EXPERIMENTAL STUDY ON THE EFFECTS OF CYCLOPHOSPHAMIDE ON THE HEALING OF THE ORAL INCISED WOUND (항암제 Cyclophosphamide가 구강창상 치유에 미치는 영향에 관한 실험적 연구)

  • To assure that cyclophosphamide has been considered something that must be effective to retard wound healing, the Author has studied clinically and histopathologically. All rabbits were incised on the mucogingival junction of anterior teeth in mandible about 1 Cm. in length with mucosa and periosteum layer. The 21 rabbits within 6 months old, weighing about 1.5-1.8 kg were divided into a experimental group and a control group. 1) Group 1 (Experimental group)-After 14 rabbits dieted as the control group were injected with Endoxan (=Cyclophosphamide) in dosage of 30mg per 1kg-body weight intramuscularly, the surgery was performed. 2) Group 2 (Control group)-the surgery in this group was performed without Endoxan. The rabbits were sacrificed on the 1st, 3rd, 5th, 7th, 10the, 14th day after surgery and microscopic slides were made with H-E stain. The results might be summarized as follows; 1) Cyclophosphamide effected to retard healing process on the incised wound of oral mucosa and connective tissue. 2) Control wounds were healed by the 7th day after surgery. 3) Comparison between control and experimental wound did not show significant differences 14th day after surgery.

Extensive Bilateral Subcutaneous Emphysema after Dental Treatment: Two Case Reports

  • We report two rare cases of extensive bilateral subcutaneous emphysema that occurred during ordinary dental procedures. An air-driven high-speed handpiece, routinely used in dental procedures may cause subcutaneous emphysema when high pressure air is introduced into the loose connective tissues below the dermal layer. The first case occurred with surgical extraction of the lower third molar. The air introduced into the fascial spaces near the surgical field spread to the contralateral spaces, as well as the neck and chest areas. The second case also showed extensive bilateral subcutaneous emphysema caused by the introduction of compressed air from the handpiece during crown preparation without any invasive procedure. Cases where the emphysema extends beyond the treatment site to involve the contralateral cervicofacial areas have been rarely reported. Predicting the occurrence of subcutaneous emphysema is difficult, so it is important to exercise caution during routine dental treatment. If significant bilateral cervicofacial swelling is suspected to be due to subcutaneous emphysema, prompt diagnosis with securing the patient's airway will be necessary.

  • In adult patients, the corrections of their malocclusion may be more difficult and require longer treatment time due to thicker layer of cortical bone and reduced blood supply. Recently, various methods such as surgery, implant for anchorage and corticotomy have been tried to overcome these problems. Corticotomy is a surgical technique in which a fissure is made through the cortical bone that surrounds a tooth so that the tooth is embedded within a block of bone that is connected to adjacent blocks through only the medullary bone. Technique of corticotomy has been widely used for correction of maxillary transverse deficiency, but hasn't actively in other fields of orthodontics. We applied corticotomies in many types of orthodontic treatment and had satisfactory results. We suggested clinical application of corticotomy in adult orthodontics to reduce treatment period and to achieve better stability after orthodontic treatment.

Melanin Pigmentation In Gingiva (멜라닌 색소 침착의 치료)

  • Oral melanin pigmentation is common in some races and ethnic groups. The gingivae are the most frequently pigmented intra-oral tissues. Melanin pigmentation is the result of melanin granules, produced by melanoblasts intertwined between epithelial cells at the basal layer of the epithelium. We present one case of melanin pigmentation of the gingiva of 26-year old female. Melanin depigmentation method is applying a 90% phenol solution to deepithelize pigmented areas, gingivectomy, epithelial abrasion, bone denudation, and split thickess flap. We chose epithelial abrasion using round diamond bur. The patient satisfies the result and have almost no pain and discomfort. But repigmentation potential must be noticed to patient.

  • Optimum characteristics of digital X-ray sensor components were analyzed to develop intra- oral dental digital X-ray image sensor using indirect method. Parametric analysis was carried out to optimize the phosphor thickness and the fiber optic plate (FOP) coupling to charge coupled device (CCD). X-ray absorption and light diffusion in the phosphor layer were analyzed by the Monte Carlo method. Real time X-ray image was obtained with prototype X- ray image sensor using general CCD camera with 1∼10 Ip/mm resolution. It has been previously shown that large resolution degradation in X-ray images was caused by miss alignment of FOP to CCD and optical adhesive selection. In this study, we reported that X-ray image quality was greatly improved by using optimized characteristics of alignment device and phosphor thickness.

