혈우병 환아에서의 구강외과 수술 마취관리 -증례 보고- (Anesthetic Management of the Oral Surgery in a Child with Hemophilia A - A case report -)

  • Hemophilia A is the most common congenital bleeding disorder, which is sex-linked disease, caused by a deficiency of clotting factor VIII. We experienced a case of alveolorrhaphy using iliac bone graft under general anesthesia for the correction of bilateral cleft alveolus in 10-year-old boy with hemophilia A. Factor VIII activity in this patient was 0.7%, on the severely deficient level, and aPTT was 100 seconds. Just before operation, he received 1,750 units of factor VIII intravenously for loading dose. After we confirmed his factor VIII activity improved to 95% and aPTT to 38.4 seconds, operation was begun. No more transfusion was needed during the operation. In his postoperative care, he received 50 units/kg a 12 hours for 3 days and 30 units/kg a 12 days for 2 days. His factor VIII activity was maintained at 57-139% during his hospitalization. He was discharged without any anesthetic complication. So we report this successful case of anesthetic management for the oral surgery in a child with hemophilia A.

아데노이드편도절제술에서 술전${\cdot}$술후 경구용 스테로이드의 효과 (The Effect of Preoperative and Postoperative Oral Steroid in Adenotonsillectomy)

  • Postoperative pain, poor oral intake and various complaints of adenotonsillectomized patients are the main problems for the otolaryngologist. Steroids have been advocated to reduce morbidity after adenotonsillectomy, but the results are conflicting. This study is to determine the effect of preoperative and postoperative oral steroid. Material and Methods : A prospective, randomized study was performed on 40 patients from 4 to 13 years of age. 20 patients undergoing adenotonsillectomy received steroid(experimental group), others undergoing adenotonsillectomy did not receive steroid(control group). An oral prednisolone was administered preoperatively for 3 days and postoperatively 7 days. A dosage was determined by patient's weight. Postoperatively each patients was examined for weight loss as well as for subjective signs of pain, oral intake, activity, mouth odor and analgesic usage. Results: experimental group showed decreased morbidity in view of postoperative pain, oral intake with statistical significance (p<0.05). The usage of analgesics was decreased in steroid group. but, activity, mouth odor and body weight showed no statistical difference. Conclusion: The results showed that preoperative and postoperative oral steroid affect the postoperative morbidity in adenotonsillectomy.

Antimicrobial Activity of Continentalic Acid from Aralia cordata Against Enterococcus Strains

  • Continentalic acid (CA, (-)-pimara-8(14), 15-diene-19-oic acid) was isolated from the roots of Aralia cordata (Araliaceae) using bioassay-guided fractionation of a crude chloroform extract. The antibacterial activity of CA against Enterococcus faecalis and Enterococcus gallinarium was estimated by determining minimum inhibitory concentrations (MICs) and minimum bactericidal concentrations (MBCs). CA exhibited potent activity against standard vancomycin-resistant enterococci (VRE) and vancomycin-susceptible enterococci (VSE), with MICs and MBCs values between 4 and $8{\mu}g/mL$ and 4 and $16{\mu}g/mL$, respectively. This compound exhibited potent activity against strains of VRE, which are highly resistant to clinically useful antibiotics. These findings suggest that continentalic acid may be useful in controlling enterococcal infection.

  • Objective : This dissertation was designed to evaluate the effect of BCL(refinded Bambusae Caulis in Liqua-men) oral administration and herbal acupuncture on alcohol metabolism and liver function. Methods : Mice were damaged by a large quantity of alcohol and received treatment of either BCL $1mg/kg$ in oral or BCL $250{\mu}g/kg$ in herbal acupuncture-BL18 BL19 bilateral. and then such parameters as GOT, GPT, catalase and superoxide dismustase(CuZn-SOD, Mn-SOD) were measured. Results : 1. Compared with control group, the activity of GOT, GPT in serum was significantly reduced and the proper degree of alcohol in serum was not significantly differ from oral administration group and herbal acupuncture group. 2. The activity of catalase in liver cell tissue and the activity of CuZn-SOD in liver cell tissue, compared with control group, was not sigificantly affected either by oral administration and herbal acupuncture group. But the activity of Mn-SOD was significantly increased in oral administration group, while it was not the case in acupuncture group. Conclusion : we consider that BCL oral administration and herbal acupuncture is highly effective in recovering alcohol metabolism and liver disfunction induced by alcohol.

