Vascular Morphometric Changes During Tumor Growth and Chemotherapy in a Murine Mammary Tumor Model Using OCT Angiography: a Preliminary Study

  • To develop a biomarker predicting tumor treatment efficacy is helpful to reduce time, medical expenditure, and efforts in oncology therapy. In clinics, microvessel density using immunohistochemistry has been proposed as an indicator that correlates with both tumor size and metastasis of cancer. In the preclinical study, we hypothesized that vascular morphometrics using optical coherence tomography angiography (OCTA) could be potential indicators to estimate the treatment efficacy of breast cancer. To verify this hypothesis, a 13762-MAT-B-III rat breast tumor was grown in a dorsal skinfold window chamber which was applied to a nude mouse, and the change in vascular morphology was longitudinally monitored during tumor growth and metronomic cyclophosphamide treatment. Based on the daily OCTA maximum intensity projection map, multiple vessel parameters (vessel skeleton density, vessel diameter index, fractal dimension, and lacunarity) were compared with the tumor size in no tumor, treated tumor, and untreated tumor cases. Although each case has only one animal, we found that the vessel skeleton density (VSD), vessel diameter index and fractal dimension (FD) tended to be positively correlated with tumor size while lacunarity showed a partially negative correlation. Moreover, we observed that the changes in the VSD and FD are prior to the morphological change of the tumor. This feasibility study would be helpful in evaluating the tumor vascular response to treatment in preclinical settings.

안저 출혈 시 형광안저혈관조영술 및 대체 검사의 유용성 (The Usefulness of Fluorescein Angiography and Alternative Tests for Assessment of Fundus Hemorrhage)

  • 안저 출혈은 망막 조직과 혈관의 이상을 의미한다. 따라서 안과 의사는 안저의 출혈성 변화가 발생하면 이에 따른 치료 계획을 수립하기 위해 치료 전과 치료 중간, 치료 이후 등 병변의 진행상황을 파악하기 위해 여러 가지 안과 검사의 출혈 성 정도의 평가를 위해 오더를 지시한다. 현재 가장 유용하고 보편적인 안저검사에는 빛간섭단층촬영(OCT), 안저촬영(FP), 형광안저혈관조영술(FAG) 등이 있다. 중증 안저 출혈에 대한 치료 계획을 수립하기 위한 기존의 형광안저혈관조영술 검사에는 한계가 있다. 저자들은 광각형광혈관조영술을 이용하여 동공주위촬영과 5-quadrant 방법을 수행할 것을 제안한다. 이 방법을 사용하면 신속히 검사 부위를 결정하고 최대한 출혈의 반경을 피해 안과 의사에게 손상된 조직과 이상 혈관의 범위를 제공할 수 있다. 그런데도 불구하고 안과 의사가 안저 출혈이 매우 심각하여 광각형광혈관조영술이 무의미하다고 판단하는 경우가 있다. 이런 경우 대체검사로 안과초음파 및 망막전위도의 오더가 발생한다. 따라서 우리는 안 초음파 및 망막전위도 검사 필요성에 대해 당위성을 인정해야 하고, 정확하게 수행해야 한다.

Ocular manifestations in a patient with de novo Fabry disease

  • Fabry disease (FD) is an X-linked, recessively inherited, rare, progressive, disorder of glycosphingolipid metabolism affecting multiple organs resulting in organ dysfunction. It is rare to find only one FD affected subject with a de novo mutation. Here we report a case of a 41-year-old Asian male diagnosed with de novo FD. Comprehensive ophthalmological evaluation was performed using slit lamp, color fundus photography, optical coherence tomography, fluorescein angiography, and indocyanine green angiography. On slit lamp examination, cornea verticillata and slightly tortuous, and aneurysmal dilatation of inferior bulbar conjunctival vessels were observed. Other imaging modalities showed unremarkable findings. Cornea verticillata and inferior bulbar conjunctival vascular abnormalities may be detected earlier than other ocular abnormalities in de novo FDs like hereditary FDs.

18F-FDG PET/CT를 통해 진단된 주폐동맥 협착 소견의 폐동맥 육종 (Pulmonary artery sarcoma manifesting as a main pulmonary artery stenosis diagnosed by 18F-FDG PET/CT)

  • Pulmonary artery sarcoma (PAS) is a rare and fatal disease that often mimics chronic thromboembolic pulmonary hypertension (CTEPH); therefore, diagnosis of PAS is often delayed. Herein, a healthy 74-year-old man was presented with a 4-month history of dyspnea. Chest computed tomography showed wall thickening and stenosis in the main pulmonary artery as well as in both proximal pulmonary arteries. In order to differentiate between unusual CTEPH, vasculitis, and PAS, we performed right heart catheterization and pulmonary angiography. The mean pulmonary arterial pressure was 21 mmHg, and there was severe pulmonary artery stenosis. Thrombi on the pulmonary arterial wall lesions were observed in intravascular ultrasound and optical coherence tomography. Furthermore, the patient had a history of deep vein thrombosis. Therefore, we diagnosed unusual CTEPH. After 6 months of rivaroxaban anticoagulation therapy, a chest X-ray revealed a left lower lobe lung mass, and a positron emission tomography later showed hypermetabolic lesions in the main pulmonary artery wall, in both pulmonary arteries walls, in the lung parenchyma, and in the bones. A biopsy of the right proximal humerus lesion revealed undifferentiated intimal sarcoma. Pulmonary sarcoma is rare, but should be considered when differentially diagnosing main pulmonary artery wall thickening and stenosis. A positron emission tomography may aid in this diagnosis.

