A Novel Melanin-Targeted 18F-PFPN Positron Emission Tomography Imaging for Diagnosing Ocular and Orbital Melanoma

  • Objective: 18F-N-(2-(Diethylamino)ethyl)-5-(2-(2-(2-fluoroethoxy)ethoxy)ethoxy) picolinamide (18F-PFPN) is a novel positron emission tomography (PET) probe designed to specifically targets melanin. This study aimed to evaluate the diagnostic feasibility of 18F-PFPN in patients with ocular or orbital melanoma. Materials and Methods: Three patients with pathologically confirmed ocular or orbital melanoma (one male, two females; age 41-59 years) were retrospectively reviewed. Each patient underwent comprehensive 18F-PFPN and 18F-fluorodeoxyglucose (18F-FDG) PET scans. The maximum standardized uptake value (SUVmax) of the lesion and the interference caused by background tissue were compared between 18F-PFPN and 18F-FDG PET imaging. In addition, the effect of intrinsic pigments in the uvea and retina on the interpretation of the results was examined. The contralateral non-tumorous eye of each patient served as a control. Results: All primary tumors (3/3) were detected using 18F-PFPN PET, while only two primary tumors were detected using 18F-FDG PET. Within each lesion, the SUVmax of 18F-PFPN was 2.6 to 8.3 times higher than that of 18F-FDG. Regarding the quality of PET imaging, the physiological uptake of 18F-FDG PET in the brain and periocular tissues limited the imaging of tumors. However, 18F-PFPN PET minimized this interference. Notably, intrinsic pigments in the uvea and retina did not cause abnormal concentrations of 18F-PFPN, as no anomalous uptake of 18F-PFPN was detected in the healthy contralateral eyes. Conclusion: Compared to 18F-FDG, 18F-PFPN demonstrated higher detection rates for ocular and orbital melanomas with minimal interference from surrounding tissues. This suggests that 18F-PFPN could be a promising clinical diagnostic tool for distinguishing malignant melanoma from benign pigmentation in ocular and orbital melanomas.

  • Ciliary body neoplasms are uncommon and have been described infrequently in the dog. We report successful treatment of three cases of canine ciliary body tumors that were diagnosed histologically as adenoma, adenocarcinoma, and malignant melanoma, respectively. They were presented with typical clinical signs that include glaucoma, anterior segment inflammation, and vision loss. On orbital ultrasound. very echodense masses involved in the ciliary body structure were revealed. Iridocyclectorny and enucleation were performed in lieu of attempts at orbital biopsy.

안와종양의 임상경험 (Clinical Experiences of the Orbital Tumors)

  • 영남대학교 의과대학 부속병원 안과에서 1984년 3월부터 1994년 8월까지 병리조직검사로 확인된 안와종양 95례를 대상으로 임상적 고찰을 실시하였다. 안와종양의 발생빈도는 성별로는 남자가 46명(48.4%), 여성이 49명(51.6%)으로서 유의한 차이를 보이지 않았으며, 연령분포는 양성종양의 경우 전 연령층에 걸쳐 고루 분포하였고, 악성종양의 경우 10세 미만과 50-60대에 특히 많은 발생을 보였다. 광학 현미경으로 살펴본 병리조직학적 검사상 양성 대 악성의 비율은 82% : 18%로서 다른 연구에서 보다 양성종양의 비율이 높았다. 종류별로는 피부지방종(20례, 21%), 유피낭(11례, 11.6%), 점액류(8례, 8.4%), 지방종(7례, 7.4%), 다형생 선종(5례, 5.3%) 순이었다. 양성종양은 성인의 경우 점액류, 유피낭, 다형성 선종, 해면혈관종 순의 발생빈도를 보였고, 소아의 경우 피부지방종, 유피낭, 지방종, 망막모세포종 순의 발생빈도를 보였다. 악성종양은 망막모세포종 3례, 악성 흑색종 2례, 피지선암 2례, 상악동암 2례 등이었다. 악성종양중 사망한 경우는 17례 중 8례 (47.0%)로서 모두 이차성 혹은 전이성 안와종양이었다.

  • Purpose: Many advances have been made in lower eyelid reconstruction surgical procedures after tumor ablative therapy. These include skin grafts, local flaps, free flaps, and skin expansion. When a full-thickness defect of the lower eyelid is reconstructed with many free flaps, ectropion and deformity of the medial and lateral canthal areas are common late complications caused by gravitational descent. The radial forearm free flap is widely used because of its lack of bulk, ease of dissection, malleability, and hairlessness. This report introduces a novel method for preventing ectropion using a composite radial forearm free flap reconstruction and palmaris longus suspension technique. Methods: A 70-year-old man had a malignant melanoma on his left lower eyelid. The patient was referred to our department after a biopsy confirmed the initial diagnosis. A full-thickness wide resection with a 25 mm free margin was performed, and a $5{\times}8cm$ radial forearm flap was elevated with a vascularised palmaris longus tendon. The palmaris longus tendon was fixed to the medial and lateral orbital rim perisoteum and the deep temporal fascia. The buccal mucosa was grafted to reconstruct the inner conjunctival layer. The pedicle vessels were anastomosed to the left superficial temporal artery and vein. Results: The postoperative clinical course was uneventful. The flap showed good texture and color match. No ectropion was noted 14 months after surgery and the tumor did not recur. The patient was quite satisfied with the final outcomes. Conclusion: Use of a radial forearm free flap and the palmaris longus tendon is an effective method for a full-thickness lower eyelid reconstruction.

Through-and-through Nasal Reconstruction with the Bi-Pedicled Forehead Flap

  • Background Nasal reconstruction is one of the most difficult challenges for the head and neck surgeon, especially in the case of complex full thickness defects following malignant skin tumor resection. Full-thickness defects require demanding multi-step reconstruction. Methods Seven patients underwent surgical reconstruction of full-thickness nasal defects with a bi-pedicled forehead flap shaped appropriately to the defect. Patients were aged between 58 and 86 years, with a mean age of 63.4 years. All of the tumors were excised using traditional surgery, and in 4 of the patients, reconstruction was performed simultaneously following negativity of fresh frozen sections of the margins under general anesthesia. Results Nasal reconstruction was well accepted by all of the patients suffering non-melanoma skin tumors with acceptable cosmetic outcomes. The heart-shaped forehead flap was harvested in cases of subtotal involvement of the nasal pyramid, while smaller defects were reconstructed with a wing-shaped flap. No cartilaginous or osseous support was necessary. Conclusions This bi-pedicled forehead flap was a valid, versatile, and easy-to-implement alternative to microsurgery or multi-step reconstruction. The flap is the best indication for full-thickness nasal defects but can also be indicated for other complex facial defects in the orbital (exenteratio orbitae), zygomatic, and cheek area, for which the availability of a flap equipped with two thick and hairless lobes can be a valuable resource.
