FABRICATION OF TISSUE ENGINEERED MYO-MUCOSAL FLAP BY GRAFTING THE COMPLEX OF AUTOLOGOUS ORAL KERATINOCYTES AND PLATELET RICH PLASMA(PRP) IN A RAT MODEL (백서에서 자가 구강점막세포와 혈소판 농축 혈장의 이식에 의한 점막 근 피판의 조직공학적 제작)

  • Backgrounds: To overcome limited amount of autogenous mucosa for the reconstruction of various mucosal defect including oral mucosal defect, tissue engineered mucosa has been recently introduced. However, introduced conventional technique of tissue engineered mucosa still have serious pitfalls such as long fabrication time, fragility of the reconstructed mucosa, and complexity of the technique. Aim of the study: To examine whether the complex of preconfluent autologous keratinocytes and autologous PRP(Platelet rich plasma) can reconstruct oral mucosa on the muscular flap with easier and faster way compared to conventional mucosal tissue engineering technique. Materials and methods: One day before the operation, oral mucosa(3mm in diameter) were taken and treated for extraction of oral keratinocytes according to the routine manner. The day of operation, oral keratinocytes were prepared in the laboratory and then moved to the operating theater. Autologous PRP was also prepared and then mixed with oral keratinocytes just before grafting on the prepared muscular flap. After keratinocyte-PRP complex was seated, then a sterilized rubber sheet was placed on the graft and the elevated skin flap was replaced and sutured. Biopsies were proceeded at 3, 5, 7, 14 and 21 days. Tissue samples were evaluated clinically, histologically, and immunohistochemically. Results: All of the oral keratinocyte-PRP complexes were successfully grafted on the recipient sites(100%). On 3 days after the operation, 1-2 continuous epithelial layer and many inflammatory cells were observed. On 5 days after the operation, increase of layers of keratinocyte was observed with less inflammatory response. Thickness of the layers was gradually increased from 7 to 21 days after the operation. Cytokeratin confirms epithelium in every specimen. Conclusions: Preconfluent graft of autogenous oral keratinocytes mixed with autogenous PRP have successfully reconstructed myo-mucosal flap. This technique could be a useful alternative for oral mucosal reconstruction in the near future.

The Temporal and Spatial Expression of the Cytokeratin in Keratinocytes during Cutaneous Wound Healing on the Amphibian(Bombina orientalis) (양서류 피부 상처회복과정 중 각질화세포 cytokeratin의 분포)

  • The system of wound healing is very complex biological processing that includes inflammatory, reepithelialization, and matrix construction. For identification of the transitional pathway of the keratinocytes, we have employed immunohistochemical analysis using cytokeratin antibody after wounding. Epithelium in skin of the frog(Bombina orientalis) was examined with transmission electron microscopy. Cytokeratin was expressed in normal basal and gland cavity cells. At 3-hour basal layer cells were strong positive, however cells of the upper layer were negative reaction. Day1 and 2 after post-wounding, regenerating epithelial cell layer was positive reaction, especially basal layer cells were strong positive. At day 10 after wounding, the degree of positive reaction to basal cells of regenerating epithelial tissue was equal to day 7 wound tissue. At day of 19th, basal and spinous layer cells were strong positive reaction. Regenerating epithelial cells were positive but some basal cells were strong positive at day 27. From this result, we identified that the migration of the keratinocytes in amphibian skin wounds is initiated from basal layer fells and the keratinocytes migrate into basal and middle of the wound area.

MICROLEAKAGE AND MARGINAL HYBRID LAYER OF DENTIN ADHESIVES (상아질 접착제의 미세누출과 변연부 혼화층)