구강암에 대한 약용식물 추출물의 항암효과에 관한 연구 (STUDIES ON ANTI-ORAL CANCER ACTIVITIES OF MEDICINAL PLANT EXTRACTS)

  • Treatment of oral cancers with chemotherapeutic agents are evaluated as an effective method for remission to reduce cancer proliferation nowadays. But, minimization of side-effects such as bone marrow suppression, gastrointestinal toxicity and renal damage is another problem to be solved. Thus, a possible approach to develop a clinically applicable chemotherapeutic agents is to screen anticancer activity among traditional medicinal plants which have been used for thousands of years with very low side-effects in orient. In this study we focused on screening anti-oral cancer activities among 14 traditional medicinal plant extracts that revealed anticancer activities on other solid tumors. The results were as follow : 1. Methanol extract of Lepidium apetalum showed the highest anti-oral cancer activity against A253 cells. At concentration of $4{\mu}g/ml$, the cell viability was 48% under our experimental condition. $IC_{50}$ value obtained was $4{\mu}g/ml$. 2. Methanol extract of Coptis japonica and Solanum nigrum were effective on KB cells. Cell viability observed were 62% and 67% at concentration of $4{\mu}g/ml$, and $IC_{50}$ values were $12{\mu}g/ml$ and $10{\mu}g/ml$ respectively. 3. When the methanol extract of Lonicera caerule was combined with $2{\mu}g/ml$ of cisplatin, the anticancer activity was synergistically increased. One hundred ${\mu}g/ml$ of Lonicera caerule showed 92%(alone) or 59%(combined with cisplatin) cell viabilities. $IC_{50}$ value of Lonicera caerule extract against KB cells was reduced from $301{\mu}g/ml$ to $126{\mu}g/ml$ when combined with $2{\mu}g/ml$ of cisplatin. 4. Medicinal plant extracts effective on both A253 and KB cells were Coptis japonica, Lepidium apetalum, Solanum nigrum, Caesalpiniae Lignum, Curcuma aromatica.

한국 전통 된장의 구강미생물에 대한 항균효과 (Anti-microbial Activity of Korean Fermented Soybean Paste (Doen-jang) Against Oral Microbes)

  • This study was performed to investigate the anti-microbial activity of extract from Korean fermented soybean paste (doen-jang) against 16 types of oral microbes, and to determine the minimum inhibition concentration (MIC) of the extract for three major microbes causing human oral diseases (Streptococcus mutans, Porphyromonas gingivalis, and Candida albicans). The extract was prepared using ethyl acetate and it was treated with the oral microbes at a concentration of 5.00 mg/ml (0.5%). The anti-microbial activity and MIC were measured using broth dilution method. Significant reduction of microbial activities of 16 types of oral microbes occurred when the soybean paste extract was added to the broth compared to the control (p<0.01), and striking inhibition (more than 99%) was observed in ten types. S. mutans, which causes dental caries, showed MIC at a concentration of 1.25 mg/ml for the extract. P. gingivalis, which causes adult periodontal disease, showed MIC at a concentration of 2.50 mg/ml for the extract. C. albicans, which causes denture stomatitis and angular stomatitis, showed MIC at a concentration of 20 mg/ml for the extract. These results indicate that ethyl acetate extract of doen-jang showed strong anti-microbial effect against 16 types of oral microbes, and the anti-microbial effect of the extract against oral microbes was stronger against bacteria than against fungi. The anti-microbial effect might be possibly enhanced by the fermentation of soybeans.

  • Cancer stem cells have stem cell-like features, such as the ability for self-renewal and differentiation but show unlimited growth because they have the lost normal regulation of cell growth. Cancer stem cells and normal stem cells have similar features. They show high motility, diversity of progeny, robust proliferative potential, association with blood vessels, immature expression profiles, nestin expression, epidermal growth factor (EGF)-receptor expression, phosphatase and tensin homolog (PTEN) expression, hedgehog pathway activity, telomerase activity, and Wnt pathway activity. On the other hand, with cancer cells, some of these signaling pathways are abnormally modified. In 1875, Cohnheim suggested the concept of cancer stem cells. Recently, evidence for the existence of cancer stem cells was identified. In 1994, the cancer stem cells' specific cell surface marker for leukemia was identified. Since then, other specific cell surface markers for cancer stem cells in solid tumors (e.g. breast and colon cancer) have been identified. In oral cancer, studies on cancer stem cells have been performed mainly with squamous cell carcinomas. Oral cancer specific cell surface markers, which are genes strongly expressed in oral cancer and cancer stem cell specific side populations, have been identified. Cancer stem cells are resistant to radiotherapy and chemotherapy. Therefore, to eliminate malignant tumors efficiently and reduce the recurrence rate, therapy targeting cancer stem cells needs to be performed. Currently, studies targeting the cancer stem cells' specific signaling pathways, telomerase and tumor vasculatures are being done.

