A Study on Sinusitis in Pediatric Patients : the Analysis of Age and the Classification of Korean Medical Clinical Type (한방병원에 내원한 부비동염 환아의 연령 분석 및 임상 유형 분류)

  • Objectives The purpose of this study is to analyze sinusitis patients who visited the department of pediatrics, OO Korean medicine hospital by using their age and to classify the clinical type by Korean medical theory. Methods The study was conducted based on 178 cases that consisted of sinusitis patients (from 1 to 15 years old) who visited OO Korean medicine hospital from March 2014 to March 2015. We analyzed the age of patients and classified them by the clinical type by reviewing patient's charts. After that, we compared the results with the results of previous studies. Results and Conclusions 178 Patients were studied. 2-years-age group was 19.7% of the study group which was higher than that of previous studies. The Lung-Kidney Yin Deficiency (肺腎陰虛) group was 33.1%, Wind-Heat (風熱) group was 29.8% and the Wind-Cold (風寒) group 18.5%, Spleen-Lung Qi Deficiency (脾肺氣虛) group was 15.7% and the Heart-Spleen Qi Deficiency (心脾氣虛) group was 2.8% of the study group. Children are full of Yang but lack of Yin (陽常有餘陰常不足) so they easily transform into heat and fire (化熱化火). The patients who had sinusitis were most likely to suffer from the common cold for more than a week, once a month. 45.1% of the people from the study group was suffered from common cold more than a week, and 43.8% of the people got common cold once a months. About 21.6 % and 18.3% of the people got common cold twice a month and once every 2 months, respectively. The remaining 15.7% got cold during the season changes.

A Case Series on Clinical Characteristics of Patients Who Underwent Bone Age Test and Relationships between Methods of Bone Age Measurements (골연령 검사를 시행한 환자들의 임상적 특성과 골연령 측정 방법 간의 상관성에 대한 후향적 연구)

  • Objectives The purpose of this study is to analyze the characteristic of the 157 pediatric patients who visited a Korean medical hospital for bone age (BA) measurement and to compare the BA measurement using Tanner-Whitehouse 3-based analysis software and specialists in pediatric Korean medicine to determine the relationships. Methods The study included 157 pediatric patients who visited a Korean medicine hospital for BA measurements from June 2021 to June 2023. They were analyzed to determine the characteristics and agreement of BA measurements using Tanner-Whitehouse 3-based analysis software and specialists using classification by age, intraclass correlation coefficient (ICC), and Bland-Altman plots. Results In total, 61.8% of the study population were boys and 38.2%, girls, with an average age of 11.32 ± 1.87. The types of growth were various, and the average of bone maturity was 0.48 ± 1.13 as concluded by analysis software and 0.38 ± 1.15 as concluded by the specialists. The ICC of two methods were excellent: 0.995 (overall), 0.996 (boys) and 0.994 (girls). However, BA determined by analysis software tended to be higher than that determined by specialists when the patients were young. Conclusions This study showed that patients visited the hospital for BA measurement regardless of their height, and the agreement of BA between the two methods can be used as a reference. However, there was differences in the trends between age and sex, which should be interpreted with caution.

Study on Development a Personal Health Record Application of Atopic Dermatitis in Korean Medicine (아토피피부염 개인건강기록 앱 개발에 관한 연구)

  • Objective : The purpose of this study is to develop a personal health record(PHR) application of atopic dermatitis in Korean Medicine(KM). Methods : We have identified the items necessary to provide an PHR application that helps to record and manage the symptoms of an atopic dermatitis in KM. We also derived the symptom collection process and method and applied it to the application. Results : In this study, the types of symptoms collected for atopic dermatitis were derived. Symptoms include daily check, stool/urine/sleep, daily emotion, meal management, symptom photographs, SCORAD, quality of life, progress check, original symptom, pediatric health check, weakness check, and subjective symptoms. The recording cycle can be divided into the first, daily, weekly, specific day, and subjective. We developed the PHR application of atopic dermatitis in KM by deriving the type of symptoms and symptom recording process. The app organized menus into dashboards, checklists, daily checks, and health records. Conclusions : We developed a PHR application for atopic dermatitis by deriving symptom collection items of atopic dermatitis and developing symptom collection process and collection technique. The app does not make an accurate diagnosis of atopic dermatitis symptoms, but it helps facilitate symptom collection and helps to identify or predict a person's health condition. It can also be used for medical treatment through sharing symptoms with Korean medicine. Patients are able to communicate in both directions on a daily, weekly, self-aware basis, at the request of a doctor, to record their symptoms and use them for medical purposes. If the doctor asks for a progress check to refer to the next examination, it can be written through the system. This manages atopic dermatitis in daily life and can be used in the clinical field.