  • Cystic hygroma is a special form of lymphangioma. It most often occurs in the neck of children. We observed a 14-year-old male whose chief complaint was a abnormal swelling of the right mandibular angle area. The computerized tomography showed the well-defined nonenhancing cystic mass on right submandibular gland area and anterior triangle of the neck. The histopathologic fidings exhibited the multiloculatedfluid-filled cysts, which are lined a flattened layer of endothelial cells with foci of lymphocytes found lying adjacent to the lining. After evaluation of above findings, the mass was diagnosed as a cystic hygroma in the neck.

A CASE REPORT OF THE MANDIBULAR INFECTED BUCCAL CYST

  • The author observed a cystic lesion which had new radiographic entities in the 6 years old male patient and obtained the following features. 1. The main clinical feature is the swelling. 2. The lesion is situated on the buccal surface of a erupting mandibular first molar. 3. The affected molar is tilted, so that the apices are adjacent to the lingual plate of the mandible. 4. The affected molar is of normal morphology, caries-free and vital. 5. The radiographic feature is the cystic radiolucency which is extended over the buccal surface of the roots of the affected molar and slightly inferior to the root apices. 6. The dental papilla of the affected molar shows the lamina dura of relatively normal density and definitition. 7. The buccal surface shows the laminated new bone formation. 8. The inferior concave border of the lesion is delineated by a thick and calcified layer of cortical bone. 9. The lesion causes displacement of the developing second molar of which anterior cortex is partially destroyed.

  • A study was made primarily to investigate vertical and horizontal distortion of the image at the apical region of the dental roots in orthopantomographs. The subjects consisted of two dry skulls with radiopaque materials attached to root surface. Measuring of the width and length of each predetermined point at 23 teeth was performed in dry skulls and radiographic films. The results obtained were as follows; 1. There was overall magnification of image in the vertical dimension. And anterior portion had greater magnification rate than posterior portion, while lower anterior portion had less magnification rate than upper anterior portion. 2. There was reduction of the image in the horizontal dimension of the teeth, because of the position relation between dry skull and image layer of the orthopantomograph. 3. There was a significant difference in distortion rate between the oposite teeth. 4. Cervical portion of the tooth had more decreased rate of horizontal distortion than apical portion.

Effect of EB-PVD Coated Si/HA Film Thickness on Surface Characteristics of Ti-35Nb-10Zr Alloy

  • In this study, effect of EB-PVD coated Si/HA film thickness on surface characteristics of Ti-35Nb-10Zr alloy was investigated. The Ti-35Nb-10Zr alloy was fabricated by arc melting method. The Si/HA layers were coated with 0.8 wt.% of Si in pure HA by EB-PVD method. The coating thickness was consisted with 100 - 300 nm for each group, the surface characteristics was analyzed by FE-SEM, EDS, XRD, XRF and corrosion test. The Si/HA coating layer was well deposited on the alloy surface by EB-PVD, the thickness was correlative factor with HA peaks and corrosion resistance value.

  • Bony defects may be found as a result of congenital anomalies, traumatic injury, automobile collisions and industrial accidents in the maxillofacial area. Such conditions are often associated with severs functional and esthetic problem. Various surgical procedure has been utilized in attempts to repair and reconstruct bony defects. Bone is a complex, living, constantly changing tissue. The architecture and composition of cancellous and cortical bone allow the skeleton to perform its essential mechanical functions. Periosteum covers the external surface of bone and consists of two layers : an outer fibrous layer and an inner more cellular and vascular layer. The inner osteogenic layer or cambium layer can form new bone while the outer layer firms part of the insertions of tendons, ligaments and muscles. This study was under taken to evaluate bone healing process on partial defect of calvarial bone with or without periosteum in rat. We made calvarial defects of different size(4mm, 6mm, 8mm) with periosteum or without periosteum in rat to study the effect of defect size on healing process. Control and experimental groups sacrified at 1, 2, 4, 6, 8 weeks, postoperatively. We examed the specimens by gloss findings, light microscophy, and fluorescent microscophy. The results were as follows. 1. Gloss findings: Control groups are larger bony defects than experimental groups after 2 weeks, and than control groups advanced healing of defected bone but experimental groups are lesser after 4, 6 weeks. After 8 weeks, bone defect has not been identified in control and experimental groups. 2. Light microscope: All defects of control groups are larger bony defects than experimental groups after 2 weeks. And than control groups show smaller defect after 4 weeks. After 8 weeks, the control group reveal pin-point sized, hardly identifiable defect space and the experimental group reveal small, but definite defect space. 3. Fluorescent microscope : Each week, new bone formation of control group is very similar to the experimental group. In this study, Osteogenesis of calvarial bone defects with periosteum or without periosteum was examined for 8 weeks in rats. The replaced periosteum had batter new bone formation than the removed periosteum.
