산후조리원의 표준화 관리 지침을 위한 연구 - 산모와 신생아 관리현황을 중심으로 - (A Study for the Development of Standardized Management Manuel in Sanhujoriwon - Centered on the Management of Women & Newborn -)

  • The purpose of this study was to develop the frame for standardized manual for Sanhujoriwon by finding out the status of the management of women & newborn in Sanhujoriwon, like postpartum care center. The subjects were 95 staffs of 22 Sanhujoriwon agreed on oral consent, in Seoul and Bundang, Korea. Data were collected from Sep. 15 to Oct. 24, 2001. The instrument used for this study was a structured questionnaire consisted of 16 items of general characteristics & educational characteristics about postpartum care, 31 items of degree of management of women's postpartum care (Cronbach's $\alpha$.93 ), 24 items of degree of management of newborn's care(Cronbach's $\alpha$.94 ), 10 items of methods of management of women's postpartum care, 8 items of methods management of newborn's care. The data were analyzed by the SPSS/PC+ program using frequency, percentage, mean, standard deviation, t-test, ANOVA and Pearson correlation coefficient. The results of the study were as follows ; 1. The mean of the degree of management for postpartum women was 4.1. The mean score for the degree of management of general postpartum care for women was 4.00 & traditional postpartum care(Sanhujori) was 4.20. The degree of management of traditional postpartum care was higher than general postpartum care for women. 2. The degree of management for newborn : The mean score for the degree of management of newborn was 4.37. 3.There was a strong positive correlation between general postpartum care and traditional postpartum care($r=.744^{**}$), and postpartum mother care and newborn care($r=.798^{**}$). 4.The basic frame for the management of the women and newborn in Sanhujoriwon. 1) For women: Integrated postpartal care Physical management : Vital sign & BP check, contraction of uterus, form and amount of lochia, management of personal hygiene, management of breast & breast-feeding, management of postpartum exercise, prevention of infection, symptom & sign of high risk and prevention & management of high risk condition; Emotional-psychological management: assessment and management of mother-baby attachment, emotional state; Educational management : education of vaccination schedule, urinary incontinence, rearing infant, breast-feeding ; and Environmental management : temperature, humidity, disinfection, cleaning, light, infection control integrated with 6 principles of Sanhujori. 2) For newborn Physical management : check of vital sign, management of umbilical cord, jaundice, prevention of infection, management of diaper rash; Emotional-psychological management : assessment of sleep, crying, activity, response of mother-baby attachment; and Environmental management : temperature, humidity, disinfection, cleaning, light, infection control integrated with 6 principles of Sanhujori. In conclusion Sanhujoriwon must be health care center for the postpartum women and newborn. Therefore, the establishment of various laws and regulations in such a way to meet the realistic needs of Sanhujoriwon as a health care center for women and infants future health should be done. The standardized management manual based on the results is absolutely required above all.

소양면 지역사회 환경기초조사 (A Basie Community Health Survey in Rural Korea (Soyang-Myun))

  • 1. Introduction Community medicine with the concept of comprehensive medical care and an ideal medical care delivery system not only for an individual or family but for the whole community has emerged. In April 1970, the Presbyterian Medical Center started a hospital based community health service project in order to improve the health of the people in rural areas. Prior to commencing a comprehensive medical care system, a family survey was needed. The major objective of this survey was to obtain information concerning the people and their environment so as to be able to plan and implement a comprehensive medical care program in Soyang-Myun. 2. Survey Method An interview using a family record form was carried out for each household. This family record form was designed to get information about demography, family planning, environmental sanitation and vital statistics. Prior to beginning, the members of the survey team were trained in interviewing techniques for three days. The team consisted of a public health nurse, four nurse-aides, a sanitarian and four health extension workers who are working in our project, The survey was carried out during the period November 1971 to March 1972. 3. Project area 1) Population of Soyang-Myun was 11,668; male, 5,962 and female, 5,706.   • The introduction of cone-beam computed tomography(CBCT) and computer software in orthodontics has allowed orthodontists to provide more accurate diagnosis and treatment. The most common use of CBCT imaging allows orthodontists to visualize the precise position of supernumerary or impacted teeth, especially impacted canines. In doing so, the exact angulation of impaction and proximity of adjacent roots can be evaluated by orthodontists, allowing them to choose vector forces for tooth movement while minimizing root resorption. Even though 2-dimensional panoramic images can be used to view the position of the impacted canines, they have limitations because it is not possible to evaluate the impacted tooth position 3-dimensionally. An accurate knowledge of root position improves the determination of success in orthodontic treatment. Nowadays, considering the fast pace of technological development, a combination of intraoral scanning, digital setups, custommade brackets and wires, and indirect bonding may soon become the orthodontic standard. In this paper, this will be discussed along with the digital models.