Study on Weak Children Information Collection Using Personal Health Record (PHR) (개인건강기록 (PHR)을 이용한 허약아 정보 수집 연구)

  • Objectives The Korean Medicine (KM) PHR platform is a personalized healthcare service which allows individuals to keep and manage their own health records. When parents are reporting for their children from their memories, there is high possibility of recall errors. In these cases, it could be useful for doctors to collect the patient's symptoms through PHR platform. In this study, we aimed to investigate the clinical significance of the PHR by using the KM PHR platform in the pediatric clinic. Methods The PHR platform was used to collect child health information from parents and child care teachers. The collected data were analyzed in comparison with the results of screening by pediatrician. Results A total of 58 children were recruited, 44 of which health information were collected from their parents and their child care teachers. The remaining 14 children's health information were collected from their parents only. As a result the parents tended to perceive their children weaker than the child care teacher. Compared to other organs, there was a only significant difference in the heart weakness score and spleen weakness score in the comparison of the weak and healthy children. Conclusions Although the study was conducted on a small group of subjects, and used PHR platform developed specifically for adults to indirectly input child's symptoms, and analyzed their health information, there was a difference in health records between information providers. Development of PHR platform for children is needed to collect more reliable information.

Korean Translation and Validation of the Paediatric Allergic Disease Quality of Life Questionnaire (PADQLQ) (Paediatric Allergic Disease Quality of Life Questionnaire (PADQLQ)의 한국어판 개발과 신뢰도, 타당도에 대한 연구)

  • Objectives In pediatrics of Korean Medicine, it is crucial to evaluate the overall symptoms of children, especially in the children who have serious troubles in their life. In the era when diagnosis in Korean Medicine is quite suitable to evaluate multiple allergic diseases, and when growing numbers are in needs, development of an objective measurement scale for diagnosis of allergic disease in Korean Medicine became essential. Therefore, in the study, pediatric Allergic Disease Quality of Life Questionnaire (PADQLQ) was translated into Korean version with validations on some aspects. Methods In this study, we translated and validated the PADQLQ and established psychometric evidence on its utility in Korean children with similar problems. 71 children, age of 6 ~ 18 with allergic disease has participated. Results The internal consistency reliability of the PADQLQ by Cronbach's ${\alpha}$ was 0.94. In three domains (Practical, Physical, Emotional), the Cronbach's ${\alpha}$ was 0.77, 0.90 and 0.69. The Spearman's rank correlation coefficient between PADQLQ total score and VAS was 0.75 (p<0.001). And, Structural validity was evaluated by the ANOVA test. QoL differences of the four groups in the three domains (practical, physical, emotional) were significant. The item discriminative indices were ranged from 0.1651 to 0.8188. Conclusions Korean version PADQLQ is highly reliable and valid for children with allergic diseases. This is significant to assess the relative seriousness of the allergic diseases and to evaluate the efficacy of different treatments in Korean Medicine.

Fundamentals and Research Trend of Laser Acupuncture (레이저침의 원리와 연구동향 분석)

