한국의 보건소 근무 치과위생사의 공중구강보건사업 수행업무실태와 대학교육간의 연관성 (Usefulness of Dental Hygiene Education Curriculum in the Public Oral Health Service Performance in Korea)

  • 본 연구에서는 현행 치위생과 구강보건관리 영역 교육과정이 보건(지)소 업무 수행에 미치는 활용도를 알아보고자 하였다. 이에 치위생과 31개 대학의 구강보건관리 영역 교과목의 시간 수를 조사 분석하고, 보건(지)소에 근무하는 치과위생사 96명을 대상으로 보건소에서 구강보건교육학 및 실습, 치면세마론 및 예방치과학 실습, 공중구강보건학 실습 및 구강보건통계학 영역의 업무빈도, 교육내용의 유용성, 중요성, 강화성에 관한 설문조사를 실시하여 치위생과의 교육과정과 보건(지)소 업무 사이의 연관성을 분석하여 다음과 같은 결과를 얻었다. 1. 치위생과 교과목 관련 보건(지)소 업무로는 치면열구전색이 주 1회 이상 가장 많이 실시하고, 교육내용이 매우 유용한 것으로도 치면열구전색이라고 가장 많이 응답하였다. 매우 중요한 교육내용은 대상자별 잇솔질법, 교육내용이 더욱 강화되어야 한다고 생각하는 항목으로는 대상자별 구강보건교육이라고 가장 많이 응답하였다. 2. 업무빈도와 교육내용의 유용성과의 연관성에서, 업무빈도가 가장 높은 치면열구전색이 교육내용 중에서 가장 유용한 것으로 나타났고, 교육내용이 중요한 대상자별 잇솔질교육, 치면열구전색, 대상자별 구강보건관리는 더욱 강화되어야 한다고 나타났다. 또한 업무빈도가 높은 업무일수록 더욱 강화되어야 한다고 나타났다. 3. 구강보건교육학 및 실습 영역은 업무빈도에 비해 교육 학습시간수가 적으므로 더욱 강화되어야하고, 치면세마론 및 예방치과학 실습은 불소도포, 치면열구전색, 대상자별 구강건강관리 또한 학습시간수의 부족으로 더욱 강화되어야 한다고 나타났다.

  • The purpose of this study was to analyzes the Job of Dental Hygienists in Dental (Clinics) Hospitals the Capital region. This study analyzes the degree of job importance and education-training need about and task, task according to work place and work age. The results are as follows : (1) Job importance of dental hygienists were order 'photographing in Dental Radiology', 'Management of Dental clinic', 'Oral prophylaxis', in case education-training need was order 'dental health insurance', 'Oral prophylaxis', 'Management of Dental clinic'. duty more than 5.0 of job importance and education-training need was as 'dental health education', 'Oral prophylaxis', 'preventive dental treatment', 'dental assistance (cooperation)', 'photographing in Dental Radiology', 'dental health insurance', 'Management of Dental clinic', Duty of practice centering in Dental (Clinics) Hospitals except 'Public oral health'. (2) Job importance and education-training need of task increased most of job importance in proportion to education-training need. (3) No significantly between dental hospital hygienist and dental clinic hygienist difference of job importance and education-training need according to work place. but 'Management of Dental clinic' and 'dental health insurance' of dental hospital hygienist lower than dental clinic hygienist. (4) The results job importance compare less 3 years to more 3 years of dental hygienists were perceive significantly 'dental health education', 'Public oral health', 'dental health insurance', 'Management of Dental clinic' the other hand, education-training need was perceive significantly 'preventive dental treatment'.