New approach of maxillary protraction using modified C-palatal plates in Class III patients

  • Maxillary protraction is the conventional treatment for growing Class III patients with maxillary deficiency, but it has undesirable dental effects. The purpose of this report is to introduce an alternative modality of maxillary protraction in patients with dentoskeletal Class III malocclusion using a modified C-palatal plate connected with elastics to a face mask. This method improved skeletal measurements, corrected overjet, and slightly improved the profile. The patients may require definitive treatment in adolescence or adulthood. The modified C-palatal plate enables nonsurgical maxillary advancement with maximal skeletal effects and minimal dental side effects.

2005년 치과의사 흡연 실태와 금연교육에 대한 제언 (Proposal for the smoking prevalence among dentists and smoking cessation in 2005)

  • By organizing the results of study questioned 1,620 dentists working nationwide in 2005, we concluded the followings. 1. The smoking rate of dentist is 29.69%. 2. 60.9% of current dentist smokers responded that they started smoking at the age of 20-24, during their dental school ages. 3. 67.41% of respondents have no idea of cyber antismoking campaign launched by Korean Dental Association, and 94.07% of respondents didn t receive formal education about smoking cessation. 4. 59.01% of respondents want to educate patients about smoking cessation in dental clinics but couldn t help putting it off for practical reasons. The reasons include their busy office hour(36.30%), difficulties in collecting proper smoking cessation documents(25.10%), and lack of their own knowledge in renunciation of smoking. (18,83%) 5. As dental healthcare personnels, they agreed with their status and duty of leading role in national renunciation of smoking. (R>80%) 6. 83.34% of dentists agreed that if educational antismoking program is arranged by Korean Dental Association, they should receive the education.

Nonsurgical maxillary expansion in a 60-year-old patient with gingival recession and crowding

  • Maxillary transverse deficiency often manifests as a posterior crossbite or edge-to-edge bite and anterior crowding. However, arbitrary arch expansion in mature patients has been considered to be challenging due to the possible periodontal adverse effects such as alveolar bone dehiscence and gingival recession. To overcome these limitations, nonsurgical maxillary expansion of the basal bone has been demonstrated in young adults. However, the age range for successful orthopedic expansion has remained a topic of debate, possibly due to the underlying individual variations in suture maturity. This case report illustrates nonsurgical, miniscrew-assisted rapid palatal expansion (MARPE) in a 60-year-old patient with maxillary transverse deficiency accompanied by anterior and posterior crossbites, crowding, and gingival recession. The use of MARPE allowed relief of crowding and correction of the crossbite without causing significant periodontal adverse effects.

아동·청소년 치과 주치의 사업의 시행현황과 발전방안 (Study of the implementation and development of a child·adolescent dental care service)

  • This study focused on the status of dental care development for children and adolescents. In contrast to the chronic disease management project, this research intended to provide basic data for the expansion of the system. We summarized the status of the system's expansion since 2012. It analyzed research reports, papers, related literature and books on the system of children and adolescent dental care service. The literature analysis classifies year, publication, title and published location. The current state of the system is listed as location, target, support funds, etc. Implementation of the project was first discussed in 2007. Initial planning focused on the Oral Health Policy Research Society of the Gunchi. Effective measures were formulated in 2008 under the center of the Gunchi. It is time to discuss the dental care system's direction and development, as well as future aims based on a beneficial program of preventive care. A system of dentistry should be introduced to benefit the entire population.

Z세대 치위생과 학생의 노인에 대한 태도에 영향을 미치는 요인 (Factors influencing attitude toward the elderly of generation Z dental hygiene students)

  • Objectives: In an aging society, the demand for dental medical institutions among the elderly is increasing. There will be an increased demand for high-quality oral health services. Therefore, the purpose of this study was to investigate factors influencing attitudes toward the elderly of Generation Z dental hygiene students. Methods: The participants in this study were 198 dental hygiene students. The survey was conducted from June 1 to 30, 2020, using a structured questionnaire. The questionnaire consisted of knowledge (24 items), attitude (20 items), and experience (11 items) of the elderly. The collection data were compared and analyzed using analysis of variance and multiple regression. Results: Factors influencing attitudes toward the elderly of dental hygiene students appeared positive in those with a lot of experience and higher knowledge of social area, in case of one grade. However, the attitude toward the elderly appears negative in those with a low knowledge of physical area. Conclusions: In order to improve Generation Z dental hygiene students' attitudes towards the elderly, it is necessary to develop a course related to the elderly and a program related to high-quality elderly contact experience opportunities. Socially, it is necessary to promote healthy elderly life through internet broadcasting and YouTube.

