Post-Marketing Surveillance Study of the Safety and Efficacy of Mesoglycan Prescribed in Primary and Secondary Care of Patients with Vascular Disease (혈관질환 환자의 예방치료에 있어 메소글리칸의 안전성 및 유효성에 관한 시판 후 조사에 대한 연구)

  • The aim of this study was to evaluate the safety and efficacy of treatment of mesoglycan. This study was conducted between September 2003 and May 2004. A total of 292 patients with vascular disorders including cerebral infarction were enrolled. The patients were administered with 50-100mg of mesoglycan by an oral route everyday for eight weeks. Subjective physicians' assessments of efficacy had ratings of 'improvement', 'invariability', 'exacerbation' and 'not being able to assess'. An improvement was observed in 241 patients out of 274; 18 patients were rules out. The efficacy rate was influenced statistically significantly by the duration of therapy (p=0.0392) and daily mean drug dose (p<0.0001). The adverse events were reported in 8 patients (9 cases) out of 292 patients: skin/appendages disorders (0.7%), liver/biliary system disorders (0.7%), cardiovascular system disorders (0.7%), neurologic disorders (0.3%). respiratory disorders (0.3%) and gastrointestinal system disorders (0.3%). There showed no serious adverse events. These results indicate that mesoglycan was well tolerated and effective for the prevention of vascular disorders.

  • The study was conducted to identify the relationship between the degree of water intake and xerostomia, halitosis, and oral microorganisms in 50 elderly living in the community and to utilize them as basic data for the development of a water intake enhancing program. The relationship between the general characteristics of the subjects and the variables was found to be statistically significant in both S. mutans and P. intermedia. There were statistically significant differences in P. intermedia among three groups with different water intakes. Based on the results of this study, it is necessary to study the improvement of oral health after development and application of water intake enhancing program.

Isolation of Methylotrophic Actionmycetes Capable of Producing Antagonistic Activity Against Oral Resident Bacteria and Screening of Mutants (구강상재균을 중심으로 항균력을 나타내는 메탄올 자화방선균의 분리 및 변이주 생산)

  • In order to select an effective antibiotic substance against oral resident bacteria, we were isolated from soil and texonomically analyzed. Seven hundred and eighteen strains were isolated on humic acid- vitamin agar(HV agar) and 220 strains were on methanol medium from three each paddy forest, field and riverside soil samples. So, during the screening of antibiotics from soil, we isolated microorganisms showing powerful antagonistic activity against oral resident bacteria. Microorganism was tested against 25 strains of bacteria, yeast and fungi. Among them, No. 248 strain exhibited the most strongly growth inhibition. So, the taxonomical analysis the isolated strain was found to be unknown Actinomyces sp. and was named No 248. A production of the antibiotics from No. 248 begins at the early exponential phase developed at the 72th hour under the optinum conditions. The property of No. 248 antimicrobial compound was very stable under acid(pH 3.0) and alkali(pH 10.0) treatment, but it was instable in heat treatment at $120^{\circ}C$. For the improvement of antibiotic activity, two mutants were isolated from strain No. 248 by the treatment of mutagenic agents, NTG and hydroxylamine. As a result, the mutant strains excreted the potent antibiotics to inhibit the growth of Candida albicans.

Satisfaction on the practical training of public institution's staffs in cyber oral health service (공공기관 근무자의 사이버 구강보건사업실무과정 교육에 대한 만족도)

  • Objectives : This thesis aims to provide basic materials for exploring trends and operational strategy of the cyber training. To achieve study purpose, it analyzed the satisfaction of trainees in regard to the education of field training course of Cyber Oral Health Promotion Program, established and operated between 2007 and 2009. Methods : This study conducted questionnaire surveys after the completion of training to utilize the satisfaction level of 989 respondents among 1,310 employees of public institutions who completed the field training course of Cyber Oral Health Promotion Program. Results : Respondents showed satisfaction on the training course throughout three years in terms of satisfaction of instructional methods (screen characteristics, educational techniques, and so on) according to educational contents, the connection and realization of online education and service satisfaction, necessary for learning in association with educational guidance, offered to learners by a tutor and rapidity in questions and answers. A majority of respondents in all years answered that they participated in training program voluntarily for self-development and improvement in work ability, and they thought that the completion of course would be helpful to their current work and future work. In addition, cyber training program was primarily conducted in the working place by 72.5%, and the most difficult thing in cyber training was to combine work and learning by 60.6%. Conclusions : As shown in the results above, workers in public institutions were satisfied with cyber job training and it was evaluated that cyber job training would be helpful to performing their actual work. Therefore, it is needed to collect and evaluate more diverse requirements of trainees with regard to cyber job training, and the development and operation of job training program that reflects these results sufficiently is required.