치과 병·의원에서의 구강보건교육업무에 대한 직무분석 (A Job Analysis on Oral Health Education Task in Dental Hospitals and Clinics)

  • 본 연구는 치과위생사들의 업무실태 및 비중을 조사함으로써 직무의 개선을 통한 체계적이고 바람직한 업무수행을 위한 기초 자료를 제공하고자 시도되었다. 본 연구대상자의 일일 업무비중을 분석한 결과, 진료협조업무의 비율이 35.45%로 가장 높게 나타났고, 치주처치업무 21.54%, 행정 및 관리업무 16.08%, 구강보건교육업무 15.88%, 예방처치업무 11.23% 순으로 나타났다. 구강보건교육내용 및 구강보건교육자료의 종류별 활용비율에서는 대상자별 잇솔질 교육법이 3.70점으로 가장 높게 나타났고, 구강보건위생용품사용법 3.51점, 정기검진에 대한 교육법 3.26점, 치과질환 예방교육법 3.13점, 식이조절교육법 1.39점 순으로 나타났으며, 악치모형물이 38.0%로 가장 많이 이용하는 것으로 나타났고, 구두로 설명 23.9%, 구강내 카메라 12.2% 순으로 나타났다. 따라서 치과위생사의 고유 업무라 할 수 있는 구강보건교육 및 구강질환의 예방관련 업무비율이 높이 않아 이에 지속적인 관심과 활용가치가 높은 다양한 프로그램을 수행하여 향후 환자의 구강보건교육의 질 향상을 도모하기 위한 제도수립이 필요하다고 사료된다.

성인들의 구강보건과 건강.삶의 질의 관련성 연구 - 영남지역 일부 주민들을 대상으로 - (Research about Relationship between the Quality of life, Oral Health and Total Health of adults)

  • The purpose of this research is to increase adults' quality of life by improving oral health. Total 572 people participated in this study in Young-Nam area. Survey research method was used to measure various tools of participants' general index, subjective view of oral health status, oral health knowledge, oral health practice, dietary pattern, OHIP-14, THI, QOL for analyzing the effect on quality of life. The result of the study is summarized as follows; 1. Oral health knowledge has positive effects on oral health practice (0.442), dietary pattern(0.343). Diet has positive effects on OHIP-14(0.187) and OHIP-14 has significant positive effects on THI(0.564). THI also has positive effects on QOL(0.380). But oral health practice index has slight effects on OHIP-14(0.056). 2. Indirect effect indexes on QOL are OHIP-14(0.214), diet(0.040), oral health knowledge(0.019), oral health practice(0.012) listed in order of effect size, and the biggest direct effect on QOL is from THI(0.308). The result of this study shows oral health and total health are important to improve the quality of life. And the knowledge about oral health dietary pattern should be important to improve oral health. But, even with high level of oral health knowledge, the oral health practice is at low level. Health education developing program should be needed. It is required to present basic data which represent adults with national view by extracting groups using sample probability methods as of next task of this study.

국내 장애인의 구강 관련에 대한 문헌고찰(2000년-2015년) (Literature review on oral health among people with disabilities in Korea (2000-2015))

  • Objectives: This study consisted of a literature review on oral health among people with disabilities, and to present the factors that should be considered in developing an oral health program for people with disabilities. Methods: The key words 'people with disabilities' and 'oral health' were searched in 4 Korean academic journals and 5 online search engines and a total of 635 papers were identified. Duplicate papers were removed, and the literature selection criteria were applied to the remaining papers. Finally, a total of 45 papers were used in the review. Results: First, people with disabilities were the most common research subjects, followed by dental students, dental hygiene students, dentists, dental hygienist and guardians and special education teachers. Second, the most frequently studied research topic was dental examination, followed by oral health behavior and behavior of using dental clinics. Third, research purposes included the status of oral health, the quality of oral health, dental treatment, the use of oral health service, and oral hygiene behavior and perception. Fourth, the most frequently discussed policy task was expansion of research subjects, followed by oral hygiene and policy development. Conclusions: To enhance the oral health of people with disabilities who have difficulty in keeping their teeth clean and healthy on their own, development of an oral hygiene training program is required not only for people with disabilities, but also for guardians and teachers. It is equally important to examine oral health behavior that could potentially affect the status of oral health and create a more accurate and systematic oral hygiene method. In addition, the government, together with various other research institutes, should conduct an oral health survey of a representative sample of people with disabilities to determine oral health status and facilitate improvements to oral hygiene programs.

