The Influence of Family Support on Hope of the Patients with Stroke (가족지지가 뇌졸중 환자의 희망에 미치는 영향)

  • The purpose of this study is to identify the influence of family support on hope of the patients with stroke. Subjects were 53 in-patients with stroke in two general hospitals and one oriental medicine hospital located in Seoul and Kwang-Ju. The instruments used for this study : The family support scale developed by Kang Hyun Suk(1985). The hope scale developed by Nowotny(1989). The Data were analyzed with descriptive statistics, t-test, ANOVA. Pearson's correlation and simple regression. Data had been collected from December 24, 1998 to January 31, 1999. The results of this study were as follows The mean score of family support was 43.94 and the mean score of the hope was 80.89. The relationship between family support and the hope of the patients with stroke revealed a significant correlation (r= .560, p= .0001). The variables influencing family support and the hope of the patients with stroke were as follows : There was significant difference between family support and general characteristic factor, which was family chief caregiver (p= .002). There was no significant difference between hope and general characteristic factors. but There were significant differences between one of the subscales of hope, 'confidence in the outcome' and general characteristic factors, which were age(p= .021), perceived disability effect(p= .027). There were significant differences between one of the subscales of hope 'possibility of future', and general characteristic factors, which were age(p= .016), education(p= .018). There was significant difference between one of the subscales of hope. 'spiritual belief', and general characteristic factors, which was religion(p= .002). There was significant difference between one of the subscales of hope, 'active involvement', and general characteristic factors, which was family chief caregiver (p= .012). It was found that the higher the degree of perceived family support, the higher the degree of hope. Clearly, the perceived family support can contribute significantly and positively to hope of the patients with stroke. Therefore, nurses should plan interventions to inspire hope level of patients with stroke by family support. The above results may be used as the basic data to seek more efficient way of elevating nursing practice and qualify for the patients with stroke.

The Effects of Neurofeedback Training on Brain Function Quotient of Elderly with Long-term Care Insurance Service (뉴로피드백 훈련이 장기요양시설노인의 뇌기능지수에 미치는 효과)

  • Purpose: Recently, Neurofeedback training system that based on biofeedback of brain wave was introduced. This study was performed to identify the effects of the improvement of brain function by Neurofeedback training on elders(the 2nd or 3rd grade of long-term care insurance services). Methods: A quasi-experimental design using a nonequivalent control group, pre-post test was used. Total 11 elderly were enrolled in this study (experimental group 5, control group 6). The intervention was conducted 3 times a week for 30 minutes from January to June, 2012 (total 60 times). Chi-square test and Mann-Whitney U-test were used to analyze the data. Results: After the Neurofeedback intervention, attention quotient (AQ), anti-stress quotient (ASQ), emotion quotient (EQ) and brain quotient (BQ) of the experimental group were significantly better than those of the control group. Conclusion: The findings indicate that the Neurofeedback training program was effective in reducing fatigue by AQ, increasing the physical and mental stress resistance by ASQ, emotional balance by EQ and improving of total brain function by BQ. Therefore Neurofeedback training be used as an effective training intervention for the health of elderly in geriatric facility.

The Effect of Changes in Patellar Height Using Infra-Patellar Strap on the EMG Activity of Quadriceps Muscles During a Squat Exercise in Adults With Patellar Baja (하위 무릎뼈를 지닌 성인들을 대상으로 쪼그려 앉기 운동 시 하위 무릎뼈 스트랩을 이용한 무릎뼈 높이 변화가 넙다리네갈래근의 근 활성도에 미치는 영향)

  • The purpose of this study was to assess the influence of patellar height on quadriceps muscles' electromyography (EMG) activity during a squat exercise in adults with patella baja. For the study, we recruited 15 volunteers who had patella baja on the right side. We measured the EMG activity of the right rectus femoris, vastus medialis oblique, and vastus lateralis muscles during squat exercises under two conditions, specifically with and without an infra-patellar strap. The infra-patellar strap was applied below the tested patella to elevate the patella to a normal height. A paired t-test was used to compare the effects of patella height on EMG activity of the quadriceps muscles. The EMG activity of the rectus femoris (RF), vastus medialis oblique (VMO), and vastus lateralis (VL) muscles were significantly decreased during the squat exercise with the infra-patellar strap compared to the same exercise without the infra-patellar strap (p<.05), while the VMO/VL ratio was not different significantly between two conditions (p>.05). The findings of this study suggest that an infra-patellar strap may benefit people with patellar baja, as changes in patellar height could improve the efficiency of the quadriceps muscles.