  • Objectives : Being introduced in the 1980s, laser acupuncture studies have been steadily carried on such as its thermal effects on human body. However, the mechanisms of effect are not yet clearly explained. Generally, laser acupuncture could be favorable for pediatric patients or people under special conditions (hemophilia, needle-phobia, pathogen-carriers, etc.) because of lower risk of infection, bleeding, pain or fainting. But there are still problems for using laser acupuncture equivalently as the traditional metal needle. The goal of this study is to review the fundamentals, history and present trend of laser acupuncture study, and to suggest how to develop laser acupuncture as an oriental medical device. Method : We performed literature search using PubMed(http://www.ncbi.nlm.nih.gov/pubmed) and Scopus(http://www.scopus.com). Search conditions are laser acupuncture, laser needle, LLLT(as title or abstract) or Litscher G(as author). Main target studies are physical fundamentals, clinical trials, physiological results and device proposals. Results : Many research have shown the similarities of laser acupuncture and traditional metal acupuncture and have reported clinical and physiological effects of laser acupuncture. But, still laser acupuncture is not widely used in Korea due to high-cost of equipments and the lack of clear understanding of laser devices. Additionally, there are still challenges in standardization and device development. Conclusions : Frequencies, laser modes, waveguides, adaptor types, polarization and signal modulations are suggested as future study subjects on laser acupuncture. More various studies and efforts for device development have to be performed to provide oriental medical doctors with evidence based confidence about usage of laser acupuncture.

The Clinical Study in Children with Common Cold (in summer & winter) (하절기(夏節期)와 동절기(冬節期)의 감모환아(感冒患兒)에 대한 임상적(臨床的) 고찰(考察))

  • The common cold is the most common pediatric infectious disease and occupies a great number of outpatients in oriental pediatrics. Because children's common cold has characteristic symptoms, we studied 276 children suffering from common cold Oriental Medicine Hospital in Kyungsan University and analyzed sex, age, chief complaints, characteristic symptoms.(period : summer 2001. 6.1.-2001.8.31, winter 2001.12.1-2002. 2. 28) The results were as follows : 1. Male to female ratios were 17: 1 in summer, 1.2: 1 in winter, 1.4: 1 in total 2. Age distribution of children from 0 to 1 was 20.29%, from 1 to 3 was 39.49%, from 3 to 6 was 32.97%, from 6 to 10 was 7.27%, over 10 was 0%, from 0 to 6 was 92.75%. 3. Frequency of chief complaints In summer : cough was 22.28%, nasal discharge was 18.39%, fever was 16.58%, secretion and sputum were 14.50%, sweating was 6.99%, nausea or vomitting were 4.92% etc. In winter : cough was 24.20%, nasal discharge was 18.03%, secretion and sputum were 16.43%, occlusion of nares was 11.64%, fever was 9.58%, nausea or vomitting were 3.88% etc. Frequency of chief complaints has seasonal difference. 4. Children's common cold has characteristic symptoms and seasonal difference. 5. Children's common cold has characteristic clinical subtype.

Survey on the Actual Conditions of Patients in Semyung University Attached Oriental Medical Hospital Night Clinic. (세명대학교 부속한방병원 야간 진료실 내원환자의 실태분석)

  • This study on the visits of the oriental medical night clinic of patients, was made to be used as reference data by examining and analyzing statistically the many actual conditions of patients who had been visited in Semyung University Attached Oriental Medical Hospital during the period from April, 1998 to March, 1999. The purpose of this survey was to understand the characteristics of patients, to evaluate the roles of Night Clinic of oriental medical hospital, and contribute to the systemic and efficient management of night clinic service. The results obtained were as follows: I. Distribution of sex: male 53% (421 cases), female 47% (379 cases) 2. The age distribution of patients showed the highest in under ten, followed by the thirties and fifties, forties and twenties in order. 3. The monthly distribution of patients showed the highest in October 1998, followed by May 1998 and February 1999 in order. 4. The daily distribution of patients showed the highest in Sunday, followed by Saturday, Monday, Friday and Tuesday in order. 5. The regional distribution revealed the highest in Jecheon with 76% of all patients, followed by Danyang in order. 6. The distribution of arrival time showed the highest in 7:00 pm~0:00 am (27%), followed by 5:30 pm~7:00 pm (23%) in order. 7. The admission rate in patients was 17%. Among them direct visiting rate was 85%. Circulatory systemic disease was the highest. 8. The highest incidences by disease were of motor system with 48% (394cases), followed by diseases in circulatory system with 19% (l48cases). The majority of the patients 67% was connected with two diseases. 9. The chief complaint of pediatric diseases was febrile seizure with 64% (32cases), digestive disease was abdominal pain with 44% (90cases), circulatory diseases was motor disturbance with 43% (83cases), motor system disease was leg pain with 37% (l19cases), respiratory disease was fever with 46% ( 41 cases). 10. In the treatment method, a major portion of treatment methods was acupuncture treatment with 32% (373cases), followed by acupuncture & herbal medicine treatment with 23% (275cases). The most commonly used herbal medicines were Hangsapyunguisan(l6%) and Ojeoksan(l4%).

