing activity in oriental hospital by characteristics 1) Nursing activity by unit For patient-oriented nursing care, there was a statistically significant difference among 8 units. In the critical care unit, the patient-oriented nursing care was mostly performed. For function-oriented nursing care, there was statistically significant difference among 8 units. In the physical therapy unit, the function-oriented nursing care was mostly performed. 2) Nursing activity in oriental hospital For all kinds of nursing activity, there was a statistically significant difference between shifts. In night shift patient-oriented nursing care, function-oriented nursing care, and other kinds of nursing care was mostly performed. Patient-oriented nursing care included indirect care, treatment set management, and the breaktime. In day shift, staff-oriented nursing care was performed. 3) Nursing activity in oriental hospital For all kinds of nursing activity, there was not a statistically significant difference by day. The day has not impact on nursing activity. 3. the meaning of activity in oriental hospital The results of this study show that the patient-oriented care was vital check, input! output check, medication, bedsore prevention, nasogastric feeding, oral care, catheterization care, perinatal care, nursing care associated with acupuncture and moxacautery, observation (fever, sweating), heat and cold application communication with patent and family, and patient room management In conclusion, two issues associated with the findings of the research appeated to be involved in the difficulty of nursing activity in hospital. The first issue was the nursing care in oriental hospital provided by nurses who have a limited understading of the principles of oriental medicine. The second issue was the deficiency of systematic guideling for oriental nursing activity. The findings suggest the need to develop the systematic guideline for oriental nursing activity. The need of continuing education for nurses who work in the oriental hospital, and the need of nursing education including oriental nursing are critical.