일부 만 30세 이상 성인에서 구강건조증 관련요인 분석 (The Associated Factors with Xerostomia in Adults Aged 30 Years and Over)

  • 본 연구는 구강건조증 인식도와 관련요인을 분석하여 구강건조증 예방 및 환자관리법에 대한 기초자료를 제공하기 위하여 실시하였다. 2012년 8월 1일부터 9월 30일까지 인천 및 경기지역에 거주하는 만 30세 이상 성인 240명을 대상으로 설문조사를 시행하였으며, t검정과 일원배치분산분석 및 다중선형회귀분석을 시행한 결과 다음과 같은 결론을 얻었다. 1. 일반적 특성과 구강건조증 인식도의 관련성을 분석한 결과 51세 이상이고 직업이 없으며, 월 평균 수입이 300만원 미만이라고 응답한 집단에서 구강건조증을 더 많이 인식하였다(p<0.05). 2. 건강상태와 구강건조증 인식도의 관련성을 분석한 결과 전반적으로 구강건강이 좋지 않으며, 치아우식증과 치주질환 및 구내점막질환 증상이 2개 이상인 집단에서 구강건조증을 더 많이 인식하였다. 또한 전반적으로 전신건강이 좋지 않으며, 만성질환에 이환되고 매일 약물을 복용하는 집단, 수면상태에 만족하지 않고 절망감을 인식한 집단에서 구강건조증을 더 많이 인식하였다(p<0.05). 3. 삶의 질과 구강건조증 인식도와의 관련성을 분석한 결과 저작능력, 대화능력 및 일상활동에 문제가 있고, 통증 불편함이 있으며 불안 우울한 집단에서 구강건조증을 더 많이 인식하였다(p<0.001). 4. 전신건조감과 구강건조증 인식도와의 관련성을 분석한 결과 피부와 눈, 입술 및 비강 내 점막이 자주 건조한 집단에서 구강건조증을 더 많이 인식하였다(p<0.001). 5. 구강건조증 인식도와 연관성의 강도를 분석한 결과 구강건조증 인식도는 삶의 질($\beta$=0.436)과 가장 연관성이 높았 다. 다음은 매일 복용 약물 수($\beta$=0.239), 전신건조감($\beta$=0.200), 절망감 인식($\beta$=0.160), 구내점막질환 증상 수($\beta$=0.099)의 순으로 나타났다(p<0.05). 이상의 연구결과로 볼 때 구강건조증은 삶의 질 저하를 초래할 수 있으므로 사회적 관심이 필요하며, 특히 구강보건인력은 구강건조증의 원인이나 부작용 및 치료법 등에 대한 관심도를 더욱 높여야 할 것이다. 또한 계속구강건강관리체계의 운영을 통해 구강건조증 예방 및 완화법에 대한 교육을 더욱 강화하여야 하고, 구강건조증에 대한 환자의 느낌이나 타액분비저하로 인해 나타나는 구강 내 변화에 세심한 관심과 철저한 모니터링이 수반되어야 할 것이다.

한국청소년의 성실태고찰 (Implication for Korean Adolescent Sexuality)

  • Rapid socio-cultural and economic changes in the country has brought with it changes in the society's value system. For a traditional society that is increasingly being exposed to modernization but where sex norms are still very restrictive, the adolescent sexual mores takes on added significance. Adolescents are caught between two opposing forces, the changing environment that allows for freer and liberal mores and the traditional society that cannot keep pace with the changing environment and therefore demands resistance to changes. This paper focuses on problems of adolescent sexuality in this country and considers the countermeasures for the existing problems. Amongst the problems are: (a) increasingly younger age of the adolescents who start sexual intercourse (b) non-use of contraception, (c) unwanted pregnancies, (d) increase in the number of induced abortion and (e) increase in the number of unwanted children and unmarried mothers. The Korean adolescent's sexual behavior seems to follow that of the developed countries. In other words, many western modes of life and sexual values seem to bave been copied in Korea and yet Korean adolescents lack in their knowledge of sex related matters such as reproductive physiology and contraception. Among middle and high school students, female students are reported to have less knowledge on sex than male students according to a 1988 survey by KIPH. Even among the unmarried famale factory workers, only 42.5 percent replied they know of the condom, and 25.1 percent and 23.1 percent said they had knowledge of spermicide and menstrual regulation respectively. However, 14.9 percent and 13.9 percent reported that they had a knowledge of the loop and female sterilization respectively according to the 1984 study by KIPH. Among the middle school students 0.8 percent said they had experience in sexual intercourse, while 7.3 percent of the high school students reported having had sexual intercourse. The sexual intercourse experience rate among the unmarried female factory workers is 37.8 percent. Among those female factory workers with sexual experience, 46.7 percent had more than one sex partners. Only 39.1 percent of male students and 18.9 percent of female students among those with sexual intercourse experience have used contraceptives. mostly condoms and oral pills 45.1 percent of female factory workers with sexual intercourse experience used contraceptives such as pills, condoms and rhythm methods. The pregnancy experience rate among the female factory workers who had experience in sexual intercourse is 29.5 percent, which is 11.1 percent among the total respondents. Out of the 102 pregnant female workers, 98 workers(96.1 percent) terminated their pregnancy by induced abortion and 2 workders(2 percent) in natural abortion, while 1 worker(1 percent) was in pregnancy and another 1 worker had normal birth that was subsequently sent to orphanage. In order to cope with the problem of adolescent sexuality, a drastic and strong policy measures should be taken by the government. The most effective countermeasure to the adolescent sexual problems appears to the education. The sex and population education in the school is very much in need. In addition, sex education program through mass media and at the job sit-should be promoted for a healthy development of adolescents' sexual behavior. Also, the existing national family planning program, which has focused on the married couples, should be extended to the unmarried people in its scope and contents of the program.
