CUTTING EFFICACY OF Er:YAG LASER AND CONVENTIONAL BUR IN DECIDUOUS AND PERMANENT TEETH (Er:YAG laser와 Conventional bur의 유치와 영구치 치아삭제효과 비교)

  • This study was conducted to observe the microscopic structures of cavities formed after ablation of primary teeth, permanent teeth, enamel and dentin in using a bur and cavities formed after ablation using laser and the following results were obtained after comparing the effects of ablation. Using a #330 bur and Er:YAG laser irradiated at 150 mJ, 200 mJ, 250 mJ and 300 mJ all at the frequency of 5 Hz, 1 mm enamel and dentin samples were ablated and the ablation time was measured. In order to measure the surfaces ablated, 5 each of primary teeth and permanent teeth were ablated using a #330 bur and Er:YAG laser at 150 mJ, 200 mJ, 250 mJ and 300 mJ for 1 sec and the cross section and vertical section were observed. The following results were obtained : 1. Cutting time of Er:YAG laser was longer than that of conventinal high-speed bur regardless of teeth type. 2. Cutting on enamel, Cutting time of conventional high-speed bur in deciduous teeth was longer than in permanent teeth(P<0.05). But Er:YAG laser was not showed any difference between the deciduous and permanent teeth(P>0.05). 3. Cutting on dentin, Cutting time of conventional high-speed bur in permanent teeth was longer than deciduous teeth. Er:YAG laser of 150 mJ, 5 Hz in permanent teeth was longer than in deciduous teeth(p<0.05). But laser of other power did not showed mean difference. 4. The cavity surface treated with the convetional high-speed bur revealed a relatively flat appearance, almost covered with a debris-like smear layer. Cavity wall showed striped appearance because of blade of bur. 5. The cavity surface treated by the Er:YAG laser system was irregular or rough surface with the absence charring, carbonization, or cracking of the dentin. In addition, there was an absence of a smear layer. Cavity floor was round and relatively smooth. According to these results, cutting time of Er:YAG laser was almostly same in permanent and deciduous teeth, but more effective in dentin than enamel. Cutting the sample, Er:YAG laser was needed more time than conventional bur. But SEM findings suggested that laser device produced favorable surface characteristic(i.e, no smear layer, irregular surface, cracking).

The relationship between dental implant stability and trabecular bone structure using cone-beam computed tomography

  • Purpose: The objective of this study was to investigate the relationships between primary implant stability as measured by impact response frequency and the structural parameters of trabecular bone using cone-beam computed tomography(CBCT), excluding the effect of cortical bone thickness. Methods: We measured the impact response of a dental implant placed into swine bone specimens composed of only trabecular bone without the cortical bone layer using an inductive sensor. The peak frequency of the impact response spectrum was determined as an implant stability criterion (SPF). The 3D microstructural parameters were calculated from CT images of the bone specimens obtained using both micro-CT and CBCT. Results: SPF had significant positive correlations with trabecular bone structural parameters (BV/TV, BV, BS, BSD, Tb.Th, Tb.N, FD, and BS/BV) (P<0.01) while SPF demonstrated significant negative correlations with other microstructural parameters (Tb.Sp, Tb.Pf, and SMI) using micro-CT and CBCT (P<0.01). Conclusions: There was an increase in implant stability prediction by combining BV/TV and SMI in the stepwise forward regression analysis. Bone with high volume density and low surface density shows high implant stability. Well-connected thick bone with small marrow spaces also shows high implant stability. The combination of bone density and architectural parameters measured using CBCT can predict the implant stability more accurately than the density alone in clinical diagnoses.

Radioprotective effects of dimethyl sulfoxide in the artificial skin reconstructed with cultured human cells (배양된 사람 세포를 이용해 제작한 인공피부에서 방사선조사에 대한 dimethyl sulfoxide의 보호 효과)

  • Purpose: To evaluate cultured human artificial skin as an experimental model for studying radiation effects in vitro. Materials and Methods: The skin was constructed by culturing keratinocytes over collagen lattice which made by culturing fibroblasts. Two groups were irradiated to gamma rays at single dose of 25 Gy with or without 3.5% of DMSO. Ultrastructures were investigated by electron microscopy after irradiation. The number of epidermal layers and expression of cytokeratin (CK) 14 & 10 were also seem by light microscopy. Results: At 2 days after irradiation in experimental group without DMSO, necrotic cells were rarely found in the spinosal layer and undercornified cells were visible in the homey layer. Similar findings were also found in experimental group with DMSO but in mild form. The number of epidermal layers in experimental group without DMSO were significantly fewer than other group. CK 14 expressed in all the layer excluding homey layer but CK 10 expressed over 3∼4 basal layers. Such patterns of CK expression were similar to all groups. It is suggested that structures of the keratinocytes and epidermal formation could be disturbed by irradiation in artificial skin and that DMSO can protect these damages. Conclusion : Therefore this work could be used as an organotypic experimental model in vitro using human cells for studying radiation effect in skin. Furthermore structural findings provided in this study could be used as useful basic data in further study using this model.