A Case Study of Nocturnal Enuresis in Two Children by Focusing Sleep-wake Transition (수면-각성이행에 초점을 둔 소아 야뇨증 치험 2례)

  • Objectives The purpose of this study is to report the case of nocturnal enuresis in two children treated by using hypothesis that awareness during sleep may improves symptoms. Methods Enuresis children usually are not even aware of nocturnal urea due to excessive sleep. During the sleep cycle, enuresis children do not tend to have sleep-wake transition period compared to normal children. Ephedrine, the main ingredient of Ephedra sinica (麻黃), has a wakening effect. Two pediatric patients, who were suffering from nocturnal enuresis, were administered by Tosatang-gamibang (菟絲湯加味方) added Ephedra sinica (麻黃) or Boatang-gamibang (補兒湯加味方) added Ephedra sinica (麻黃) while correcting unhealthy voiding habit. Results As a result of the treatment, two pediatric patients were recovered without relapse. Conclusions In this report, we have proven that Tosatang-gamibang (菟絲湯加味方) added Ephedra sinica (麻黃) or Boatang-gamibang (補兒湯加味方) added Ephedra sinica (麻黃) and correcting voiding habit can considerably be effective in recovering nocturnal enuresis.

A study on the Jinyinbohetangxia verse of the medication instructions in the Xioaeryaozhengzhijue (『소아약증직결(小兒藥證直訣)』에 기재된 처방 복용법 중 금은화박하탕하(金銀花薄荷湯下) 구절에 대한 고찰)

  • Objectives : This study aims to clarify the interpretation of the verse "jinyinhuabohetangxia(金銀花薄荷湯下)" in Zhouxuehai(周學海)'s corrective commentary in the Xiaoeryaozhenzhijue(小兒藥證直訣), by examining traditional pediatric texts in relation to the verse. Methods : For each of the three possible interpretations of the instructive phrase for taking herbal formulas-Lonicera flower with peppermint(金銀花+薄荷), gold and silver with peppermint(金銀+薄荷), and gold coin peppermint(金錢薄荷)-applications of these herbs in the various pediatric books were analyzed to determine the most appropriate interpretation. Results & Conclusions : The formulary composition of Liangjingwan(凉驚丸) and Wusewan(五色丸) seems to sufficiently target the pathology of pediatric convulsion(驚癎) and heart malnutrition(心疳). Furthermore, it seems unnecessary to add Lonicera flower(金銀花) to the gastrointestinal fragility(脾胃虛弱) of the malnourished(疳). Some of the herbal formulas in the pediatrics chapter of the Zhengzhizhunsheng(證治准繩) include Lonicera flower(金銀花) to treat only carbuncles(癰腫, 腫毒), while other books do not include it in their formulas. On the other hand, instructions to take water boiled with gold and silver appear in many texts, including the Xiaoeryaozhenzhijue(小兒藥證直訣), and sometimes include gold leaf(金箔) directly in the formula. Descriptions of gold and silver pots with instructions on how to make herbal formulas were found as well, which supports what Zhouxuehai(周學海) commented on the Xiaoeryaozhenzhijue(小兒藥證直訣). Although gold coin peppermint(金錢薄荷) is currently used as Lianqiancao(連錢草) in China, it was difficult to verify its identity as Jinqianbohe(金錢薄荷), which seems to have been recognized as a type of mint in the past. No reported anticonvulsant studies of Lonicera flower(金銀花), gold and silver(金銀), and Lianqiancao(連錢草) were found. Peppermint(薄荷) was ubiquitous in instructions for taking herbal formulas treating pediatric convulsions. Therefore, the instruction to take water boiled with gold and silver, or to take the herbal medicine in water boiled in a gold and silver pot, rather than using Lonicera flower(金銀花), could be considered as an interpretation closer to the original author's intent.