Effects of soft occlusal appliance therapy for patients with masticatory muscle pain

  • Background: The options for stabilization appliance therapy for masticatory muscle pain include soft occlusal and hard stabilization appliances. A previous study suggested that hard stabilization appliance therapy was effective for patients with local myalgia who developed long facets on their occlusal appliances. The objective of this study was to identify patients in whom a soft occlusal appliance should be used to treat masticatory muscle pain by analyzing the type of muscle pain present and patient factors that influenced the effectiveness of this treatment. Methods: The study included 42 patients diagnosed with local myalgia or myofascial pain according to the Diagnostic Criteria for Temporomandibular Disorders Diagnostic Decision Tree. The analysis of patient factors included variables believed to be associated with temporomandibular disorders. First, a temporary screening appliance was used for 2 weeks to assess each patient for bruxism during sleep. Soft appliance therapy was then started. For each patient, the effectiveness of the appliance was evaluated according to the intensity of tenderness during muscle palpation and the treatment satisfaction score at one month after starting treatment. Results: Data from 37 of the 42 patients were available for analysis. Twenty-five patients reported satisfaction with the appliance. In logistic regression analysis, the odds ratio for reduction of facet length was 1.998. Nineteen patients showed at least a 30% improvement in the visual analog scale score. The odds ratio for local myalgia was 18.148. Conclusion: Soft appliance therapy may be used in patients with local myalgia. Moreover, patients who develop short facets on the appliance surface are likely to be satisfied with soft appliance therapy. Soft appliance therapy may be appropriate for patients with local myalgia who develop short facets on their occlusal appliance.

A study on the incremental oral health care of C pediatric clinic using a Dentocult-SM test (C소아치과의원의 개량형 Dentocult-SM검사를 이용한 계속관리에 관한 조사 연구)

  • The research was conducted to 100 child patients selected by random sampling, which got a Dentocult-SM test in the first visit and then was being continuously managed, out of child patients of a pediatric clinic located in Gyeonggi-do. The period of there search is one year from June 2007 to May 2008, Using Dentocult-SM test, we analyzed the correlation between the distribution of dental plaque, a streptococcus mutans in saliva and condition of dental caries cavity in the teeth of child patients, then we measured the distribution of a streptococcusmutans. According to SM score, we applied incremental oral heath care for child patients to clinical and obtained the following results, 1. In terms of the age of child patients in research, the number of 3 years old patients was 29(lst ranked), the number of 2 years old patients was 28(2nd ranked). 2. The result of SM score showed that female child patients(52.0%) was higher than male ones in negative, male child patients(52.0%) was higher than female ones in mild, female child patients(68.2%) was higher than male ones in moderate, male child patients(57.1%) was higher than female ones in severe. 3. At the first visit, the SM score showed statistically remarkable difference between dt and dmft. We can also confirm the average of severe is the highest. 4. At the second visit, the SM score showed statistically remarkable difference among dt, ft, and dmft index We can also confirm the average of severe is the highest. 5. At the third visit, The SM score showed statistically remarkable difference among dt, ft, and dmft index We can also confirm the average of severe is the highest. 6. The comparison of dmft index differences to SM score showed statistically no remarkable difference in incremental oral heath care for negative and mild, In addition to that, we can confirm that the incremental oral heath care makes statistically remarkable differences in moderate and severe. 7. The comparison of dt index differences to SM score showed statistically no remarkable difference in incremental oral heath care for negative, mild, and moderate, In addition to that, we can con firm that the incremental oral heath care makes statistically remarkable differences in severe. 8. The comparison of mt index differences to SM score showed statistically no remarkable difference in incremental oral heath care for mild and moderate, In addition to that, we can confirm that the incremental oral heath care makes statistically remarkable differences m severe. 9. The comparison of ft index differences to SM score showed statistically no remarkable difference in incremental oral heath care for mild, In addition to that, we can confirm that the incremental oral heath care makes statistically remarkable differences in negative, moderate, and severe. 10. According to the comparison of dmft index to the age, the 4 years old patients showed the highest number(5.50 in the first visit and 6,08 in the second one). In the third visit, the 6 years old patients showed the highest number(7.00). By the above results, we can find that the incremental oral heath care by SM score makes the results of oral care better. Therefore, the improvement or maintenance in oral health of child patients needs continuing personal oral health management and regular systematic management focused on prevention by the specialist.