Jaborandi extract와 Sorbitol이 구강미생물 증식에 미치는 영향 (Antimicrobial activity of jaborandi extract and sorbitol to oral microbes)

  • Objectives : The aim of the study is to investigate the antimicrobial activity of jaborandi and sorbitol to various oral microbes. Methods : Jaborandi leaves contain pilocarpine. The conditions for extraction were optimized on the basis of substances, temperatures and pHs. Total pilocarpine content after extraction was analyzed by HPLC. The herbal antimicrobial activity of jaborandi and sorbitol were evaluated for oral microbes containing ATCC 25175 S. mutans, ATCC 13419 S. salivarius, ATCC 6249 S. mitis, ATCC 33398 S. equi, ATCC 29213 S. aureus, ATCC 14053 C. albicans. Results : The optimum conditions for highest yielding extraction were pilocarpine content after boiling at $100^{\circ}C$ for 1 hour at pH 3. The level of total pilocarpine content was analyzed at 833 mg/kg by HPLC. The most effective antimicrobial activity was obtained by combination of pilocarpine and sorbitol rather than pilocarpine, menthol and sorbitol, respectively. Conclusions : This results supported the preventive oral health care using safe and convenient jaborandi herb.

Cytotoxic Effect of Syringic Acid on Human Oral Epithelioid Carcinoma Cells

  • This study was undertaken to clerify the cytotoxic effect of syringic acid by colorimetric assay on human cancer cells. For the evaluation of cytotoxicity of syringic acid, the cell viability and cell adhesion activity of syringic acid on cancer cells, human oral epithelioid carcinoma cells were determined using by colorimetric assays such as MTT (3-[4,5­dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide) assay and XTT (2,3-bis-[2-methoxy-4-nitro-5-sulfophenyl]­2H-tetrazolium-5-caboxanilide) assay, respectively after human oral epithelioid carcinoma cells were treated with syringic acid for 48 hours. In this study, the cell viability of syringic acid on human oral epithelioid carcinoma cells showed a significant decrease by MTT assay compared with control, and also, the cell adhesion activity by XTT assay was decreased significantly in these cells after cells were treated with various concentrations of syringic acid for 48 hours. $MTT_{50}\;and\;XTT_{50}\;were\;282.3\;{\mu}M\;and\;418.8{\mu}M$ syringic acid, respectively. These results suggest that syringic acid shows midcytotoxic effect on human oral epithelioid carcinoma cells by the decreasement of the cell viability and the cell adehision activity assessed by colorimetric assay in these cultures.

매실추출물(PME)의 구강 미생물에 대한 항균작용 (Antimicrobial activity of Prunus mume extract to oral microbes)

  • Objectives : Prunus mume has been used for the folk medicine from old times. The purpose of the study was to investigate the antimicrobial activity of prunus mume extract to various oral microbes. Methods : This study was carried out to examine the antimicrobial effect of Prunus mume extract against oral microbes. Data were collected using a Dentocult SM Strip mutans and Dentocult LB Strip mutans from April 5 to May 4, 2013. A total of 36 experimental and 32 control group were selected for this study. Results : The MIC of Prunus mume extract was tested for 0.39% in S. mutans, S. salivarius and S. auerus, 0.78% in S. mitis, S. equi and E. coli. In vivo, experimental group showed significantly the lower Streptococcus mutans levels by the use of the gum contained Prunus mume extract from 15 days compared with control group(p=0.012). The reduction was more significant in Lactobacilli level of the experimental group than the control group(p=0.022). Conclusions : These findings suggest that the oral products containing Prunus mume extracts is effective in preventing oral diseases.