Surface Characterization According to the Bias Voltage of the TiAgN Coating Film Layer Formed by the AIP Process (AIP법으로 형성된 TiAgN 코팅필름의 바이어스전압에 따른 표면 특성 분석)

  • The implanting of metal products is performed with numerous surface treatments because of toxicity and adhesion. Recently, the surface modification of metal products has been actively studied by coating the surface of the TiC or TiN film. We prepared a Ti(10%)Ag Target which may be used in dental oral material by, using the AIP(arc ion plating) system TiAgN coating layer that was deposited on Ti g.23. The purpose of this study was to establish the optimal bias voltage conditions of the coated TiAgN layer formed by the AIP process. The TiAgN coatings were prepared with different bias voltage parameters (0V to -500V) to investigate the effect of bias voltage on their mechanical and chemical properties. The SEM(scanning electron microscope), EDS(energy dispersive X-ray spectrometer), XRD(X-ray diffraction), micro-hardness, and potentiodynamic polarization were measured and the surface characteristics of the TiAgN coating layers were evaluated. The TiAgN coating layer had different mechanical characteristics based on the bias voltage, which also showed differences in thickness and composition.

Effects of Black rice (Oryza sativa L.) Aleurone layer on Cyclophosphamide-induced Immunosuppression in Mice (Cyclophosphamide에 의해 유도된 면역 저하 마우스에서 흑미 (Oryza sativa L.) 호분층의 면역 활성 효과)

  • Anthocyanins are considered as one of the major functional components including cyanidin-3-glucoside and peonidin-3-glucoside anthocyanins of black rice (Oryza sativa L., BR) aleurone layer (OAL) extract. The immunomodulatory biological effects of OAL has rarely been evaluated. This study has extensively studied the immunomodulatory effects of BR in immunosuppressed mice by CPA. The immunostimulatory effects of BR extract in in vivo at 150 (OAL-1) and 300 mg/kg body weight (OAL-2) for OAL groups were measured and compared to the normal group (CON) ingested with distilled water only or other groups. After the 14 days of oral administration, immune related organ weights, serum immunoglobulin (Ig) A and IgG, and cytokines (IL-12, TNF-α) levels, splenocytes proliferation rate, and NK cell activity were evaluated as immune related biomarkers. The serum IgA and IgG values in the OAL-treated groups increased to the level of the CON. OAL significantly and effectively controlled the IL-12 and TNF-α levels and NK activity compared to the negative control (NC) group. The results suggest that BR aleurone layer with immunomodulatory activities has a great potential as a functional food by itself or as a supplement.

Clinical Evaluation of Simultaneous Implants Placement Following Augmentation of the Maxillary Sinus with Deproteinized Bovine Bone (탈단백 우골을 이용한 상악동 거상술 후 즉시 임플란트 식립에 대한 임상적 평가)

  • Purpose: Placement of endosseous implants in the atrophic maxilla is often restricted because of the lack of supporting bone. In this article, augmentation of the maxillary sinus floor with deproteinized bovine bone to enable insertion of endosseous implants is described. The technique is aimed at providing a cortical layer on top of the graft to ensure a reliable seal of the maxillary sinus and to achieve optimal stability of the bone graft in case of simultaneously placement of dental implants. Methods: The procedure was used in 200 patients (839 implants), using deproteinized bovine bone. The mean follow-up was 28.5 months. No inflammation of the bone grafts nor of the maxillary sinus occurred. The patients received implant supported overdentures or bone-anchored bridges. Results: The survival rate of implant restoration of this study was 97.6%. The total average of marginal bone loss in radiographs was $0.20{\pm}0.38$ mm. Insufficient primary stability, bony quality, and infection were thought to be associated factors in the failed cases. Conclusion: This study documented that deproteinized bovine bone, when used as a grafting material for augmentation of the sinus floor, may lead to proper osseointegration of a endosseous implant.