  • The purpose of this study was to compare microleakage and marginal hybrid layer in class V restorations using two one-bottle adhesives and one self-etching adhesive. Class V cavity preparations with occlusal margins in enamel and gingival margins in dentin were pre-pared on buccal and lingual surfaces of 30 extracted human molar teeth. Prepared teeth were randomly divided into three treatment groups (n=30) and restored with three adhesives and composites: Single Bond/Filtek Z-250 (Group 1), Prime&BondNT/Esthet.X (Group 2), UniFil Bond/UniFil F (Group 3). For microleakage, samples were stored in room temperature water for 24 hours, thermocycled stained with 2% methylene blue dye, sectioned into halves, scored and analysed using Mann-whitney test and Wilcoxon signed rank sum test. For marginal hybrid layer, samples were sectioned into halves, treated with 10% phosphoric acid for 5 seconds, stored in 5% NaOCL solution for 24 hours, dried and gold coated. Occlusal and gingival margins of each sample were inspected under SEM. The results of this study were as follows ; 1. Microleakage at the occlusal margins was not evident in group 1 and group 2, but it showed in group 3 (p<0.05). 2. Microleakage in group 1 and group 3 was significantly lower than in group 2 at gingival margins (p<0.05). 3. Microleakage at gingival margins was greater than at occlusal margins in group 1 and group 2, but microleakage at occlusal margins was greater than at gingival margins in group 3 (p<0.05). 4. In group 1 and group 2, no gaps at occlusal margins showed. But gaps showed in group 3. Occlusal margins were free from a hybrid layer in all groups 5. The thickness of the marginal hybrid layers was 2.5~5 $\mu\textrm{m}$ thick in group 5 $\mu\textrm{m}$ thick in group 2 and 1.5 $\mu\textrm{m}$ thick in group 3. 6 There was no corelation between microleakage and thickness of marginal hybrid layer. In coclusion, the effect of dentin adhesives on microleakge in class V composite restorations was excellent when one-bottle adhesives were applied on enamel margin, and it was good when a self-etching adhesive was applied on dentinal margin. There was no corelation between microleakage and thickness of marginal hybrid layer.

  • For the effective treatment of aphthous stomatitis, the matrix type mucoadhesive patches containing triamcinolone acetonide have been formulated. The drug layer was obtained by drying the polymer gel which was prepared with carbomer 934P, ammoniomethacrylate copolymer, titanium dioxide and polyethylene glycol 400. The effects of the content of additives on physical characteristics of the polymer gel and the drug layer were evaluated. The addition of carbomer increased the yield point and the zero-shear viscosity of polymer gel as well as the thickness, the water absorption ratio, the adhesive time and $T_{50%}$ of drug layer. The adhesive time and the water absorption ratio of drug layer were also improved by the addition of ammoniomethacrylate copolymer, but the addition of titanium dioxide had decreased the zero-shear viscosity of polymer gel and the adhesive time of drug layer.

EVIDENCE OF INTRAEPITHELIAL CGRP IMMUNOREACTIVE NERVE FIBERS DURING REEPITHELIALIZATION OF EXTRACTION WOUND OF RAT (흰쥐의 발치와 재상피화에 따른 상피내 CGRP 면역양성 신경섬유의 분포변화)

  • The purpose of this study was to investigate the distribution pattern of CGRP immunoreactive nerve fibers in the healing mucosa of extracion wound. Maxillary 1st molars of rats were extracted. All extraction sites and adjacent tissues of 3 groups of rats(1-week, 2-week and 4-week groups) were removed en bloc and processed for immunostaining and were subjected to light microscopic examination. The results obtained were as follows; In 1-week group, there was no difference in the distribution pattern of CGRP immunoreactive nerve fiber between epithelial margin adjacent to extraction socket and normal gingival epithelium. In 2-week group, some CGRP-immunoreactive nerve fibers were seen in epithelial layer. In 4-week group, many intercellular CGRP immunoreactive nerve fibers were abundant in all layers of immature epithelium characterized by scab on the mucosa and thick keratinized cell layer with irregular surface. Intraepithelial CGRP immunoreactive nerve fibers were reduced to normal level in adjacent mature epithelium. These results suggest that density of CGRP immunoreactive nerve fibers are increased transiently in epithelium during reepithelialization process and CGRP released from these nerve fibers may play an important role in the reepithelialization in the wound healing.

Tissue Level Based Deep Learning Framework for Early Detection of Dysplasia in Oral Squamous Epithelium

  • Deep learning is emerging as one of the best tool in processing data related to medical imaging. In our research work, we have proposed a deep learning based framework CNN (Convolutional Neural Network) for the classification of dysplastic tissue images. The CNN has classified the given images into 4 different classes namely normal tissue, mild dysplastic tissue, moderate dysplastic tissue and severe dysplastic tissue. The dataset under taken for the study consists of 672 tissue images of epithelial squamous layer of oral cavity captured out of the biopsy samples of 52 patients. After applying the data pre-processing and augmentation on the given dataset, 2688 images were created. Further, these 2688 images were classified into 4 categories with the help of expert Oral Pathologist. The classified data was supplied to the convolutional neural network for training and testing of the proposed framework. It has been observed that training data shows 91.65% accuracy whereas the testing data achieves 89.3% accuracy. The results produced by our proposed framework are also tested and validated by comparing the manual results produced by the medical experts working in this area.