The Usefulness of the 24hrs Urine 17-KS.17-OHCS as an Index for the Differentiation of Deficiency Syndrome of the Kidneys in Stroke Patient (뇌졸중 환자의 신허 진단 지표로서 24시간 요중 17-KS, 17-OHCS의 유용성에 대한 검토)

  • Background and Purpose : Relationship between 17-KS.17-OHCS in 24hrs urine and Deficiency Syndrome of the Kidneys had been examined, but the study about 17-KS.17-OHCS in stroke patients was rare6'. In this study, we aimed to investigate the usefulness of 24hrs urine 17-KS.17-OHCS in stroke patients as an index for the Differentiation of Deficiency Syndrome of the Kidneys. Subjects : 66 stroke patients(male : female =2 9 : 37) were selected, they were admitted in the hospital of oriental medicine, Kyunghee university(from November 1 st, 1998 to May 30th, 2000). Their age was over 65 years. The patients who had renal malfunction, hyperthyroidism, hypothyroidism were excluded and who took chlorpromazine, spironolactone, digoxin, reserpine, hormonal agent were also excluded. Methods : After we selected the patients, we investigated the Differentiation of Syndrome by use of Diagnostic Paper and examined the level of 17-KS.17-OHCS in 24hrs urine. We compared Deficiency Syndrome with non-Deficiency Syndrome of the Kidneys using of 17-KS.17-OHCS in 24hrs urine. Results : 1. Stroke did not affect 17-KS.17-OHCS excretion in 24hrs urine. 2. In 24hrs urine, 17-KS of male stroke patients and 17-OHCS of female stroke patients were lower in patients diagnosed as a Deficiency Syndrome than non-Deficiency Syndrome of the Kidneys(p<0.05). 3. Among Deficiency Syndrome of Yin, Yang, Yang and Yin of the Kidneys group, there was no differentiation of 17-KS.17-OHCS in 24hrs urine(p>0.05).

Study of Efficacy and Safety of Ginseng Seed Oil in Heathy Subjects Who Have Mild Liver Dysfunction : A Randomized, Double Blinded, Placebo-Controlled Study (경증의 간 기능 이상 소견을 보이는 건강한 성인에 대한 인삼종자오일의 유효성 및 안전성 연구 : 무작위배정, 이중눈가림, 위약대조 연구)

  • Objectives: The purpose of this study was to investigate the effectiveness and safety of ginseng seed oil in healthy subjects who have mild liver dysfunction. Methods: A randomized, double blinded, placebo-controlled trial was conducted. A total of 167 subjects visited Semyung University Hospital from July 1st, 2016 to June 10th 2017. Except for the 103 excluded subjects, 64 subjects were randomized into one of the two groups: an treatment group(n=33) and control group(n=31). Subjects were randomly given either ginseng oil seed capsules or indistinguishable placebo capsules(2 capsules per dose, twice per day). Laboratory tests(aspartate aminotransferase, alanine aminotransferase, gamma glutamyl transpeptidase, triglyceride, total cholesterol, high density lipoprotein-cholesterol, low density lipoprotein-cholesterol) were performed to evaluate the effectiveness after 6, 12 weeks of treatment. Vital sign, laboratory test were performed to assess safety at every visit. Results: There were no significant differences in efficacy between treatment group and control group. There were some adverse events with no significant difference in symptoms and frequency between treatment group and control group. Conclusions: Although the efficacy of ginseng seed oil was not proved, ginseng seed oil did not worsen liver function and proved its safety. More study of ginseng seed oil and clinical trials are necessary to increase the usefulness of above-ground parts of ginseng.

Randomized Controlled Clinical Trials in the Journal of Korean Society of Chuna Manual Medicine for Spine and Nerves: A Quantitative and Qualitative Analysis (척추신경추나의학회지에 수록된 무작위대조임상 연구의 양적, 질적 분석)

  • Objectives: The aim of this study is to assess the quantity and quality of randomized controlled clinical trials (RCTs) published in the journal of Korean Society of Chuna Manual Medicine for Spine and Nerves (KSCMSN). Methods: All relevant RCTs were selected and extracted. Data extract of RCTs from all the articles published in the jounal of KSCMSN up to now, quantity assessment was made on the study design, sample size, intervention, control group and medical condition. of the consolidated standards of reporting trials (CONSORT) check list. Assessment was performed by 2 independent reviewers and disagreement was discussed based on concensus. Results: Among the 276 articles, 10 RCTs were published. 1st articles were published in 2003 and half of RCTs were published in 2011. All RCTs were parallel 2-arm designed. Average sample size was 29.2 per study and 14.6 per arm. Main intervention was consisted as acupuncture 40%, pharmacopuncture 30% and Chuna 30%. Average of adequacy of CONSORT check list was 10.3% and overall reports were insufficient. Conclusions: Though RCTs published in jounal of KSCMSN were increasing, the quality remains low. KSCMSN should make a effort to follow the CONSORT statement and improve the quantity and quality of studies.