EXPERIMENTAL STUDY ON THE EFFECT OF X-IRRADIATION IN THE RAT PERIODONTAL LIGAMENT (X선조사가 발육치근막에 미치는 영향에 관한 실험적 연구)

  • The author studied on the effects of X-ray irradiation to the development of periodontal ligament in gestation rats. They were irradiated in their abdomen with 100, 200 and 300 rads respectively in one shot irradiation with deep radiation therapy equipment (MAXIMAR 250-Ⅲ), In 7th, 14th, 21th and 28th day after delivery, those new born rats were respectively sacrificed with ether anesthesia and removed of their mandibles. After removal, those mandibles were fixed in 10% neutral buffer formalin, decalcified with 5% trichloroacetic acid for 5 days and embedded with paraffine. Staining was performed with H-E, Van Gieson, Mallory azan, Bielshowsky-Gomori silver stain and Halmi's oxytalan fiber stain. The results were as follows: 1. Before tooth eruption, all the fiber: components in dental sac were almost always oriented near the outer enamel epithelial \layer. But in irradiated new born rats, those collagen fiber orientation was more irregular than those of control groups, and this phenomenon was more severe in proportion to the amount of irradiation in the gestation period. 2. Before tooth eruption, the connective tissue fibers in periodontal ligament were stained with lighter in the irradiated groups than those of control groups. Oxytalan fibers of irradiated groups were thin and splitting pattern of their fiber morphology to compare with those of control groups. 3. After tooth eruption, the periodontal ligament fibers of irradiated groups were oriented functionally and their morphology was thick, fine and heavy staining. Oxytalan fibers were revealed with oblique parallel arrangement in the periodontal ligament of irradiated groups.

Metastatic Lung Carcinoma Involving the Periodontium : Report of a case (폐암이 치주 조직에 전이된 증례보고)

  • The oral cavity is easily accessible for direct exposure of a malignant disease. 1 percent of the oral malignant tumors are of metastatic origin and approximately 10 percent to 25 percent of the 1 percent fraction originate from the lungs. A case of metastatic lung carcinoma to the gingiva in a 88-year-old male is reported. He complained of pain and swelling between right maxillary 1st premolar and 2nd molar. Although surgical excision of the lesion has been done, the gingival lesion developed as a quickly growing mass and recurred 2 weeks after surgical excision. The gingival mass was histopathologically diagnosed as an undifferentiated carcinoma. Epithelial layer was continuous without ulceration and it seems that the cancer cells are originated from primary tumor. Infiltrated cancer cells were pleomorphic and dyskeratotic. The cells had 2 or more nuclei, not showing squamous or glandular differentiation. Immunohistochemical study revealed the cells originated from the epithelial cells. The prognosis is poor, because prognosis depends on surgical elimination of the primary tumor.

  • The purpose of this study was to investigate the postnatal development of pulpal innervation with the tooth development and eruption process in developing rat mandibular molars (postnatal 1, 5, 10, 15, 20, 25, 35day-old and adult rats). Immunohistochemical method was carried out for the detection of nerve fibers, using the antibody against calcitonin gene-related peptide(CGRP). The results were as follows: The feature of CGRP-IR nerve fibers were shown in a bead-like appearance. The time of nerve entering into the dental papilla of tooth follicle began at the occured advanced dentinogenesis. The development of Raschkow plexus began at the formative stage of the roots and was accelerated at the stage of the crown emerged into the oral cavity. The number of nerve fibers entering the odontoblastic layer increased with the tooth eruption and mastication. The development of innervation was shown to be related with the stage of the development of individual teeth rather than the chronological age of the rat.

Effect of HA Crystals Precipitated by Hydrothermal-Treatment on the Bioactivity of Ti-6Al-7Nb Alloy (열수처리에 의해 석출된 HA 결정이 Ti-6Al-7Nb 합금의 생체활성에 미치는 영향)

  • This study was to investigate the surface properties of electrochemically oxidized Ti-6Al-7Nb alloy by anodic spark discharge technique. Anodizing was performed at current density 30 $mA/cm^2$ up to 300 V in electrolyte solutions containing $DL-{\alpha}$-glycerophosphate disodium salt hydrate($DL-{\alpha}$-GP) and calcium acetate (CA). Hydrothermal treatment was done at $300^{\circ}C$ for 2 hrs to produce a thin outermost layer of hydroxyapatite (HA). The bioactivity was evaluated from HA formation on the surfaces in a Hanks' solution with pH 7.4 at $36.5^{\circ}C$ for 30 days. The size of micropores and the thickness of oxide film increased and complicated multilayer by increasing the spark forming voltage. Needle-like HA crystals were observed on anodic oxide film after the hydrothermal treatment at $300^{\circ}C$ for 2 hrs. When increasing $DL-{\alpha}$-GP in electrolyte composition, the precipitated HA crystals showed the shape of thick and shorter rod. However, when increasing CA, the more fine needle shape HA crystals were appeared. The bioactivity in Hanks' solution was accelerated when the oxide films composed with strong anatase peak with presence of rutile peak. The increase of amount of Ca and P was observed in groups having bioactivity in Hanks' solution. The Ca/P ratio of the precipitated HA layer was equivalent to that of HA crystal and it was closer to 1.67 as increasing the immersion time in Hanks' solution.