  • In the study of magnification of image in the focal trough of panoramic radiography, (Yoshida company Panoura - Eight - s), a series of 63 x-ray films were taken with the 8-19 metal pins placed in the holes of the plastic plate, measured and evaluated by 4 observes. The author analyzed the vertical and horizontal magnification rate in the corrected focal trough. Results were as follows: 1. For vertial measurements, magnification rates were minimum 10% maximum 36% and the magnification for image of medial side was larger than that of image of lateral side from image layer. 2. For horizontal measurements, magnification rates were minimum-14% maximum 46% and images of medial side from focal trough were magnified and images of lateral side from focal trough were retrenched. 3. When moved 10㎜ downward occlusal layer, interspace was somewhat narrow between the pins and upper sides of pins were horizontally magnified but images of the end parts of pins showed tapered form. 4. When moved 10㎜ downward from the occlusal layer, opposite images showed overlapping.

A huge glandular odontogenic cyst occurring at posterior mandible

  • The glandular odontogenic cyst is a rare lesion described in 1987. It generally occurs at anterior region of mandible in adults over the age of 40 and has a slight tendency to recur. Histopathologically, a cystic cavity lined by a nonkeratinized, stratified squamous, or cuboidal epithelium varying in thickness is found including a superficial layer with glandular or pseudoglandular structures. A 21-year-old male visited Dankook University Dental Hospital with a chief complaint of swelling of the left posterior mandible. Radiographically, a huge multilocular radiolucent lesion involving impacted 3rd molar at the posterior mandible was observed. Buccolingual cortical expansion with partial perforation of buccal cortical bone was also shown. Histopathologically, this lesion was lined by stratified squamous epithelium with glandular structures in areas of plaque-like thickening. The final diagnosis was made as a glandular odontogenic cyst.

ANTICANCER EFFECTS OF CAESALPINIA SAPPAN EXTRACTS ON ORAL CARCINOMA AND OSTEOSARCOMA CELLS (소목 추출물의 구강암 및 골육종 세포주에 대한 항암작용에 관한 연구)

  • Anticancer effect of methanol extract of Caesalpinia sappan L. on oral carcinoma(KB) and osteosarcoma(HOS) cells were investigated in this study. In order to elucidate the anticancer mechanism of Caesalpinia sappan L, we analyzed telomerase inhibitory effect of the methanol extract of Caesalpinia sappan L. In addition, we prepared 5 fraction samples according to its polarity differences and analyzed anticancer effects on oral carcinoma and osteosarcoma cells. Following results are obtained in this study. 1. 50% cell proliferation inhibitory value($IC_{50}$) of the methanol extract of Caesalpinia sappan L. against oral carcinoma(KB) cells and osteosarco ma(HOS) cells were $9.0{\mu}g/ml\;and\;10.9{\mu}g/ml$, respectively. 2. The methanol extract of Caesalpinia sappan L. showed inhibitory effect of telomerase which is required for cancer cell immortality. Therefore, it seems that the anticancer effect of methanol extract of Caesalpinia sappan is at least partially due to telomerase inhibitory effect. 3. Five fraction samples were prepared according to its polarity and 88.7% of ingredient of total methanol extract was transferred to ethylacetate fraction. Thin layer chromatography analysis showed that dichloromethane fraction contained ingredient with relatively high polarity and ethylacetate fraction contained similar ingredient found in total methanol extract. 4. Anticancer effect was observed in n-hexane, dichloromethane, and ethylacetate fractions. The highest anticancer effect was found in dichloromethane fraction which had $IC_{50}$ value of $4.4{\mu}g/ml$ and > $4.0{\mu}g/ml$ against oral carcinoma(KB) cells and osteosarcoma(HOS) cells, respectively.

ORAL PEMPHIGUS VULGARIS: A CASE REPORT (구강내 발생한 심상성 천포창 환자의 치험례)

  • Pemphigus vulgaris is a chronic autoimmune intraepithelial blistering disease with oral mucosal manifestations that very often precede the skin lesions. The vesicles or bullae are produced by an acantholytic process, detachment of differentiating keratinocytes from one another in the epithelial stratum spino sum or spinous cell layer. The pathogenesis of this disease is initially manifested by IgG(mainly) binding to desmosome(desmoglein 3 or 1) in the intercellular spaces of epithelium. This autoantibody binding caused the release of a plasminogen activator(a proteolytic enzyme) from keratinocytes. This ultimately results in cell to cell separation. The mainstay therapy of pemphigus vulgaris is systemic corticosteroids and immunosuppressive agents to eliminate the pathogenic autoantibodies from circulation. A 41-year old woman presented with a 1.5 year history of oral ulceration. There were no lesions on the skin or other mucosal sites. Histology and immunostaining were consistent with pemphigus vulgaris. Control of oral ulceration and normal oral function were achieved after systemic corticosteroids and immunosuppressive agents were instituted.