일개대학 National Competency Standard (NCS) 교과목 운영에 관한 연구: 예방치학 및 실습 교과목 중심으로 (Development and implementation of national competency standard: based on preventive dentistry and practice)

  • Objective: The purpose of the study is to investigate the curriculum development and operation based on national competency standard (NCS). Methods: The duty of the dental hygienist was analyzed based on DACUM by ten experts in January, 2011. The duty model of the dental hygienist was inspected after duty analysis. The subjects of choice were preventive dentistry and practice. The satisfaction with the subjects were carried out from March to June, 2015. Results: The duty analysis of dental hygienist by DACUM produced preventive dental treatment(11 tasks), oral health education(3 tasks), comprehensive dental hygiene treatment(6 tasks) and 12 categories(156 tasks). Preventive dental treatment was divided into preventive dentistry and practice, oral health education was changed into oral health education and practice, and comprehensive dental hygiene treatment was replace by comprehensive dental hygiene and practice. The contents of preventive dentistry and practice included outline, learning objective, related knowledge and self evaluation. Professional evaluation required mutual experience and evaluation of the students. The mutual evaluation of the students was $4.61{\pm}0.506$(dental plaque control) and $1.80{\pm}0.316$(tooth brushing). The professional evaluation was $1.73{\pm}0.274$(dental plaque control) and $1.60{\pm}0.322$(tooth brushing)(p<0.01). The satisfaction with preventive dentistry and practice was $4.61{\pm}0.506$(improvement in practical work ability), $4.58{\pm}0.511$(knowledge improvement) and $4.55{\pm}0.572$(NCS educational environment) in order. Conclusions: The operation of NCS curriculum is considered to improve practical work ability and to solve skill mismatch between dental industries and educational training institutions.

119 구급대원의 구강악안면 외상환자의 지식, 태도, 수행에 관한 연구 (A study on the knowledge, attitudes, and performances of 119 EMTs regarding oral and maxillofacial trauma patients)

  • Purpose: This descriptive research study aimed to identify and strengthen the knowledge, attitudes, and performances related to first aid for patients with oral and maxillofacial trauma among 119 EMTs working at 119 safety centers and local stations in Jeollanam-do, South Korea. Methods: An online questionnaire was distributed among 144 119 EMTs working at fire stations in Jeollanam-do from August 1 to August 15, 2022. Results: The factors that influenced the knowledge of oral and maxillofacial trauma among the 119 EMTs included qualification (β=.113, p=.005) and performance (β=.837, p=.005), and those influential to attitudes were qualification (β=0.194, p=.005), ordinary interest (β=0.242, p=.005), and the need for education (β=.285, p=.005). The only factor that influenced the performance of first aid for patients with oral and maxillofacial trauma was knowledge (β=.900, p=.005). Conclusion: Considering the nature of first aid for patients with oral and maxillofacial trauma, it is necessary to organize practical knowledge and educational content tailored to the characteristics of each task. This should be simultaneously conducted with practice-oriented simulation education.

학교구강보건실 담당자의 업무 실태 및 양치시설에 대한 의견 조사 (The recognition on toothbrushing facilities and job status of facility workers in school dental clinic)