A Case of Ocular Toxocariasis Successfully Treated with Albendazole and Triamcinolon

  • We present a case of ocular toxocariasis treated successfully with oral albendazole in combination with steroids. A 26-year-old male visited the authors' clinic with the chief complaint of flying flies in his right eye. The fundus photograph showed a whitish epiretinal scar, and the fluorescein angiography revealed a hypofluorescein lesion of the scar and late leakage at the margin. An elevated retinal surface and posterior acoustic shadowing of the scar were observed in the optical coherence tomography, and Toxocara IgG was positive. The patient was diagnosed with toxocariasis, and the condition was treated with albendazole (400 mg twice a day) for a month and oral triamcinolone (16 mg for 2 weeks, once a day, and then 8 mg for 1 week, once a day) from day 13 of the albendazole treatment. The lesions decreased after the treatment. Based on this study, oral albendazole combined with steroids can be a simple and effective regimen for treating ocular toxocariasis.

In Vivo Enhanced Indocyanine Green-Photothermal Therapy for a Subconjunctival Tumor

  • Indocyanine green (ICG) is a dye approved for use in clinical diagnostics. ICG remains in the intravascular space following intravenous administration, due to its ability to rapidly bind to the plasma proteins, and its therapeutic potential has been studied in well-vascularized cutaneous tumors. Here we have evaluated the clinical response of a subconjunctival tumor to photothermal therapy (PTT) using an ICG-enhanced near-infrared diode laser and its adverse effects, in a rabbit. 22 male New Zealand white rabbits with subconjunctival tumors were enrolled (control group 6, laser-only group 8, laser-with-ICG group 8). Rabbits in the laser-with-ICG group received ICG (twice, 2 mg/kg each time, intravenously) directly followed by irradiation with a diode laser (λ = 810 nm). Rabbits in the laser-only group were irradiated with the diode laser. ICG angiography, ultrasonography, and pathologic examination were performed to evaluate PTT response at specific time points (0, 2, and 4 weeks after PTT). Two weeks after initial treatment, the eight rabbits treated by laser with ICG showed a 100% response rate. There was no clinical response in both laser-only and control groups. ICG-PTT is a potential and effective palliative therapeutic modality for subconjunctival tumors.

  • Central serous chorioretinopathy may induce poor eyesight and serous retinal detachment. However, its exact cause has not been well established thus far. It can be associated with systemic high-dose corticosteroid treatment mainly for young and middle-aged men and may spontaneously regress or recur after withdrawal from corticosteroid. After corticosteroid administration for Behcet's disease, it is necessary to identify any ocular symptoms. Behcet's disease can lead to the development of ocular complications, such as uveitis, hypopyon, retinal vasculitis, optic neuritis, angiogenesis, secondary cataract, and glaucoma. It is possible to diagnose any of these complications via optical coherence tomography and digital indocyanine green angiography. It is easy to neglect an ocular symptom that may appear after a low-dose corticosteroid treatment as an ocular complication in patients with Behcet's disease. Thus, we report on a case concerning high-dose corticosteroid treatment with a literature review.

단안에 국소적으로 발현된 색소정맥옆망막맥락막 위축 1예 (A Case of Unilateral Focal Pigmented Paravenous Retinochoroidal Atrophy)

  • 목적: 단안에 국소적으로 발생한 색소정맥옆망막맥락막 위축(pigmented paravenous retinochoroidal atrophy) 1예를 경험하여 이를 보고하고자 한다. 증례요약: 46세 여자 환자가 건강검진상에서 발견된 우안 망막 이상 소견으로 내원하였다. 나안시력은 양안 모두 1.0이었다. 세극등현미경으로 시행한 전안부 검사상 특이 이상 소견은 관찰되지 않았다. 안저검사상 우안의 상측 망막정맥을 따라 국소적으로 분포된 뼈 조각 모양의 색소침착이 동반된 망막맥락막위축 소견이 관찰되었다. 안저자가형광상 우안의 망막맥락막위축의 경계에서 다소 과형광을 보이면서 병변의 대부분은 저형광 소견을 보였다. 빛간섭단층촬영상 해당 병변은 망막색소상피층의 위축 및 뭉침이 혼재되어 있었고, 맥락막층에서 맥락막모세혈관층이 얇아진 소견을 보였다. 형광안저촬영상 병변 내 창문비침 및 형광차단이 혼재되어 관찰되었으며 이는 망막색소상피층의 위축 및 뭉침 부위와 일치하였다. 결론: 단안에 국소적으로 발생한 색소정맥옆망막맥락막 위축 1예를 경험하여 이를 국내 최초로 보고하는 바이다.