Effect of Toothbrushing Facilities on PHP Index and Oral Health-Related Behaviors in Middle Schools (양치시설 설치여부에 따른 중학생의 구강환경관리능력 및 구강건강관리 행태 변화)

  • This study aims to analyze the effects of tooth-brushing facilities (TBF) at middle schools that installed TBF in 2012 as part of the oral health project in Seongdong-gu, Seoul. The subjects were the first graders at A middle school that installed TBF previously and B middle school that installed TBF in 2012. The results are as follows: DMFT, DMFS and CPI didn't show significant difference by schools (p>0.05), but code (0) was higher and code (2) was lower in female students than male students (p<0.05). As to PHP index, the students at A school with TBF indicated better oral hygiene (p<0.01). In the research on before and after installing TBF, the oral hygiene was improved significantly more after the installation than before (p <0.01). In the comparison on the oral health-related behavior, the percentage of not brushing after lunch was higher B school than A school and the frequency of toothbrushing after lunch was higher A school than B school (p<0.01). About the reasons why not to brush after lunch, there was difference in 'lack of the place' as A school showed 2.6% while B school without TBF indicated 14.2% (p<0.01). About the changes of the oral health-related behavior before and after TBF installation, the percentage of not brushing after lunch increased from 78.7% to 83.8% after the installation. The average frequency of tooth-brushing after lunch for one week also decreased from 0.50 to 0.34 (p>0.05). Among the reasons why not to brush after lunch, 'lack of the place' significantly reduced, but 'because other friends don't do it' increased greatly after the installation (p <0.05). Only with the improvement of oral health knowledge, we cannot expect either clinical or behavioral significance. Not only schools but policies of governmental agencies and financial support, cooperation of community-related groups for program development and evaluation will contribute altogether to helping teenagers to habitualize proper oral health management.

  • Objectives: The purpose of the study is to investigate the effects of life habits of some elders on subjective symptoms of periodontal disease. Methods: The subjects of the study were 287 elderly people in Jeonju, Jeollabuk-do by convenience sampling method. A questionnaire was completed by one to one interview method from January 17 to September 10, 2014. The questionnaire consisted of demographical characteristics and subjective symptoms of periodontal diseases by healthy life habits, dietary life habits, and oral healthy life habits. Data were analyzed by SPSS 18.0. Results: Smokers tended to have higher subjective symptoms of periodontal diseases than the non-smokers because smokers ingested more alcohol than the nonsmokers. The smokers tended to consume less meat, fish, beans and vegetables. The smokers did not brush their teeth frequently. They did not visit the dental clinics frequently for the scaling services. Conclusions: The development of oral health improvement program is very important and necessary for the elderly people and the program will enhance the health-related quality of life in the elderly.

Consumer Needs for HMR Product Development of Chinese Elderly Consumers (중국 고령친화형 HMR 제품 개발을 위한 소비자 요구도)

  • This study analyzed consumer needs for the development of elderly-friendly HMR products in China. In developing the products, items needed to be improved, types and packages of the necessary products, additional improvements and food preferences were investigated. According to the demographic characteristics of elderly Chinese consumers, these was analyzed as well. A survey was conducted on a total of 370 elderly people, and data analysis was performed using SPSS 18.0 program. Items related to product quality safety, such as "marks of origin by ingredient", "rich nutrients", "expansion of letter size of packaging", "rich nutrients" and "clear manufacturing date and expiration date", were required to develop HMR products. The types of products that should be developed were porridge and noodles, and the packaging types that should be developed were eco-friendly packaging and recycled packaging. In addition, "low-calorie HMR", "development of various seasonings", "use of eco-friendly food ingredients" and "HMR to digest easily" should be improved. The demand for product improvement and food preference showed significant differences according to the degree of education, monthly income and oral health. The results were intended to prepare basic data for setting the direction when developing elderly-friendly HMR.

The Effect of Professional Tooth Cleaning and Plaque Control Instruction according to the Smoking Behavior (흡연이 전문가치면세정술 및 세균막관리교육 효과에 미치는 영향)

  • Objectives: Smoking is related to periodontal disease and periodontal therapy. So the aim of this study was to investigate the effects of professional tooth cleaning and plaque control instruction (PT & PCI) for smoking behavior. Methods: A total of 151 adults were investigated using the O'Leary Plaque Index (PI), $L\ddot{o}e$ & Silness gingival index (GI) and the number of sextants possessing periodontal pocket (SPP). And adults were given a through dental scaling and Watanabe method for dental plaque control. Follow up examination were conducted after 3 months and compared the pre and post- status. The collected data were analyzed with t-test, paired t-test and one-way analysis of variance. Results: Regardless of smoking behavior, improving effects were identified after PT & PCI on PI, GI and SPP in the whole population. However, the effects of GI improvement were significant in the smoking group alone; those of PI improvement were most significant in the non-smoking group; and those of SPP improvement were more significant in non-smoking and pre-smoking groups than in the smoking group. The shorter period of smoking and the smaller amount of smoking, the greater effects of PT & PCI by smoking-related characteristics. Conclusion: Smoking cessation instruction should necessarily be included in oral health education in that smoking is an important factor to consider in prevention of periodontal diseases and periodontal therapies.