Overview of the Interaction between Warfarin and Korean Herbal Medicine (와파린과 상호작용하는 한약재에 대한 고찰)

  • Background and Objective : Warfarin is the standard anticoagulation treatment for atrial fibrillation, venous thromboembolism (VTE), and mechanical heart valves. Close monitoring of the International Normalized Ratio (INR) is required due to the drug's very narrow therapeutic window. Many factors can affect INR levels. Drug and food interactions are frequently cited as causes of adverse events with warfarin. We discussed interactions between herbs and warfarin studied in this research. Methods : In this review, PubMed was used to search medical journals. Keywords "warfarin AND interaction" were applied. Results : 55 articles were included. The possibility of correlation between warfarin and single herbal medicines such as Salviae Miltiorrhizae Radix, Angelicae Gigantis Radix, Ginseng Radix Alba, Lycii Fructus, Ginkgo Folium, Menthae Herba, Trigonellae semen was suggested. Furthermore, some herbal compounds interacting with warfarin were reported. The conclusion of studies reporting the effect of herbal medicine on warfarin were controversial due to small size or quality of research. Conculsions : We suggest that we should prescribe therapeutic herbal medicines to patients using warfarin more carefully and do INR follow-up regularly.

  • Frozen shoulder was diagnosed with this study 14 patients were enrolled. Patients who volunteer to visit the configuration is more than six months and pain were studied in a chronic illness. Frozen shoulder to patients based on these results the effect of KTO treatment is a diet that is valid for a unified model and the exact effect can be enhanced, be combined with other treatments and therapies are deemed necessary. When applied to patients with Frozen shoulder KTO with an increased range of motion of the shoulder can result in a reduction of pain.

  • Osteoarthritis(OA) is a widespread, slowly developing disease with a high prevalence rate increasing with age. The most common big joints involved in OA is the knee, where the disease particulary strikes, causing difficulties in rising from a chair. climbing stairs, kneeling. standing, and walking. The primary complaints of these patients include pain, stiffness. instability, and loss of function. The purpose of this study was to assess the impact of self-reported symptoms and limited flexion, and limited extension on functional impairment in patients with knee osteoarthritis. In the data collection, a total of 206 who had visited the physiotherapy unit at six different departments of orthopedics were interviewed in K City from June to September, 2000. The results of the study summarized are as fellows: 1. There was a significant difference among different degree group of limited flexion(p<0.05). A group difference was demonstrated between <5 ${\sim}$ 15 and 16 > (F=16.21, p<0.05). In the relationship between the degree of limited flexion and the level of functional impairment, the h igher the range of limited flexion, the lower the level of functional impairment. 2. In the relationship between the degree of limited extension and the level of functional impairment, a group difference was revealed between the non-limitation group and the <10 and 11 < However, no significant difference was noted between 11 ${\sim}$ 20 and 21 >(F= 13.37, p<0.05). In conclusion, finding above suggest that functional impairment is closely correlated with limited tlexion and extenion

Life-Style Habits in a High-Risk Area for Upper Gastrointestinal Cancers: a Population-Based Study from Shanxi, China

  • Background: Cancer is a burden on humanity and ranks as a leading cause of morbidity and mortality in China. Shanxi province has its unique cancer patterns and the burden is increasing. In this study, we aimed to assess the pattern of dietary habits and life-style in Shanxi, a high-risk area for upper gastrointestinal cancers in China and further evaluate the trends in cancer incidence and mortality based on registered data. Materials and Methods: Data on lifestyle, diet, physical activity were obtained from the household health survey at Zhongyang from 2013 to 2015. Cancer diagnoses were reported to Shanxi Center for Disease Control and Prevention (SCDCP). Population-based cancer incidence data and mortality data of 2012 were collected from the SCDCP. All incidence and death rates were expressed per 100,000 populations. Univariate analysis was performed using the Chi-squared test or Fisher's exact test. Results: Overall, deficiencies in fresh fruits and vegetable food, and intake of hot food, salted food, or pickled food are serious problems in Shanxi, especially in rural areas. Upper gastrointestinal cancers were the most commonly diagnosed cancers, and the incidence in rural areas is higher than those in urban areas. Cervical cancer is the most common cancer for females. Moreover, the agespecific incidence exhibited an increased trend before 40 years old. Consistent with the previous literature, our epidemiological investigation results suggest that lifestyle, nutrition deficient, and infections were major risk factors for upper gastrointestinal cancers or cervical cancer in Shanxi. Facing a serious situation, we further explored defensible recommendations for the general public in order to promote changes in environments that support healthful eating and physical activity habits, to reduce cancer risk. Conclusions: Our results present the current cancer trends in Shanxi and its related etiologic risk factors and provide a theoretical basis to guide public health efforts to prevent and control cancers in the province.