  • Objectives : This study aims to analyze the tasks, recognition and obstacles in operation of school dental clinics and to examine opinions on installation, operation and prerequisites for toothbrushing facilities. Methods : It conducted a survey t o the persons in charge at 378 school dental clinics in Korea and total 127 sheets, excluding incompletely answered data, were used for analysis by using SPSS 18.0. Results : Two regular dental hygienists visit school dental clinics 2-3 times per week and work 4-6 hours per week on average. Their tasks include oral health education, toothbrushing instruction, oral examination, sending school newsletters, and dental sealants. The obstacles of operation include excessive workload other than the work for the school dental clinic, lack of dentists, and lack of cooperation of principals and teachers in school. The persons in charge think that the chief task of the school dental clinic is the continuous oral health management, and it effectively affects students' oral health improvement. Most of them were for the installation of toothbrushing facilities. They said that it will be effective in students having an adequate toothbrushing habit and their toothbrushing rate increasing higher. They thought that if the school dental clinic is changed to toothbrushing facilities, it will improve students' oral health management. The prerequisites for toothbrushing facilities are the support of manpower in charge, principal's support, and development of operational programs. Conclusions : The most effective function of school dental clinics is constant oral health management. However, when public health doctors are reduced and dental sealants get included in health insurance, the budget of local government will decrease and then it will eventually reduce the work of school dental clinics. Therefore, it is needed to enhance support for school dental clinics or install a toothbrushing facilities rather than a school dental clinic.

구강건강증진 관점에서의 치과의료서비스와 치과공포의 연관성 (Relationships between Dental Fear and Dental Services Utilization with Respect to Oral Health Promotion)

  • Objectives: This research is to provide the basic data for the improvement on oral health by examining the relationship between the dental fear and the dental service utilization, and to discuss a strategy for dental health in order to lower the level of dental fear. Methods: Data were collected by conducting a structured survey of 1,607 people between the age of 13 and 70 who have experiences in visiting a dental institution. Both univariate and multivariate analyses were employed to analyze the relationship between the number of times dental service and dental fears. Results: The average level of dental fear the respondents reported was $52.16{\pm}15.71$ and 31.5% of the response was at the level of high dental fear. A strong physiologic response during the dental treatment was muscle tension and the stimulations that strongly arouse the dental fear were the anaesthetic needle and the sound of drill. A result of analysis on the relationship between the dental fear and the dental service utilization shows that as the level of dental fear became higher, the number of times for the dental service utilization for the last year had reduced. Also when a patient has an experience of putting off or canceling an appointment due to dental fear and as a physiologic response during the dental treatment became stronger, the result shows a tendency that the number of times for the dental service utilization for the last year had reduced. Conclusions: It has been observed that dental fear is one of the main barrier to use dental service. Therefore, a physio-psychological factor like the dental fear should be included in the task of promotion of dental health, and in analyzing the dental service utilization behavior. A discussion about how to lower dental fear was made with respect to oral health promotion. At individual level, improvement of communication skill by dental service providers should be made with rearrangement of clinic environment into more cozy one.

산학연계형 치위생학과 주문식 교육과정 연구 (A Study on Industry-University Cooperative Customized Curriculum of Dental Hygiene Study)

  • 최근 사회의 급격한 변화는 사회내 모든 부분에 반영되어 성과와 효율에 대한 무한 경쟁의 모습으로 나타나고 있다. 교육 분야도 이 같은 상황은 예외가 아니어서 수요자의 요구를 충족시키기 위한 형태로 대학간 전공간 경쟁이 심화되고 있다. 주문식 교육제도의 도입은 바로 이러한 특성을 반영한 대학의 생존전략 중에 하나이다. 치위생학의 주문식 교육과정에 대한 연구결과 치과병의원의 관리자들은 치과진료보조분야의 직무를 치과위생사의 주요업무라고 생각하는 것으로 나타났다. 반면 치과위생사들은 구강보건교육을 주요업무로 생각하고 있었으나 실제에 있어서는 본인의 의사와 달리 치과병의원 관리자의 의사를 따르는 것으로 나타나 1차적으로 산업체의 요구에 따른 산업체 현장실습강화가 필요한 것으로 분석되었다. 그리고 이와 함께 치과위생사의 주요 역할인 예방치과처치, 구강보건교육을 강화함으로써 병의원 내에서 치과위생사의 지위가 안정적으로 정착되게 하여 직장생활을 통해 자아실현의 기회를 제공할 필요도 있음을 알 수 있었다.
